Oats as a Functional Food

Oat (Avena sativa) belongs to the grass (Poaceae) family. It has been recognized as a healthful and nutritious cereal containing high concentration of soluble fiber compounds such as β-glucan, which are beneficial for digestive system and preventive against colon rectal cancer help to maintain an optimal weight due to high fiber content. It also contains antioxidants, vitamins, phenolic acids sterols and phytic acid which has essential role in our body functioning. The key cholesterol lowering ingredient in oats is soluble fiber. β-glucans, the most important cereal non-starch. β-glucan, is abundant in oat kernels and exhibits a high viscosity at relatively low concentrations. Viscosity is an important rheological property of β-glucan and is associated with beneficial physiologic responses that mediate appetite regulation. Oats were traditionally eaten as porridge with salt or sugar were added recent modern product innovation has created an array of oat products (Oat breads, oat cakes and oats milk etc.). Oats are used as an additive in Cereal breakfast, baked goods, oat milk and oat powder is incorporated in many Dairy products like (low-fat ice creams, yoghurts, cheese and other fermented drinks). It is stated that 3 g/day oat β-glucan reduced the total and LDL cholesterol by five to ten per cent.

Colon Rectal Liver Metastases: The Role of the Liver Transplantation in the Era of the Transplant Oncology and Precision Medicine

The development of liver metastases in colon rectal cancer has a strong impact on the overall survival (OS) of the patient, with a 5-year survival rate of 5% with palliative treatment. Surgical resection combined with pharmacological treatment can achieve a 5-year OS rate of 31–58%. However, in only 20% of patients with colon rectal liver metastases (CRLMs), liver resection is feasible. In highly selected patients, recent trials and studies proved that liver transplantation (LT) for non-resectable CRLM is a surgical option with an excellent long-term OS. The paper aims to review the indications and outcome of LT for CRLMs, with a special focus on immunosuppressive therapy and the management of local and extrahepatic recurrence after LT.

Current Therapeutic Strategies in BRAF-Mutant Metastatic Colorectal Cancer

Around 8–12% of patients with advanced colon rectal cancer (CRC) present with BRAF alterations, in particular V600E mutation, which is associated with right-side, poorly differentiated and mucinous type tumors. The presence of BRAF mutation (BRAF-mt) has been identified as a hallmark of poor prognosis and treatment optimization in this patient subgroup is an important goal. Currently, the standard of care is an aggressive strategy involving triplet chemotherapy and anti-VEGF agents, but new therapeutic approaches are emerging. Very promising results have been obtained with targeted therapy combinations, such as anti-BRAF agents plus anti-EGFR agents. Furthermore, around 60% of BRAF-mt patients show a strong association with high microsatellite instability (MSI-H) and immune checkpoint inhibitors could represent the new standard of care for this subgroup. The focus of this review is to summarize current strategies for BRAF-mt CRC treatment and highlight new therapeutic options.

Narrative Literature Review on Risk Factors Involved In Breast Cancer, Brain Cancer, Colon Rectal Cancer, Gynecological Malignancy, Lung Cancer, and Prostate Cancer

In this study, we describe the most critical risk factors for different malignancies, including breast, prostate, lung, colon-rectal carcinoma, among others and those with risk factors that are modifiable. We revised the literature review about risk factors involved in the genesis of cancer in various databases, including articles indexed in PUBMED, SCOPUS, PMC, and Google Scholar. Knowing risk factors is a mode of fighting malignancy. Diet, lifestyle, practises, and laboratory/clinical interventions were among risk factors of diverse malignancy. We conclude that risk factors can prevent the development of many malignancies in a century where this conundrum is raising disproportionably. It is treated here is to make the public aware of the modifiable risks of cancers.

Proctor-guided virtual reality–enhanced three-dimensional video-assisted thoracic surgery: an excellent tutoring model for lung segmentectomy

Objective: Lung segmentectomy using video-assisted thoracoscopic surgery (VATS) is an effective strategy to treat early-stage lung cancer. The objective of this case report is to show the efficacy of Hyper Accuracy 3D™ (HA3D) reconstruction as a tool for trainee surgeons and expert surgeons to perform complex procedures. Methods: An 84-year-old man was treated for colon-rectal cancer. During follow-up, a pulmonary nodule on the right anterior upper lobe suspected for intestinal metastasis was revealed by a computed tomography scan. According to functional tests and radiology, a right anterior upper lobe segmentectomy was planned. HA3D lung reconstruction was used during surgery. Results: Using the HA3D virtual model, the procedure was performed with healthy tissue sparing, ensuring safe resection margins. No postoperative morbidities were noted. The patient referred good pain control. The hospital stay was 6 days. Conclusions: VATS segmentectomy is a technically demanding procedure. HA3D lung reconstruction can help surgeons effectively perform the resection, aiding at individuating intersegmental planes, bronchi, and vessels, guaranteeing oncologic radicality and safe surgical margins, and preserving respiratory function.

High-Fat Diet Promotes DSS-Induced Ulcerative Colitis by Downregulated FXR Expression through the TGFB Pathway

Ulcerative colitis is one of the IBD which cause a chronic intestinal inflammation and dysfunctional of the mucosal barrier. For now, the incident of UC was steadily increased all over the world. It has become a novel independent risk factor of several severe diseases especially colon-rectal cancer. However, the etiology of UC was still obscure. Previous studies show that high-fat diet contributed to the pathogenesis of immune system dysregulation, and farnesoid X receptor (FXR) was also implicated in the pathogenesis of various inflammatory symptoms. Yet, their inner roles in the pathogenesis of UC have not been mentioned. In this study, we aim to investigate the role of FXR in UC. High-fat diet (HFD) promotes the progression of DSS-induced UC, shows an increasing secretion of bile acid in serum, and leads to a downregulation of FXR target genes (FXRα, Shp, and lbabp). Adding FXR agonist FexD rescues the phenotype induced by high-fat diet, whereas TGFBRI inhibitor SB431542 abrogates the restoration by FexD in DSS-induced UC mice. To further verify the relationship between the FXR and TGFB signaling pathway, we made a UC-HFD model in the Caco2 cell line. Results shows the same conclusion that FXR mitigate UC inflammation through a TGFB-dependent pathway. These results expand the role of FXR in ulcerative colitis and suggest that FXR activation may be considered a therapeutic strategy for UC.

Colon-rectal Cancer and Lars Management - Forecast of refundability of health services in Apulia in 2019

This article, taken from the ending research thesis of the studies in Business Administration in Pegaso University, derives from the lack of detailed data on colon-rectal cancer in Apulia, in 2019. The clinical, economic and statistical study introduced has given the possibility to examine the neoplasia incidence in Apulia reality, to estimate for the first time the Lars major phenomenon (Low Anterior Resection Syndrome) after a rectum surgical treatment. These goals have been achieved starting from the national data analysis provided by Airtum and Aiom; and operating through benchmarking, among the most virtuous Italian realities, it has been possible to estimate the forecast of refundability of health services generated by the treatment of colon cancers, rectal cancers and of the Lars phenomenon management. We can see the incidence of 2,802 total cases of colon-rectal cancer so divided: 1,920 colon cancer cases and 882 rectal cancer cases. The Lars phenomenon, according to the most recent medical literature, presents 444 cases, considering that 90 % of the 882 rectum tumors will have a surgery; 56% of these develops Lars major. Assuming to treat all cases of Lars major, it would potentially be possible a refundability of 63,492 Euros. While, as regards the refundability obtained from the treatment of the 2,802 colon-rectal cancer total cases, it could be hypothesized a total refundability of 18,897,408 Euros.

The Obesity-Related Gut Bacterial and Viral Dysbiosis Can Impact the Risk of Colon Cancer Development

An incorrect food regimen from childhood is suggested to negatively impact the gut microbiome composition leading to obesity and perhaps to colon rectal cancer (CRC) in adults. In this study, we show that the obesity and cancer gut microbiota share a characteristic microbial profile with a high colonization by mucin degraders species, such as Hafnia alvei and Akkermansia muciniphila. In addition, the species Clostridium bolteae, a bacterium associated with insulin resistance, dyslipidemia, and inflammation, has been associated with the presence of oncogenic Human Polyomaviruses (HPyVs). Merkel cell Polyomavirus (MCPyV) and BK Polyomavirus (BKPyV) were the most frequently oncogenic viruses recovered in the gut of both obese and tumor patients. Considering the high seroprevalence of HPyVs in childhood, their association with specific bacterial species deserve to be further investigated. Data from the present study highlight the presence of a similar microbiome pattern in CRC and obese subjects, suggesting that obese microbiome may represent an opportunity for tumorigenic/driver bacteria and viruses to trigger cell transformation.
