The developed molecular and genetic method for analysis of the carcinogenic potential of Helicobacter pylori based on determining the degree of phosphorylation of the cytotoxin-associated protein of the bacterium (CagA) has been presented. The degree of phosphorylation of CagA protein of Helicobacter pylori is estimated by determining the number and type of so-called EPIYA (Glu-Pro-Ile-Tyr-Ala) motifs at the carboxyl end of the CagA protein region using PCR (polymerase chain reaction) and sequencing of the CagA gene locus. The proposed method can be used to form groups of patients who need additional examination and follow-up observation for the purpose of early prediction of pathological conditions associated with the infectious process caused by Helicobacter pylori strains dominating in the Republic of Belarus. The determination of the number and type of EPIYA motifs can be used as an additional criterion for detection of risk groups for gastrointestinal diseases.

The analysis of the influence of predictors on the probability of retinal detachment in children of the Minsk region was carried out on the basis of a study of the medical records of 660 children's patients treated at the Health care Institutions "4th Children's City Clinical Hospital" and “Minsk Regional Children's Clinical Hospital” for the period 2009–2019. Of these, 313 patients were older than 1 year with an established diagnosis of retinal detachment, 107 children under the age of 1 year, 240 children older than 1 year without retinal detachment at the time of treatment and the presence of predictors of its development (comparison group). To assess the influence of factors, a methodology based on the calculation of the odds ratio was used. The most significant risk factors were assigned a score equivalent from one to four, which allowed quantifying the probability of retinal detachment and identifying risk groups depending on the total score. The use of the proposed method allowed the children of the comparison group to identify changes in the vitreoretinal interface, which required primary laser preventive treatment in 46.7% of cases (113 eyes) during the three-year follow-up period, in 45.4% of children (109 eyes) it was limited only to dynamic observation without treatment. In 95.0% of children in the comparison group, it was possible to prevent the development of retinal detachment.

Abstract Background: To analyze the difference of serum gastrin-17 levels in different sexs, ages, and body mass index (BMI) of healthy people, and to explore the correlation between gastrin 17 and pepsinogen, in addition to study the influence of Helicobacter pylori infection and various inflammatory factors on the secretion level of gastrin-17. Methods: 531 subjects who received physical examination in our center from April 2019 to December 2019 were enrolled in the study. All the staff were tested for gastrin 17 (G17), pepsinogen I (PGI), pepsinogen II (PGII), PGI / PGII (PGR), Helicobacter pylori (Hp) and C-reactive protein (CRP) and other inflammatory factors. To compare the difference of G17 secretion in different populations and its correlation with PG, then to understand the HP infection and the influence of inflammatory indicators on G17. Results: There was no significant difference in the secretion level of G17 in different sex, age and BMI (P > 0.05); G17 was positively correlated with PGI and PGII, but negatively correlated with PGR; the level of G17 in Helicobacter pylori positive patients was 10.16±12.84, which was significantly higher than that in negative patients(3.27±6.65), P =0.017, 95% CI: 1.713 (1.100, 2.668); the increase of serum amyloid A(SAA) in different inflammatory indicators was the high-risk factor of G17 abnormality, P=0.016, 95% CI: 2.692 (1.202, 6.028), obviously CRP and erythrocyte sedimentation rate (ESR) had no effect on G17 abnormalities. Conclusions: The secretion of G17 is closely related to PG and HP. Combined screening is helpful for early screening of gastrointestinal diseases in healthy or high-risk groups of gastric cancer, but the influence of inflammatory indicators on G17 should be excluded to improve the reliability of the results.

Aim of the research. To study the prevalence of H. pylori cytotoxin-associated gene A (CagA) strain and estimate the activity of associated gastritis in schoolchildren with dyspepsia syndrome in the Republic of Tyva. Materials and Methods. We studied the prevalence of H. pylоri CagA strain by performing the cross-sectional examination for picked up randomly 1064 schoolchildren aged of from 7 to 17 years, residing in the Republic of Tyva. We used the technique of determination of blood IgG to CagA antigen of H. pylori in 218 children, including 131 cases with dyspeptic complaints. We also executed esophagogastroduodenoscopy including biopsy sampling. Results. CagA-seropositive children accounted for 47.2%. We noted the increase in the activity of antral gastritis in H. pylori-infected schoolchildren with the maximum being related to CagA-seropositive schoolchildren. Gastritis activity in the body of stomach in H. pylori-infected children was higher as compared to non-infected children, but as a whole it was lower than in antral region of the stomach. Besides that we revealed specific features of gastritis activities in the body and in the antral region of stomach in children in ethnic populations. The peculiarities are referred to the presence of the differences between the mentioned parameters in alien schoolchildren and the absence of such differences in the Tyvan schoolchildren. Conclusion. Widely spread H. pylori, in particular CagA strain of the microorganism, plays the adverse role in the development and especially progressing of gastritis in schoolchildren residing in the Republic of Tyva. The importance of the adverse impact of the infection is associated with the ethnicity of children.

Relevance. If we reduce the treatment exclusively to the eradication of Helicobacter pylori (Hp), the question of monitoring the results of anti-Hp therapy is controversial, given the widely known data on the detection of Hp, including virulent strains of Hp, in most healthy individuals.The aim of the study: to track the dynamics of stomach colonization with various Hp strains in patients with gastric ulcer and duodenal ulcer (PUD) immediately after the use of standard three-component anti-Hp therapy (AHT) and 1.5–2 months after AHT.Methods. Genotyping of Hp strains was carried out by the VNTR method together with the determination of the cagA gene.Results. Assessment of the results of AHT in the form of «eradication of Hp — re-detection of Hp» (i. e., without taking the determination of Hp strains into account) showed less reliability in the differences than the «eradication — preservation of the strain or change of the strain» score in patients with PUD; in addition, the differences between the initial bacteriological picture and that observed immediately after AHT were on the verge of reliability, while the differences between the initial bacteriological picture and that observed after 1.5–2 months had a high degree of reliability. The number of eradications increased (eradication achieved immediately after AHT was not preserved, except for one) and the number of cagA-containing strains decreased (due to new eradication detected at late follow-up periods and due to a change in strains) in long-term follow-up in patients with PUD.Conclusion. Since all patients with PUD achieved clinical remission, which lasted for the next 1.5–2 months, the success of AHT should not be unambiguously associated with the eradication of Hp; the restoration of the organism's colonization resistance to Hp after a course of therapy is more likely.

Background Studies have reported agreement between computed tomography (CT) and renography for the determination of split kidney function. However, their correlation with post-donation kidney function remains unclear. We compared CT measurements with renography in assessment of split kidney function (SKF) and their correlations with post-donation kidney function. Methods A single-centre, retrospective cohort study of 248 donors from January 1, 2009-July 31, 2019 were assessed. Pearson correlations were used to assess post-donation kidney function with renography and CT-based measurements. Furthermore, we examined high risk groups with SKF difference greater than 10% on renography and donors with post-donation eGFR less than 60 mL/min/1.73m2. Results 62% of donors were women with a mean (standard deviation) pre-donation eGFR 99 (20) and post-donation eGFR 67 (22) mL/min/1.73m2 at 31 months of follow-up. Post-donation kidney function was poorly correlated with both CT-based measurements and renography, including the subgroup of donors with post-donation eGFR less than 60 mL/min/1.73m2 (r less than 0.4 for all). There was agreement between CT-based measurements and renography for SKF determination (Bland-Altman agreement [bias, 95% limits of agreement] for renography vs: CT volume, 0.76%, -7.60–9.15%; modified ellipsoid,1.01%, -8.38–10.42%; CC dimension, 0.44%, -7.06–7.94); however, CT missed SKF greater than 10% found by renography in 20 out 26 (77%) of donors. Conclusions In a single centre study of 248 living donors, we found no correlation between CT or renography and post-donation eGFR. Further research is needed to determine optimal ways to predict remaining kidney function after donation.

The literature review presents international statistical data on the rates of both completed and failed suicide attempts in various countries. The works lists the reasons contributing to suicide risk, factors triggering suicidal behavior among which are: gender, age, marital status, severe diseases, psycho-emotional state, economic, social reasons and others. Also, in consideration of the coronavirus disease pandemic spreading around the world, the work adduces the results of the studies on the COVID-19 impact on people including not only patients but also medical personnel severely strained in the situation of the pandemic which have been performed across countries. We have made a conclusion about the necessity and possibilities to prevent suicide attempts, have analyzed the experience of other countries in the organization of assistance to this segment of the population, have advanced forth an opinion on the appropriateness of the development of the national integrated suicide risk prevention policy in the Republic of Belarus.

Legal events, one of which is the death of a person, will cause legal consequences on how to follow up the management of the rights and obligations of someone who has died. Islam teaches a set of shari’ah laws regarding inheritance and will. The Medan Religious Court had determined the compulsory heir but it was sued by another heir. On the other hand, based on the Decision of the Supreme Court Number: 485K/Ag/2013, the plaintiff was declared as the compulsory heir and decided that the defendants were not. This certainly can lead to legal uncertainty. Regarding to this case, the author discusses the settlement of will in inheritance dispute because it raises several problems regarding the factor of inheritance dispute, settlement of inheritance dispute, and implementation of the will. The research in this thesis applies a normative juridical method which is an approach through legal research by examining the principles of law and law comparison. Based on the research results, the conclusion that can be drawn is that the factor causing inheritance dispute is the determination of inheritance to adopted child. Settlement of inheritance dispute, against the Decision of the Supreme Court of the Republic of Indonesia Number: 485K/Ag/2013, invalidates the determination of heirs and states that the Decision of the Religious Courts has no legal force. In addition, related to the settlement of the testament to the inheritance dispute against the Decision of the Supreme Court of the Republic of Indonesia Number: 485K/Ag/2013, the judge has determined to give a wasiyah waajibah (binding will) to the adopted child a maximum of 1/3 of the testator’s inheritance.

In South Africa the modern outlook towards time may be said to have started in 1948. Both the two major observatories, The Royal Observatory in Cape Town and the Union Observatory (now known as the Republic Observatory) in Johannesburg had, of course, been involved in the astronomical determination of time almost from their inception, and the Johannesburg Observatory has been responsible for the official time of South Africa since 1908. However the pendulum clocks then in use could not be relied on to provide an accuracy better than about 1/10 second, which was of the same order as that of the astronomical observations. It is doubtful if much use was made of even this limited accuracy outside the two observatories, and although there may – occasionally have been a demand for more accurate time, it was certainly not voiced.
