Нами получен патент на полезную модель Республики Казахстан №5782 от 22.01.2021, который с большой эффективностью используется для обрезания крайней плоти. Для отсечения листков крайней плоти применяется устройство для пирографии Stayer, снабженный трансформатором, генерирующий напряжение от 0 до 5 В., с частотой тока 50Гц, мощностью 40Вт., с твердым рабочим наконечником, регулируемым температурой от 450ºС до 750ºС. Применение устройства для пирографии Stayer для обрезания крайней плоти позволяет сократить время оперативного вмешательства, уменьшить травматичность воздействия и улучшить косметический эффект. За 2018-2020 годы было выполнено 500 операций с применением устройства для пирографии Stayer. При этом, среднее время операции составляло 10 минут. У всех детей достигнут хороший непосредственный и отдаленный клинический результат. We have obtained a patent for a utility model of the Republic of Kazakhstan №5782, 22.01.2021., which is used with great efficiency in circumcision of the foreskin. To cut off the foreskin sheets, a Stayer pyrography device is used, equipped with a transformer that generates a voltage from 0 to 5 V., with a current frequency of 50 Hz, a power of 40 W., with a solid working tip, adjustable temperature from 450 ° C to 750 ° C. The use of the Stayer pyrography device for circumcision of the foreskin allows you to reduce the time of surgical intervention, reduce the traumatic impact and improve the cosmetic effect. In 2018-2020, 500 operations were performed using the Stayer pyrography device. The average operation time was 10 minutes. All children achieved a good immediate and long-term clinical outcome.

When Sinn Féin and the IRA emerged in Northern Ireland in 1969 they used a combination of revolutionary politics and violence to an effort to overthrow British rule. Today, the IRA is in a state of ‘retirement’, violence is a tactic of the past, and Sinn Féin is a co-ruler of Northern Ireland and an ever growing political player in the Republic of Ireland. This is one of the most startling transformations of a radical violent movement into a peaceful political one in recent times. So what exactly changed within Irish republicanism, what remains the same, and, crucially, what caused these changes? Where existing studies explain the decision to end violence as the product of stalemate or strategic interplay with the British state, this book draws on a wealth of archival material and interviews to argue that moderation was a long-term process of increasing inclusion and contact with political institutions, which gradually extracted moderate concessions from republicanism. Crucially, these concessions did not necessitate republicans forsaking their long-term ethno-national goals. The book also considers the wider implications of Irish republicanism for other cases of separatist conflict, and has significance for the future study of state responses to violent separatism and of comparative peace processes.

Few-layer exfoliated black phosphorus (Ex-BP) has attracted tremendous attention owing to its promising applications, including in electrocatalysis. However, it remains a challenge to directly use few-layer Ex-BP as oxygen-involved electrocatalyst because it is quite difficult to restrain structural degradation caused by spontaneous oxidation and keep it stable. Here, a robust carbon-stabilization strategy has been implemented to prepare carbon-coated Ex-BP/N-doped graphene nanosheet (Ex-BP/NGS@C) nanostructures at room temperature, which exhibit superior oxygen evolution reaction (OER) activity under alkaline conditions. Specifically, the as-synthesized Ex-BP/NGS@C hybrid presents a low overpotential of 257 mV at a current density of 10 mA cm−2 with a small Tafel slope of 52 mV dec−1 and shows high durability after long-term testing.

Controversy is common on environmental issues, with carbon taxation in Canada a current example. This paper uses Canada as a case study for analysis based around balanced presentation, a technique developed some time ago, yet largely forgotten. Using the method, analysis is shifted away from the point of controversy to a broader quantitative question, with comparative data employed from official government sources. Simple quantitative analysis is applied to evaluate emission trends of individual Canadian provinces, with quantitative metrics to identify and confirm the application of relevant emission reduction policies by individual jurisdictions. From 2005 through 2019, three provinces show consistent downward emission trends, two show consistent upward trends, and the remaining five have no trends, showing relatively “flat” profiles. The results clarify, in terms of diverse emission reduction policies, where successes have occurred, and where deficiencies or ambiguities have existed. Neither carbon taxation nor related cap-and-trade show any association with long-term reductions in overall emissions. One policy does stand out as being associated with long-term reductions, namely grid decarbonization. The results suggest a possible need within Canada to rethink emission reduction policies. The method may be relevant as a model for other countries to consider as well.

AbstractSometimes, a rather high stress drop characterizes earthquakes induced by underground fluid injections or productions. In addition, long-term fluid operations in the underground can influence a seismogenic reaction of the rock per unit volume of the fluid involved. The seismogenic index is a quantitative characteristic of such a reaction. We derive a relationship between the seismogenic index and stress drop. This relationship shows that the seismogenic index increases with the average stress drop of induced seismicity. Further, we formulate a simple and rather general phenomenological model of stress drop of induced earthquakes. This model shows that both a decrease of fault cohesion during the earthquake rupture process and an enhanced level of effective stresses could lead to high stress drop. Using these two formulations, we propose the following mechanism of increasing induced seismicity rates observed, e.g., by long-term gas production at Groningen. Pore pressure depletion can lead to a systematic increase of the average stress drop (and thus, of magnitudes) due to gradually destabilizing cohesive faults and due to a general increase of effective stresses. Consequently, elevated average stress drop increases seismogenic index. This can lead to seismic risk increasing with the operation time of an underground reservoir.

Introduction: Cystine stone development is relatively uncommon among patients with urolithiasis, and most studies have reported only on small sample sizes and short follow-up periods. We evaluated clinical courses and treatment outcomes of patients with cystine stones with long-term follow-up at our center. Methods: We retrospectively analyzed 22 patients diagnosed with cystine stones between January 1989 and May 2019. Results: The median follow-up was 160 (range 6–340) months, and the median patient age at diagnosis was 46 (range 12–82) years. All patients underwent surgical interventions at the first visit (4 extracorporeal shockwave lithotripsy, 5 ureteroscopy, and 13 percutaneous nephrolithotripsy). The median number of stone events and surgical interventions per year was 0.45 (range 0–2.6) and 0.19 (range 0–1.3) after initial surgical intervention. The median time to stone events and surgical intervention was 2 years and 3.25 years, respectively. There was a significant difference in time to stone events and second surgical intervention when patients were divided at 50 years of age at diagnosis (p = 0.02, 0.04, respectively). Conclusions: Only age at a diagnosis under 50 was significantly associated with recurrent stone events and intervention. Adequate follow-up and treatment are needed to manage patients with cystine stones safely.

Abstract Background Since 2010, laparoscopic transanal total mesorectal excision (TaTME) has been increasingly used for low and very low rectal cancer. It is supposed to improve visibility and access to the dissection planes in the pelvis. This study reports on short- and long-term outcomes of the first 100 consecutive patients treated with TaTME in a certified German colorectal cancer center. Patients and methods Data were derived from digital patient files and official cancer registry reports for patients with TaTME tumor surgery between July 2014 and January 2020. The primary outcome was the 3-year local recurrence rate and local recurrence-free survival (LRFS). Secondary endpoints included overall survival (OAS), disease-free survival (DFS), operation time, completeness of local tumor resection, lymph node resection, and postoperative complications. The Kaplan–Meier method was employed for the survival analyses; competing risks were considered in the time-to-event analysis. Results During the observation period, the average annual operation time decreased from 272 to 178 min. Complete local tumor resection was achieved in 97% of the procedures. Major postoperative complications (Clavien–Dindo 3–4) occurred in 11% of the cases. At a median follow-up time of 2.7 years, three patients had suffered from a local recurrence. Considering competing risks, this corresponds to a 3-year cumulative incidence rate for local recurrence of 2.2% and a 3-year LRFS of 81.9%. 3-year OAS was 82.9%, and 3-year DFS was 75.7%. Conclusion TaTME is associated with favorable short and long-term outcomes. Since it is technically demanding, structured training programs and more research on the topic are indispensable.

In the last two decades, variably doped strontium barium niobate (SBN) has attracted a lot of scientific interest mainly due to its specific non-linear optical response. Comparably, the parental compound, i.e., undoped SBN, appears to be less studied so far. Here, two different cuts of single-crystalline nominally pure strontium barium niobate in the composition Sr0.61Ba0.39Nb2O6 (SBN61) are comprehensively studied and analyzed with regard to their photoconductive responses. We present conductance measurements under systematically varied illumination conditions along either the polar z-axis or perpendicular to it (x-cut). Apart from a pronounced photoconductance (PC) already under daylight and a large effect upon super-bandgap illumination in general, we observe (i) distinct spectral features when sweeping the excitation wavelength over the sub-bandgap region as then discussed in the context of deep and shallow trap states, (ii) extremely slow long-term relaxation for both light-on and light-off transients in the range of hours and days, (iii) a critical dependence of the photoresponse on the pre-illumination history of the sample, and (iv) a current–voltage hysteresis depending on both the illumination and the electrical-measurement conditions in a complex manner.

The efficacy of a short leg walking cast in the treatment of chronic plantar heel pain was assessed for 32 patients with 37 involved extremities treated over a 2-year period. All patients had failed numerous other treatment modalities and had been symptomatic for an average of 1 year. Long-term follow-up for 24 patients with 28 involved extremities revealed complete resolution of pain for 7 extremities (25%), improvement for 17 (61%), and no improvement for 4 (14%). Ten (42%) patients were completely satisfied with cast treatment, 3 (12%) were satisfied with reservations, and 11 (46%) were dissatisfied. Casting appears to be a reasonable option for patients with recalcitrant heel pain and should be offered before surgical intervention.
