OBJECTIVE To compare the timing of serum anti-drug antibodies in adult and pediatric age groups, males and females, treated for inflammatory bowel disease or arthritis with adalimumab or infliximab by retrospectively combining data collected during a 2-year therapeutic drug monitoring period. METHODS Four hundred thirty sera were divided in groups collected at 0, 3, 6, 12, and 24 months (T0, T3, T6, T12, and T24) after initiation of therapy and assayed for drug and relative anti-drug antibodies levels. At each time point, the percentage of sera presenting anti-drug antibodies, as well as the drug concentrations, were calculated and correlated with patient age and sex. RESULTS Anti-drug antibodies were present in 31.5% of sera and were significantly higher in the pediatric age group than in the adult age group, through all time points. The percentages of sera showing anti-drug antibodies were significantly different as early as 3 months and were sera from pediatric female group. The percentages of sera showing anti-drug antibodies reached the highest value at 6 months in the pediatric age group and at 12 months in the adult age group. CONCLUSIONS Sera from pediatric had an earlier presence of anti-drug antibodies than adults. In particular, pediatric females sera showed the fastest anti-drug antibodies development.

Objective.To compare pediatric and adult age groups in terms of postoperative bleeding and pain following tonsillectomy performed by thermal welding system (TWS).Method.The study consisted of 213 patients, of whom 178 were children and 35 were adults. The mean age of the pediatric patients (81 girls and 97 females) was6.7±2.4years (range 3–13 years) and the mean age of the adults (20 males and 15 females) was21.8±7.07years (range 15–41 years). All of the patients were evaluated in terms of postoperative bleeding and pain following tonsillectomy performed by TWS.Results.Bleeding was detected in the late postoperative period in 11 pediatric and 7 adult patients and of them 2 pediatric and 3 adult patients controlled under general. Postoperative bleeding was significantly less prevalent in the pediatric age group compared to the adult age group (P=0.04). Likewise, postoperative pain was significantly less prevalent in the pediatric age group as compared to the adult age group (P<0.001).Conclusion.Both postoperative bleeding and pain following tonsillectomy performed by TWS were more prevalent in the adult age group compared to the pediatric age group.

A retrospective study of 672 sampled records of patients of a major institutions providing Chinese traditional medicine in Singapore reveals that 97.3% of the patients were Chinese, with a dialect group distribution following closely that of the Singapore Chinese. The male-to-female patient ratio was 1.0:1.13. There were fewer patients of the pediatric age group and more of those from age 30 upwards. Some 43% of the patients sought treatment for ''infective'' and ''internal, emotional and weakness'' diseases. The prevalent disease conditions seen among patients from the different age groups are also analyzed and discussed in this paper.

Abstract Purpose: To investigate the short-term results of accelerated crosslinking (A-CXL) treatment for progressive keratoconus in the pediatric and adult age groups. Materials and methods: The records of the 62 eyes of 40 patients who had undergone the A-CXL procedure (9 mV/cm2, 10 min) for progressive keratoconus between January 2015 and January 2019 were evaluated retrospectively. The patients were divided into 2 groups as the pediatric group (aged 17 years or less) and the adult group (aged 18 years or more) for statistical analysis. Pre- and post- 12th month A-CXL best-corrected visual acuity (BCVA), maximum keratometry (Kmax), sim K1, sim K2, corneal thickness at the thinnest point (thCT), and corneal astigmatism (CA) values of the patient groups were recorded. Results: The 29 eyes of 16 patients were included in the pediatric group and the 33 eyes of 24 patients were included in the adult group. The mean age was 13.50±3.05 years in the pediatric group and 23.58±4.37 years in the adult group. A significant improvement in BCVA and a significant decrease in thCT values were present in both groups 12 months after the surgery compared to the preoperative period. A decrease was present in the Kmax, sim K1, sim K2 and CA values in the pediatric group, but was not statistically significant. The decrease in Kmax, sim K1 and sim K2 values compared to the preoperative period was significant in the adult group, but the decrease in CA values was not significant. When the two groups were compared at the end of 12 months, only the sim K1 value was significantly lower in the adult group, and there was no significant difference between the other measurements. Conclusions: Better visual acuity improvement, a higher flattening rate, and less progression occur after 12 months with A-CXL treatment for progressive keratoconus in the adult age group compared to the pediatric age group.

Periodic fever with aphthous stomatitis, pharyngitis, and adenitis (PFAPA) is an autoinflammatory disease manifested as recurrent febrile episodes associated with one of the following cardinal features: aphthous ulceration, pharyngitis, and cervical adenitis. It was initially described in children and thought to be a disease of pediatric age group. Few adult cases were also reported in the literature. We describe the case of a 39-year-old female affected by PFAPA who presented with a history of febrile episodes associated with aphthous ulceration, stomatitis, and tonsillitis for 4 years. The febrile episodes occurred at a regular interval of 4 weeks and resolved within 5 days. The patient underwent tonsillectomy without any significant improvement. The patient responded only to a single high dose of steroid during the attack. Although PFAPA was initially thought to be a disease of pediatric age group, it should be considered in patients with recurrent febrile illness in all age groups.

Germ cell tumors (GCT) are rare neoplasms which are seen in all age groups and both sexes. Gcts in the pediatric age group show considerable variation from their adult counterparts in relation to site, histological type, clinical and radiological features and response to treatment. Extragonadal gcts are much more common in the pediatric age group with the most common sites being sacrococcygeal region, head and neck, retroperitoneum and mediastinum. Unusual sites like the vagina and urinary bladder have also been reported. Therefore, the varied presentations and different treatment options should be considered.

Abstract Background and Aim of Study Coagulopathy is a common occurrence following traumatic brain injury (TBI). Various studies have reported the incidence and risk factors of coagulopathy and their correlation with poor outcome in adult as well as pediatric age group. In our study, we aim to analyze trauma-induced coagulopathy in adult and pediatric patients. Methods Adult (> 18 years) and pediatric (< 18 years) patients of TBI admitted in the intensive care unit of a trauma center of a tertiary care center had been studied from August 2015 to March 2018. Patients were further subdivided into moderate and severe TBI based on Glasgow Coma Scale (GCS) of 9 to 12 and < 9, respectively. Coagulation profile (prothrombin time [PT], activated partial thromboplastin time [APTT], thrombin time, fibrinogen, and D-dimer) and arterial blood gas (ABG) analysis were done on day of admission and on days 3 and 7. Coagulation profiles were analyzed in both the age groups, and risk factors were studied and correlated with the mortality and morbidity based on the Glasgow outcome score. Results Two hundred patients including 143 adults and 57 pediatric patients were included. Mean age among the adult and pediatric population was 31.51 ± 16.83 and 11.5 ± 5.90 years, respectively. In adults, 96 (83.62%) out of 116 in severe TBI group and 20 (74.07%) out of 27 in moderate TBI group developed coagulopathy, and in pediatric age group, 14 (70%) out of 20 in moderate TBI and 30 (81.08%) out of 37 in severe TBI developed coagulopathy. Midline shift was significantly associated with coagulopathy in both the age groups (p value < 0.039). Mortality was not significantly different in patients with coagulopathy between the age groups, but improved status as per the Glasgow outcome score was more in pediatric age group. Conclusion The development of coagulopathy is a frequent complication in patients with moderate to severe TBI in both age groups. Even though it is not closely associated with death in this study, it may be regarded as a marker of injury severity.

Background: The skull diploic venous space (DVS) represents a potential route for cerebrospinal fluid (CSF) diversion and absorption in the treatment of hydrocephalus. The goal of this study was to carry out a detailed characterization of the drainage pattern of the DVS of the skull using high-resolution MRI, especially the diploic veins draining to the lacunae laterales (LLs) since the LLs constitute an important channel for the CSF to access the superior sagittal sinus and subsequently the systemic circulation. The objective was to identify those skull regions optimally suited for an intraosseous CSF diversion system. Methods: High-resolution, T1-weighted MRI scans from 20 adult and 16 pediatric subjects were selected for analysis. Skulls were divided into four regions, that is, frontal, parietal, temporal, and occipital. On each scan, a trained observer counted all diploic veins in every skull region. Each diploic vein was also followed to determine its final drainage pathway (i.e., dural venous sinus, dural vein, LL, or indeterminate). Results: In the adult age group, the frontal and occipital skull regions showed the highest number of diploic veins. However, the highest number of draining diploic veins connecting to the lacunae lateralis was found in the frontal and parietal skull region, just anterior and just posterior to the coronal suture. In the pediatric age group, the parietal skull region, just posterior to the coronal suture, showed the highest overall number of diploic veins and also the highest number of draining diploic veins connecting to the LL. Conclusion: This study suggested that diploic venous density across the skull varies with age, with more parietal diploic veins in the pediatric age range, and more occipital and frontal diploic veins in adults. If the DVS is ultimately used for CSF diversion, our anatomical data point to optimal sites for the insertion of specially designed intraosseous infusion devices for the treatment of hydrocephalus. Likely the optimal sites for CSF diversion would be the parietal region just posterior to the coronal suture in children, and in adults, frontal and/or parietal just anterior or just posterior to the coronal suture.

Recent data from the Type 1 Diabetes Exchange has highlighted the urgent need for better glycemic control in the pediatric age group. Fortunately, ongoing research on insulin pumps, continuous glucose monitors, and artificial pancreas systems continues to improve our ability to manage type 1 diabetes in all age groups, while reducing the burden of care at the same time. Here we discuss the status of diabetes technology research as well as future directions and goals of these projects.

Background: Hypothyroidism (HT) in pediatric age group is of prime importance because of it’s varied presentations. Missing a diagnosis of hypothyroidism is disastrous since it results in mental retardation (MR). Hence, it is mandatory to know the various types of clinical presentation in clinical practice.Methods: Study includes 30 children who were diagnosed to have hypothyroidism. Detail history were taken thorough examination carried out and thyroid function tests were obtained in all the cases. Results were statistically analyzed in Microsoft office excel 2007.Results: Majority of the children were diagnosed at the age of 1-3 month of age. Males and females were almost equally affected, 70% of the children had constipation, 26.7% of the cases presented with umbilical hernia, feeding difficulties were seen in 23.3%, abdominal distention and prolonged jaundice were seen in 20% of the cases. 10% of the cases presented with short stature and 6.6% children showed obesity.Conclusions: Detailed history and examination of each infant will help in early diagnosis of congenital hypothyroidism (CH). High index of clinical suspicion should be made when any of clinical features of hypothyroidism are present.
