Effort-Reward Imbalance and Intention to Retire Early in Australian Healthcare Workers

Objectives – Retaining older workers in the Australian Healthcare System in rural areas is critical in preventing an imminent workforce crisis. Traditionally, healthcare workers have retired earlier than other groups. Using an internationally recognised model of job stress, the aim of this study was to examine the influence of Effort-Reward Imbalance (ERI) in predicting psychological strain and intention to retire in older Australian healthcare workers. Method – Utilising a cross-sectional design, data was obtained from 295 women and men aged over 55 years, working as professional healthcare workers in a rural Australian setting. ERI was measured using the 23-item scale developed by Siegrist (Siegrist, 2002); psychological strain was measured using the 12-item version of the General Health Questionnaire (GHQ) and a question on retirement intentions was developed. Results – The results confirm that those employees who experience high ERI and higher levels of overcommitment (OVC) experience psychological strain. Support for the three-way interaction was mixed: at low levels of OVC, higher levels of effort and increasing reward decreased strain, yet at high levels of OVC, increasing reward increased strain. This study could not confirm an association between intention to retire early and effort, reward, OVC or strain.

Background: The effort-reward imbalance (ERI) model is widely used in job stress research. However, few studies using this model have been conducted in developing countries. This study tested the extrinsic and intrinsic hypotheses regarding the burnout of healthcare workers in China with the ERI model.Method: Job stress was assessed by Siegrist’s ERI questionnaire, and burnout was evaluated by the Maslach Burnout Inventory-General Survey (MBI-GS). A total of 1,505 effective respondents were included in the final study. Multiple and hierarchical linear regression was used to analyze the association between components in the ERI model and burnout.Results: Emotional exhaustion and cynicism were positively correlated with ERI and overcommitment. Professional efficacy was positively related to ERI but not to overcommitment. ERI was the determining factor of emotional exhaustion and cynicism. Overcommitment moderated the relationship between ERI and emotional exhaustion and between ERI and cynicism.Conclusion: Changing workplace conditions and increasing personal resilience might alleviate burnout among hospital workers in China. The links between professional efficacy and stressful work environment need further exploration.

Minimum Standards Service is basic service quality regulation that is entitled to be obtained by every citizen. Elderly people must obtain 100% health services. Various factors can lead to low visits to the elderly posyandu in the Gading Puskesmas area in 2017. Objective: to find out the relationship between family support, knowledge, and elderly activities in the elderly posyandu in Gading Surabaya puskesmas. Method: The type of research is observational analytic with a cross-sectional design. Data processing and analysis is done by univariate and bivariate. Data collection techniques by conducting simple random sampling with a sample of 94 elderly people. Location and time of research from December 2018 at the posyandu in the Gading Surabaya health center area. Results: There is a significant value of 0,000 <0,05, so it can be concluded is have a relationship with family support, knowledge of elderly with activity following the elderly posyandu at Gading Surabaya health center

AbstractObjectiveFollowing adverse work conditions, health consequences can be explained by an imbalance between the effort made and the reward received. We investigated the association between extra effort, perceived reward, and post-traumatic stress disorder (PTSD). The Effort-Reward Imbalance Model was used to examine whether extra effort at work in the aftermath of a workplace-related terrorist attack affected the risk of PTSD and the effects of reward for extra effort from a leader or colleagues.MethodsCross-sectional data were collected 10 months after a terrorist attack in Norway in 2011. Out of 3520 Ministry employees invited, 1927 agreed to participate. Employees reported any extra effort performed as a result of the bomb explosion and any reward received from a leader or colleagues. PTSD was assessed with the PTSD Checklist.ResultsEmployees who reported extra effort displayed increased risk for PTSD (odds ratio [OR]=1.71, 95% confidence interval [CI]: 1.15-2.55, P=0.008). Perceived reward for extra effort from a leader was associated with lower risk for PTSD (OR=0.39, 95% CI: 0.23-0.64, P<0.001) but not perceived reward from colleagues.ConclusionsExtra effort may increase the risk of PTSD, but reward from a leader may mitigate this effect. The Effort-Reward Imbalance Model appears to be an appropriate approach that may contribute to understanding of the etiology of work-related PTSD. Background: The aging population is increasing rapidly; therefore, paying attention to the needs of older people is necessary. Sexual satisfaction, which is needed to feel happy, is often neglected in older women. Objective: This study aims to evaluate the relationship between sexual satisfaction and mental health in older women. Materials and Methods: In this analytical study with a cross-sectional design, 195 married women aged ≥60 years covered by urban and rural health centers of Guilan, Iran were recruited using a cluster sampling method. A demographic form, the 28-item General Health Questionnaire (GHQ-28) and a Sexual Satisfaction Questionnaire were completed by participants and collected data were analyzed in SPSS v. 22 software using descriptive statistics (frequency, mean, standard deviation, median), and Mann-Whitney U, Kruskal-Wallis and Spearman correlation tests. Results: There was a significant relationship between sexual satisfaction and mental health in older women (P<0.0001). 51.2%, 27.7% and 21.08% of participants had low, moderate and high sexual satisfaction, respectively. Sexual satisfaction was directly related to monthly income (P<0.0001) and women’s and their husbands’ education (P<0.0001 and 0.006) while it was inversely related to physical illness (P=0.030), medication use (P=0.009), age (P<0.0001), number of children (P=0.022), and duration of marriage (P<0.0001). The median score of mental health in participants was 38 (ranging 24-53) and the percentage of participants with mental problems (79.4%) was higher than those without mental problems (20.6%). Conclusion: Mental health and sexual satisfaction in older women are directly related to each other; therefore, providing effective and appropriate sexual care programs is recommended for them.

ABSTRACT Objective: to analyze the combined use of models for the evaluation of work-related psychosocial aspects and their association with the prevalence of Minor Psychics Disorders among nursing workers. Method: cross-sectional study with a sample of 285 nursing workers. Data collection was performed through the application of a structured sociodemographic and occupational questionnaire and the Demand-Control-Support, Effort-Reward Imbalance and Self-Reporting Questionnaire. Descriptive analysis and a multiple logistic regression were performed. Results: the prevalence of suspicion of minor psychics disorders was 32.6%. The dimensions of both models were associated with mental health. The full Effort-Reward Imbalance and Demand-Control and Social Support models predict Minor Psychics Disorders to a greater extent than the combined use of partial models. Conclusion: it was found that the Effort-Reward Imbalance model captured better the magnitude of the Minor Psychics Disorders in this sample of workers compared to the Demand-Control and Social Support model. However, the concomitant use of the theoretical models revealed unique contributions in the evaluation of Minor Psychics Disorders. Considering the complexity of mental illnesses, it is important that different factors be evaluated.

Peptic ulcer is an upper gastrointestinal tract disease caused by hypersecretion of acids and pepsin of gastric mucosa. Peptic ulcer disease can be caused by several factors such as smoking, fast food, alcoholic beverages, NSAIDs and Helycobacter pylori. The purpose of this research was to obtain the distribution of antiulcer and itsrationality on peptic ulcer inpatients at the RSUD Sultan Syarif Mohamad Alkadrie Pontianak. This study is a descriptive observational study with cross sectional design. Data from medical record collected retrospectively.  This research using medical record peptic ulcer inpatients from January to December in 2017. The samples used were 34 of 44 patients who met  the  inclusion criteria. Based on results, the distribution of anti ulcer were omeprazole (2.94%), pantoprazole (73.53%), lansoprazole (26.47%), ranitidine 5.89%, 58.82% antacids and 85.29% sucralfat. The result of rationality evaluation on criteriaof appropriate indication (100%);appropriate drug (55.88%); appropriate patient (97.06%); and appropriate (61.76%). Overall,the rationality of treatment that meets all four rational treatment criterias was 78.68%.

Abstract Background Safeguarding the psychological well-being of healthcare workers (HCWs) is crucial to ensuring sustainability and quality of healthcare services. During the COVID-19 pandemic, HCWs may be subject to excessive mental stress. We assessed the risk perception and immediate psychological state of HCWs early in the pandemic in referral hospitals involved in the management of COVID-19 patients in Uganda. Methods We conducted a cross-sectional survey in five referral hospitals from April 20–May 22, 2020. During this time, we distributed paper-based, self-administered questionnaires to all consenting HCWs on day shifts. The questionnaire included questions on socio-demographics, occupational behaviors, potential perceived risks, and psychological distress. We assessed risk perception towards COVID-19 using 27 concern statements with a four-point Likert scale. We defined psychological distress as a total score > 12 from the 12-item Goldberg’s General Health Questionnaire (GHQ-12). We used modified Poisson regression to identify factors associated with psychological distress. Results Among 335 HCWs who received questionnaires, 328 (98%) responded. Respondents’ mean age was 36 (range 18–59) years; 172 (52%) were male. The median duration of professional experience was eight (range 1–35) years; 208 (63%) worked more than 40 h per week; 116 (35%) were nurses, 52 (14%) doctors, 30 (9%) clinical officers, and 86 (26%) support staff. One hundred and forty-four (44%) had a GHQ-12 score > 12. The most common concerns reported included fear of infection at the workplace (81%), stigma from colleagues (79%), lack of workplace support (63%), and inadequate availability of personal protective equipment (PPE) (56%). In multivariable analysis, moderate (adjusted prevalence ratio, [aPR] = 2.2, 95% confidence interval [CI] 1.2–4.0) and high (aPR = 3.8, 95% CI 2.0–7.0) risk perception towards COVID-19 (compared with low-risk perception) were associated with psychological distress. Conclusions Forty-four percent of HCWs surveyed in hospitals treating COVID-19 patients during the early COVID-19 epidemic in Uganda reported psychological distress related to fear of infection, stigma, and inadequate PPE. Higher perceived personal risk towards COVID-19 was associated with increased psychological distress. To optimize patient care during the pandemic and future outbreaks, workplace management may consider identifying and addressing HCW concerns, ensuring sufficient PPE and training, and reducing infection-associated stigma.
