      RELATIONSHIP OF MARITAL STATUS, APOE ε4, AND TYPES OF PHYSICAL ACTIVITIES ON COGNITIVE DECLINE IN ELDERLY WOMENABSTRACTIntroduction: The number of elderly population in Indonesia continues to increase every year, especially women. Elderly women are at higher risk of experiencing decreased cognitive function.Aims: To determine the relationship between the characteristics  of the subject, vascular risk factors, APOE ε4 genotype, and the type of physical activity with the decline of cognitive function in elderly women.Methods: A cognitive cohort study of 114 elderly women who had been observed for a mean of ±2.5 years, as part of the Active Aging study at Atma Jaya Catholic University. The independent variables of this study included were age, education, marital status, vascular risk factors (hypertension, diabetes, dyslipidemia, and BMI), APOE ε4, and types of physical activity. Cognitive function is measured using Mini Mental State Examination (MMSE).Results: There were 114 subjects with mean age was 71.22±7.297 and 33.3% experienced a decline in cognitive function. Age, marital status (single), and type of physical activity (cooking) have a significant relationship with Relative Risk (RR) of 4,45; 0,12; and 0,334.Discussions: Factors related to cognitive decline in elderly women were age. Lansia perempuan berisiko lebih tinggi mengalami penurunan fungsi kognitif.Tujuan: Mengetahui hubungan antara karakteristik subjek, faktor risiko vaskuler, genotip APOE ε4, dan jenis aktivitas fisik terhadap penurunan fungsi kognitif pada lansia perempuan.Metode: Penelitian kohort terhadap 114 lansia perempuan >60 tahun yang diikuti fungsi kognitifnya selama rerata ±2,5 tahun, sebagai bagian dari penelitian Active Aging di Universitas Katolik Atma Jaya. Variabel bebas penelitian ini meliputi usia, pendidikan, status perkawinan, faktor risiko vaskuler (hipertensi, diabetes, dislipidemia, dan IMT), APOE ε4, dan jenis aktivitas fisik. Fungsi kognitif diukur menggunakan Mini Mental State Examination (MMSE).Hasil: Didapatkan 114 subjek dengan rerata usia 71,2±7,3 dan 33,3% mengalami penurunan fungsi kognitif. Analisis multivariat menunjukkan usia, status perkawinan (single) , dan jenis aktivitas fisik (memasak) memiliki hubungan bermakna dengan risiko relatif (RR) masing-masing 4,46; 0,12; dan 0,334.Diskusi: Faktor risiko gangguan fungsi kognitif pada lansia perempuan adalah usia.    EFFECTS OF HOUSEWORK IN PREVENTION OF COGNITIVE DECLINE IN ELDERLY WOMEN WITH LOW EDUCATIONABSTRACTIntroduction: Physical activities have a role in preventing or slowing down cognitive decline. In Indonesia, physical activity in the elderly women is dominated with house work chores.Aims: This study was aimed to determine the relationship between house chores with cognitive function in elderly women.Methods: A cross sectional analytic descriptive of 65 women aged 60 years and over with highest education level of junior high school that live in Kalianyar, West Jakarta, between October to November 2014. Physical activity was measured by questionnaire about nine housework activities that Indonesian women usually do and cognitive function was assessed by Mini-Mental State Examination (MMSE).Results: In this study of 65 subjects with a mean age of 64.31 years and low education level, there was a significant relationship between the higher amount of physical activity and better cognitive level. The types of physical activity that had a significant relationship with better cognitive function was cooking, grocery shopping, and washing dishes, with the frequency of cooking and grocery shopping had a significant relationship.Discussion: Cooking and grocery shopping activity have a relationship with better cognitive function.Keywords: Cognitive function, daily physical activity, elderly, house choresABSTRAKPendahuluan: Aktivitas fisik dianggap memiliki peran dalam pencegahan atau perlambatan penurunan fungsi kognitif. Di Indonesia, aktivitas fisik pada lansia perempuan didominasi dengan pekerjaan rumah tangga.Tujuan:  Mengetahui  hubungan  aktivitas  fisik pekerjaan  rumah  tangga  terhadap  fungsi  kognitif  pada  lansia perempuan.Metode: Penelitian deskriptif analitik potong lintang terhadap lansia (>60 tahun) perempuan yang mengenyam pendidikanakhir paling tinggi tamat SMP dan tinggal di Kelurahan Kalianyar, Jakarta Barat, pada bulan Oktober-November 2014. Aktivitas fisik diukur menggunakan kuesioner sembilan aktivitas pekerjaan rumah tangga yang dilakukan lansia perempuan Indonesia dan penilaian fungsi kognitif menggunakan Mini-Mental State Examination (MMSE).Hasil: Pada penelitian terhadap 65 subjek dengan rerata usia 64,31 tahun berpendidikan rendah, terdapat hubungan bermakna antara jumlah aktivitas fisik yang dilakukan dengan fungsi kognitif yang lebih baik. Adapun jenis aktivitas fisik yang memiliki hubungan bermakna dengan fungsi kognitif yang lebih baik adalah memasak, berbelanja, dan mencuci piring, terutama frekuensi memasak dan berbelanja yang lebih sering memiliki hubungan secara bermakna.Diskusi: Aktivitas memasak dan berbelanja berhubungan dengan fungsi kognitif yang lebih baik.Kata kunci: Aktivitas fisik sehari-hari, fungsi kognitif, lansia, pekerjaan rumah tangga

THE RELATIONSHIP BETWEEN ELEVATED C-REACTIVE PROTEIN AND COGNITIVE FUNCTION DETERIORATION OF GERIATRIC OUTPATIENTSABSTRACTIntroduction: Geriatric patients have different characteristics compared to other patients in general, in which they potentially experiencing cognitive impairment, decreasing of physiological and functional status, also the immunological system. This situation causes various inflammatory reactions that play a role in the development of neurodegenerative diseases, including the increase production of acute phase protein called C-reactive protein (CRP).Aims: To determine the association  between the CRP serum concentration  elevation and the deterioration  of geriatric outpatients’s cognitive function at Neurology Clinic Sebelas Maret University Hospital.Methods: A cross-sectional study of geriatric patients at the Neurology Clinic of Sebelas Maret University Hospital, Surakarta.   The cognitive state was evaluated using Mini-Mental State Examination (MMSE) and those with MMSE score<24 were considered cognitively declined. Concentrations of serum CRP were measured. Multiple logistic regression analysis was used to calculate Odds ratios (ORs) for cognitive decline, adjusted for the covariates of age, sex, body mass index, and disease history.Results: There were 73 participants with mean age was 65.6 years old. There were 56,2% who were experiencing a cognitive decline. Relative to the lowest (first) quartile of CRP concentration, adjusted ORs were 1,44 for the second, 1,97 for the third, and 2,33 for the highest quartiles (p=0,035). The association between CRP levels and decreased cognitive function was found to be significant after adjusting for covariates. When data were stratified by sex, the association between CRP concentration and cognitive decline was observed in women.Discussions: This research  suggested an association  between higher CRP concentration  and lower cognitive function. Keadaan ini menimbulkan berbagai reaksi inflamasi yang berperan dalam perkembangan penyakit neurodegeneratif, antara lain peningkatan produksi protein fase akut bernama C-reactive protein (CRP) oleh hepar.Tujuan: Mengetahui hubungan antara peningkatan kadar CRP dengan penurunan fungsi kognitif pasien geriatri rawat jalan di Poliklinik Saraf RS Universitas Sebelas Maret.Metode: Penelitian potong lintang terhadap pasien geriatri di Poli Saraf RS Universitas Sebelas Maret, Surakarta. Evaluasi fungsi kognitif menggunakan Mini-Mental State Examination (MMSE); dengan skor <24 dikategorikan mengalami penurunan fungsi kognitif. Kadar serum CRP masing-masing subjek juga diukur. Analisis regresi logistik berganda digunakan dalam menghitung rasio Odds (RO) untuk penurunan fungsi kognitif, dengan pengaturan kovariat usia, jenis kelamin, indeks massa tubuh, dan riwayat penyakit sebelumnya.Hasil: Didapatkan 73 subjek dengan rerata usia 65,6 tahun. Sebanyak 56,2% subjek mengalami penurunan fungsi kognitif. Dengan perbandingan relatif terhadap kuartil konsentrasi CRP terendah (pertama), RO yang telah diatur adalah 1,44 untuk kuartil kedua; 1,97 untuk kuartil ketiga; serta 2,33 untuk kuartil keempat (p=0,035). Hubungan kadar CRP dan penurunan fungsi kognitif ditemukan signifikan setelah disesuaikan dengan kovariat. Setelah dilakukan sub-analisis berdasarkan jenis kelamin, peningkatan kadar CRP terhadap penurunan kognitif secara signifikan terjadi pada subjek perempuan.Diskusi: Penelitian ini menunjukkan adanya hubungan antara peningkatan kadar CRP dengan penurunan fungsi kognitif. Metabolic Syndrome (MetS) is a cluster of conditions, each of which represents a risk factor for cardiovascular disease: central obesity, hyperglycemia, dyslipidemia and hypertension. Any of these conditions and MetS itself have been associated to Alzheimer's Disease and Vascular Dementia. In recent years there is a growing evidence for the role of physical activity in preventing metabolic diseases and cognitive decline. In our research we assessed the prevalence of MetS in a sample of 154 elderly people. Furthermore, we evaluated cognition (with Mini Mental State Examination, MMSE)  and the physical activity level in every patient. We found a significant association between MetS, borderline cognitive impairment and sedentary lifestyle.

The role of cholesterol in Cognitive Decline is less known as compared to other pathologies linked with dyslipidaemia. However, Cholesterol is well-known to have multiple functions in normal functioning of the brain and the aging process. This study therefore aimed at finding a co-relation, if any, between Cholesterol and age-related cognitive decline.To assess Cognitive Function of subjects and compare it with respective blood cholesterol levels. The study included 50 elderly subjects from Maharashtra (females: 12%) assessed using Marathi version of Mini Mental State Examination (MMSE) and their cholesterol blood concentrations obtained using a Lipid Profile investigation. Cholesterol levels and cognitive function was then co-related using statistical analysis.The subjects with a higher Total Cholesterol & LDL Cholesterol had slightly less scores on the Mini Mental State Examination (MMSE) Test. However, the difference was found to be significant.Based on the results obtained, no relevant co-relation could be found between Blood Cholesterol Levels and Cognitive Function

AbstrakGangguan kognitif merupakan masalah yang sering terjadi pada golongan usia lanjut. Prevalensi gangguan kognitif tinggi pada negara yang memiliki populasi usila yang tinggi. Indonesia merupakan negara keempat dunia yang memiliki populasi usila tertinggi dan diperkirakan akan menjadi ketiga tertinggi pada 2020. Antara usaha preventif yang dilakukan adalah dengan beraktivitas fisik. Penelitian ini dilakukan untuk melihat apakah ada hubungan antara tingkat aktivitas fisik terhadap fungsi kognitif pada usila. Jenis penelitian ini adalah studi cross sectional dengan metode non probability sampling. Populasi penelitian adalah usila ≥60 tahun yang berada di Kelurahan Jati Kecamatan Padang Timur dan memenuhi kriteria inklusi dan eksklusi. Total sampel seramai 51 orang dengan 13 laki-laki dan 38 perempuan. Tingkat aktivitas fisik dinilai menggunakan General Practice Physical Activity Questionnaire (GPPAQ) dan fungsi kognitif dinilai menggunaan Mini Mental State Examination (MMSE). Data diolah dengan uji statistik chi square menggunakan program SPSS. Hasil univariat didapatkan persentase usila yang aktif sebanyak 29.4% dan yang kurang aktif 70.6%. Persentase usila dengan fungsi kognitif yang normal sebanyak 82.4% dan yang mengalami penurunan 17.6%. Hasil bivariat didapatkan ada hubungan antara tingkat aktivitas fisik dengan fungsi kognitif dimana nilai p = 0.044 (p < 0.05). Penelitian ini menunjukkan terdapat hubungan antara tingkat aktivitas fisik dengan fungsi kognitif usila di Kelurahan Jati Kecamatan Padang Timur.Kata kunci: fungsi kognitif, aktivitas fisik, usia lanjutAbstractCognitive impairment is common and highly age-related in the world. The prevalence of cognitive impairment is high in the country with high proportion of elderly. Indonesia is the fourth country with the highest population of elderly and estimated to be the third highest in 2020. Little attention has been paid to the identification of modifiable lifestyle habits for its prevention. This study aimed to determine whether there is a relationship between the level of physical activity and cognitive function in the elderly. This study was a cross sectional study with non probability sampling method. The study population was elderly ≥ 60 years who reside in the Village of Jati, East Padang district and fit the inclusion and exclusion criteria. There were 51 samples with 13 males and 38 females. The level of physical activity was measured by using General Practice Physical Activity Questionnaire (GPPAQ) and the cognitive function was assessed by using Mini Mental State Examination (MMSE). Data processed by the chi-square statistical tests using SPSS. Results of univariate analysis level of physical activity showed the percentage of active elderly is 29.4% and less active is 70.6%. Elderly with normal cognition is 82.4% and with impaired cognition is 17.6%. Results of bivariate analysis showed a significant relationship exists between the level of physical activity and cognitive function in the elderly p = 0.044 (p < 0.05). This study showed that there is a relationship between the level of physical activity and cognitive function in the elderly.Keywords: cognitive function, physical activity, elderly

AbstractFrailty is considered a multidimensional geriatric syndrome, manifested by the accumulation of age-associated deficits. The consequences of frailty transitions are still understudied. This study evaluated the influence of frailty transitions on cognitive function in the older adult population. We used data derived from the Korean Longitudinal Study of Aging (KLoSA) (2008–2018) on older adults aged ≥ 65 years. Frailty was assessed using a validated Korean frailty measure known as the frailty instrument (FI), and cognitive function was measured using the Korean version of the Mini-Mental State Examination (K-MMSE). Transitions in frailty and their relationship with cognitive function were investigated using lagged generalized estimating equations (GEE), t-tests, and ANOVA. Respondents who experienced frailty transitions (those with ameliorating frailty), those who developed frailty, and whose frailty remained constant, were more likely to have a lower cognitive function than those who were consistently non-frail. Older age, activities of daily living (ADL) disability, and instrumental ADL disability were more negatively associated with declining cognitive function, especially in the “frail → frail” group. Changes in all individual components of the frailty instrument were significantly associated with impaired cognitive function. The results suggest an association between frailty transitions and cognitive impairment. Over a 2-year span, the remaining frail individuals had the highest rate of cognitive decline in men, while the change from non-frail to frail state in women was significantly associated with the lowest cognitive function values. We recommend early interventions and prevention strategies in older adults to help ameliorate or slow down both frailty and cognitive function decline.

Abstract Background Cognitive dysfunction in idiopathic interstitial pneumonia (IIP) is an important clinical co-morbidity that is associated with impaired lung function. The aim of the work is to assess cognitive function in major IIP and to find out the relation between cognitive dysfunction and the oxygenation parameters. Results Fifty individuals were involved in the study; 30 patients with major IIP and 20 healthy individuals. Patients with IIP had significantly lower mini mental state examination (MMSE) score compared to the control group (P < 0.001). Wechsler Deterioration Index (WDI) revealed that 33.3% (n = 10) of the patients with IIP had sure cognitive impairment and 26.6% (n = 8) had ongoing cognitive deterioration. Patients with idiopathic pulmonary fibrosis (IPF) had lower cognitive function than other IIP. Conclusion There is an impairment of cognitive function in patients with major IIP, particularly in IPF, as measured by WDI and MMSE. Further large studies are needed to assess the possible predictors of cognitive impairment and their effects on the patients’ outcome.

Abstract Background: Anticholinergic medication use is linked with increased cognitive decline, dementia, falls and mortality. The characteristics of the population who use anticholinergic medication are not known. Here we estimate the prevalence of anticholinergic use in England’s older population in 1991 and 2011, and describe changes in use by participant’s age, sex, cognition and disability. Methods: We compared data from participants aged 65+ years from the Cognitive Function and Ageing Studies (CFAS I and II), collected during 1990-1993 (N=7,635) and 2008-2011 (N=7,762). We estimated the prevalence of potent anticholinergic use (Anticholinergic Cognitive Burden [ACB] score=3) and average anticholinergic burden (sum of ACB scores), using inverse probability weights standardised to the 2011 UK population. These were stratified by age, sex, Mini-Mental State Examination score, and activities of daily living (ADL) or instrumental ADL (IADL) disability. Results: Prevalence of potent anticholinergic use increased from 5.7% (95% Confidence Interval [CI] 5.2-6.3%) of the older population in 1990-93 to 9.9% (9.3-10.7%) in 2008-11, adjusted odds ratio of 1.90 (95%CI 1.67 – 2.16). People with clinically significant cognitive impairment (MMSE [Mini Mental State Examination] 21 or less) were the heaviest users of potent anticholinergic in CFAS II (16.5% [95%CI 12.0-22.3%]). Large increases in the prevalence of the use medication with ‘any’ anticholinergic activity were seen in older people with clinically significant cognitive impairment (53.3% in CFAS I to 71.5% in CFAS II). Conclusions: Use of potent anticholinergic medications nearly doubled in England’s older population over 20 years with some of the greatest increases amongst those particularly vulnerable to anticholinergic side-effects.
