Background: The prevalence of asthma is higher in icteric babies. It is thought that eosinophilia and asthma are complications of phototherapy rather than hyperbilirubinemia. Objectives: Since there is little data in this regard, we evaluated the relationship between phototherapy and the level of eosinophils in this study. Methods: In this cross-sectional prospective study, we assessed icteric neonates admitted to the neonatal and intensive care unit of Children’s Medical Center of Tehran during 2017 - 2018. Laboratory data including serum bilirubin and count of blood cell differentiation for eosinophils, before, during, and after phototherapy besides demographic data like age, gender, gestational age, and duration of phototherapy were collected. Results: Totally, 163 neonates (52.1% male and 47.9% female) with a mean age of 5.49 ± 4.01 days, enrolled in the study. The prevalence of term neonates was 81% and 19% were preterm. The mean of serum total bilirubin was 17.91 ± 3.37 mg/dL. Compared to the mean value of eosinophils before phototherapy, this value in the first (P = 0.001) and second (P < 0.001) days were significantly higher. In female neonates, there was a significant increase in eosinophils in the first (P = 0.001) and second (P < 0.001) day of the post-treatment period. However, in male subjects, this increase was significant only in the second day after phototherapy (P = 0.001). In neonates less than one week of age, this count was significantly increased on the first (P = 0.003) and second (P < 0.001) day after phototherapy. In cases with hemolytic hyperbilirubinemia, eosinophils count before phototherapy and the second day was significant (P = 0.006). The mean of eosinophils in first and second days after phototherapy was not correlated with the severity of hyperbilirubinemia but there is a significant negative correlation between the mean total bilirubin of patients with mean eosinophils before phototherapy. Conclusions: Our findings showed that there was a significant relationship between hyperbilirubinemia and its therapeutic phototherapy with an increasing level of peripheral eosinophils count in neonates.

Abstract Background The consequences of obstetric fistula on affected women are more than the medical condition. It has extensive physical, psychological, social, and economic consequences on them. Obstetric fistula affects the entire health and entire life of women. Women suffering from obstetric fistula are often abandoned by her partner, relatives, and the community. This study aimed to determine the quality of life of obstetrics fistula patients before and after surgical repair. Methods Institutional-based prospective, before and after study design was conducted in the Jimma University Medical Center from November 1, 2019–October 30, 2020. A face-to-face interview was conducted with fistula patients who visited Jimma University Medical center, fistula clinic during the study period. All fistula patients were included in the study. Accordingly, 78 women who underwent surgical repair were interviewed. The means and the standard deviation were computed using conventional statistics formulas. The unpaired t-test was used to compare two independent means, and one-way analysis of variance (ANOVA) was used to compare the quality of life before repair and after a successful repair. Linear regression analysis was done for identifying determinants of quality of life. A P value of 0.05 will be considered statistical significance. Result The overall quality of life of women was 58.17 ± 7.2 before the surgical repair and 71.20 ± 10.79 after surgical repair. The result indicates there is a significant difference in the mean value of pre and post-operative (P < 0.001). The overall satisfaction of women with their health status before the surgical repair was 22.5 ± 1.30and it has increased to 53.0 ± .90after surgical repair. The physical health dimension score was 16.51 ± 5.27 before the surgical repair while it has increased to 21.77 ± 5.38 after the surgical repair. The score of the social domain before the surgical repair was 5.19 ± 1.34 and it has increased to 7.13 ± 3.67 after the surgical repair. The score of the environmental health domain was 17.41 ± 2.89 before the surgery while it also increased to 21.65 ± 4.04 after the surgical repair. The results have shown there was a significant difference in the mean values of pre and post-operatives in both social and environmental scores (P < 0.001). The score of the psychological health domain before the surgery was 19.06 ± 1.46 and it was increased to 19.84 ± 3.21 after the surgical repair. The result showed there is a significant difference in mean value pre and post-operative (P = 0.048), though it is a slight improvement compared to other domains. Conclusion The overall quality of life of the patient with fistula was improved after successful surgical repair. Although all domains of quality of life had shown significant improvement after successful surgical repair, the psychological domain showed slight improvement.

Objective. To evaluate the association between serum total bilirubin concentration (STBC) and diabetic retinopathy (DR) among the Chinese northeastern population. Methods. A cross-sectional study was conducted in Liaoning between January 2015 and May 2017. Results. A total of 742 subjects (419 men and 323 women) with type 2 diabetes mellitus (DM) who visited an ophthalmic clinic were included in this study. The mean age of the subjects was 59.55 ± 10.63 years, and 43.5% of the subjects were women. The mean of DM duration was 11.01 ± 7.35 years. STBC were negatively correlated with DM duration, urea nitrogen, serum creatinine, uric acid, and urine microalbumin. After adjusting for confounding factors, as a continuous variable, STBC was inversely associated with the risk of DR in total subjects (OR: 0.95, 95% CI: 0.93–0.99). When STBC was used as a tertiary variable, compared with the first tertile, the OR in the third tertile was 0.37 (95% CI: 0.22–0.64) in total subjects. Conclusion. Our results demonstrate that a significant negative association was found between STBC and DR. STBC might be an early clinical marker for predicting the occurrence of DR.

  Fractional exhaled nitric oxide (FeNO) is a noninvasive marker of inflammation, used for monitoring asthma. The aim of this study was to compare FeNO, asthma control test (ACT), and lung function test (spirometry) in children aged 8-15 years. This observational, cross-sectional study was performed on76 asthmatic children (age, 8-15 years), who were referred to the Department of Immunology and Allergy, Children's Medical Center, Tehran, Iran during 2012-2013. Patients were matched for sex and age. The recruited patients were selected via consecutive sampling. FeNO was measured with a portable electrochemical analyzer and forced spirometry was performed according to the American Thoracic Society (ATS) guidelines. The ACT questionnaire was used and completed for all the patients.  The mean FeNO was 28.5±29.1 ppb, and the mean ACT score was 19.8±3.6. FeNO was significantly correlated with forced expiratory volume (FEV1) (r, 0.232; p=0.049) or 25-75% maximum expiratory flow (MEF 25-75) (r, -0.304; p=0.009). FeNO showed no significant correlation with ACT score or FEV1/forced vital capacity (FVC) (p>0.05). Additionally, there was no significant correlation between FeNO and changes in FEV1 and MEF 25-75% before and after the administration of bronchodilators (p>0.05). To improve asthma control, childhood ACT, FeNO, and spirometric tests can be used as complementary tools in clinical practice to detect children with poorly controlled asthma.

Metabolic syndrome is a congregation of central obesity, dyslipidemia, raised bloodsugar levels, increasing the individual’s susceptibility to Type II Diabetes and cardiovasculardiseases. Objectives: (1) To determine the prevalence of metabolic syndrome in young, urban,female population. (2) To determine the risk factors in poor, urban, female population. StudyDesign: This was a descriptive cross sectional study. Setting: The department of Gynae/Obst Unit II KMDC/Abbasi Shaheed hospital. Period: One year starting from January 2016 toDecember 2016. Material and Method: Approval was taken from ESRC of KMDC. All healthyasymptomatic married/single women between 18-49 years of age were included while women<18 or >50 years of age, diabetic, hypertensive or having bleeding disorders were excludedfrom study. Laboratory data included blood sugar, triglycerides, HDL-cholesterol, collected byphlebotomist from the participants in fasting state through venipuncture. A Chi-square test wasapplied to evaluate the association of demographic group variables and metabolic syndrome.P-value <0.05 was considered as statistically significant. There was no conflict of interest. Result:A total of 343 participants were recruited. The socio and demographic data is summarized inTable-I. The prevalence of Metabolic syndrome was found to be high. 227(66.2%) of participantswere having Metabolic syndrome according to NCEP ATP III criteria. 63(18.4 %) had history ofPIH while 52(15.2%) had family history of hypertension and 126(36.7 %) had family historyof both Hypertension and Diabetes. 232 (67.6 %) of women had sedentary life style and only3(0.9%) practiced aerobic exercises. 287(83.7%) had their waist circumference of >80cm, themean systolic blood pressure was 127.5 +-23.76 while the mean diastolic blood pressure was86.99+-57.36. The mean of BMI was at higher level 30.97+-6.41. Obesity is the most commonrisk factor for Metabolic syndrome. The mean of fasting blood sugar was 105.08+-42.16which was on higher side. The mean of Triglycerides 142.43+-61.12 and HDL 39.04+-12.45were within normal limits. Increased prevalence was observed in women who had PIH duringpregnancy and childbirth 25.1% v 5.2%(p value=0.001). Conclusion: Prevention and treatmentof metabolic syndrome is a big challenge. Lifestyle interventions should begin from the earlychildhood to reduce weight and to prevent development of obesity and metabolic syndrome.

In Indonesia, the prevalence of cataract is 80 %. The main purpose of phacoemulsification with theimplantation of intraocular lens (IOL) is to improve visual acuity on cataract patient. The outcome ofcataract surgery should be evaluated based on visual acuity and quality of vision. Contrast sensitivityexamination can provide important information about the visual capabilities of individuals. The aim of thisresearch is to find out the contrast sensitivity value on cataract patients after phacoemulsification with theimplantation of hydrophobic aspheric monofocal IOL at FMC Hospital, Sentul on December 2018 untilJanuary 2019. This research used cross sectional descriptive design. The contrast sensitivity test isconducted on subject that fulfill the inclusive and exclusive criteria. This test is conducted using Vistech6500. The mean value of contrast sensitivity of 50 eyes at 1.5 cpd was 184.11, at 3 cpd was 235.5, at 6 cpdwas 207.360, at 12 cpd was 119.830, and at 18 cpd was 52.914. The result, the mean contrast sensitivity of50 eyes at the 1.5 and 3 cpd is higher compared to maximum value of Vistech 6500. However at 6, 12 and 18cpd the mean contras sensitivity depleted compared to maximum value of Vistech 6500. The result of meancontrast sensitivity based on age was the middle-aged (45-59 years) had a better contrast sensitivitycompare than young old (60-74 years) and very old category (75-90 years).

Chemotherapy shows high effectiveness, but also has side effects, including mucositis. Mucositis can cause pain, difficulty sleeping, eating disorders, mood, and activity, which has implications for the quality of life of children. The purpose of this study was to identify the incidence of mucositis and the factors that influence the incidence of mucositis in cancerous children receiving chemotherapy treatment. This research method is descriptive correlational analysis with cross sectional design. Consecutive sampling technique was used to establish respondents as research samples so as to get 60 respondents. Data were analyzed univariately and bivariately. Bivariate analysis was performed with Chi square test and 2 mean difference test to see differences in the mean values of mucositis before and after chemotherapy. The results showed that almost all cancer children who received chemotherapy had 53 people (88.3%) and a small portion, 7 people (11.7%) had no mucositis. There was a significant difference in the mean value (p = 0,000) between before and after chemotherapy with an increase in the average mucositis value of 3.12. The research shows that there is a significant relationship (p <0.05) between previous mucositis experience (p = 0,000), type of cancer (p = 0.025), type of chemotherapy (p = 0.010), and duration of therapy (p = 0.027) and the incidence of mucositis. Meanwhile nutritional status was not related to the incidence of mucositis (p = 0.077). Nurses, as health workers who most often contact with patients, should be able to improve nursing care in cancer children who get chemotherapy in minimizing the occurrence of mucositis by conducting routine mucositis and oral care assessments.

Hepatitis has become a health problem in the world. The hepatitis virus infected many people. According to the teacher of MTsN 02 Bondowoso more than 20 students have hepatitis A viral infection. The purpose of this research was to know the differences of students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study used pre-experimental (pre-post test design). This study used stratified random sampling technique, 127 students from 270 sample involved this research,and 143 students was excluded. We used questionnaires to collect data. The results showed that the mean value of the students 'knowledge about hepatitis A before counseling in MTsN 02 Bondowoso 2015 was 83.96 with the lowest value of 37.5 and the highest value was 100. The mean value of the students' knowledge about hepatitis A after counseling in MTsN 02 Bondowoso 2015 was 93.21 with the lowest value waf 62.5 and the highest value was 100. Paired t test showed that t (-9.07) > t table (1.98), the null hypothesis (H0) was rejected. There was a difference between students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study showed that routine counseling by healthcare provider was important to prevent hepatitis A infection.; Keywords: counseling, knowledge of students, hepatitis

AbstractSurvival of breast cancer patients has improved, and treatment-related changes regarding metabolic profile deterioration after neoadjuvant systemic treatment (NST) become important issues in cancer survivors. We sought to compare metabolic profile changes and the neutrophil-to-lymphocyte ratio (NLR) between patients undergoing neoadjuvant chemotherapy (NCT) and neoadjuvant endocrine therapy (NET) 3 years after the treatment. In a prospective, randomized, phase III trial which compared 24 weeks of NCT with adriamycin and cyclophosphamide followed by docetaxel and NET with goserelin and tamoxifen (NEST), 123 patients in the Asan Medical Center were retrospectively reviewed to evaluate metabolic changes, such as body mass index (BMI), blood pressure (BP), total cholesterol (TC), fasting glucose, and the NLR. The mean age of patients was 42 years. The changes in BMI, serum glucose, and TC during NST and after 3 years were significantly different between NCT and NET. The proportion of overweight + obese group and the mean BMI were significantly increased during NCT (26.6% to 37.5%, 22.84 kg/m2 to 23.87 kg/m2, p < 0.05), and these attributes found to have normalized at the 3-year follow-up. In the NET group, BMI changes were not observed (p > 0.05, all). There were no differences in changes over time among in the Hypertension group during NCT and NET (p = 0.96). The mean value of serum TC and fasting glucose significantly increased (< 0.05, both) during NCT and decreased 3 years after NCT (p < 0.05); however, no significant changes were observed in the NET group. The NLR was increased from 1.83 to 3.18 after NCT (p < 0.05) and decreased from 1.98 to 1.43 (p < 0.05) after NET. Compared with minimal metabolic effect of NET, NCT worsens metabolic profiles, which were recovered over 3 years. The NLR was increased after NCT but decreased after NET.

Background:Foot disease is a common problem in rheumatoid arthritis (RA). Therapeutic decisions are often based on clinical examination (CE) alone, which can be adversely affected by factors such as deformity, obesity, and peripheral edema. Ultrasonography (US) has previously been shown to be more sensitive than CE for detecting synovitis and tenosynovitis in RA forefeet, but few data exist for the hindfoot and ankle.Objectives:The aim of this study was to compare CE and US for the detection of hindfoot and ankle synovitis and tenosynovitis in patients with established RA.Methods:We conducted a cross-sectional study including patients with RA (ACR/EULAR 2010). Demographic data and disease parameters were collected. CE was performed by a rheumatologist for the presence or absence of tenderness, swelling, and mobility restriction of both ankles. The following tendons were examined for tenosynovitis: tibialis anterior (TA) and posterior (TP), fibularis longus (FL), and brevis (FB) (assessed together). In a second time, US examination of the tibiotalar, talonavicular, and subtalar joints and the same tendons as CE was performed by a blinded radiologist experienced in musculoskeletal imaging using a Philips HD11 device with a high-frequency linear transducer. The presence or absence of synovitis and tenosynovitis was recorded, and the composite synovitis score (power doppler / grayscale ultrasound (PDUS)) was measured for each joint. The US score of each patient was defined by the sum of the composite scores of the joints studied (0-30). A p-value <0.05 was considered significant.Results:Sixty-two feet were examined in 31 RA patients (25 women and six men) with a mean age of 54.8±10.8 years old [32-70]. The mean disease duration was 8.5±7.2 years [1-37]. Rheumatoid Factor (RF) and Anti-Citrullinated Peptides Antibodies (ACPA) were positive in 61.3% and 83.8% of cases. The mean DAS28 ESR was 3.8±1.5 [0.6-7].Clinical examination of ankles revealed tenderness in 57.4% of cases, swelling in 38.8% of cases, and restriction in the range of motion in 11.1% of cases. TA tenosynovitis was noted in 14.8% of cases, TP tenosynovitis in 22.2% of cases, and FL and FB tenosynovitis in 31.5% of cases.US showed tibiotalar synovitis in 59.3% of cases, talonavicular synovitis in 64.8% of cases, and subtalar synovitis in 46.3% of cases. TA tenosynovitis was noted in 5.6% of cases, TP tenosynovitis in 22.2% of cases, and FB and FL tenosynovitis in 25% and 11.1% of cases respectively.An association was found between clinical tenderness and US synovitis of the tibiotalar joint (p=0.013) and the talonavicular joint (p=0.027). No association was noted between clinical swelling and US synovitis in these joints.No association was noted between clinical and US tenosynovitis of TA (p=0.279), TP (p=0.436), FB (p=0.495) and FL (p=0.315).Conclusion:Clinical examination of RA ankles may be challenging and needs to be coupled with US, which is more sensitive and accurate in the detection of synovitis and tenosynovitis.Disclosure of Interests:None declared

Background:The lipid paradox is termed the decreased cholesterol level in rheumatoid arthritis (RA). Nevertheless, the apolipoprotein levels are usually higher than a healthy person and are predictors of cardiovascular events.Objectives:We aimed to describe lipid abnormalities in RA patients and to look for predictor factors of these changes.Methods:The prospective study was carried out on patients with RA who met the 2010 American College of Rheumatology (ACR)/European League Against Rheumatism (EULAR) classification criteria. These patients were followed in the rheumatology department of the Kassab Institute.We collected the socio-demographic data, biological and immunological parameters.The lipid assessment included: a measurement of total cholesterol (TC), HDL, LDL, and triglycerides (TG). Lipoproteins APOA1 and APOB were measured. All data were collected after patient consent.Results:Of the 47 patients recruited, 78.7% were female. The mean age was 52.5 ±11.06 [32-76]. The average RA progressed from 86.25 ±63 months [5-288] and was erosive in 81.6% of cases. The rheumatoid factor (RF) was positive in 57.8% of patients, and citrullinated antipeptide antibodies (ACPA) were present in 62.2%. Eight patients had a previous CV history.Mean TC was 4.42 ±1.3 [1.2-7.58], mean HDL was 1.38 ±0.73 [0.18-4.10], mean LDL was 2.55 ±1.16 [0.24-5.54]. The mean TG value was 1.28 ±0.6 [0.24-5.54]. TC elevation was found in 9.1% of cases, HDL in 21.3% of cases, LDL in 5.5% of cases, and TG in 16.4% of cases. Mean APOB/APOA1 ratio was 0.67 ±0.18 [0,46-1,11]. LDL elevation was associated to a high DAS28 (p=0.06, r=0.512). APOA1 was associated to a low DAS28 (p=0.04, r=-0.642).The mean value of APO A1 was 1.36 ±0.21 [0.84-1.81], that of APOB was 0.90 ±0.22 [0.58-1.40]. APOA1 values were lower in patients with high-level LDL (p=0.767). The APOB value was associated with lipid disturbance without significant correlation (p=0.291).Conclusion:Lipid test abnormalities can be found in RA patients outside of any known CV risk factors. APOA1 seems to have a protective effect. Screening and treatment of these abnormalities can prevent CV risk.References:[1]Miguel Bernardes and al.
