Prenatal care consists of practices considered to be effective for the reduction of adverse perinatal outcomes. However, studies have demonstrated inequities in pregnant women's access to prenatal care, with worse outcomes among those with lower socioeconomic status. The objective of this study is to evaluate access to and utilization of prenatal services in the Sistema Único de Saúde (SUS - Unified Health System) in the city of Rio de Janeiro and to verify its association with the characteristics of pregnant women and health services. A cross-sectional study was conducted in 2007-2008, using interviews and the analysis of prenatal care cards of 2.353 pregnant women attending low risk prenatal care services of the SUS. A descriptive analysis of the reasons mentioned by women for the late start of prenatal care and hierarchical logistic regression for the identification of the factors associated with prenatal care use were performed. The absence of a diagnosis of pregnancy and poor access to services were the reasons most often reported for the late start of prenatal care. Earlier access was found among white pregnant women, who had a higher level of education, were primiparous and lived with a partner. The late start of prenatal care was the factor most associated with the inadequate number of consultations, also observed in pregnant adolescents. Black women had a lower level of adequacy of tests performed as well as a lower overall adequacy of prenatal care, considering the Programa de Humanização do Pré-Natal e Nascimento (PHPN - Prenatal and Delivery Humanization Program) recommendations. Strategies for the identification of pregnant women at a higher reproductive risk, reduction in organizational barriers to services and increase in access to family planning and early diagnosis of pregnancy should be prioritized.

Calls to address crack-cocaine use in Brazil among homeless and street-frequenting populations who are in urgent need of health services have questioned the capacity of the Brazilian Unified Health System to attend to the nation’s most marginalized citizens. In recent years, Brazil has launched several actions to escalate care for substance users, yet many obstacles hindering accessibility and effectiveness of services remain. Paradoxically, these actions have been implemented in the context of a growing economic crisis, and expanding services for a population of poor and stigmatized substance users while cutting other government programs tends to elicit harsh criticism from citizens. In consequence of such prospects, this commentary aims to discuss barriers marginalized substance users face in accessing health services that are at risk of worsening with government cutbacks. Using Rio de Janeiro as an example, we explore two primary issues: the resource-strained, under-staffed and decentralized nature of the Brazilian Unified Health System and the pervading stigma that bars vulnerable citizens from official structures and services. Abandoning initiated government efforts to increase access to health services would risk maintaining vulnerable citizens at the margins of public structures, inhibiting the opportunity to offer this population humane and urgently needed treatment and care.

Objetivos: determinar e analisar necessidades de saúde especiais entre crianças hospitalizadas em hospital de alta complexidade, no Rio de Janeiro. Método: estudo transversal e descritivo desenvolvido com 21 familiares de crianças hospitalizadas em unidade intensiva e enfermarias de cuidados agudos, na rede de atenção de alta complexidade. Em 2019, aplicou-se o children with special healthcare needs Screener  (CS Screener®) -  versão brasileira na entrevista com 21 familiares. Resultados: as crianças hospitalizadas (21/36) apresentaram necessidades e demandas de serviços de saúde relacionadas à doenças e transtornos comportamentais correspondendo a 76,19%. O cuidado da criança dependia do Sistema Único de Saúde e de Assistência social. Quatro possuíam dispositivos tecnológicos implantados; oito eram acompanhadas por especialistas na atenção psicossocial e educação especial. Conclusão: essas crianças se reinternavam frequentemente, ocupando a maioria dos leitos hospitalares do Sistema Único de Saúde, principal referência para a continuidade dos cuidados de enfermagem e uso contínuo de medicamentos.

O presente trabalho teve por objetivo verificar o conhecimento dos cirurgiões dentistas da rede de atenção primária, secundária e terciária do município de Palmas, Tocantins, acerca do fluxo de assistência aos pacientes com diagnóstico de câncer bucal no Sistema Único de Saúde, desde o início do tratamento até a sua conclusão. Para isso, foi aplicado um questionário semiestruturado aos grupos de profissionais envolvidos diretamente com a rede de atenção, avaliando o nível de entendimento dos cirurgiões dentistas sobre o fluxo de atendimento. Foram feitas as análises qualitativa e quantitativa. Os resultados da análise quantitativa mostraram que houve diferenças no entendimento dos profissionais dos grupos envolvidos na pesquisa em relação ao conteúdo abordado no questionário. Conclui-se que, no município de Palmas, existe a necessidade de uma maior integração entre os cirurgiões dentistas envolvidos na rede de atenção ao paciente com câncer de boca, e que a introdução de um programa de cooperação interinstitucional voltado a esses pacientes poderia contribuir com o fluxo de acompanhamento e com a qualidade de vida do doente durante e após o tratamento oncológico.   Palavras-chave: Neoplasias Bucais; Sistema Único de Saúde; Atenção à Saúde. ABSTRACT The objective of this study was to verify the knowledge of dental surgeons in the primary, secondary and tertiary care network of the city of Palmas, Tocantins, Brazil, about the care of patients with oral cancer diagnosis in the Unified Health System, from the beginning of the treatment to completion. A semi-structured questionnaire was applied to the groups of professionals directly involved in the care network, evaluating the level of understanding of the dentists about the care. Qualitative and quantitative analyzes were performed. The results of the quantitative analysis showed that there were differences in the understanding of the professionals of the groups involved in the research, in relation to the content addressed in the questionnaire. It is concluded that, in the city of Palmas, there is a need for greater integration among dental surgeons involved in the care network for patients with oral cancer, and that the introduction of an inter- institutional cooperation program for  these patients could contribute to the follow-up flow and the quality of life during and after cancer treatment. RESUMOO Sistema Único de Saúde (SUS), um dos maiores sistemas públicos de saúde do mundo, foi instituído pela Constituição Federal de 1988 que, em seu artigo 196, coloca: “a saúde é direito de todos e dever do Estado”. Assim, este artigo objetivou avaliar a percepção dos moradores do Bairro Novo Israel em relação ao atendimento primário oferecido à comunidade na UBS Frei Valério Di Carlo. Para tanto, foi realizada uma pesquisa de natureza aplicada, com base em modelo quantitativo, sendo posta em prática uma pesquisa de campo que foi operacionalizada por meio da aplicação de um questionário e teve como amostra 100 usuários de uma unidade básica de saúde localizada na Zona Norte da cidade de Manaus. Os resultados da pesquisa indicam uma insatisfação dos usuários em relação ao tempo de espera para o atendimento. No entanto, estão satisfeitos com os serviços oferecidos pela Unidade.Palavras-chave: Sistema Único de Saúde, Avaliação, Satisfação dos Usuários. ABSTRACTThe Unified Health System (SUS), one of the largest public health systems in the world, was established by the Federal Constitution of 1988, which states in its article 196: "health is the right of everyone and the duty of the State." Thus, this article aimed to evaluate the perception of the residents of Bairro Novo Israel in relation to the primary care offered to the community at UBS Frei Valério Di Carlo. OBJECTIVE Replicating the training program in non-verbal communication based on the theoretical framework of interpersonal communication; non-verbal coding, valuing the aging aspects in the perspective of active aging, checking its current relevance through the content assimilation index after 90 days (mediate) of its application. METHOD A descriptive and exploratory field study was conducted in three hospitals under direct administration of the state of São Paulo that caters exclusively to Unified Health System (SUS) patients. The training lasted 12 hours divided in three meetings, applied to 102 health professionals. RESULTS Revealed very satisfactory and satisfactory mediate content assimilation index in 82.9%. CONCLUSION The program replication proved to be relevant and updated the setting of hospital services, while remaining efficient for healthcare professionals.

ABSTRACT Objective: to know health professionals’ perceptions about care actions provided to children with Congenital Zika Virus Syndrome and their families. Methods: this is a qualitative study, carried out in a capital of center-western Brazil, based on the Unified Health System theoretical precepts. Data were collected in September and October 2020, through audio-recorded interviews with 12 health professionals from a specialized service and submitted to analysis of content, thematic modality. Results: the implementation of care actions with these children occurs through multidimensional assessment of children and their families, use of the Unique Therapeutic Project, therapeutic interventions for the development of children and the communication and exchange of interprofessional and family experiences, in addition to considering professionals’ prior knowledge and their search for it. Final considerations: children with CZS and their families need individualized, frequent, integrated and continuous care.
