Durable and temporary mechanical circulatory support (MCS) use is growing for a range of cardiovascular indications. Kidney dysfunction is common in persons evaluated for or receiving durable or temporary MCS, and portends worse outcomes. This kidney dysfunction can be due to pre-existing kidney chronic kidney disease (CKD), acute kidney injury (AKI) related to acute cardiovascular disease necessitating MCS, AKI due to cardiac procedures, and due to acute and chronic MCS effects and complications. Durable MCS, with implantable continuous flow pumps, is used for long-term support in advanced HF refractory to guideline directed medical and device therapy, either permanently or as a bridge to heart transplantation. Temporary MCS-encompassing in this review intra-aortic balloon pumps (IABPs), axial flow pumps, centrifugal flow pumps, and venoarterial ECMO-is used for diverse situations: high risk percutaneous coronary interventions (PCI), acute decompensated heart failure (HF), cardiogenic shock, and resuscitation following cardiac arrest. The wide adoption of MCS makes it imperative to improve understanding of the effects of MCS on kidney health/function, and of kidney health/function on MCS outcomes. The complex structure and functions of the kidney, and the complex health states of individuals receiving MCS, makes investigations in this area challenging, and current knowledge is limited. Fortunately, the increasing nephrology toolbox of non-invasive kidney health/function assessments may enable development and testing of individualized management strategies and therapeutics in the future. We review technology, epidemiology, pathophysiology, clinical considerations, and future directions in MCS and nephrology.

Happiness has become one of the indicators in measuring the level of success of economic development lately. Happiness as an indicator of development success is in line with the new schools of thought in economics, namely economics of happiness and economics of behavior. This happiness indicator is in line with the sustainable development goals driven by the United Nations agency. This study aims to analyze the effect of macroeconomic variables on happiness in Indonesia. The macroeconomic variables are gross regional domestic product per capita, poverty, provincial government expenditures for the education function and for the health function. Happiness data is obtained from the results of a survey of measuring the level of happiness conducted by the Central Statistics Agency which issues a happiness index in 33 provinces in Indonesia. The form of this research is descriptive-quantitative and sourced from secondary data obtained from the publications of the Central Statistics Agency, the Ministry of Education, the Ministry of Health and the National Development Planning Agency. Data analysis uses a panel data regression approach, with serial data in 2014 and 2017 for the number of observations in 33 provinces in Indonesia. The results of the research analysis show: (1) gross regional domestic product per capita has a positive and significant effect on happiness in Indonesia; (2) poverty has a negative and significant effect on happiness in Indonesia; (3) provincial government spending on education function has a positive and significant effect on happiness in Indonesia; (4) provincial government spending on health function has a positive and insignificant effect on happiness in Indonesia. Breast cancer chemotherapy, although very potent against tumour tissue, results in significant cardiovascular toxicity. The focus of research in this area has been predominantly towards cardiotoxicity. There is limited evidence detailing the impact of such treatment on the vasculature despite its central importance within the cardiovascular system and resultant detrimental effects of damage and dysfunction. This review highlights the impact of chemotherapy for breast cancer on the vascular endothelium. We consider the most likely mechanisms of endothelial toxicity to be through direct damage and dysfunction of the endothelium. There are sharp consequences of these detrimental effects as they can lead to cardiovascular disease. However, there is potential for exercise to alleviate some of the vascular toxicity of chemotherapy, and the evidence for this is provided. The potential role of exercise in protecting against vascular toxicity is explained, highlighting the recent in-human and animal model exercise interventions. Lastly, the mediating mechanisms of exercise protection of endothelial health is discussed, focusing on the importance of exercise for endothelial health, function, repair, inflammation and hyperlipidaemia, angiogenesis, and vascular remodelling. These are all important counteracting measures against chemotherapy-induced toxicity and are discussed in detail.

ABSTRACT Introduction Studies have shown that physical exercise is beneficial to people’s overall physical and mental health, but few research reports on the effects of different physical exercises on people’s human health. Object The paper explores the difference in human health function between people who adhere to traditional health sports and those who rarely exercise and provide a scientific basis for applying and promoting traditional health sports in TCM “prevention of disease”. Methods The paper surveyed 526 people who regularly participate in physical exercises and rarely exercise. The exercise items are divided into Tai Chi/Tai Chi sword group, Health Qigong Baduanjin group, Health Qigong Wuqinxi group, and Health Qigong Yijin group. Warp group, walking/jogging group. Results There are differences in the mental indicators of the people in different exercise groups. The overall average percentage levels of and NK cells in each exercise group and the tiny exercise group are different, and the difference is statistically significant (P<0.05). Conclusions Persisting in physical exercise is beneficial to the balance of health and function of the population. Level of evidence II; Therapeutic studies - investigation of treatment results.

Biological aging research is expected to reveal modifiable molecular mechanisms that can be harnessed to slow or possibly reverse unhealthy trajectories. However, there is first an urgent need to define consensus molecular markers of healthy and unhealthy aging. Established aging hallmarks are all linked to metabolism, and a ‘rewired’ metabolic circuitry has been shown to accelerate or delay biological aging. To identify metabolic signatures distinguishing healthy from unhealthy aging trajectories, we performed nontargeted metabolomics on skeletal muscles from 2-month-old and 21-month-old mice, and after dietary and lifestyle interventions known to impact biological aging. We hypothesized that common metabolic signatures would highlight specific pathways and processes promoting healthy aging, while revealing the molecular underpinnings of unhealthy aging. Here, we report 50 metabolites that commonly distinguished aging trajectories in all cohorts, including 18 commonly reduced under unhealthy aging and 32 increased. We stratified these metabolites according to known relationships with various aging hallmarks and found the greatest associations with oxidative stress and nutrient sensing. Collectively, our data suggest interventions aimed at maintaining skeletal muscle arginine and lysine may be useful therapeutic strategies to minimize biological aging and maintain skeletal muscle health, function, and regenerative capacity in old age.

Although the association between childhood sexual abuse (CSA) and various aspects of sexual difficulties is well established, little is known about the association between CSA and adult sexual fantasies. The current rapid review searched for studies that reported on CSA and sexual fantasies through PubMed, PsycInfo, and Violence & Abuse Abstracts databases. Included in the review were empirical studies involving a population of adults who experienced CSA before the age of 18 and which reported on survivors’ sexual fantasies. The impact of CSA on adult sexual fantasies was found across three main dimensions: prevalence of sexual fantasies, content, and appraisal of the fantasies. Overall, 13 studies that addressed the sexual fantasies of survivors of CSA were identified. This review found an association between CSA and adult sexual fantasies, indicating that survivors of CSA are more likely to report: unrestricted sexual fantasies, more atypical sexual fantasies, more sexual fantasies that involve force, and more fantasies that include elements of sadomasochism, submissiveness, and dominance. Survivors of CSA also begin having sexual fantasies at a significantly earlier age and report their sexual fantasies as being significantly more intrusive than do nonabused subjects. When treating CSA survivors, therapists should acknowledge that a history of CSA can impact the survivors’ sexual fantasies. Further studies with adult survivors of CSA are needed to determine how these sexual fantasies develop subsequent to the abuse, how they are perceived by survivors, and what their effect is on survivors’ and their partners’ sexual health, function, and satisfaction.

AbstractFreshwater ecosystems provide a large number of benefits to society. However, extensive human activities threat the viability of these ecosystems, their habitats, and their dynamics and interactions. One of the main risks facing these systems is the overexploitation of groundwater resources that hinders the survival of several freshwater habitats. In this paper, we study optimal groundwater paths when considering freshwater ecosystems. We contribute to existing groundwater literature by including the possibility of regime shifts in freshwater ecosystems into a groundwater management problem. The health of the freshwater habitat, which depends on the groundwater level, presents a switch in its status that occurs when a critical water level (‘tipping point’) is reached. Our results highlight important differences in optimal extraction paths and optimal groundwater levels compared with traditional models. The outcomes suggest that optimal groundwater withdrawals are non-linear and depend on the critical threshold and the ecosystem’s health function. Our results show that the inclusion of regime shifts in water management calls for a reformulation of water policies to incorporate the structure of ecosystems and their interactions with the habitat.

The spread of the COVID-19 in Indonesia has changed various aspects of the life of the Indonesian people. The lifestyle of the people in the city of Surabaya which is advanced and modern makes the community, especially women, pay attention to their appearances. This causes the trend of masks circulating in society today to become increasingly diverse following the wishes of consumers. This research wants to see how the phenomenon of buying masks by women in Surabaya has become part of the lifestyle during the pandemic. The method used was data collection techniques carried out in two stages, primary namely questionnaires and in-depth interviews as a complement to secondary information, namely with data from various sources such as journals, books, web, and other data center agencies. The respondents interviewed were selected purposively with predetermined criteria. After all, data is processed, the data findings are interpreted and analyzed using the consumerism theory of Jean Baudrillard. This research resulted in the conclusion that most women in the city of Surabaya bought this type of cloth mask. Women in the city of Surabaya consider that masks not only have a health function as the main factor but also must have a fashion function.
