Assessment of the antidiabetic potential of Gymnemic acid as α-amylase and α-Glucosidase inhibitor using invitro and insilico tools

Diabetes Mellitus is a metabolic disorder characterized by elevated blood glucose level caused due to deficiency of insulin secretion or insulin action. The management of the blood glucose level is an important approach in the control of diabetes mellitus complications. There are many divergent therapeutic strategies in the management of Type II diabetes. The inhibition of carbohydrate hydrolyzing enzymes such as α-amylase and α-glucosidase can be used to lower postprandial blood glucose levels. The objective of the present study is to evaluate the inhibitory potential of Gymnemic acid in various concentrations on α-amylase and α-glucosidase activities. Gymnemic acid and voglibose show the IC50 value of 68.12 and 46.23 on alpha amylase and IC50 value of 65.77 and 36.67 on alpha glucosidase activity respectively. Insilico docking study between α-amylase and α-glucosidase with gymnemic acid and voglibose depict the similar amino acid binding sites that substantiate that gymnemic acid might be used as a natural and cost-effective alternative to the synthetic drug. This study throws light on the anti-diabetic action of phytomedicine, Gymnemic acid, a component of Gymnema sylvestre.

Objective: Diabetes mellitus is a leading cause of death and disability in the world and its prevalence is predicted to rise to 10% by 2030. Hence, this study is conducted with objectives to find out the effect of aerobic exercises and resisted exercises on blood glucose levels in type 2 diabetes mellitus (T2DM) subjects and to compare the effect of both exercises on blood glucose level.Method: The comparative study was conducted at Krishna Institute of Medical Sciences Deemed University, Physiotherapy department, Karad. 30 participants with age group between 30 and 65 years were taken. Subjects were selected as per inclusion and exclusion criteria. Group A (15) participants were given aerobic exercise on static bicycle, and Group B (15) participants were given resistance training using dumbbells and weight cuffs for 5 days/week for 4 weeks. Diet recommendations were given to every participant.Results: Statistical analysis was performed using paired and unpaired t-test. Analysis showed statistically extremely significant difference in fasting blood glucose level and postprandial blood glucose level in both the groups (p≤0.0001).Conclusion: Thus, this study concludes that both aerobic exercises and resistance training prove to be beneficial in controlling blood glucose levels in T2DM subjects.

This study aims to investigate the influence of patient characteristics on the choice of insulin therapy in type 2 diabetes mellitus outpatients in Denpasar municipality. This is a descriptive analysis study using the patient's medical records as research material. Patients who meet inclusion and exclusion criteria are being recorded based on their medical records. Characteristics that are taken are age, gender, fasting blood glucose level (FBG), 2-hour postprandial blood glucose level (2-hours PPG) and HbA1c values of patients. Types of insulin therapy gained from patient medical records and drug use report in pharmacy. Characteristics data and type of insulin analyzed using correlation test to determine the effect of the patient characteristics on the selection of insulin therapy. 43 patients became the research subject. Males gendered patients (72.09%) and the patients aged less than 65 years (90.70%) are the dominant characteristics of the research subjects. The average value of FBG of patients is 212 mg / dL; 2-hours PPG 280 mg / dL and HbA1c 10.1%. There is a correlation between sex, age, HbA1c value and FBG with the type of insulin obtained by patients (p <0.05). Based on the results of statistical tests, age and gender have a strong correlation on insulin choice, HbA1c and FBG level has a moderate influence and 2-hours PPG have a weak correlation. Patient characteristics had an influence on the type of insulin choice for diabetes mellitus type 2 outpatient in the Denpasar municipality.

Abstract: Blood glucose must be maintained in a constant concentration. Hyperglycemia, an increase of blood glucose level, can be a symptom of diabetes mellitus. One of the factors that affect the blood glucose level is physical activity. Low physical activity can be influenced by work. Sedentary lifestyle is identic with office workes. Office workers do not enough time to do physical activity. This study was aimed to describe the fasting blood glucose level among office workers. This was a descriptive study with a cross sectional design. Respondents were obtained by using total sampling method. There were 52 respondents in this study consisted of 25 males and 27 females. The results showed that 45 respondents (86.54%) had normal fasting blood glucose levels, 5 respondents (9.62%) had high fasting blood glucose levels (hyperglycemia), and 2 respondents (3.84%) had low fasting blood glucose levels (hypoglycemia). The maximum value was 243 mg/dL, the minimum value was 63 mg/dL, the median value was 83 mg/dL, the average value was 94.42 mg/dL, and standard of deviation was 37.85 mg/dL. Conclusion: Most office workers had normal blood glucose levels.Keywords: fasting blood glucose, office workers Abstrak: Glukosa darah dalam tubuh manusia harus dijaga dalam konsentrasi yang konstan. Kadar glukosa darah dalam tubuh yang meningkat (hiperglikemia) dapat menjadi gejala penyakit diabetes mellitus. Salah satu faktor yang memengaruhi kadar glukosa darah ialah aktivitas fisik. Aktivitas fisik yang kurang dapat dipengaruhi oleh pekerjaan. Pola hidup sedentary lifestyle identik dengan pekerja kantor dewasa ini. Pekerja kantor tidak memiliki waktu untuk terlibat dalam aktivitas fisik yang cukup. Penelitian ini bertujuan untuk mengetahui gambaran kadar glukosa darah puasa pada pekerja kantor. Jenis penelitian ialah deskriptif dengan desain potong lintang. Pemilihan responden dilakukan dengan metode total sampling. Responden terdiri dari 25 orang laki-laki s dan 27 orang perempuan. Hasil penelitian mendapatkan 45 responden (86,54%) memiliki kadar glukosa darah puasa normal, 5 responden (9,62%) memilki kadar glukosa darah puasa tinggi (hiperglikemia), dan 2 responden (3,84%) memilki kadar glukosa puasa rendah (hipoglikemia). Hasil pengukuran kadar glukosa darah puasa mendapatkan nilai maksimum 243 mg/dL, nilai minimum 63 mg/dL, nilai median 83 mg/dL, nilai rata-rata 94,42 mg/dL dan standar devisiasi 37,85 mg/dL. Simpulan: Sebagian besar pekerja kantor masih memiliki kadar glukosa darah puasa yang normal. Kata kunci: glukosa darah puasa, pekerja kantor

The study was performed to explore the suppressive effect of Jew's mellow leaves (JML) on postprandial blood glucose levels in rats and humans. A soluble dietary fiber (SDF) was extracted from the freeze-dried JML powder. An elevation of the postprandial blood glucose level in rats given 1% or 2% JML-SDF solution orally together with 20% glucose solution was significantly suppressed as compared with that observed in the control rats given only glucose solution. When seven healthy young male adults ingested 225 mL of JML mixed juice containing 15 g of freeze-dried powder with 75 g of glucose in the fasting state in the morning, the elevation of the postprandial blood glucose level was significantly suppressed as compared with the control subjects. The diffusion rate of glucose and the permeation rate of glucose in the cultured Caco-2 cells were both significantly reduced by the addition of appropriate amounts of JML-SDF when compared to the controls. These results indicate that the effective substance in JML for suppressing blood glucose elevation is a kind of mucilaginous SDF. The mechanism by which this suppression occurs may be largely attributable to the delayed absorption of glucose from the intestinal membrane in the upper digestive tract by viscous SDF.

Barley (Hordeum vulgareL.) is a well-known cereal plant. Young barley leaf is consumed as a popular green-colored drink, which is named “Aojiru” in Japan. We examined the effects of barley leaf powder (BLP) and insoluble fibers derived from BLP on postprandial blood glucose in rats and healthy Japanese volunteers. BLP and insoluble fibers derived from BLP suppressed the increment of postprandial blood glucose levels in rats (), and increased the viscosity of their digesta. The insoluble fibers present in BLP might play a role in controlling blood glucose level by increasing digesta viscosity. In human, BLP suppressed the increment of postprandial blood glucose level only in those which exhibited higher blood glucose levels after meals (). BLP might suppress the increment of postprandial blood glucose level by increasing digesta viscosity in both of rats and humans who require blood glucose monitoring.

Prevention of complications of diabetes mellitus requires good control of diabetes mellitus. Based on Basic Health Research Data of the Ministry of Health, in the end of 2013, the number of diabetic patients in Indonesia had reached 9.1 million and this number continues to grow which in 2030, the number is predicted to reach 21.3 million. This study aimed to identify the paramaters of diabetes mellitus control. This descriptive study involved 30 diabetic patients in coastal communities of Surabaya who were randomly selected. Data collection was undertaken using an observation sheet and diabetes control measurement tools. The data were analyzed descriptively. The results showed that diabetes control were mostly poor based from the indicator of fasting blood glucose level (181.77±68.728) and two hours postprandial blood glucose level (277.07±111.884). Meanwhile, total cholesterol (196.43±60.640), blood pressure (systolic 125±16.969 and diastolic 78±8.469), and Body Mass Index (23,257±3,231) were good. Education on Self Care Management will support the control of diabetes mellitus.

Diabetes mellitus (DM)is a disorder of carbohydrate metabolism which characterized by increased blood glucose level,which also increase the salivary glucose level. Glucose is a good medium for the growth of microorganisms includingC.albicans. One of the most common infections of the oral mucosa of patientswith DM is candidiasis. Chronic candidiasismay develop to be a malignancy. This study aimed to determine the correlation of salivary glucose levels with bloodglucose levels to the occurrence of oral candidiasis in patients with DM. Analytic observational study was carried outthrough the determination of blood glucose levels just by way of strip using a glucometer and salivary glucose levels with"GODPAP test enzymatic kolometrik". Oral candidiasis was determined in presenting C.albicans on 30 patients with DMwith seeding Saboround agar and gram staining. The results showed r = 0.228, which is higher salivary glucose levelsfollowed by high levels of blood glucose, and microscopic examination of Candida spores found in the whole sample,as well as Candida hyphae on two samples of the study. It was concluded that there is a correlation between salivary glucose levels and blood glucose levels, and glucose levels affect the occurrence of oral candidiasis in patients with DM.

The food glycemic index is a scale or number of foodstuffs that if consumed can have an impact on blood glucose levels so it can be used as a way to control blood glucose levels. The purpose of this study to determine the relationship of glycemic index of food with blood glucose level at diabetes mellitus type 2 in working area of Ranotana Weru Health Center. This research is an observational research using cross sectional research design with sample consist of 34 respondents who fill up the criteria of inclusion and exclusion. Data retrieval is done through interviews using food frequency questionnaire form (FFQ) and blood glucose levels obtained from the examination using autocek. Univariate analysis is done by frequency distribution and bivariate analysis using Fisher’s Exact Test. The results showed that of 34 respondents most of the respondents have blood glucose levels when the uncontrolled ≥180 mg/dl and consumed a high food glycemic indexs of 28 respondents (82,3%). While respondents have controled blood glucose levels of <180 mg/dl and consumed a low food glycemic index is 5 respondents (14,7%). Average blood glucose levels of respondents is 237,74% mg/dl. The statistical test is done obtained that there was correlation of food glycemic index with blood glucose level with value p = 0,000 (p <0,05). Conclusion, there is correlation of food glycemic index with blood glucose level in type 2 diabetes mellitus in working area of Ranotana Weru Health Center.

Glucose is an important carbohydrate which is more absorbed into the bloodstream as glucose and other sugar is changed into glucose in the liver. Blood glucose level is closely related to diabetes mellitus. Diabetes mellitus is a disease that arises in a person which is indicated by the presence of blood glucose levels exceeding normal (hyperglycemia) due to the deficiency of the insulin hormone in the body. The design of this research was using RAL with 24 male rats that divided into 6 treatments which were normal with the giving of Na-CMC 0.5% 1mL/200 gram BB, negative with the giving of Na-CMC 0.5% 1mL/200 gram BB, positive with the giving of glibenclamide of 0.45 mg/kg BB and 3 other treatments by giving catfish biscuit (Formula A, Formula B, and formula C) with 4 replications. All treatments were injected with monohydrate alloxan at a dose of 150 mg/kg BW, except for the normal treatment. The results showed that the formulation of catfish biscuit can lower the blood glucose level of white rats. The composition of biscuit with formula C has the highest decreasing percentage which was 80.20% to the blood glucose level of white rats.

BACKGROUND: Glucocorticoids (GCs) are expected to inhibit the synthesis and release of proinflammatory cytokines, which induces local pain. Serious side effects or complications are considered rare with single-dose GC use. However, the amount of systemic absorption and the side effects induced by local GC injections are not well understood. OBJECTIVES: We measured the changes in glucose levels after single-does dexamethasone injection with nerve blockade using a continuous glucose monitoring system (CGMS) in non-diabetes mellitus (DM) patients and investigated the risk factors for hyperglycemia. STUDY DESIGN: This is a cohort study. SETTING: This study was conducted at Gifu University Hospital in Japan. METHODS: Forty-six non-DM patients who underwent elective lumbar or sacral nerve root pulsed radiofrequency or lumbar medial branch of the posterior primary rami conventional radiofrequency with dexamethasone (0.1 mg/kg) were analyzed. The patients underwent monitoring of their interstitial glucose using a CGMS. Hyperglycemia was defined as a blood glucose level ≥ 200 mg/dL. The area under the curve (AUC) where the blood glucose level was over 200 mg/dL was calculated and analyzed. The risk factors of hyperglycemia were determined using an applied ordinal regression model analysis with the AUC as the objective variable and 4 factors (age, body mass index, glucose level just before GC injection, and glycosylated hemoglobin) as explanatory variables. The blood glucose levels were predicted by a nonlinear regression model. RESULTS: The AUC and maximum glucose level were higher on the first day than after the second day. None of the 4 factors were predictors of hyperglycemia. The glucose level before the procedure was associated with the predicted blood glucose level on the first day (P = 0.042). However, the 95% upper confidence limit of the maximum predicted blood glucose level was less than the safety margin. The predicted blood glucose levels returned to the usual level after the second day. LIMITATIONS: First, GCs are metabolized by cytochrome p450 3A4, and it is possible that the inhibition of this pathway decreases the clearance of GCs. Some of our patients were taking medications that influence this cytochrome pathway. Second, we cannot eliminate the possibility of stress-induced hyperglycemia. Finally, we were unable to record the exact meal timing and calories the patients had consumed. CONCLUSIONS: The blood glucose levels were higher than usual on the first day following a local dexamethasone injection, but the levels were not critical in most cases. Because we cannot predict which patients will develop hyperglycemia, we must determine whether or not GCs can be safely administered and inform patients about potential complications.
