The aim of the study was to analyze the effectiveness of treatment of patients using the Twin-Forse apparatus in adulthood. A case of a 25-year-old patient was treated with a retroposition of the lower jaw, distal occlusion of the posterior teeth, sagittal incisal disocclusion, deep incisal disocclusion, narrowing of the deformation of the dental alveolar arches, protrusion of the upper and lower incisors, crowded position of the lower teeth. Fixed orthodontic appliances of the Damon Q system (Ormco, USA) were used. Half a year after the fixation of the bracket system, a Twin-Forse apparatus (Ortho Organizers Inc., USA) was installed. After half a year of wearing, the device was removed, the patient was assigned to wear intermaxillary elastics to achieve maximum fissure-tubercular contacts. It has been found that the use of the Twin-Forse device in adult patients helps to achieve optimal functional and aesthetic results.

Subject. The depth of the overbite increases with age. With the loss of teeth secondary deformations of the dentition, dysfunctions, teeth wearing, periodontal disease and temporomandibular joint (TMJ) dysfunctions occurs, facial aesthetics is affected. The urgency of the topic depends on the high prevalence of anomaly and self-destruction of the orofacial system (OFS) with deep overbite in adults. The aim was to identify the factors of decompensation of the dentition in adults with deep overbite. Methodology. Clinical (questioning, examination, functional tests), anthropometric (photo analysis, casts’ analysis, Shimbachi index) methods and X-ray analysis(cephalometry, analysis of zonograms of TMJ, orthopantomography) were performed for 84 adult patients with deep overbite more than 3 mm (GRP) and Angle Class I or II. All patients were divided into 2 main groups according to the size of overjet: OG-1 ― 42 people with a normal overjet (2 ± 2 mm) between the incisors; OG-2 ― 42 people with an enlarged overjet (more than 4 mm). We used the MSUMD classification of malocclusion (1991). The results of the examination of the patients in 2 main groups were compared with each other and with the average structure of the OFS in patients with physiological occlusion. We have identified specific clinical, anthropometric, and radiological groups of symptoms of decompensation in OFS, which are common for adults with GRP. Conclusions. In adult patients with GRP the main factor of decompensation in OFS is lower jaw displacement. In the development of displacement of the lower jaw a few factors are important: transversal disproportion of the dentition; sagittal jaw disproportion; loss of posterior teeth. The loss of the teeth, the reduction of the interalveolar height and the associated functional and aesthetic problems are adaptive mechanisms for the displacement of the mandible.

AbstractFixed orthodontic appliances are common and effective tools to treat malocclusion. Adverse effects of these appliances, such as dental caries and periodontitis, may be associated with alteration of the microbiome. This study investigated the impact of these appliances on the dynamics of the oral microbiome. Seventy-one patients were selected. Supragingival plaque samples were collected before placement (T0) and six months after placement (T1). Saliva samples were collected at T0 and T1, and then when appliance removal (T2). Microbial DNA was analyzed by 16S rRNA meta-sequencing. The diversity analysis indicated dynamic changes in the structure of the oral microbiome. Taxonomic analysis at phylum level showed a significant increase in Bacteroidetes and Saccharibacteria (formally TM7) and decrease in Proteobacteria and Actinobacteria over time, in both plaque and saliva. Genus level analysis of relative abundance indicated a significant increase in anaerobic and facultative anaerobes in both plaque and saliva. Fixed orthodontic appliances induced measurable changes in the oral microbiome. This was characterized by an increase in relative abundance of obligate anaerobes, including periodontal pathogens. It can be concluded that this dysbiosis induced by fixed orthodontic appliances is likely to represent a transitional stage in the shift in microbiome from healthy to periodontitis.

Abstract Objectives Adults with fixed orthodontic appliances are increasing nowadays. Compared with adolescents, adults present biological differences that might influence treatment duration. Therefore, the aim of the study was to compare duration of treatment with fixed appliances between adults and adolescents. Materials and methods Eight databases were searched up to September 2019 for randomized and non-randomized clinical studies comparing treatment duration with fixed appliances in adolescents and adult patients. After duplicate study selection, data extraction, and risk of bias assessment with the Cochrane ROBINS-I tool, random effects meta-analyses of mean differences (MD) and their 95% confidence intervals (CIs) were performed, followed by assessment of the quality of evidence with GRADE. Results A total of 11 unique studies (one prospective and 10 retrospective non-randomized) with 2969 adolescents and 1380 adult patients were finally included. Meta-analysis of 7 studies found no significant difference in the duration of comprehensive treatment with fixed appliances (MD = − 0.8 month; 95% CI = − 4.2 to 2.6 months; P = 0.65; I2 = 92%) between adults and adolescents. Similarly, both distalization of upper first molars with skeletal anchorage for class II correction and the retraction of canines into the premolar extraction spaces lasted similarly long among adults and adolescents. On the other hand, alignment of palatally displaced canines lasted considerably longer in adults compared to adolescents (1 study; MD = 3.8 months; 95% CI = 1.4 to 6.2 months; P = 0.002). The quality of evidence for the meta-analysis was low due to the inclusion of non-randomized studies with considerable risk of bias. Conclusions While existing evidence does not indicate a difference in the overall duration of treatment with fixed appliances between adults and adolescents, the alignment of palatally displaced canines lasted significantly longer in adults. However, our confidence in these estimates is low due to the risk of bias in the included studies. Trial registration PROSPERO: (CRD42019148169)

BackgroundThere is limited data on the beneficial effects of probiotics on the gingival health of patients undergoing treatment with fixed orthodontic appliances. This study aims to compare the effect of probiotic tablets combined with regular oral hygiene versus regular oral hygiene alone on gingival status in these patients. The effect of probiotic intake on plaque formation and salivary microbiome composition will be also assessed.Methods and analysisThis is a 3 month single-centre, single blind (clinical and laboratory examiners), parallel group randomised controlled two arm superiority trial. Fifty paediatric patients attending the Postgraduate Orthodontic Clinic at the Hamdan Bin Mohammed College of Dental Medicine (HBMCDM), Mohammed Bin Rashid University of Medicine and Health Sciences (MBRU), Dubai, United Arab Emirates, who meet the eligibility criteria will be recruited. Block randomisation with 1:1 allocation and concealment of allocation will be carried out. The treatment group will receive probiotic tablets containingStreptococcus salivariusM18 andLactobacillus acidophilustogether with regular oral hygiene versus the control group on regular oral hygiene alone. Clinical examination and collection of saliva for microbiome assay will be carried out at baseline and end of study. Self-reporting by patients will be used to document acceptability and adverse effects. Statistically significant decrease in gingival bleeding on probing in the treatment group will be classified as primary outcome of treatment success. Statistically significant reduction in Plaque Index, Gingival Index and shift in the composition of the oral microbiome in favour of beneficial bacteria are secondary outcomes indicative of efficacy of probiotic intake.Ethics and disseminationEthical approval for the study has been granted by the HBMCDM, MBRU, Institutional Review Board (Reference #: MBRU-IRB-2018–015). Study findings will be disseminated via publication in peer-reviewed journal.Trial registration numberISRCTN95085398

This study is dedicated to a comparative analysis of effectiveness of using orthodontic appliances when treating class II malocclusion in children and adolescents. It was shown that treatment with the Invisalign system with a change of the lower jaw position is the most effective method for correcting class II malocclusion in growing patients. Using Invisalign aligners showed not only the best efficacy according to teleroentgenograms. but also made the treatment process comfortable for patients at all its stages. Moreover, higher level of compliance was demonstrated with treatment using the Invisalign system compared to the Twin-block appliance.

Relevance. Deep overbite is one of the most frequent malocclusions in children and teenagers of different ages. Distal occlusion with a deep overbite is the most common (59.6-67.6% according to the literature) in early mixed dentition. Most Russian and international authors state in their research that the severity of malocclusion increases with age. The treatment of the pathology in children is relevant due to deterioration of the deep overbite with age, worsening of functional disorders.Materials and methods. The study analyses the treatment results of a child with a deep bite using the authors’ technique.Results. The study demonstrated the advantages of the authors’ technique. The treatment allowed achieving normal dentoalveolar heights in the posterior mandible on molar and premolar eruption, establishing the correct relationship between the upper and lower dental arches that promoted harmonious development of the child’s maxillofacial area. The treatment regulated the length and width of the upper dental arch and corrected its form. The active intrusion of the lower incisors and extrusion of the lower posterior teeth were observed during the treatment. The curve of Spee was restored, and an increased overlap reduced. Occlusal contacts of posterior teeth improved, and deep overbite decreased. The technique is easy to use and allows curing 9-12-year-old children in a short period (7-10 months).Conclusions. The suggested technique treats deep overbite as well as creates conditions for the further harmonious development of the maxillofacial area. Thus, it can be recommended for the treatment in early mixed dentition.
