Development and Implementation of a Novel Prehospital Care System in the State of Kerala, India

AbstractMost low- and middle-income countries (LMICs) have struggled to find a system for prehospital care that can provide adequate patient care and geographical coverage while maintaining a feasible price tag. The emergency medical systems of the Western world are not necessarily relevant in developing economic systems, given the lack of strict legislation, the scarcity of resources, and the limited number of trained personnel. Meanwhile, most efforts to provide prehospital care in India have taken the form of adapting Western models to the Indian context with limited success. Described here is a novel approach to prehospital care designed for and implemented in the State of Kerala, India. The Active Network Group of Emergency Life Savers (ANGELS) was launched in 2011 in Calicut City, the third largest city in the Indian State of Kerala. The ANGELS integrated an existing fleet of private and state-owned ambulances into a single network utilizing Global Positioning System (GPS) technology and a single statewide call number. A total of 85 volunteer emergency medical certified technicians (EMCTs) were trained in basic first aid and trauma care principles. Public awareness campaigns accompanied all activities to raise awareness amongst community members. Funding was provided via public-private partnership, aimed to minimize costs to patients for service utilization. Over a two-year period from March 2011 to April 2013, 8,336 calls were recorded, of which 54.8% (4,569) were converted into actual ambulance run sheets. The majority of calls were for medical emergencies and most patients were transported to Medical College Hospital in Calicut. This unique public-private partnership has been responsive to the needs of the population while sustaining low operational costs. Introduction. In Ukraine, in conditions of insufficient funding and too slow reforms, an inefficient health care system (HE) has developed, which is not able to fully meet the requirements of the time and overcome such negative factors as low logistics, low staff costs, underdeveloped system health insurance, etc. Goal. The purpose of the article is to specify the concept of PPP in relation to the specifics of the health sector, as well as to clarify the definition of PPP in the field of health. Method (methodology). In the process of research the methods of scientific cognition were used, in particular monographic, bibliosemantic, historical, dialectical, logical generalization, system analysis. Results. The inefficiency of public administration and financing of the health care system has been stated, which creates obstacles to improving the quality of health care. Foreign experience in attracting private investment in the industry, which will create the foundations for improving the level and quality of health care, maintaining social stability, improving citizens' access to health services. It is established that this will also have a positive effect on optimizing public spending and accelerating the reform of the industry as a whole. A new definition of the term "public-private partnership" in the field of health care has been proposed, which can be used for further scientific and practical developments. The necessity of reforming the industry with the use of alternative ways of attracting investments, namely the mechanisms of public-private partnership, is substantiated. It was found that a promising model of PPP in the field of health care can be the implementation of a private partner reconstruction of a state (municipal) medical institution, as well as obtaining the right to operate and maintain the infrastructure of such an institution during the payback period. It was found that the interaction of the state and private business in the field of health care can also be manifested in the participation of private institutions in providing free medical care to the population. Possible options for cooperation between the state and business within the PPP in the field of health care in Ukraine are considered. Priority is given to the development of health infrastructure based on attracting private investment and long-term interaction between the state and business.

AbstractLimited information is available regarding the functioning of DOTS (Directly Observed Treatment Short course) center for tuberculosis treatment under public private partnership with a medical college hospital. The present study was done to assess the various parameters related to the effectiveness of follow up treatment and its outcome in a DOTS center attached to a medical college hospital in coastal South India.Reports of patients with tuberculosis diagnosed and treated under Revised National Tuberculosis Control Programme at KS Hegde Medical College hospital DOTS center under Mangalore TB unit were reviewed retrospectively for the period from July 2004 to June 2011. Data was described in proportion and chi square test was applied to assess the association of cases with age group and sex. Among 60 patients serially registered under DOTS, 41 (68.33 %) cases were females and 19 (31.67%) cases were males. Maximum number of cases belonged to 15-29 years age group (20, 33.3%) and the difference observed between the categories of TB and age group was found to be significant (P<0.001). Overall 23 (38.33%), 12 (20%) and 25 (41.67%) cases received Cat-I, Cat-II and Cat-III treatment respectively. Success rate for new smear positive cases was 92.85% (13/14) and for Re-treatment smear positive cases was 88.89% (8/9). Sputum conversion rate among new smear positive cases (n= 14) at 2 months and 3 months was 85.71% and 92.86% respectively. Sputum conversion rate for Re-treatment cases (n=9) at 2 months and 3 months was 55.56% and 88.89% respectively. RNTCP is running successfully in this DOTS center with very low treatment failure rate and better outcome indicators, highlighting the importance of public private partnership of poorly functioning DOTS centers with medical colleges.

Modern economic conditions and the integration of the national economy into the global economy require structural changes and a search for a new model of relationship between the public and private sectors. The relevance of this article is determined by the fact that the existing scientific concepts have not formed a general understanding of the theoretical aspects for the implementation of public-private partnership at the railway transport, have not established development priorities and tasks of the state in the implementation of the mechanism of public-private partnership at the railway transport. Realization of the projects capable of maintaining implementation of the development priorities of the railway industry requires significant investments of private business and financial, organizational and legal support of the state. The authors substantiate the necessity of using public-private partnership at the railway transport of Ukraine for solving the most urgent current problem – renewal of the material and technical resources and bringing them in line with the requirements of the leading European countries. The proposed priority-partnership approach and the obtained results open up new opportunities for further research into the solution of the problem regarding the implementation of public-private partnership at the railway transport.

The article is devoted to the development of such a form of state property management as a public-private partnership. Public-private partnership is one of the most widespread format of interaction between the state and business. The article analyzes the legal foundations and trends in the development of this form of interaction. Modern methods for assessing risks, benefits and effects for different stakeholders of projects implemented in the form of PPPs are considered. The authors propose their approach to building a financial model and assessing the effects of PPP projects. Taking into account risks, the fair price of participation in the PPP project for both parties is estimated. The proposed financial model can be used to calculate the concession fee in tariff-regulated projects where the state is supposed to participate, usually with the subsequent transfer of the ownership for the created object to the state. The results of the work can be used by departmental and regional centers of public-private partnerships, by specialists of private investor companies and tariff regulation bodies when formulating the financial and economic justification for public-private partnership projects, justifying its preference in comparison with other forms of state budget investments.

Many countries are moving to a new technological structure, caused by the 4th industrial and technological revolution. This transition is accompanied by the transformation of the previously existing territorial organization of the economy, infrastructure and resettlement, which relates to issues of urban development policy. In this case, coordination of the interests of the state and business is required. An effective tool in this area is public-private partnership (PPP). The main goal is the development of tools for the implementation of innovative urban development policy in the technological and social transformation of the economy. The methodological basis is the methodologies, approaches in the works of foreign and Russian scientists in the field of PPP research in the implementation of innovative urban policy. The main results of the study are following: 1.The modern task of urban development policy is the formation of a comfortable environment that will ensure a high standard of living for the population. 2. For the implementation of this task, the “adaptive territory” approach is suggested. 3. The modern tool for this approach is PPP. The main conclusions and recommendations can be used to justify the mechanisms for implementing innovative urban development policy in order to harmonize the interests of the state and business in the implementation of the “adaptive territory” approach, which meets the modern development paths of the global economy.
