Although most of our patients will enter the consulting room quite quietly, often in a depressive state, having contained their sadistic impulses, a tiny fraction of those with whom we work will attack us in a variety of chilling ways. In this article, the author describes in detail two particularly terrifying clinical experiences in which a patient either threatened to kill him or actually sullied his consulting room with bodily fluids. Drawing upon his psychotherapeutic encounters not only with intellectually disabled patients and forensic patients but, also, with those who presented as ordinary “normal-neurotics”, the author considers the phenomenology of these “bomb”-like explosions and explains how he attempted to maintain a classical psychoanalytical focus of understanding, which consisted of a careful scrutiny of the countertransference and a firm commitment to the interpretation of unconscious material, whilst under attack. Furthermore, he examines the essential role of speaking with experienced colleagues who will provide essential supervision or assistance during these challenging chapters of clinical practice. The author also considers the many ways in which “bombs” can be hurled not only by the more obviously dangerous or disturbed individuals but, also, with surprising frequency, by those with no criminal history whatsoever, who, upon first encounter, often present as reasonably healthy.

In Yakutia, natural floodplain meadows and steppes have long been involved in economic activities. To develop methods of rational use, one needs to constantly consider the state of vegetation, mainly its constituent populations. Our research focuses on studying the dynamics of the vitality of the populations of the Agrostis diluta Kurcz. and steppe Psathyrostachys caespitosa (Sukaczev) Peschkova cereals. These species play an essential role in the composition of vegetation cover. We determined the vitality coefficient IVC. Our study of populations over several years has revealed the dynamic mobility of the vital structure, which is of adaptive significance and ensures the sustainability of populations. We have found that the vital structure of the cereal populations of the meadow Agrostis diluta and the steppe Psathyrostachys caespitosa during long-term studies (2007–2013 and 2007–2016, respectively) is heterogeneous. In unfavorable humidification conditions, we characterize years as depressive. Under favorable conditions of humidification, they quickly turn into thriving plants. In addition to the weather of the vegetation periods, habitat specificity affects the vitality of species populations that differ in ecological and coenotic characteristics. The Q quality index and IVC correspond to each other by year and form a clear descending order during the transition from a thriving to a depressive state. Currently, the vital state of the populations of both species is satisfactory.

Abstract The morphological structure, size-age variability and dynamics of age-related fertility of producers of Baikal omul of pelagic, near-bottom and coastal morpho-ecological groups during the depressive state of the population were studied. There is an improvement in the indicators of the growth rate of fish, maturation and an increase in absolute individual fertility against the background of the dilution of the population and the release of the feeding area. A comparative characteristic of some morphological features of fish among morphological-ecological groups is given. The number of gill rakers on the first gill arch significantly distinguishes different morpho-ecological groups and ranges from 36 to 43 pieces for the near-bottom, from 40 to 46 pieces for the coastal and from 44 to 53 pieces for the pelagic morpho-ecological groups. It is noted that the spawning populations of the coastal and pelagic groups are mainly represented by young individuals. With age, the proportion of females increases in all groups. The largest indices of the pectoral, abdominal and anal fins, responsible for movement in the vertical plane, rotation and stop, are noted in the near-bottom group.

This cross-sectional study aims to clarify the relationship between the number of behavioral and psychological symptoms in long-term care recipients and family caregivers’ depressive states. Participants were 38 family caregivers who had provided care for their relatives for 6 years or more; they were recruited from in-home care settings in Aichi, Gifu, and Shiga Prefectures, Japan. Participants answered a self-administered questionnaire assessing their depressive state using the Japanese version of the Center for Epidemiologic Studies for Depression Scale (CES-D). They also answered questions inquiring about behavioral and psychological symptoms of care recipients, including resistance to care, irritability, and feelings of persecution. Using CES-D scores, 11 participants were categorized as depressed and 27 as non-depressed. Depressive symptoms were significantly greater in those with more behavioral and psychological symptoms, following adjustment for confounding factors. The odds ratio of being in a depressive state was 9.43 higher for those with more than 4 behavioral and psychological symptoms compared to those with none, showing a distinct threshold for the influence of behavioral and psychological symptoms on depressive state. Knowing the number of behavioral and psychological symptoms of care recipients may help quickly identify depressed caregivers and alleviate depressive symptoms.

Chemotherapy and other invasive therapies are often limited by side effects, pain and negative experiences that can limit adherence to the therapy itself. Such negative components add to the patient's depressive state due to the disease. This research project proposes the use of deep sedation during chemotherapy sessions or other disabling therapies in the treatment of tumors or other severe diseases. The proposed protocol provides for an ad hoc hospitalization which could be during the night, during the day or limited to a few hours. Administration during sleep eliminates the memory and the negative impact the treatment has on the rest of the patient's daily life. This approach also agrees with the evidence of the circadian rhythm of cellular repair processes, which is greater at sunrise and sunset and linked to a good quantity and quality of sleep. In conclusion, this project aims to reduce the negative impact and increase the adherence to and efficacy of the therapy itself.

Abstract Depressive state is a common complication of spinocerebellar ataxia type 3 (SCA3). To the best of our knowledge, cases of SCA3 presenting with cenesthopathy have not been described. Here, we present a case of a severe depressive state with cenesthopathy and delusion in a young Japanese man with SCA3. A 43-year-old Japanese man with SCA3 developed a severe depressive state with associated cenesthopathy and delusion. He was treated with escitalopram (10 mg/day) and olanzapine (2.5 mg/day). Computed tomography showed atrophy of the cerebellum, bilateral superior cerebellar peduncle, and tegmentum of the pons. Single-photon emission computed tomography demonstrated reduced blood flow in the cerebellum, vermis, and brainstem. After 8 weeks, his depressive state and delusion improved; however, his cenesthopathy persisted. We encountered a case of a severe depressive state with cenesthopathy and delusion in a young Japanese man with SCA3. This case supports previous studies that the cerebellum could have a role beyond motor functions.

<b><i>Introduction:</i></b> Psychological disorders, such as depression, are markedly prevalent in patients with airway diseases. In this study, we assessed the effect of treatment with dupilumab, an IL-4 receptor α chain antibody, on depressive symptoms in a cohort of patients with asthma with eosinophilic chronic rhinosinusitis (ECRS). <b><i>Methods:</i></b> The study participants, diagnosed with asthma and ECRS, were assessed for symptoms and quality of life (QOL) scores for asthma and ECRS and medications. The Patient Health Questionnaire-9 (PHQ-9) scores were used to evaluate the depressive state. The depressive symptoms were compared with asthma and ECRS symptoms both at the time of initiation and after 4 months of dupilumab treatment. <b><i>Results:</i></b> Ultimately, 31 patients were included in the study. Most patients demonstrated a depressive state that was correlated with the nasal symptom score. In the evaluation 4 months after dupilumab treatment, the PHQ-9 score was significantly reduced, and the decrease was remarkable in patients whose nasal symptom score was reduced by 50% or more. Additionally, the PHQ-9 scores in patients with improved nasal and asthma symptoms were significantly reduced. <b><i>Discussion/Conclusion:</i></b> Dupilumab may improve QOL in patients with bronchial asthma with ECRS by reducing depressive symptoms through the improvement of clinical symptoms.
