Abstract Purpose [18F]FDG-PET/CT scanning can help detect metastatic infectious foci and reduce mortality in patients with Staphylococcus aureus bacteremia (SAB), but it is unknown if patients with SAB and an indication for prolonged treatment because of possible endovascular, orthopaedic implant, or other metastatic infection still need [18F]FDG-PET/CT. Methods In a retrospective single-center cohort study, we included all consecutive adult patients with SAB between 2013 and 2020 if an [18F]FDG-PET/CT scan was performed and antibiotic treatment was planned for ≥ 6 weeks prior to [18F]FDG-PET/CT. We aimed to identify patients for whom treatment was adjusted due to the results of [18F]FDG-PET/CT, and assessed concordance of [18F]FDG-PET/CT and clinical diagnosis for infected prosthetic material. Results Among 132 patients included, the original treatment plan was changed after [18F]FDG-PET/CT in 22 patients (16.7%), in the majority (n = 20) due to diagnosing or rejecting endovascular (graft) infection. Antibiotic treatment modifications were shortening in 2, iv-oral switch in 3, extension in 13, and addition of rifampicin in 4 patients. Ninety additional metastatic foci based on [18F]FDG-PET/CT results were found in 69/132 patients (52.3%). [18F]FDG-PET/CT suggested vascular graft infection in 7/14 patients who lacked clinical signs of infection, but showed no infection of prosthetic joints or osteosynthesis material in eight patients who lacked clinical signs of such an infection. Conclusion [18F]FDG-PET/CT can help refine treatment for SAB in patients with clinically suspected endovascular infection or vascular grafts, even if 6 weeks treatment is already indicated, but can be safely omitted in other patients who are clinically stable.

Ti-based materials are widely used for dental and orthopaedic implant applications due to their adequate mechanical properties, corrosion behaviour and biocompatibility. However, these materials are biologically inert and display poor wear resistance. In one of the most studied processes that aims to overcome these drawbacks, Ti surfaces are often covered by anodic oxide films with the incorporation of bioactive agents such as Ca and P. Although there are several works on the tribocorrosion behaviour of MAO-treated Ti surfaces, the influence of electrolyte composition on the corrosion kinetics under sliding is yet to be fully understood. In the present work, anodic oxide films were produced on cp-Ti surfaces with different calcium acetate concentrations in the electrolyte. Tribocorrosion behaviour was investigated by reciprocating sliding tests performed in 8 g/L NaCl solution at body temperature, under potentiostatic conditions. The results showed that higher concentrations of calcium acetate had a detrimental effect on tribocorrosion kinetics, however, they resulted in less mechanical damage due to alterations in the topography and structure of the MAO layer.

Post-operative infections in orthopaedic implants are severe complications that require urgent solutions. Although conventional antibiotics limit bacterial biofilm formation, they ignore the bone loss caused by osteoclast formation during post-operative orthopaedic implant-related infections. Fortunately, enoxacin exerts both antibacterial and osteoclast inhibitory effects, playing a role in limiting infection and preventing bone loss. However, enoxacin lacks specificity in bone tissue and low bioavailability-related adverse effects, which hinders translational practice. Here, we developed a nanosystem (Eno@MSN-D) based on enoxacin (Eno)-loaded mesoporous silica nanoparticles (MSN), decorated with the eight repeating sequences of aspartate (D-Asp8), and coated with polyethylene glycol The release results suggested that Eno@MSN-D exhibits a high sensitivity to acidic environment. Moreover, this Eno@MSN-D delivery nanosystem exhibited both antibacterial and anti-osteoclast properties in vitro. The cytotoxicity assay revealed no cytotoxicity at the low concentration (20 μg/ml) and Eno@MSN-D inhibited RANKL-induced osteoclast differentiation. Importantly, Eno@MSN-D allowed the targeted release of enoxacin in infected bone tissue. Bone morphometric analysis and histopathology assays demonstrated that Eno@MSN-D has antibacterial and antiosteoclastic effects in vivo, thereby preventing implant-related infections and bone loss. Overall, our study highlights the significance of novel biomaterials that offer new alternatives to treat and prevent orthopaedic Staphylococcus aureus-related implantation infections and bone loss.

Abstract Background: With regard to the ceramic hip joint implant, given the concerns in ceramic about the alumina brittleness and zirconia instability, is there any alternative material solution for the orthopaedic implant? Beyond the metastable oxide ceramics, along the echelon of advanced technical ceramics, looking at the non-oxide ceramic, the silicon nitride could be an excellent candidate for the joint implant’s application. The purpose of this study is to investigate the safety, effectiveness and to demonstrate the potential of this silicon nitride hip implant. Methods: According to the related ISO (International Organization for Standardization) standards, a series of in-vitro (nine) & in-vivo (five) tests, which had been accomplished for the aforementioned aim. Especially, the total hip replacement in pigs had been achieved, as per the authors’ knowledge, this is the first time to apply the THA (Total Hip Arthroplasty) in the big animal. Results: Refer to the ISO 6474-2, in comparison with the current monopolized German product, this silicon nitride ceramic hip implant has high strength, high hardness, excellent fracture toughness, lower density, better wear resistance, good biocompatibility, inherent stability, corrosion resistance and bioactivity, bone integration capability. Conclusions: This silicon nitride ceramic will be an admirable alternative solution with superior comprehensive property that can withstand the toughest conditions in the most demanding applications like in orthopedic and beyond.

Stainless steel and titanium alloys are common materials for orthopaedic implants. However there is a lack of information and studies on magnetic remanence of  implants used in clinical practice. The aims of this study are to investigate the composition and the presence of magnetic remanence for these two orthopaedic implant materials. These two factors may cause implant instability and heat problems as well as degradation of the images quality if the patients undergo magnetic resonance imaging (MRI) examination. The magnetic hysteresis loop and remanence status of stainless steel and titanium alloy orthopaedic implants were investigated with a vibrating sample magnetometer (VSM). Both samples of stainless steel and titanium alloy had been exposed to external magnetic fields up to 1 T (10000 G) and 1.4 T (14000 G), respectively. The compositions of these two orthopaedic implant materials were studied using a scanning electron microscope with energy dispersive X-ray analysis (SEM-EDX). The results of the study demonstrated that ferrous and nickel compositions in stainless steel alloy orthopaedic implants contributed to the residual magnetism, as shown in the hysteresis loop. The titanium alloy orthopaedic implant sample does not contain any ferromagnetic elements. After exposure to a magnetic field, the stainless steel values of retentivity, coercivity and magnetisation are significantly higher compared to those of the titanium alloy. The stainless steel orthopaedic implant sample demonstrates a typical hysteresis loop that suggests the existence of magnetic remanence. In contrast, the titanium alloy orthopaedic implant sample showed no significant remanence phenomenon. By considering the existence of magnetic remanence in the implant is important as potential effect on the MRI image quality.

Background. Since there is no national arthroplasty registry in the Russian Federation, the exact number of arthroplasty operations performed is unknown. Aim of the study to evaluate the epidemiology of primary, revision hip and knee arthroplasty and periprosthetic joint infection in Russian Federation. Materials and Methods. The inquires were made to 85 subjects, 73 federal state budget institutions and 30 private hospitals. Results. Answers were received from 76 subjects of Russian Federation, 41 federal and 18 private hospitals. The studied indicators revealed to be pretty much heterogenic. The decrease of operations number from 20% to 40% in subjects of Russian Federation due to SARS-CoV-2 pandemic forced us to make no analysis for these 2020 indicators in Russian Federation generally. 147 061 primary hip and knee arthroplasties were performed in 2019 in Russian Federaion. 7 770 revison hip and knee arthroplasties were done. 6 606 cases of severe orthopaedic implant-assosiated infection were cured including 4 282 cases post hip and knee arthroplasties that constitutes 2,91 % from primary arthroplasties. Conclusions. The frequency of periprosthetic joint infection in Russian Federation correlates to the world data. High complexity for patients routing with implant-assosiated infection and strong demand for Federal Center of implant-assosiated orthopaedic infection are revealed.

AbstractPoly-ether-ether-ketone (PEEK) has attracted increasing attention as a promising orthopaedic implant material owing to its excellent mechanical properties and biocompatibility. However, its antibacterial properties must be improved as an implant material. In this study, PEEK was sulfonated to obtain a porous surface, and graphene oxide (GO) was deposited to form a coating with antibacterial activity and biocompatibility. After PEEK was sulfonated for different durations, GO was deposited on the surface to prepare the coating (SPEEK-GO), which was then characterised using scanning electron microscopy (SEM), Raman spectroscopy, and contact angle measurements. The in vitro study included antimicrobial and cellular tests. The results showed that the PEEK sulfonated using a 10-min treatment exhibited a uniform porous structure and provided a better basal surface for the deposition of GO. The SPEEK-GO coating displayed strong antibacterial activity against two common dental pathogens. It exhibited good adhesion and proliferation of MC3T3-E1. Moreover, it showed osteogenic differentiation as bone implant material.

Abstract. Slow-growing Gram-positive anaerobic bacteria (SGAB) such as Cutibacterium acnes are increasingly recognized as causative agents of implant-associated infections (IAIs) in orthopaedic surgeries. SGAB IAIs are difficult to diagnose because of their non-specific clinical and laboratory findings as well as the fastidious growth conditions required by these bacteria. A high degree of clinical suspicion and awareness of the various available diagnostic methods is therefore important. This review gives an overview of the current knowledge regarding SGAB IAI, providing details about clinical features and available diagnostic methodologies. In recent years, new methods for the diagnosis of IAI were developed, but there is limited knowledge about their usefulness in SGAB IAI. Further studies are required to determine the ideal diagnostic methodology to identify these infections so that they are not overlooked and mistakenly classified as aseptic failure.
