Evidence-Based Homoeopathic Treatment of Acute Herpes Zoster (Shingles): A Case Report

AbstractHerpes zoster (HZ), also known as shingles, occurs due to reactivation of varicella zoster. It is manifested by unilateral, painful, vesicular rashes following a dermatome. Homoeopathy is well known for its tremendous results in chronic cases. This article presents the evidence-based significant result of single homoeopathic remedy in an acute case of HZ within a short time period without any post-herpetic neuralgia which usually follows the HZ for months. Three to four doses of Hypericum 30 CH provided significant relief within a day and complete recovery within 10 days. Modified Naranjo score is 9, indicating definite causal attribution. This case proves that by selecting appropriate similimum, promising and positive results are achieved by homoeopathy, be it acute or chronic cases in a safe and gentle way. No such existing homoeopathic literature provides any characteristic information particularly related to HZ in Hypericum. So, future research and clinical trials could be beneficial for better validation of Hypericum as a therapeutic for HZ.

Introduction. Chickenpox is a common pediatric disease, while herpes zoster (HZ) is rare among children, especially among infants. HZ in infancy may appear after intrauterine or postnatal infection with varicella-zoster virus (VZV). We report on a case of HZ in an immunocompetent infant who had a history of chickenpox in early infancy. Case outline. A seven-month-old male infant was presented with skin changes in the left T1 and T2 dermatomes. Skin changes appeared eight days after the infant had a mild left-arm traction injury. The patient?s medical history revealed that he had a mild form of chickenpox at the age of three and a half months. After the clinical diagnosis of HZ was made, he was treated with oral acyclovir 20 mg/kg every six hours for five days and had complete recovery without any sequelae. Conclusion. Risk factors for pediatric HZ are immunosuppression and chickenpox during the first year of life. Local trauma is a reported risk for VZV reactivation among adults. To our best knowledge, our case is the first reported pediatric case in which the injury of the left arm precedes HZ appearance. Routine vaccination against chickenpox may be an important preventive measure because herd immunity will protect infants and immunocompromised children from getting chickenpox and thus HZ.

Herpes zoster infection is a painful worldwide disease. The inappropriate and delayed treatment causes prolongation of the disease with debilitating symptoms and postherpetic neuralgia. Herpes zoster is commonly known as shingle's caused by varicella zoster virus (VZV). Current studies have shown that exosomes play a significant role in pathogenesis as well as the spread of herpes. Combination therapy has shown pain reduction, reduction in burning, and arrest progression of the disease. This technique is cost effective and easy to apply. In Ayurveda herpes can be correlated with Pittaj visarpa. Principle of treatment is Raktmokshan (Bloodletting) and Pradeha (Local application of medicines). A 50-year-old male presented in OPD of Kayachikitsa at Datta Meghe Ayurved Medical College Hospital and Research Centre. We treated his Herpes zoster on the basic principles of management of Vata-Pittaj Visarpa. A rapid decrease in pain severity was observed after 7 days of Combination Therapy according to Zoster Specific Brief Pain Inventory (ZBPI) Questionnaire. Lesions of Herpes Zoster were found completely crusted and healed within 2 weeks. Repeated application of Ayurvedic Formulation Lepa along with internal medications gave complete relief. Adaptive trials are necessary to discover further practical applicability of Ayurveda and modern treatment together. Future research on the effect of combination therapy on exosomes is necessary.

Abstract Purpose of Review To assess the pleiotropic effects of ketogenic diets (KD) on glucose control, changes in medication, and weight loss in individuals with type 2 diabetes, and to evaluate its practical feasibility Recent Findings KD results in improved HbA1c already after 3 weeks, and the effect seems to persist for at least 1 year. This is associated with a reduction in glucose-lowering medications. The weight loss observed after a short time period seems to be maintained with a long-term diet. Adequate support (supportive psychological counseling, enhancing positive affectivity, reinforcing mindful eating) is necessary to achieve a benefit and to assure adherence. Summary Despite the documented decrease in HbA1, a definitive causal effect of KD remains to be proven. KD should be performed under strict medical supervision. Future research should clarify how compliance can be maximized and how ketosis can be optimally monitored.

Herpes zoster is a common infection caused by reactivation of the varicella zoster virus. In most patients the disease runs a painful but benign course with complete recovery. A unique case of trigeminal ophthalmic herpes zoster presented to The Toronto Hospital, Toronto, Ontario. This patient had a unilateral full thickness defect of scalp and skull with secondary osteomyelitis and opacification of the eye. After appropriate medical therapy and surgical debridement, reconstruction was undertaken using a latissimus dorsi free flap and split thickness skin grafts. This achieved a satisfactory reconstruction with obliteration of the diseased tissue. On follow-up several months later, there was erosion through the central area of the skin graft. This appears to be secondary to trauma caused by the patient frequently scratching the area, and that the original defect likely had a factitious component of the same etiology.

Due to the world-wide aging population, there is a need for specialist neurological knowledge, treatment and care. Stroke treatment is effective in reducing mortality and disability, but it is still not available in many areas of the world. We describe the set-up process of a specialized Neuroscience, Stroke and Rehabilitation Centre in Brunei Darussalam (BNSRC) in cooperation with a German hospital. This study details the setup of a stroke-, neurological intensive care- and neurorehabilitation unit, laboratories and a telemedical network to perform all evidence-based stroke treatments. All neurological on-site services and the telemedical network were successfully established within a short time. After setup, 1386 inpatients and 1803 outpatients with stroke and stroke mimics were treated. All evidence-based stroke treatments including thrombolysis and hemicraniectomy could be performed. It is possible to establish evidence-based modern stroke treatment within a short time period by a transcontinental on-site and telemedical cooperation.

This study aims to determine the implementation of the Intensive Course (IC) program in Islamic boarding schools to identify the challenges, strengths, and weaknesses faced in its execution. The method in this research uses qualitative research exploration. Three data collection techniques were used: in-depth interviews through a semi-structured list of questions developed by related literature, observation, and documentation. The results of this study describe the implementation of the IC program during the pandemic. The challenges in its implementation consist of students, teaching materials, and a relatively short time, the advantages and disadvantages of the learning program. This study only describes one Islamic boarding school in Kediri, namely the Syarif Hidayatullah Islamic Boarding School. These results are helpful as an overview of IC during a pandemic and a reference for future research. This study provides evidence-based research information, especially on its implementation at the Syarif Hidayatullah Islamic boarding school.

The recent increase in the aging population, specifically in the United States, has raised concerns regarding treatment for mental illness among older adults. Late-life depression (LLD) is a complex condition that has become widespread among the aging population. Despite the availability of behavioral interventions and psychotherapies, few depressed older adults actually receive treatment. In this paper we review the research on refining treatments for LLD. We first identify evidence-based treatments (EBTs) for LLD and the problems associated with efficacy and dissemination, then review approaches to conceptualizing mental illness, specifically concepts related to brain plasticity and the Research Domain Criteria (RDoc). Finally, we introduce ENGAGE as a streamlined treatment for LLD and discuss implications for future research.

According to the latest forecasts, it will take 10 years for the world economy to get back to “decent shape”. Some more critical estimates suggest that the whole western world will have a “colossal mess” within the next 5–10 years. Regulators of some major countries significantly and over a short time‑period changed their forecasts for the worse which means that uncertainty in the outlook for the future persists. Indeed, the intensive anti‑crisis measures have reduced the severity of the past problems, however the problems themselves have not disappeared. Moreover, some of them have become more intense — the eurocrisis, excessive debts, global liquidity glut against the backdrop of its deficit in some of market segments. As was the case prior to the crisis, derivatives and high‑risk operations with “junk” bonds grow; budget problems — “fiscal cliff” in the US — and other problems worsen. All of the above forces the regulators to take unprecedented (in their scope and nature) steps. Will they be able to tackle the problems which emerge?

Ramsay Hunt syndrome is a special form of herpes zoster which is typically characterized by peripheral facial palsy and unilateral herpetic vesicles on the ear. These symptoms are often accompanied by vestibulocochlear dysfunction and other neurological and ophthalmological symptoms. The diagnosis and therapy requires a multidisciplinary approach. The authors present a typical case where the early administration of combined antiviral and systemic corticosteroid therapy led to complete recovery. The authors emphasize the importance of early diagnosis and adequate combination therapy, which improves the prognosis of this disease. Orv. Hetil., 2012, 153, 1281–1283.
