Aim of the research. To study the prevalence of H. pylori cytotoxin-associated gene A (CagA) strain and estimate the activity of associated gastritis in schoolchildren with dyspepsia syndrome in the Republic of Tyva. Materials and Methods. We studied the prevalence of H. pylоri CagA strain by performing the cross-sectional examination for picked up randomly 1064 schoolchildren aged of from 7 to 17 years, residing in the Republic of Tyva. We used the technique of determination of blood IgG to CagA antigen of H. pylori in 218 children, including 131 cases with dyspeptic complaints. We also executed esophagogastroduodenoscopy including biopsy sampling. Results. CagA-seropositive children accounted for 47.2%. We noted the increase in the activity of antral gastritis in H. pylori-infected schoolchildren with the maximum being related to CagA-seropositive schoolchildren. Gastritis activity in the body of stomach in H. pylori-infected children was higher as compared to non-infected children, but as a whole it was lower than in antral region of the stomach. Besides that we revealed specific features of gastritis activities in the body and in the antral region of stomach in children in ethnic populations. The peculiarities are referred to the presence of the differences between the mentioned parameters in alien schoolchildren and the absence of such differences in the Tyvan schoolchildren. Conclusion. Widely spread H. pylori, in particular CagA strain of the microorganism, plays the adverse role in the development and especially progressing of gastritis in schoolchildren residing in the Republic of Tyva. The importance of the adverse impact of the infection is associated with the ethnicity of children.

Abstract We examined the prevalence and correlates of Helicobacter pylori (H. pylori) infection according to cytotoxin-associated gene A (CagA) phenotype, a main virulence antigen, among the ethnically diverse population groups of Jerusalem. A cross-sectional study was undertaken in Arab (N = 959) and Jewish (N = 692) adults, randomly selected from Israel's national population registry in age-sex and population strata. Sera were tested for H. pylori immunoglobulin G (IgG) antibodies. Positive samples were tested for virulence IgG antibodies to recombinant CagA protein, by enzyme-linked immunosorbent assay. Multinomial regression models were fitted to examine associations of sociodemographic factors with H. pylori phenotypes. H. pylori IgG antibody sero-prevalence was 83.3% (95% confidence interval (CI) 80.0%–85.5%) and 61.4% (95% CI 57.7%–65.0%) among Arabs and Jews, respectively. Among H. pylori positives, the respective CagA IgG antibody sero-positivity was 42.3% (95% CI 38.9%–45.8%) and 32.5% (95% CI 28.2%–37.1%). Among Jews, being born in the Former Soviet Union, the Middle East and North Africa, vs. Israel and the Americas, was positively associated with CagA sero-positivity. In both populations, sibship size was positively associated with both CagA positive and negative phenotypes; and education was inversely associated. In conclusion, CagA positive and negative infection had similar correlates, suggesting shared sources of these two H. pylori phenotypes.


Background Helicobacter Pylori (H. Pylori) infection is an infectious disease of the gastrointertinal tract especially the stomach and is currently associated with various systemic disorders in the body, one of which is hormonal disorders. Diabetes mellitus (DM) is an important cause of dyspepsia. Impaired gastrointestinal motor function is now recognized as a major problem in diabetes mellitus. The incidence of H. Pylori is increased in DM. Delayed gastric emptying and dysmotility are important causes of dyspepsia in diabetes. Methods This study was conducted by cross sectional method. The sample of the study was 39 patients who met the inclusion and exclusion criteria in the General Hospital H. Adam Malik Medan. Anti IgA and anti IgG H. Pylori examination by ELISA method using Chemwell. Results There were no difference in the incidence of anti IgA H. Pylori positive in patients with controlled DM and uncontrolled DM with p= 0,431. There was no difference in the incidence of anti IgA H. Pylori positive in patients with controlled DM and uncontrolled DM with p= 0,423. However, there were differences in H.Pylori infection in DM patients with dyspepsia and DM patients without dyspepsia with p = 0.001. Conclusion There were no relationship between H. Pylori infections in patients with type 2 DM.

It has been demonstrated that cross sections of bundles of hair can be effectively studied using image analysis. These studies can help to elucidate morphological differences of hair from one region of the body to another. The purpose of the present investigation was to use image analysis to determine whether morphological differences could be demonstrated between male and female human Caucasian terminal scalp hair.Hair samples were taken from the back of the head from 18 caucasoid males and 13 caucasoid females (Figs. 1-2). Bundles of 50 hairs were processed for cross-sectional examination and then analyzed using Prism Image Analysis software on a Macintosh llci computer. Twenty morphological parameters of size and shape were evaluated for each hair cross-section. The size parameters evaluated were area, convex area, perimeter, convex perimeter, length, breadth, fiber length, width, equivalent diameter, and inscribed radius. The shape parameters considered were formfactor, roundness, convexity, solidity, compactness, aspect ratio, elongation, curl, and fractal dimension.

Background: Helicobacter pylori infection and its treatment still remains a challenge to human health worldwide. A variety of antibiotics and combination therapies are currently used to treat H. pylori induced ulcers and carcinoma; however, no effective treatment is available to eliminate the pathogen from the body. Additionally, antibiotic resistance is also one of the main reasons for prolonged and persistent infection. Aim of the study: Until new drugs are available for this infection, vaccinology seems the only alternative opportunity to exploit against H. pylori induced diseases. Methods: Multiple epitopes prioritized in our previous study have been tested for their possible antigenic combinations, and results in 169-mer and 183-mer peptide vaccines containing the amino acid sequences of 3 and 4 epitopes respectively, along with adjuvant (Cholera Toxin Subunit B adjuvant at 5’ end) and linkers (GPGPG and EAAAK). Results: Poly-epitope proteins proposed as potential vaccine candidates against H. pylori include SabAHP0289-Omp16-VacA (SHOV), VacA-Omp16-HP0289-FecA (VOHF), VacA-Omp16-HP0289-SabA (VOHS), VacA-Omp16-HP0289-BabA (VOHB), VacA-Omp16-HP0289-SabA-FecA (VOHSF), VacAOmp16-HP0289-SabA-BabA (VOHSB) and VacA-Omp16-HP0289-BabA-SabA (VOHBS). Structures of these poly-epitope peptide vaccines have been modelled and checked for their affinity with HLA alleles and receptors. These proposed poly-epitope vaccine candidates bind efficiently with A2, A3, B7 and DR1 superfamilies of HLA alleles. They can also form stable and significant interactions with Toll-like receptor 2 and Toll-like receptor 4. Conclusion: Results suggest that these multi-epitopic vaccines can elicit a significant immune response against H. pylori and can be tested further for efficient vaccine development.

The aim of the study is to determine the role of geochemical factors (the content of Mg, K, Ca, Zn, Pb in soils and natural waters of the plain zone of Dagestan) in the prevalence of essential arterial hypertension (EAH) among the population of children. Materials and methods of research: an observational cross-sectional (one-step) study of the prevalence of EAH in the child population was carried out. The prevalence rates of EAH among children in the plain zone of the Republic of Dagestan are calculated for 100,000 children 0–17 years old, without taking into account gender differences. The content of elements (Mg, K, Ca, Zn, Pb) in the environment (soil, natural waters) was determined by atomic absorption spectroscopy. Results: one-way analysis of variance (One-Way ANOVA) of indicators of the content of metals (Mg, K, Ca, Zn, Pb) in the regions and objects (soils, natural waters) of the study revealed a statistically significant difference in the data (for soils: F=81,06772, p<0,0002; for natural waters: F=58,86451, p<0,00001). The dependence of children's EAH on the content of chemical elements in the objects of the biosphere of Dagestan was determined, which was expressed in an increase in the number of patients with EAH when Pb content exceeded (r=+ 0,576, r=+ 0,759) and with the decrease of Mg (r=–0,668, r=–0,173), K (r=–0,440, r=–0,636), Ca (r=–0,693, r=–0,533), Zn (r=–0,051, r=–0,827) in soils and natural waters. Conclusion: when assessing the pathogenesis of EAH in the population of children, the content of Mg, K, Ca, Zn, Pb in soil and water should be taken into account, since these microelements, entering the body with water and food, affect the elemental status of child's body.

Iron (Fe) is one of many heavy metals that is corrosive resistant, dense, and has a low melting point. If accumulated in the body, the metal can cause some medical conditions, such as irritation to skin and eyes, breathing problems, and in the long term, cancer. This research aims to know generally the spread of metallic iron (Fe) in the river Pasar in Belangwetan, Klaten. This study was conducted using an observational method in which researchers did not examine the effects of interventions. Sampling was done using purposive sampling method taken from three points, namely the upper, middle, lower. Determination of iron levels by Atomic Absorption Spectrophotometer (AAS) obtained a positive result, and the data was processed using SPSS to determine the Mean and Standard Deviation. Of the research result, it could be known the Mean score was 2.33 ppm and SD was 0.0352. The result of this research indicated that the levels of iron in the river Pasar in Belangwetan were 2.33 ppm. It means that the levels violate the regulation of the Minister of Health of the Republic of Indonesia, which is not more than 1mg/L (1ppm) in the clean water

In despite of the high clinical impact of Helicobacter pylori, its exact sources and routes of transmission are unknown. Dogs may play an imperative role in the transmission of H. pylori to humans. The current investigation was done to study the status of vacA and cagA genotypes in the H. pylori strains of dogs. One-hundred and fifty fecal samples were collected from healthy and complicated household dogs. Genomic DNA was extracted from fecal samples and presence of 16S rRNA gene was studied using the PCR amplification. Distribution of vacA and cagA genotypes were studied by the multiplex PCR. Thirteen out of 150 fecal samples (8.66%) were positive for H. pylori 16S rRNA gene. Prevalence of H. pylori in healthy and complicated dogs were 5.55% and 8.57%, respectively. Male had the higher prevalence of H. pylori (P=0.038). The most commonly detected genotypes among the H. pylori strains were vacAs1A (61.53%), cagA (38.46%), vacAm1a (38.46%), vacAs2 (30.76%) and vacAm2 (30.76%). The most commonly detected combined genotypes were s1aCagA (30.76%), s1am1a (23.07%), s2m1a (23.07%) and s2CagA (23.07%). Iranian household dogs harbor H. pylori in their fecal samples similar in genotypes of the vacA and cagA alleles which suggest that complicated and even healthy dogs may be the latent host of the H. pylori and its genotypes. However, supplementary studies are required to found the exact role of dogs as a definitive host of the H. pylori.

Context: A cross-sectional study was carried out at the Department of Pathology, Dhaka Medical Collage, Dhaka and Immunology Laboratory, Laboratory Sciences Division of ICDDR,B, Dhaka during a period of 1 year from July, 2007 to June, 2008 to determine the efficacy of endoscopic crush cytology in the detection of Helicobacter pylori infection in gastroduodenal diseases. Clinically suspected cases of gastro-duodenal lesions and who had not taken antibiotics, omeprazole or bismuth salts for at least three weeks prior to endoscopy were selected. Patients who were clinically and endoscopically suspected of having malignancy were excluded from the study. A total of 110 such subjects were consecutively included in the study. The statistics used to analyze the data were descriptive statistics and components of accuracy test.Results: The sensitivity of crush cytology in correctly diagnosing H. pylori of those who had the disease was 89.3%, while the specificity of the test in correctly differentiating those who did not have H. pylori was 92.6% when compared against histopathological examination using Giemsa stain. However, a slightly low sensitivity (86.2%) without compromising with specificity (92.3%) was obtained when the crush cytology diagnosis was compared against histopathological examination using haematoxylin-eosin (H & E) stain.Conclusion: The study concludes that the diagnostic accuracy of crush smear cytology (sensitivity and specificity) for detection of Helicobacter pylori in gastric biopsy material is comparable to histopathology. Moreover, the technique is very simple, less expensive and less time consuming which gives clinicians added advantage in making a quicker decision.Key words: Cytology; Helicobacter pylori; Gastroduodenal disease. The aim is to study the effect of Helicobacter pylori infection on risk of developing gastroesophageal reflux disease. Materials and methods - cross-sectional observational study of 1007 patients with dyspepsia syndrome who underwent videoesophagogastroduodenoscopy with biopsy and histological examination of biopsy specimens of the gastric mucosa by OLGA-system. The age, gender, overweight, cigarette smoking, presence of Helicobacter pylori infection and gastritis stage were assessed. Results - the study showed a significant decrease in the incidence of gastroesophageal reflux disease in patients with positive H. Pylori status by 4% (RR 0,68; 95% CI, 0.49-0.94, p=0,041). The risk of developing gastroesophageal reflux disease significantly higher in overweight (RR 2,62; 95% CI 2,0-3,56; р<0,001) men (RR 1,76; 95% CI 1,33-2,32; р=0,0046) who smoked cigarettes (RR 3,23; 95% CI 2,45-4,24; р<0,001) and was not associated with the patient’s age and the stage of gastritis (р>0,05). Conclusion - a significant reduction in the frequency and risk of developing gastroesophageal reflux disease in patients with Helicobacter pylori infection is demonstrated.

Editorial: Helicobacter pylori is a micro-aerophilic, helical-form gramnegative aggressive bacteria. Accordingly, the idiom “Helico” intimates its helical appearance, “bacter” symbolizes bacteria, while “pylori” denotes stomach due to the first and common site of this bacteria living. Further, Marshall B. and Warren R. observed and described it in 1982. Then, the followed investigators studied this bacterium in detail with its consequences and complexities [1]. Gastric upset (Indigestion), dyspepsia: means impaired gastric digestion. Accordingly, the patient complains of upper abdominal pain, heartburn, belching, nausea, even feeling earlier gastric fullness than expected while eating. Furthermore, there are many causes of indigestion like gastroesophageal reflux disease, ulcer disease, gastritis, and even gastric cancer. Hence, unexplained recent onset dyspepsia in older people may need additional examinations. Moreover, one of the common causes is Helicobacter pylori infection, which needs laboratory and endoscopic examination [2]. Argument Many theories investigated the etiology and pathogenesis of Helicobacter pylori infection, concerning chronic or acute gastritis. Hence, gastric upset is the main presentation of both types of gastritis. Evidences The genotype is valuable in determining the dominant Helicobacter pylori strains as the isolates were different genetically plus heterogeneous distribution. Accordingly, the vac and cag markers operate a significant function in defining clinical consequences. These virulence agents are present in a subset of Helicobacter pylori strains isolates like cagA, iceA, vacA, and ureC. Moreover, the cagA causes cytotoxins induction by the gastric epithelial cell as Interleukin 8 [3]. The molecular intercommunication researches exhibit that the act of acarus calamus in hindering biofilm formation in Helicobacter pylori is due to the inhibitory impact of phytobio-active component, β-sitosterol, on the quorum sensing molecules-ToxB, PhnB, DnaA, plus Sip. Consequently, this opinion may suggest the molecular mechanism of Helicobacter pylori in producing the acidrelated complaints and gives a clue to a new therapy [4]. Helicobacter pylori infection causes lncRNA risk impression linked to H. pylori in gastric cancer patients and can prognosticate the prediction of these patients [5]. There was a close relationship between raised serum IgE levels in Helicobacter pylori infected patients [6]. Counterargument The laboratory investigations of Helicobacter pylori infection depend on several factors like the fluctuations of serum antibody titers in a time series, the antigene detection in stool tests, the false-positive results of lab tests, or the manner of endoscopic biopsy collection. Furthermore, other factors like the variations in Cytotoxin-Associated Gene A (CagA) in East Asian patients. Moreover, the gastric nodularity or atrophy, the patient’s age, the severity of the gastric mucosal infection are causes of variations in Helicobacter pylori detection at the time of the investigation [7]. Refutation The significant markers of H. pylori, the presence of the vacuolating cytotoxin (vacA), the cytotoxin-associated gene A (cagA), which induced by the direct communication with gastric epithelium factor antigen (iceA gene), and the presence of urease C gene (ureC). Consequently, all these factors play the principal factors in deciding the gastric consequences of Helicobacter infections. Conclusion Helicobacter pylori induce gastric upset by several mechanisms to form numerous Gastric diseases.
