TRAVELLERS: A School-Based Early Intervention Programme Helping Young People Manage and Process Change, Loss and Transition. Pilot Phase Findings

Objective: This paper outlines the conceptual background and findings from the pilot phase of TRAVELLERS – an early intervention programme designed to enhance protective factors for young people experiencing change, loss and transition events and early signs of emotional distress. The pilot study aimed to determine whether TRAVELLERS was a feasible, acceptable and promising intervention for young people within secondary schools in Aotearoa/New Zealand. Method: The conceptual origins of the TRAVELLERS programme are described in terms of: adolescent mental health concerns; emerging mental health promotion theory and practice; and prevention and early intervention models. The key elements of the TRAVELLERS programme are described. The programme was piloted in two secondary schools, one rural and one urban with 34 participants (females n = 24, males n = 10). Evaluation methods included: review of programme materials; identification of potential selection tools appropriate to Year 9 students; analysis of selection questionnaire; and conduct of feedback from participants, facilitators and parents/caregivers. Results: The TRAVELLERS programme provides a means of identifying and selecting young people who may benefit from participating in an early intervention programme. The programme has achieved a statistically significant reduction in participants’ distress (p < 0.01). Young people were overwhelmingly enthusiastic about most aspects of TRAVELLERS. School personnel reported that TRAVELLERS was an appropriate and acceptable programme to the school. Conclusions: Targeted interventions provided within a supportive school environment can contribute to enhancing protective factors such as personal and interpersonal coping strategies, increased help-seeking behaviour, and young people feeling more positive about themselves and their lives. The pilot programme has been amended and prepared for a two year trial phase in 10 secondary schools during 2002–2003.

ObjectivesJigsaw is an early intervention mental health service developed by Headstrong which provides support to young people, aged 12–25 years, in 10 communities across Ireland. This study aimed to profile young people who availed of Jigsaw, in one calendar year, and to provide evidence that Jigsaw’s model facilitates the reduction of psychological distress.MethodsParticipants were 2420 young people who received support, directly or indirectly, from Jigsaw. Demographic details, including age, gender, presenting issues and referral pathways, captured on the Jigsaw Data System were described and psychological distress was assessed using the Clinical Outcomes in Routine Evaluation (CORE) questionnaires.ResultsA gender balance was almost observed and the majority of participants were between 15 and 17 years old. The most common presenting issue was anxiety and the most common referral sources were self, parent, general practitioner, school and Adult Mental Health Services (AMHS). Participants reported high levels of psychological distress pre-intervention and levels were significantly lower post intervention.ConclusionsAlthough a lack of control group limits interpretation of the study findings, this study provides emerging evidence that Jigsaw is an accessible and effective service which plays a key role in the continuum of mental health care for young people in Ireland.

Objective: The objective of this paper is to outline the conceptual background and strategy of intervention for a systematic and sustainable approach to mental health promotion in secondary schools. Method: The conceptual origins of the Gatehouse Project are described in terms of the epidemiology of adolescent mental health problems, attachment theory, education reform research and health promotional theory and practice. The elements of health promotional work are described in terms of structural change and priority setting; implementation at multiple levels within the participating schools is described. Results: The conceptual framework of the Gatehouse Project emphasises healthy attachments with peers and teachers through the promotion of a sense of security and trust, effective communication and a sense of positive self-regard based on participation in varied aspects of school and community life. A school social climate profile is derived from a questionnaire survey of students. An adolescent health team uses this information to set priorities for change within the school. Interventions may focus on the promotion of a positive social climate of the whole school or in the classroom. Curriculum-based health education is also used and based on materials that are relevant to the normal developmental experiences of teenagers. These are integrated into the mainstream curriculum and incorporate a strong component of teacher professional development. Lastly, the intervention promotes linkage between the school and broader community with a particular emphasis on the needs of young people at high risk of school drop-out. Conclusions: Educational environments are complex systems undergoing continuous and simultaneous changes. The Gatehouse Project will provide unique information on the relationship between the social environment and the emotional wellbeing of young people. More importantly it outlines a sustainable process for building the capacity of schools to promote the social and emotional development of young people.

School belonging is generally regarded as a student's sense of affiliation or connection to his or her school. Anyone who has personally navigated the sometimes torturous terrain of secondary school is able to have some level of direct understanding of the importance that belonging, fitting in, and identifying with a school holds for most people. Educators and practitioners often work with young people who feel that they do not belong to the school community, in which they attend. An absence of belonging can manifest itself in mental health concerns, school attrition, and risk taking behaviours. Opportunities for early intervention through fostering school belonging are born from a greater understanding and awareness of what school belonging is and how it is contextualised and fostered. This special issue aims to place a focus on school belonging and highlight it as a significant social issue of our time.

Research reports high levels of mental health problems faced by young people in the UK. Schools provide a range of mental health support services, although these are often not robustly evaluated. This paper aims to explore the mental health provision of secondary schools across two large regions in the North of England and provide comparisons to the mental health questionnaire scores of their pupils. Results are part of a wider study providing an overview of the mental health of secondary school pupils. Measures include the Strengths and Difficulties Questionnaire, distributed to year 8, 9, and 11 pupils attending secondary schools and a bespoke mental health service provision questionnaire for school staff at the same schools. A total of 6328 pupil questionnaires and 36 staff questionnaires were returned from 21 schools. Results showed a non-significant correlation between provision and young people’s mental health scores and highlight a range of factors to take into consideration. There is a need to improve the evaluation and recording of school-based mental health provision. Mental health difficulties in young people are prevalent in schools. Future research is needed to elucidate which types of services are most helpful in preventing, supporting, and signposting those with mental health problems.

Estimates indicate the lifespan of individuals with psychotic illness is reduced by approximately 15-20 years. Consequently there is a need to address the physical health of those who live with a mental illness, like psychosis. The Bondi Centre provides an integrated model of care to young people with a first episode of psychosis. The Keeping the Body In Mind program focuses on prevention and early intervention of physical health issues and is offered alongside treatment for mental health and social issues as part of routine care. We used body mapping, an arts-based research method, to explore the complexity of this physical health intervention. Our aim was to develop an in-depth understanding of experiences of young clients of the early intervention centre, with a particular focus on the embodied relationship between physical and mental health. Six young people engaged in creating life-sized body maps depicting their experience of the physical intervention program over four 3-hour sessions, followed by an in-depth interview. Analysis of our body maps drew on thematic analysis and narrative inquiry. The narrative trope was one of recovery, highlighting the importance of the link between body and mind, individual and community, and the balance between light and darkness. There was an emphasis on developing feelings of connectedness (to self and others), hope and optimism for the future, a sense of having an identity, and a sense of meaning and empowerment. Recovery was conceptualised as an ongoing process rather than an end product or fixed state. Involvement in the body mapping process was consistently identified as therapeutic, offering an opportunity for reflection on the journey to recovery with a focus on past, present and imagined storylines of the future.

Abstract The Office of the Commissioner for Mental Health in Malta was established in 2011 to promote and protect rights of persons with mental disorders and their carers. This advocacy role includes monitoring of involuntary care, regular reporting on quality of care and care environments, in-depth analysis and recommendations on emerging issues such as mental health literacy, multidisciplinary care plans, drug addiction services and stigma and regular networking across ministries, agencies, departments, and NGOs, breaking silos and building bridges. Data for 2018 shows that acutely ill young people (10-29 year olds) were 30% of acute involuntary admissions. Males and foreign nationals from medium and least developed countries were more frequently represented. Substance abuse, mood disorders and psychotic disorders were the more common diagnostic groups. Building resilience and providing opportunities for early intervention are key elements of better mental health and well-being in the younger generation. Six examples of good practice in adolescent and youth mental health from Malta will be presented: Youth.inc by A&Auml;¡enzija Å»għażagħ; Kellimni.com by SOS Malta; Youth Mental Health First Aid by Richmond Foundation; Research and Professional Education by ACAMH (Malta); Student Support Services at MCAST MALTA; Project Enlight! by Enlight Foundation. Two of these initiatives were recognised as best practices at European level in a peer learning exercise conducted by the Dutch Youth Institute. The recommendations are: more focused approaches towards young people with acute mental disorders with special attention to their specific needs; the identification of young people in trouble; work programmes that build resilience, life-skills and employment prospects; the intensified use of refined electronic and social media tools for promotion, prevention and early intervention; and active support and encouragement of peer group development and self-help initiatives. AbstractResearch indicates that individuals who have high levels of mental health literacy engage in help-seeking for mental health issues more in comparison to those with low mental health literacy. It is therefore important to ensure that young people are able to identify signs of psychological distress and identify appropriate supports and resources when necessary. Preliminary research has identified gender differences in help-seeking and identification of mental health problems. This study aimed to investigate the relationship between mental health literacy, help-seeking intentions and wellbeing in senior high school students. The sample consisted of 22 females and 10 male students, ranging from 16–18 years. Findings indicated a positive correlation between wellbeing and general help-seeking, as well as wellbeing and help-seeking for suicidal ideation. Gender differences were identified for aspects of mental health literacy and help-seeking intentions. Results of this research have implications for practitioners in fostering positive outcomes and developing targeted interventions towards improving mental health literacy and help-seeking behaviour in the future.

Although borderline personality disorder (BPD) usually has its onset in young people, its diagnosis is often delayed, and specific treatment is usually only offered late in the course of the disorder. Over the past two decades, the explosion of knowledge about personality disorder has shown that BPD is neither a variant of normal adolescent development, nor a ‘passing phase’ of little consequence. In fact, BPD is associated with harmful personal, social, and economic consequences. This has provided solid ground for early diagnosis and treatment of both subthreshold borderline personality pathology (‘indicated prevention’) and full-syndrome disorder (‘early intervention’). Over the past two decades, the Helping Young People Early (HYPE) programme has been developed and researched in Melbourne, Australia. HYPE is a comprehensive and integrated indicated prevention and early intervention programme for youth (15–25 years of age). HYPE is comprised of a service model and an individual therapy, and incorporates the principles of cognitive analytic therapy. HYPE primarily aims to alter the life-course trajectory of young people with borderline personality pathology by improving functional outcomes, interpersonal relationships, and self-management, developing support networks independent of mental health services, promoting adaptive help-seeking, and avoiding or reducing iatrogenic harms.
