Background & Aim of the Study: Increased levels of many biomarkers, including liver enzymes, blood urea and serum creatinine as well as glycemic markers have been reported following coronavirus (COVID-19) infection, leading to the development of acute disease. This study aims to measure and follow-up the following biomarkers (fasting blood glucose, blood urea, serum creatinine, total serum bilirubin, as well as the liver enzymes AST, ALT, and ALP) in otherwise healthy participants and patients with liver disease, renal disease and diabetes following COVID-19 infection. Materials and Methods: This is cross section study, included 144 participants who were infected with COVID-19 and admitted to the Sheikh Zayed Hospital, Baghdad, Iraq. Participants were divided into 4 study groups, Group 1: 46 participants with no pre-existing medical condition (Control), Group 2: 30 patients with existing liver disease. Group 3: 28 patients with existing renal disease and Group 4: 40 patients with diabetes mellitus. Participants were followed up for 14 days following COVID-19 infection to monitor the progression of the biochemical markers. Results: There were significant changes in serum levels of all the markers of this study between the four study groups (p<0.001). Serum ALP levels were not significantly changed within any of the four study groups. However, both ALT and AST levels were significantly changed within all the four study groups (p<0.001). The levels of TSB changes significantly within the renal group (Group 3), (p=0.017). The levels of S. Creatinine showed significant changes in all the study groups except the renal group (Group 3). The levels change significantly within all the study groups except the control group (Group 1), while fasting blood glucose levels changes significantly in the control group only (Group 1), (p=0.004). Conclusions: Following COVID-19 infection, there were significant changes in the levels of ALT, AST, S. Creatinine and B.Urea after 14 days of the disease progression. While in patients with existing renal disease, there were significant changes in the levels of TSB, AST, ALT and B. Urea following COVID-19 infection. In diabetic patients, there were significant increase in the level of   fasting blood glucose after 14 days of COVID-19 infection. there were no significant changes in serum levels of ALP and FBG in patients with chronic illnesses (liver disease, renal disease, and diabetes) when compared to control group.

Background & Aims: ProBNP is a sensitive marker of cardiac dysfunction. We assessed the concentration of circulating NT-proBNP in patients with liver cirrhosis in various stages of the disease and its correlation with markers of cardiac and renal dysfunction and with markers of liver disease severity.Patients and methods: A number of 88 patients with liver cirrhosis were included in the study, divided into 3 groups: group 1 - 18 control patients without ascites; group 2 - 35 non-azotemic patients with ascites; group 3 - 35 patients with hepatorenal syndrome. The cardiac dysfunction was assessed by measuring the NT-proBNP serum levels and the QTc interval. The markers of renal dysfunction were the estimated glomerular filtration rates - formulas involving creatinine and serum cystatin C. The Child-Pugh score was used to assess the liver disease severity.Results: The median NT-proBNP serum levels significantly increased in patients with advanced liver cirrhosis (group 3: 960 fmol/ml and group 2:  660 fmol/ml) as compared to group 1 (435 fmol/ml) (p<0.05). A significant direct correlation was found between the NT-proBNP concentration and the QTc interval (r=0.540, p<0.001). The NT-proBNP levels also correlated with the Child-Pugh score (r=0.501, p<0.01), proving the link between the cardiac dysfunction and the liver disease severity. The cardio-renal interrelation is supported by the relationship between the NT-proBNP concentration and the estimated clearances.Conclusion: The high NT-proBNP levels in patients with advanced cirrhosis indicate the presence of cardiac dysfunction, which has a role in the pathogenesis of the hepatorenal syndrome.Abbreviations: DP: diastolic pressure; GFR: glomerular filtration rate; HRS: hepatorenal syndrome; MAP:mean arterial pressure; NT-proBNP: N-terminal fragment of the prohormone B-type natriuretic peptide; proBNP: prohormone brain natriuretic peptide; SBP: spontaneous bacterial peritonitis; SP: systolic pressure; TIPS: tranjugular intrahepatic portosystemic shunt.

Introduction: The aim of the present study is to profile the serum antioxidative enzymes, catalase (CAT), glutathione peroxidase (GPx), and superoxide dismutase (SOD) level in Type II diabetes mellitus patients in comparison to healthy volunteers in the South Indian population.Methodology: A prospective, observational, case–control study was conducted for 1 year with a total of 120 patients including 90 Type II diabetes patients (case group) and 30 healthy volunteers (control group). Blood was collected from these volunteers, and serum levels of CAT, GPx, and SOD were estimated. In addition, they were also monitored for the fasting blood glucose, glycated hemoglobin (HbA1c), and postprandial blood glucose. Data were statistically analyzed applying unpaired t-test and Pearson correlation with the statistical significance of p<0.05.Results: The diabetes patient group showed significant higher levels of glycated hemoglobin, fasting blood glucose and postprandial blood glucose (p<0.0001). There was asignificant lower level in the RBC levels of superoxide dismutase in case group compared to control group 3859.00±381.8 (mean+SD) and 5862.7±209.45 (mean+SD) Units per gram Hb, (t-value 27.35, p-Value <0.0001). Catalase and Glutathione peroxidase RBC levels also showed significant lower levels in the case group compared to the control group (catalase 212.7±19.08 (mean±SD) and 396.47±10.83 (mean±SD) Units per gram Hb; T value=50.07 and p<0.0001)(Glutathione peroxidase11.7 ±01.09 (mean +SD) and 18.6 ± 01.00 (mean +SD) Units per gram Hb; t value=30.26 and p<0.0001).Conclusion: A significant reduction in serum levels of antioxidative enzymes, CAT, GPx, and SOD was observed in the South Indian Type II diabetes patient population.

In diabetes mellitus there is an increase in oxidative stress in which oxidants exceed the antioxidant system resulting from the production of oxidant-antioxidant imbalance and/or limited antioxidant defenses. Abelmoschus esculentus (“okra”) is known as lowering blood glucose vegetable by Indonesian. A. esculentus has a high antioxidant activity that can: 1) lower H2O2 level; 2) decrease the amount of ROS; 3) lower •OH level. This study tried to prove that administration of A. esculentus may decrease fasting blood glucose, serum malondialdehyde, and increase superoxide dismutase activity of diabetic male white rats. This research is a pure experimental research, with randomized post test only control group designs, held in 28 days. Thirty one rats were divided into 4 groups randomly: KN = normal control group given 0.1% Na-CMC solution; KDM = DM control group injected with STZ, followed by 0.1% Na-CMC solution; P1 = treatment group 1 injected with STZ, followed by administration of A. esculentus at a dose of 775 mg/kgBW/day; P2 = treatment group 2 injected with STZ, followed by administration of A. esculentus at a dose of 1550 mg/kgBW/day. The result: administration of A. esculentus at doses of 775 mg/kgBW/day and 1550 mg/kgBW/day can not decrease fasting blood glucose and MDA serum level of diabetic male white rats. Giving A. esculentus at a dose of 1550 mg/kgBW/day may increase the activity of SOD erythrocytes of diabetic male white rats.. Conclusions: A. esculentus has the potential to lower blood glucose and MDA serum levels, also increase erythrocyte SOD activity.

The neuropeptides comprise an important part in the nervous system interacting with endocrine and immune systems. Peptide regulators are responsible for the continuity of communicating elements, which support homeostasis, however, despite abundant research examining neuropeptides, not all specific mechanisms and features of interacting proteins with cells and immune components have been uncovered. Objective: to perform a comprehensive assessment of neuropeptide system in patients with herpes zoster. Materials and methods: 106 in-hospital patients were examined diagnosed with herpes zoster within 2016–2019 period. Control group consisted of 30 healthy age- and sex-matched volunteers. Blood serum was collected after verifying diagnosis on day 1. After discharge, patients were monitored for signs of pain syndrome and overall state within 3 months. It allowed to divide patients into 3 groups retrospectively. Group 1 — patients with herpes zoster, accompanied by mild or moderate pain syndrome; group 2 — patients with herpes zoster, accompanied by severe pain; group 3 — patients with herpes zoster, complicated by postherpetic neuralgia. Level of serum protein s100B, myelin basic protein, nerve growth factor, brain-derived neurotrophic factor, neuron specific enolase was measured by using specific reagents purchased from “R&D Diagnostics Inc.” (США). Results. it was found that level of serum protein S100B in all groups was significantly increased compared to control group, showing no inter-group differences. Amount of myelin basic protein in all study groups vs. control was significantly higher. Moreover, level of these parameters in group 2 vs. group 1 and 3 was significantly elevated. In addition, level of nerve growth factor was significantly increased in group 1 vs. groups 2 and 3, whereas in group 3 it was significantly lower than in control and group 2. Brain-derived neurotrophic factor was significantly decreased in all the study groups compared to control, showing no significant intergroup differences. Level of neuron-specific enolase was significantly increased in group 3 vs. control as well as group 1 and 2. The data obtained allowed to identify two parameters for assessing a risk of postherpetic neuralgia in acute herpes zoster, as well as provided deeper insights into the pathogenesis of neuroimmune disorders accompanying herpes zoster.

BackgroundWe aimed to understand how glycaemic levels among COVID-19 patients impact their disease progression and clinical complications.MethodsWe enrolled 2,366 COVID-19 patients from Huoshenshan hospital in Wuhan. We stratified the COVID-19 patients into four subgroups by current fasting blood glucose (FBG) levels and their awareness of prior diabetic status, including patients with FBG&lt;6.1mmol/L with no history of diabetes (group 1), patients with FBG&lt;6.1mmol/L with a history of diabetes diagnosed (group 2), patients with FBG≥6.1mmol/L with no history of diabetes (group 3) and patients with FBG≥6.1mmol/L with a history of diabetes diagnosed (group 4). A multivariate cause-specific Cox proportional hazard model was used to assess the associations between FBG levels or prior diabetic status and clinical adversities in COVID-19 patients.ResultsCOVID-19 patients with higher FBG and unknown diabetes in the past (group 3) are more likely to progress to the severe or critical stage than patients in other groups (severe: 38.46% vs 23.46%-30.70%; critical 7.69% vs 0.61%-3.96%). These patients also have the highest abnormal level of inflammatory parameters, complications, and clinical adversities among all four groups (all p&lt;0.05). On day 21 of hospitalisation, group 3 had a significantly higher risk of ICU admission [14.1% (9.6%-18.6%)] than group 4 [7.0% (3.7%-10.3%)], group 2 [4.0% (0.2%-7.8%)] and group 1 [2.1% (1.4%-2.8%)], (P&lt;0.001). Compared with group 1 who had low FBG, group 3 demonstrated 5 times higher risk of ICU admission events during hospitalisation (HR=5.38, 3.46-8.35, P&lt;0.001), while group 4, where the patients had high FBG and prior diabetes diagnosed, also showed a significantly higher risk (HR=1.99, 1.12-3.52, P=0.019), but to a much lesser extent than in group 3.ConclusionOur study shows that COVID-19 patients with current high FBG levels but unaware of pre-existing diabetes, or possibly new onset diabetes as a result of COVID-19 infection, have a higher risk of more severe adverse outcomes than those aware of prior diagnosis of diabetes and those with low current FBG levels.

Aim. To evaluate endothelial function in patients with chronic ischemic heart disease by determining the number of circulating endothelial cells in peripheral blood. Material and methods. 71 patient with typical angina class I-III and presence of obstructive coronary artery disease according to coronary angiography were assessed, coronary anatomy was assessed on the SYNTAX Score scale and the number of CEC was determined by flow cytofluorimetry. Depending on the chosen treatment strategy patients were divided into the following groups: a group of medical therapy (group 1) - 22 patients; a group of patients requiring percutaneous coronary intervention (group 2) - 25 patients; a group of patients requiring surgical revascularization by coronary artery bypass surgery (group 3) - 24 patients. The control group consisted of 20 patients without atherosclerotic lesions of the coronary arteries. Results. Study groups do not differ by sex, age, history of smoking, the presence of hypertension, MI history (р > 0.05). In the group of patients who are scheduled to perform CABG significantly more patients with diabetes mellitus (р < 0.05) (group 1 - 3 patients (13.6%), group 2 - 4 patients (16.0%), group 3 - 7 patients (29.1%), control group - 2 patients (11.8%)). Study groups reliably differ in the anatomy of coronary lesions (SYNTAX Score in group 1 - 9.4 ± 2.7, in group 2 - 19.7 ± 5.7, in group 3 - 23.5 ± 6.0), р < 0.05. The number of CECs in the study groups is significantly higher than the generally accepted norm (р < 0.05), (group 1 - 12 (10÷16), group 2 - 14 (10÷17), group 3 - 14 (11÷17), control group - 12 (10÷16)). There are no significant differences between the groups with coronary artery disease (р > 0.05). Conclusion. Study of endothelial function by flow cytometry using monoclonal fluorescently labeled antibodies to CD146 and CD45 can be considered as a criterion for noninvasive assessment of the severity of atherosclerotic lesion of the coronary bed. Aim. To investigate the association of the polymorphic variants -204A > C (rs 3808607) in the CYP7A1 gene with the development of dyslipidemias in healthy individuals, in patients with non-alcoholic fatty liver disease (NAFLD) and in those with NAFLD concurrent with hypothyroidism. Subjects and methods. DNA samples and lipidograms were examined in 180 patients, including 60 healthy individuals (Group 1), 60 patients with hypothyroidism concurrent with NAFLD (Group 2), and 60 patients with NAFLD (Group 3). All the patients underwent ultrasound examination of the thyroid gland and abdominal cavity organs; FibroMax scores were calculated. Results. All the study groups most frequently showed a homozygous AA genotype (86.6% of cases in Group 1, 80% in Group 2, and 83.3% in Group 3). The development of NAFLD in CC genotype carriers is characterized by the most pronounced changes in lipid metabolism (atherogenic index (AI), 7.32 in Group 3) compared to the genotypes AA (AI, 4.56 in Group 2 and 1.73 in Group 1) and CC (AI, 6.43 in Group 2 and 2.52 in Group 1) in functional insufficiency of thyroid hormones and relative normal conditions. Conclusion. The analysis of the relationship of polymorphic variants CYP7A1 rs 38088607 to lipid metabolic disturbances in the study groups showed that the significantly higher levels of atherogenic cholesterol fractions were determined in the CC genotype compared to AA genotype carriers and they did not depend on the presence of NAFLD and hypothyroidism. The findings make it possible to consider the AA homozygous genotype of variant mutation CYP7A1 rs 38088607 as protective against dyslipidemia. However, in functional insufficiency of thyroid hormones, the level of triglycerides is significantly higher in both genotypes, which suggests that hypothyroidism plays an essential role in the development of dyslipidemia and NAFLD.

Objective: Non-Alcoholic Fatty Liver Disease (NAFLD) is the most common cause of liver disease. This study aims to evaluate the effect of High Intensity Interval Training (HIIT) and Loquat Leaf Extract (LLE) consumption on liver enzymes in obese men with NAFLD. Methods: In this quasi-experimental study, 40 men with NAFLD (BMI: 33.92±1.82 kg/m2) participated and randomly divided into four groups: HIIT (n= 10), LLE (n= 10), LLE+HIIT (n= 10) and control (n= 10). Exercises were performed for 8 weeks, 3 sessions per week, each for 60 minutes with an intensity between 80-95% of heart rate reserve. Two capsules of 250 mg LLE was administrated daily for 8 weeks. Serum levels of Aspartate Aminotransferase (AST), Alkaline Phosphatase (ALP), and Alanine Aminotransferase (ALT) were measured before and 48 hours after the intervention. Results: The results of paired t-test showed that the post-test serum levels of ALT, AST and ALP in the LLE+HIIT (P=0.01, 0.01, 0.01) and HIIT groups decreased significantly (P=0.02, 0.01, 0.001). These changes were not significant in the control and LLE groups (P≥0.05). The results of Tukey’s test showed that serum levels of ALT, AST and ALP in the LLE+HIIT and HIIT groups had the highest significant decrease (P=0.001) compared to the control group, while ALT and ALP serum levels in the LLE+HIIT group compared to the LLE group (P=0.03) and in the HIIT group compared to the control group (P=0.02) had the lowest decrease after eight weeks of intervention. Conclusion: It seems that consumption of LLE along with high- HIIT leads to a decrease in the level of liver enzymes in men with NAFLD. It is recommended that patients with NAFLD use this technique to improve their liver conditions.

Milking is a source of change in milk yield and milk quality determination, and one of the main events of milking is the oxytocin-mediated ejection reflex. Complete ejection of milk from the udder is due to the presence of high oxytocin during milking. The aim of this study is to investigate whether oxytocin administered intramuscularly has an effect on increasing milk ejection. In the study, 28 clinically healthy lactation Holstein cows were used, the lactation numbers and lactation periods of these cows were ignored in the evaluation. The study groups are divided into four groups, each consisting of seven cows. 50 IU oxytocin was given to group-1 five days before milking in the morning and evening, group-2 received 50 IU oxytocin once a day just before milking and the group-3 was given 50 IU oxytocin once before morning milking on the 1st, 3rd, and 5th days and the group-4 is a control group, oxytocin was not given to this group. Oxytocin injection was administered intramuscularly 30 minutes before milking. In the evaluation, the last five days of milk data before the application, five days of milk data during the application period and five days of milk data after the application period were taken into consideration. As a result of the study, we can say that the udder was not completely emptied with the intramuscular administration of oxytocin.

BACKGROUND: Cryptorchidism is associated with the risk of infertility and can be a symptom of congenital sexual maldevelopment. AIM: To assess the functional status of the pituitary gland and sexual glands in boys aged 16 months with bilateral inguinal cryptorchidism and to reveal the congenital sexual maldevelopment during minipuberty. METHODS: Twenty-one boys aged 16 months (the minipuberty period) with isolated bilateral inguinal cryptorchidism and 40 healthy boys aged 23 months (the control group) were examined. The gonadal status was assessed and serum levels of sex hormones were measured. Molecular genetic testing was performed if there were indications for it. RESULTS: The results of hormone analysis were used to divide the patients into three groups: group 1 patients with normal serum levels of gonadotropin and sex hormones; group 2 patients with elevated gonadotropin level and low levels of anti-Mllerian hormone (AMH) and inhibin B, and group 3 patients with zero gonadotropin and testosterone levels and low levels of AMH and inhibin B. Group 1 patients had no functional disturbances in the pituitarygonadal system. Failure of Sertoli cells associated with a high risk of infertility was detected in group 2 patients. Group 3 patients were diagnosed with hypogonadotropic hypogonadism verified by molecular genetic tests. CONCLUSION: Hormonal testing of patients with bilateral inguinal cryptorchidism during minipuberty makes it possible to early detect the congenital sexual maldevelopment.
