We read with interest the review article by Paul V Knight and Alan McKenzie on diabetes in the elderly. They highlight the fact that close examination of the feet or fundoscopy is often not performed. We have audited the care of elderly diabetic patients on our acute wards using the BGS guidline for diabetic care.

Objective: This study analyzes the causes of death, survival, and other related factors in hospitalized elderly people with fractures over the course of one year. Methods: We followed 376 fracture patients for one year in a prospective cohort study to a reference hospital in central Brazil. The Cox regression model was used to analyze factors associated with survival. Results: The results indicate that the one-year mortality rate was high (22.9%). The independent factors linked to lower overall survival were as follows: patients aged ≥80 years with previous intensive care unit (ICU) admission and presence of comorbidities (diabetes mellitus [DM] and dementia). Conclusion: Our study results may contribute to a better understanding of the impact of fractures on the elderly population and reinforce the need to oversee age-groups, diabetic patients, and patients with complications during hospitalization.

AbstractElderly people and patients with comorbidities are at higher risk of COVID-19 infection, resulting in severe complications and high mortality. However, the underlying mechanisms are unclear. In this study, we investigate whether miRNAs in serum exosomes can exert antiviral functions and affect the response to COVID-19 in the elderly and people with diabetes. First, we identified four miRNAs (miR-7-5p, miR-24-3p, miR-145-5p and miR-223-3p) through high-throughput sequencing and quantitative real-time PCR analysis, that are remarkably decreased in the elderly and diabetic groups. We further demonstrated that these miRNAs, either in the exosome or in the free form, can directly inhibit S protein expression and SARS-CoV-2 replication. Serum exosomes from young people can inhibit SARS-CoV-2 replication and S protein expression, while the inhibitory effect is markedly decreased in the elderly and diabetic patients. Moreover, three out of the four circulating miRNAs are significantly increased in the serum of healthy volunteers after 8-weeks’ continuous physical exercise. Serum exosomes isolated from these volunteers also showed stronger inhibitory effects on S protein expression and SARS-CoV-2 replication. Our study demonstrates for the first time that circulating exosomal miRNAs can directly inhibit SARS-CoV-2 replication and may provide a possible explanation for the difference in response to COVID-19 between young people and the elderly or people with comorbidities.

The review article reflects the specific features of involutive cognitive functions in elderly people. Furthermore the basis of these changes is the natural physiological process of morphofunctional remodeling of the human body in general and of the central nervous system in particular. As a result, the elderly and senium people have cognitive decline, and in the presence of provoking medical and social factors and comorbid diseases they may have transient cognitive dysfunction.

Nurses play an essential role in the provision of health services and health promotion for the elderly people. This study aims to find out the attitude of nurses towards care of elderly people in Tribhuvan University affiliated teaching hospitals of Kathmandu valley, Nepal. A descriptive cross-sectional study was conducted among 236 nurses working in TU affiliated teaching hospitals in Kathmandu. Data was collected proportionately from four hospitals using semi structured questionnaire and “Multifactor Attitudes Questionnaires (MAQ)” for assessing attitude of nurses towards care of elderly.  Data was entered in EpiData 3.1 and analyzed using SPSS version 16. Descriptive as well as inferential statistics namely Kruskal-Wallis and Man-Whitney test were used for analyzing data. The overall response rate was 98% (236 out of 240 nurses). The mean age of respondents was 28.74 ±7.43 years.  The overall prevalence of attitude below than median value of the scale was 2.5% and more than half (51.3%) of respondents had knowledge below median value regarding care of elderly. Attitude towards elderly was found to be associated with age, working hour and qualification of the respondent. So these factors should be considered in nursing education and to improve attitude towards care of elderly people. Keywords: Attitude, Nurse, Care, Elderly, Kathmandu

Introduction: Fatigue is one of the most common causes of disability in most elderly people affected with diabetes. Considering the growing population of this group, the present study aimed to determine the effect of warm foot bath on the fatigue of the elderly people with diabetes. Methods: The randomized clinical trial study was carried out on 66 elderly diabetic patients who had referred to the Diabetes Research Center in Yazd. Eligible participants were selected by convenience sampling and randomly divided into two experimental and control groups. Participants of the experimental group performed a warm foot bathing for 20 minutes during two weeks. The Piper Fatigue Scale was completed in three steps (at the beginning of the study, two and four weeks after intervention). Data were analyzed by SPSS software, using ANOVA for repeated measures, independent t-test, Chi-Square and Fisher exact test. Results: The mean ± standard deviation of fatigue in the three phases of evaluation in the experimental group was 5.91 ± 0.81, 4.80 ± 1.19, 4.55 ± 1.57 and in the control group was 5.95 ± 0.97, 6.06 ± 1.13 and 5.79 ± 1.33, respectively. The difference in the mean of fatigue scores in both groups after the intervention was significant (p < 0.001). Conclusion: Revealing the positive effects of warm foot bathing, this method, as a non-pharmacological and easy-to-use method, is recommended to improve fatigue condition in the elderly patients with diabetes.

The number of nurses entering the nursing profession is decreasing, but demands placed upon the Health Services are increasing primarily due to the increasing number of people in our society. People are living longer and it is apparent that some extra provision must be made to care effectively for the elderly members of our society. Various studies have shown that care of the elderly is an unpopular clinical field for nurses. (Shape, 1986. Notter, 1992. Vickels 1982).

ABSTRACT Objective: to characterize the seropositive elderly for the Human Immunodeficiency Virus and Acquired Immunodeficiency Syndrome (HIV/AIDS) in their socio-demographic aspects; to understand how the elderly take care of themselves from the diagnosis of HIV/AIDS. Method: Qualitative, descriptive, exploratory research conducted at a Voluntary Counseling and Testing Center with 10 elderly people receiving treatment for HIV/AIDS. The data were analyzed according to the content analysis. Results: Data show the elderly people’s lack of knowledge about HIV/AIDS transmission, the experience of being elderly and having HIV/AIDS, caring for oneself and life after diagnosis of HIV/AIDS in their daily lives. Final considerations: The diagnosis of HIV/AIDS seropositivity in the elderly generates a blend of feelings and fears that lead to food changes, adherence to treatment and the renunciation of daily and social habits, manifested as ways of self-care.

The review article describes the characteristics of curable involutive cognitive impairment in the elderly. It is noted that mild cognitive impairment is predominantly neurodynamic in nature, but over time it can transform into a syndrome of moderate cognitive impairment, which, sometimes, is a precursor of dementia. Special attention is given to the clinical manifestations of mild and moderate cognitive impairment, diagnostic criteria, the course of the disease, and its medical and social impact.

Introduction: Global ageing is the success story of the 21st century as a result of declining fertility, mortality and improved public health interventions. With the increased elderly population, the attitude of societies about ageing changes. Positive attitudes towards the elderly people are valued, respected and experienced but most of the literature shows negative perceptions towards elderly people among the youth, especially in the form of stereotypical behaviors and ageism which can result in social exclusion, isolation and ultimately the abuse of elderly people and disempowerment of elderly people. Methods: Cross sectional research design was used to assess the attitude of youth towards the elderly people in the selected community of Lalitpur, Nepal from August 12 to September 7, 2018. Total 380 youth were interviewed by using modified Kogan’s Old People Scale and structured questions. Systematic random sampling technique was used for data collection. Results: More than half of the youth (66.1%) had positive attitude level towards the elderly. With regards to the attitude towards the care of elderly almost of all of the youth (97.9%) had positive attitude towards the care of the elderly. There was significant association between gender and attitude towards elderly (p=0.04) and occupation and attitude towards the care of the elderly (p=0.02) at 95% of significance level. Conclusion: Although more than half of the youth have positive attitude toward the elderly people still there is need to provide awareness program regarding the physical, physiological and psychological changes that occurs during the ageing period which helps in enhancing the positive attitude of the youth and thus contributing towards overall quality of life of the elderly people.
