AbstractWith respect to the tremendous deficit in surgical care plaguing developing nations, it is critical that medical outreach models be organized in such a fashion that sustainable advancements can be durably imparted beyond the duration of targeted missions. Using a didactic framework focused on empowering host neurosurgeons with an enhanced surgical skillset, a mission was launched in Managua, Nicaragua, after previous success in Kiev, Ukraine, and Lima, Peru. Unfortunately, the failure to critically assess the internal and external state of affairs of the region’s medical center compromised the outreach mission. Herein lies the visiting team’s lessons from failure and insights on facilitating effective communication with host institutions, circumventing geopolitical instability, and utilizing digital collaboration and video-conferencing tools in the post-COVID-19 era to advance the surgical care of developing regions in a fashion that can be generationally felt.

Many members of the medical profession in Mobile, Alabama, have exemplified a strong commitment to the education of their colleagues and successors, a tradition (L., traditio, “to hand over”) that dates from the early 18th century. The Mobile General (city/county) Hospital (1830 to 1970) and its successor, the Medical Center, University of South Alabama (1971 to the present), were the institutional foci of those endeavors. Because it is individuals who create, design, and vitalize institutions, this monograph is an acknowledgment of the accomplishments of those who gave that endeavor purpose, direction, and meaning, particularly with reference to the evolution of surgical education. Numerous clinical and societal forces—cultural, economic, political, and social—influenced that evolution. This compilation gives attribution to a legacy of commitment to health and medical/surgical care, education, and research within southern Alabama.

With emphasis on value-based health care, empiric models are used to estimate expected read-mission rates for individual institutions. The aim of this study was to determine the relationship between distance traveled to seek surgical care and likelihood of readmission in adult patients undergoing cardiac operations at a single medical center. All adults undergoing major cardiac surgeries from 2008 to 2015 were included. Patients were stratified by travel distance into regional and distant travel groups. Multivariable logistic regression models were developed to assess the impact of distance traveled on odds of readmission. Of the 4232 patients analyzed, 29 per cent were in the regional group and 71 per cent in the distant. Baseline characteristics between the two groups were comparable except mean age (62 vs 61 years, P < 0.01) and Caucasian race (59 vs 73%, P < 0.01). Distant travel was associated with a significantly longer hospital length of stay (11.8 vs 10.5 days, P < 0.01) and lower risk of readmission (9.5 vs 13.4%, P < 0.01). Odds of readmission was inversely associated with logarithm of distance traveled (odds ratio 0.75). Travel distance in patients undergoing major cardiac surgeries was inversely associated with odds of readmission.

Abstract Then Coalition for Quality in Geriatric Surgery has completed both alpha and beta pilots which represent the initial efforts to launch this national quality initiative to improve the care of all older adults undergoing operations. The alpha phase included hospital stakeholders rating the feasibility of implementing standards related to evidence-based high quality surgical care for older adults on the topics of eliciting patient goals, completing a preoperative frailty risk assessment, educating healthcare professional about care specific to older adults, and implementing postoperative age-friendly care models. The beta pilot phase required 9 medical centers nationally (including an academic hospital, an urban medical center, a Kaiser hospital, a VA hospital and a rural hospital) to implement the evidenced based standards for high quality surgical care of older adults. Site verification visits were completed in the Summer 2018 which evaluated the effectiveness of each medical center’s ability to implement each of the standards.

The role of multinational enterprises (MNEs) for sustainable development, particularly in developing countries, has become a topic of debate among environmental, business, and economic researchers. In that context, we try to explore the compatibility and congruence of the business operations of MNEs vis-à-vis sustainable development in the developing part of the world. By using a multitheoretical framework followed with a multicase analysis concerning emerging and developing countries, we investigate how MNEs are addressing regional requirements of sustainable development where environment, local culture, and institutional mechanism are the key concerns. The adherence to evolving institutional mechanism over the years along with willful ethical steps taken by MNEs are found to be important in improving the state of affairs in the developing nations.

Purpose: The aim of this paper is to highlight the importance of Philosophical System of Education in the developing regions. Abstract “Twinning” refers to a constructive partnership between hospitals in developed and developing nations. Such an effort may contribute immensely to capacity building for the developing nation, but one of the reasons given for the lack of sustainability is cost. We share a detailed operating cost analysis of our recent experience with an institution in Vietnam. We were awarded a 1-year $54,000 grant from the Vietnam Education Foundation (VEF) to conduct a live tele-video conferencing course on the “Fundamentals of Clinical Surgery” with Thai Binh Medical University (TBMU). In-country lectures as well as an assessment of the needs at TBMU were performed. Total financial assistance and expenditures were tabulated to assess up-front infrastructure investment and annual cost required to sustain the program. The total amount of direct money ($66,686) and in-kind services ($70,276) was $136,962. The initial infrastructure cost was $41,085, which represented 62% of the direct money received. The annual cost to sustain the program was approximately $11,948. We concluded that the annual cost to maintain a “twinning” program was relatively low, and the efforts to sustain a “twinning” program were financially feasible and worthwhile endeavors. “Twinning” should be a critical part of the surgical humanitarian volunteerism effort.

In innovative industrial companies, collaboration techniques and laboratories have been in use for years. What historically started with teleconferencing instead of business trips has now reached the status of normal tools for interdisciplinary cooperation in these companies with advances in technologies such as PCs and data broadband as well as software such as Computer Aided Design (CAD), Virtual Reality (VR) and Augmented Reality (AR). But the collaboration techniques are often installed in extra rooms, e.g. video conferencing facilities and large VR laboratories. Other technologies such as chat programs can be used by any employee directly at his workplace. Without the use of these techniques, the short development times required today in many industrial sectors cannot be achieved and even remote maintenance of machines at the customer’s would be impossible for machine manufacturers. Within a research project, digital collaboration techniques are inspected and evaluated regarding their suitability for interdisciplinary cooperation in various applications. Collaboration techniques include special hardware for graphic simulations and head-mounted displays for 360∘ visualization of digital objects. Additionally different software for cooperation with graphics, video and conference programs distributed over several locations is tested, evaluated and partly newly developed. The application cases are processes from engineering and for planning of buildings. Based on the requirements determined during the tests, a new user-friendly mobile collaboration environment was developed. This collaboration environment integrates the different technologies of graphic simulation as well as already established methods such as video transmission or video conferencing and chat programs in a suitcase system.
