Multiple sclerosis (MS) is a progressive neurological disorder, classically presenting in working age adults, including those in the Armed Forces. The Defence Medical Rehabilitation Centre (DMRC) Stanford Hall offers vocationally focused neurorehabilitation services for service personnel (SP) with MS, with the goal to minimise disability, maximise independence and remain able to work.This paper has two aims. First, it briefly provides a clinical update of MS, focusing on pathology, presentation, diagnosis and management. Finally, it will describe the role of DMRC and data from the last decade in the management of MS.Our findings suggest not all SP with MS are being referred to DMRC, and some of those who do have significant delays, potentially impacting on patient support, symptom management and occupational outcomes. It is hoped that this paper will improve awareness and recognition of MS for Armed Forces personnel.

Introduction: Several treatment options e.g. topical corticosteroids, phototherapy like narrow-band ultra violet B (NB-UVB) and psoralen+ultra violet A (PUVA) etc are available for vitiligo. But none is so effective in single but combined one is more effective and superior. Objective: To compare the efficacy of NB-UVB vs NB-UVB with topical Tacrolimus ointment (0.1%) in the treatment of Vitiligo. Materials and Methods: This descriptive cross-sectional study was conducted in Combined Military Hospital, Dhaka from October 2015 to April 2016. Total 100 patients with vitiligo were divided into 2 groups of 50 patients. NB-UVB was given for 04 weeks to Group-A. In Group-B patients, topical Tacrolimus ointment (0.1%) twice daily was advised with simultaneous NB-UVB. The patients were followed-up at the baseline of the 4th, 8th and 16th week. Results: Majority (46%) of the patients were from 25-34 years of age with a mean age of 27.4±12.6 years. Among all risk factors, Family history of Vitiligo was the most common risk factor and common skin type (Fitzpatrick) was Type IV (40%). At the end of 4th week, maximum cases of Group-B showed good response (score 3) with a mean score of 1.74. Whereas, maximum cases of Group-A showed poor response (score 1) with a mean score of 0.86. End of the 16th week, maximum cases of Group-B improved successfully. In the end, 20% patients of Group-A showed an excellent result; but it was 42% from Group-B. Conclusion: It is revealed from this study that patients treated with a combination of NB-UVB with topical Tacrolimus ointment (0.1%) showed better treatment outcome, more effective and becomes a new mode of treatment. The article analyzes the moral condition and conditions of staying of interned Ukrainian soldiers in the camp of Stshalkovo (Poland) in the second half of 1921 – early 1922. Based on the use of a wide range of archival sources, the author concludes that the camp everyday life of the wartime period was marked by significant difficulties due to its extraordinary crowding and presence in the camp of interned and captured soldiers of other formations (Balahovtsy and Red Army soldiers). The lack of any material resources, the lack of adequate and high-quality food, the lack of fuel for heating booths, anti-Ukrainian agitation, led to the fact that a part of Ukrainian soldiers lost national moral values. In order to prevent the demoralization of Ukrainian warfare, the command of the group tried to intensify cultural and educational work in the camp, but due to lack of premises and lack of resources, it slowly collapsed. Some of the interned ones did not withstand the burden of hopeless and burdensome camping munitions, and left the camp in search of better living conditions. Obviously, this was a dangerous trend, and the command of the interned troops in various ways tried to prevent it from further spreading. The leader of the group A. Pиzitsky was particularly opposed to the destructive processes in the camp. His measures were fully understood and supported by the higher military leadership of the Armed Forces and the Government of the UPR.

Objective: To assess morbidity and success of transabdominal (O’Conor) repair of vesicovaginal fistula with orwithout interposition of flap between vagina and urinary bladder. Study Design: Prospective comparative study. Place and Duration of Study: Armed Forces Institute of Urology, Rawalpindi, from Mar 2016 to Jan 2019. Methodology: Fifty five patients were randomized into group A & B by lottery method. An inclusion criterionwas single fistulous opening of ≤3cm. Complex and recurrent fistulae were excluded. Patients in group A underwent O’Conor repair without interpositional flap while in group B vesicovaginal fistulae were repaired with flap interposition. Results: Twenty two patients were randomized in group A while 27 in group B. Mean age of patients was 41.65± 11.93 years. Gynecological and obstetrical surgery was the main cause of fistula. Mean duration of surgerywas 162.7 ± 18.49 minutes. Per-operative ureteric catheterization was done in 9 (16.8%) patients. Seventeen(30.6%) patients had paralytic ileus. There was transient fever in 4 (7.6%) of cases and wound infection was seenin 3 (5.8%) of patients. Mean hospital stay was 3.4 ± 2.3 days. Cystogram was done in 35 (64.5%) of patients before the removal of per-urethral catheter. The overall success rate was 92.9%. Ten (18.2%) of patients developed denovo urgency which was managed conservatively. There was no statistical difference in both groups in terms of morbidity and success. Conclusion: In simple Vesico-vaginal fistulae repair, interposition of flap can be omitted and it does not affect the outcomes in terms of success and morbidity.

Objective: To determine if the traditional chewing stick Miswak was as effective in cleaning teeth as Toothbrush. Study Design: Cross sectional study. Place and Duration of Study: Armed Forces Institute of Dentistry, Rawalpindi, from Aug 2020 to Jan 2021. Methodology: A total of 300 subjects were included which were divided in two groups on the basis of whether they used miswak or toothbrush as a cleaning aid. Group A was toothbrush users and group B was Miswak user. Plaque Index was used to determine the cleanliness of teeth. The scores were recorded and data analyzed using SPSS-23. Results: The means and standard deviations of Plaque Index score for group A and B were 0.96 ± 0.58 and 0.98 ± 0.56 respectively. The comparison of Plaque Index score for both groups was insignificant with the p-value of 0.083. Conclusion: Within the limitations of this study, it is concluded that, no significant difference was found in the effectiveness of traditional miswak and tooth brush. It is recommended that if the technique of teeth cleaning is good then any of the abovementioned means of teeth cleaning can be used.

In the nineteenth and early twentieth centuries, many Irish doctors led successful careers in the British Empire’s military medical services. Surprisingly, Irish medical connections with the British military were not simply severed once the Irish Free State seceded from the United Kingdom in 1921, as might be expected. Rather, they rapidly grew in the 1920s and 1930s. This chapter asks why British military service continued to prove so popular among Irish doctors, making extensive use of a database of 262 Irish medical officers who served in the British forces between 1922 and 1945. The chapter reveals striking patterns in the social profile of officers, their motives, career success and the peaks and troughs of recruitment. It seems that many Irish medical officers complained that appointments in Irish hospitals were controlled by nepotism and that limited jobs were available. Several Irish publications which dispensed career advice to medical students during the 1930s not merely acknowledged, but actually recommended, opportunities in the British military services in preference to the Irish Army Medical Service - castigated for its poor pay, promotion prospects and pension entitlements. The result was an outflow of Irish medical practitioners beyond the attaining of Irish independence.

Psychological injury has been associated with military service, and this can result in a variety of mental health symptoms and disorders. A range of barriers to help-seeking have been identified in the military and mental health services have sought to address such factors through effective and efficient care and consultation. The use of eye movement desensitisation and reprocessing forms part of a repertoire of trauma-focused therapies within the UK’s Armed Forces. This article will outline the application of this approach within the British military, along with the role of specialist clinical supervision in treating those affected by operational trauma.

The UK prevalence of scarlet fever, a Group A streptococcal infection, is increasing. We present an unusual case of suspected recurrent scarlet fever in a member of the UK Armed Forces. Treatments, occupational implication and public health measures to mitigate the risk of disease spread.
