Integration of Multiple Sources of Information in Language Processing

Summary Objective: To summarize recent research and present a selection of the best papers published in 2015 in the field of clinical Natural Language Processing (NLP). Method: A systematic review of the literature was performed by the two section editors of the IMIA Yearbook NLP section by searching bibliographic databases with a focus on NLP efforts applied to clinical texts or aimed at a clinical outcome. Section editors first selected a shortlist of candidate best papers that were then peer-reviewed by independent external reviewers. Results: The clinical NLP best paper selection shows that clinical NLP is making use of a variety of texts of clinical interest to contribute to the analysis of clinical information and the building of a body of clinical knowledge. The full review process highlighted five papers analyzing patient-authored texts or seeking to connect and aggregate multiple sources of information. They provide a contribution to the development of methods, resources, applications, and sometimes a combination of these aspects. Conclusions: The field of clinical NLP continues to thrive through the contributions of both NLP researchers and healthcare professionals interested in applying NLP techniques to impact clinical practice. Foundational progress in the field makes it possible to leverage a larger variety of texts of clinical interest for healthcare purposes.

AbstractMany long-standing predominant theories of cognition have viewed higher levels of processing, such as language and cognition, as free from the influence of lower levels of processing, such as action and perception. However, many recent experiments have found evidence that the delineation between traditional modules is not so clean or precise, with motor output and language comprehension interacting much more fluidly than traditional theories predict. Evidence for this account includes findings of systematic activation of motor cortex while processing action words, as well as functional consequences of language on action and of action on language. It is worth noting that this recent spate of interest in the embodiment of cognition is not without historical precedence. Here, we will review such evidence coming from previous decades of research on the interaction between language and action, in addition to exploring the empirical results of these more recent experiments and methodologies. It seems to be the case, as evidenced from research in cognitive neuroscience and modeling, that motor representations and actions can influence language processing in predictable ways. Not only is there continuous competition between simultaneously active representations in language processing, with multiple sources of information interacting immediately, this competition is apparent in the motor output produced as a response to language.

Abstract Previous research has shown that heritage speakers struggle with inflectional morphology. ‘Limitations of online resources’ for processing a non-dominant language has been claimed as one possible reason for these difficulties. To date, however, there is very little experimental evidence on real-time language processing in heritage speakers. Here we report results from a masked priming experiment with 97 bilingual (Turkish/German) heritage speakers and a control group of 40 non-heritage speakers of Turkish examining regular and irregular forms of the Turkish aorist. We found that, for the regular aorist, heritage speakers use the same morphological decomposition mechanism (‘affix stripping’) as control speakers, whereas for processing irregularly inflected forms they exhibited more variability (i.e., less homogeneous performance) than the control group. Heritage speakers also demonstrated semantic priming effects. At a more general level, these results indicate that heritage speakers draw on multiple sources of information for recognizing morphologically complex words.

Language processing is influenced by multiple sources of information. We examined whether the performance in simultaneous interpreting would be improved when providing two sources of information, the auditory speech as well as corresponding lip-movements, in comparison to presenting the auditory speech alone. Although there was an improvement in sentence recognition when presented with visible speech, there was no difference in performance between these two presentation conditions when bilinguals simultaneously interpreted from English to German or from English to Spanish. The reason why visual speech did not contribute to performance could be the presentation of the auditory signal without noise (Massaro, 1998). This hypothesis should be tested in the future. Furthermore, it should be investigated if an effect of visible speech can be found for other contexts, when visual information could provide cues for emotions, prosody, or syntax.

AbstractProficient clinical reasoning is a critical skill in high-quality, evidence-based management of swallowing impairment (dysphagia). Clinical reasoning in this area of practice is a cognitively complex process, as it requires synthesis of multiple sources of information that are generated during a thorough, evidence-based assessment process and which are moderated by the patient's individual situations, including their social and demographic circumstances, comorbidities, or other health concerns. A growing body of health and medical literature demonstrates that clinical reasoning skills develop with increasing exposure to clinical cases and that the approaches to clinical reasoning differ between novices and experts. It appears that it is not the amount of knowledge held, but the way it is used, that distinguishes a novice from an experienced clinician. In this article, we review the roles of explicit and implicit processing as well as illness scripts in clinical decision making across the continuum of medical expertise and discuss how they relate to the clinical management of swallowing impairment. We also reflect on how this literature may inform educational curricula that support SLP students in developing preclinical reasoning skills that facilitate their transition to early clinical practice. Specifically, we discuss the role of case-based curricula to assist students to develop a meta-cognitive awareness of the different approaches to clinical reasoning, their own capabilities and preferences, and how and when to apply these in dysphagia management practice.

AbstractThe recognition, disambiguation, and expansion of medical abbreviations and acronyms is of upmost importance to prevent medically-dangerous misinterpretation in natural language processing. To support recognition, disambiguation, and expansion, we present the Medical Abbreviation and Acronym Meta-Inventory, a deep database of medical abbreviations. A systematic harmonization of eight source inventories across multiple healthcare specialties and settings identified 104,057 abbreviations with 170,426 corresponding senses. Automated cross-mapping of synonymous records using state-of-the-art machine learning reduced redundancy, which simplifies future application. Additional features include semi-automated quality control to remove errors. The Meta-Inventory demonstrated high completeness or coverage of abbreviations and senses in new clinical text, a substantial improvement over the next largest repository (6–14% increase in abbreviation coverage; 28–52% increase in sense coverage). To our knowledge, the Meta-Inventory is the most complete compilation of medical abbreviations and acronyms in American English to-date. The multiple sources and high coverage support application in varied specialties and settings. This allows for cross-institutional natural language processing, which previous inventories did not support. The Meta-Inventory is available at https://bit.ly/github-clinical-abbreviations.

The spatial arrangement of a metropolis is of utmost importance to carry out daily activities, which are constrained by space and time. Accessibility is not only shaped by the spatial and temporal dimension, but it is also defined by individual characteristics, such as gender, impairments, or socioeconomic characteristics of the citizens living or commuting in this area. This study analyzes mobility trends and patterns in the metropolitan area of Barcelona before and after the COVID-19 pandemic outbreak, with special emphasis on gender and equality. The study draws on multiple sources of information; however, two main datasets are analyzed: two traditional travel surveys from the transport metropolitan area of Barcelona and two coming from smartphone data. The results show that gender plays a relevant role when analyzing mobility patterns, as already highlighted in other studies, but, after the pandemic outbreak, some population groups were more likely to change their mobility patterns, for example, highly educated population groups and those with higher income. This study also highlights that e-activities may shape new mobility patterns and living conditions for some population segments, but some activities cannot be replaced by IT technologies. For all these reasons, city and transport planning should foster sustainable development policies, which will provide the maximum benefit for society.

The Coronavirus disease 2019 (COVID-19) pandemic has emerged as a significant and global public health crisis. Besides the rising number of cases and fatalities, the outbreak has also affected economies, employment and policies alike. As billions are being isolated at their homes to contain the infection, the uncertainty gives rise to mass hysteria and panic. Amidst this, there has been a hidden epidemic of “information” that makes COVID-19 stand out as a “digital infodemic” from the earlier outbreaks. Repeated and detailed content about the virus, geographical statistics, and multiple sources of information can all lead to chronic stress and confusion at times of crisis. Added to this is the plethora of misinformation, rumor and conspiracy theories circulating every day. With increased digitalization, media penetration has increased with a more significant number of people aiding in the “information pollution.” In this article, we glance at the unique evolution of COVID-19 as an “infodemic” in the hands of social media and the impact it had on its spread and public reaction. We then look at the ways forward in which the role of social media (as well as other digital platforms) can be integrated into social and public health, for a better symbiosis, “digital balance” and pandemic preparedness for the ongoing crisis and the future.

Persistent activity has been observed in the prefrontal cortex (PFC), in particular during the delay periods of visual attention tasks. Classical approaches based on the average activity over multiple trials have revealed that such an activity encodes the information about the attentional instruction provided in such tasks. However, single-trial approaches have shown that activity in this area is rather sparse than persistent and highly heterogeneous not only within the trials but also between the different trials. Thus, this observation raised the question of how persistent the actually persistent attention-related prefrontal activity is and how it contributes to spatial attention. In this paper, we review recent evidence of precisely deconstructing the persistence of the neural activity in the PFC in the context of attention orienting. The inclusion of machine-learning methods for decoding the information reveals that attention orienting is a highly dynamic process, possessing intrinsic oscillatory dynamics working at multiple timescales spanning from milliseconds to minutes. Dimensionality reduction methods further show that this persistent activity dynamically incorporates multiple sources of information. This novel framework reflects a high complexity in the neural representation of the attention-related information in the PFC, and how its computational organization predicts behavior.
