Trouble was brewing for Nestle in India with a lab test finding MSG in Maggi noodles, a product brand which had been adjudged ‘most powerful’ and ‘most trusted’ in India;it was being banned in different parts of the country. Paul Bulcke, CEO of Nestle SA, arrived in New Delhi to face the heat and take necessary damage control measures. The case challenges the participants to review the events leading to a total ban on all the nine variants of Maggi noodles imposed by FSSAI, the Indian Regulator, by Nestle India. It also challenges them to suggest ways of taking care of the business in future in India as well as its effects in other countries.

<p>Acute renal failure is defined as a rapid decrease in the glomerular filtration rate, occurring over a period of hours to days and by the inability of the kidney to regulate fluid and electrolyte homeostasis appropriately. AKI is a catastrophic, life-threatening event in critically ill patients. AKI can be divided into pre-renal injury, intrinsic kidney disease (including vascular insults) and obstructive uropathies. The prognosis of AKI is highly dependent on the underlying cause of the injury. Children who have AKI as a component of multisystem failure have a much higher mortality rate than children with intrinsic renal disease. Treatment of AKI is subjected to risk stratification and ongoing damage control measures, such as patients with sepsis, exposure to nephrotoxic agents, ischemia, bloody diarrhea, or volume loss, could be helped by optimizing the fluid administrations, antibiotics possessing least nephrotoxic potential, blood transfusion where hemoglobin is dangerously low, limiting the use of nephrotoxic agents including radio contrast use, while maximize the nutrition. Acute kidney injury remains a complex disorder with an apparent differentiation in pathology between septic and nonseptic forms of the disease. Although more studies are still required, progress in this area has been steady over the last decade with purposeful international collaboration.</p>

AbstractCo-circulation of Chikungunya and Dengue viral infections (CHIKV and DENV) have been reported mainly due to transmission by commonAedesvector. The purpose of the study was to identify and characterise the circulating strains of CHIKV and DENV in DENV endemic region of New Delhi during 2016. CHIKV and DENV were identified in the blood samples (n= 130) collected from suspected patients by RT-PCR. CHIKV was identified in 26 of 65 samples (40%). Similarly, DENV was detected in 48 of 120 samples (40%). Co-infection with both the viruses was identified in five (9%) of the samples. Interestingly, concurrent infection with DENV, CHIKV andPlasmodium vivaxwas detected in two samples. CHIKV strains (n= 11) belonged to the ECSA genotype whereas DENV-3 sequences (n= eight) clustered in Genotype III by phylogenetic analysis. Selection pressure of E1 protein of CHIKV and CprM protein of DENV-3 revealed purifying selection with four and two positive sites, respectively. Four amino acids of the CHIKV were positively selected and had high entropy suggesting probable variations. Co-circulation of both viruses in DENV endemic regions warrants effective monitoring of these emerging pathogens via comprehensive surveillance for implementation of effective control measures.

In 2012, the European Centre for Disease Prevention and Control (ECDC) launched the ‘European survey of carbapenemase-producing Enterobacteriaceae (EuSCAPE)’ project to gain insights into the occurrence and epidemiology of carbapenemase-producing Enterobacteriaceae (CPE), to increase the awareness of the spread of CPE, and to build and enhance the laboratory capacity for diagnosis and surveillance of CPE in Europe. Data collected through a post-EuSCAPE feedback questionnaire in May 2015 documented improvement compared with 2013 in capacity and ability to detect CPE and identify the different carbapenemases genes in the 38 participating countries, thus contributing to their awareness of and knowledge about the spread of CPE. Over the last two years, the epidemiological situation of CPE worsened, in particular with the rapid spread of carbapenem-hydrolysing oxacillinase-48 (OXA-48)- and New Delhi metallo-beta-lactamase (NDM)-producing Enterobacteriaceae. In 2015, 13/38 countries reported inter-regional spread of or an endemic situation for CPE, compared with 6/38 in 2013. Only three countries replied that they had not identified one single case of CPE. The ongoing spread of CPE represents an increasing threat to patient safety in European hospitals, and a majority of countries reacted by establishing national CPE surveillances systems and issuing guidance on control measures for health professionals. However, 14 countries still lacked specific national guidelines for prevention and control of CPE in mid-2015.

Advances in invasive radiology coupled with new damage control measures assisted in the refinement of the technique with promising outcomes. The literature continues to have substantial heterogeneity about REBOA indications, applications, and the challenges confronted when implementing the technique in a level I trauma center. Scoping reviews are excellent platforms to assess the diverse literature of a new technique. It is for the first time that a scoping review is adopted for this topic. Advances in invasive radiology coupled with new damage control measures assisted in the refinement of the technique with promising outcomes. The literature continues to have substantial heterogeneity about REBOA indications, applications, and the challenges confronted when implementing the technique in a level I trauma center. It is for the first time that a scoping review is adopted for this topic.MethodsCritical search from MEDLINE, EMBASE, BIOSIS, COCHRANE CENTRAL, PUBMED and SCOPUS were conducted from the earliest available dates until March 2018. Evidence-based articles, as well as gray literature at large, were analyzed regardless of the quality of articles.ResultsWe identified 1176 articles related to the topic from all available database sources and 57 reviews from the gray literature search. The final review yielded 105 articles. Quantitative and qualitative variables included patient demographics, study design, study objectives, methods of data collection, indications, REBOA protocol used, time to deployment, zone of deployment, occlusion time, complications, outcome, and the level of expertise at the concerned trauma center.ConclusionGrowing levels of evidence support the use of REBOA in selected indications. Our data analysis showed an advantage for its use in terms of morbidities and physiologic derangement in comparison to other resuscitation measures. Current challenges remain in the selective application, implementation, competency assessment, and credentialing for the use of REBOA in trauma settings. The identification of the proper indication, terms of use, and possible advantage of the prehospital and partial REBOA are topics for further research.Level of evidenceLevel III.

The treatment of vascular injuries remains a significant challenge of modern trauma care. This is particularly true of injuries at challenging anatomic locations or that are complex in nature. The mortality rates from these injuries can be high, despite recent progress in application of damage control management concepts. Developments of endovascular techniques have introduced new alternatives to traditional open repair strategies that may prove useful in the setting of complex vascular injury. The concept of endovascular and hybrid trauma management (EVTM) gains experience which allows replacing traditional principles of open vascular management in selected cases. Endovascular modalities, specifically intra-vascular balloon occlusion, offer a novel strategy that may be of particular use in these situations. Our present case presentation outlines an example of successful utilization of this approach and affords an opportunity to review a simplified approach using endovascular balloons as proximal and distal control measures, in order to limit the challenges represented by more extensive anatomic exposures in a victim of trauma.

Tension pneumopericardium is a rare disease that is very difficult to diagnose. A 3-year old child was run over by a minibus. Computed tomography showed bilateral pneumothorax, multiple rib fractures on both sides, and a hepatic laceration. The massive blunt thoracic trauma caused entrapped air in the pericardium, and a tension pneumoprecordium developed abruptly. We report this unique case of tension pneumopericardium in a child, and the subsequent emergency surgery and damage control measures.

Abstract The aim of the paper is to present the results of a cost analysis for artificial and natural oak regeneration in selected forest districts in Poland. This research was conducted in six forest districts with a high share of natural oak regeneration, located in south-western and central Poland. Altogether 65 plots with artificial and 35 plots with natural regeneration were analysed based on the extent of silvicultural (weeding, blank-filling and pre-commercial thinning) and protective measures (chemical and mechanical wildlife damage control, fencing) performed on the regenerated areas for the six years following forest regeneration. An intensity ratio (i.e., a proportion of the area of measures to the area of forest regeneration) as well as the mean unit costs of the measures and mean costs of the measures per hectare of forest regeneration were calculated. The results show that the total costs of silvicultural and protective measures in natural oak regeneration were considerably lower as compared to artificial regeneration. The cumulative average costs during 6 years (without costs of fencing) amounted to 1216 PLN/ ha and 6543 PLN/ha for natural and artificial regeneration respectively. Lower costs of natural oak regeneration resulted from the complete lack of expenditures on seedlings and planting, a considerably lower weeding intensity, scarce blank-filling, and the absence of mechanical wildlife damage control measures. Nonetheless, natural regeneration generated higher costs for pre-commercial thinning due to its higher intensity, earlier initiation, and higher unit costs compared to artificial regeneration, as well as chemical wildlife damage control measures, for which the unit costs were higher by over 50%. However, the higher costs of pre-commercial thinning and chemical control measures did not significantly affect the measurable financial benefits of natural regeneration. In conclusion, the matter of financial viability of natural oak regeneration should be explored in more detail over longer time spans.

Background: The coronavirus disease (COVID-19) has spread worldwide with an increasing number of patients, including pregnant women and neonates. This study aims to evaluate morbidity and mortality in the COVID-19 era compared to the preceding year in the Neonatal Intensive Care Unit (NICU) at Tamale Teaching Hospital, Ghana.Methods: This is a cross-sectional study carried out on neonates admitted to NICU between March 1st to August 31st, 2019 (pre-COVID-19 era) and March 1st to August 31st, 2020 (COVID-19 era). Multivariate logistic regression was performed to identify predictors of mortality for both periods.Results: From 2,901 neonates, 1,616 (56%) were admitted before, and 1,285 (44%) were admitted during the pandemic. Admissions decreased during the COVID-19 era, reaching their lowest point between June and August 2020. Compared to the previous year, during the COVID-19 era, admissions of patients born at TTH, delivered at home, and with infections decreased from 50 to 39%, 7 to 4%, and 22 to 13%, respectively. Referred status (OR = 3.3) and vaginal delivery (OR = 1.6) were associated with an increased likelihood of mortality. For low- birth weight neonates, admissions of patients born at TTH, with vaginal and home delivery decreased from 62 to 48%, 8 to 2%, and 59 to 52%, respectively. Neonatal infections and congenital anomalies decreased from 8 to 4%, 5 to 3%, respectively. The likelihood of mortality among referred patients increased by 50%.Conclusion: We observed a marked decrease in admissions and change in the diagnosis landscape and related mortality during the pandemic. Underlying challenges, including fear, financing, and health system capacity, might intensify delays and lack of access to newborn care in northern Ghana, leading to higher rates of lifelong disabilities and mortality. Immediate damage control measures, including an improved home-based continuum of care and equipping families to participate in the newborn care with complemented m-health approaches, are needed with urgency.

Thermal damage control of mass concrete is the key to guarantee the quality of mass projects. Based on several engineering experiences and finite element software ANSYS, the temperature field and stress field of the typical dam section of the Tengzigou hydropower station in Sichuan province were simulated. Considering the actual materials used, cooling measures, etc., maximum tensile stress and compressive stress at different time points derived from the temperature stress field during the time of concrete maintenance were calculated, and the numerical results showed that strength increment under the given conditions was much less than the actual condition. After the concrete of the dam body of the hydro power station were poured, there was no significant temperature stress crack appeared through a long-term observation, and the project condition was in tune with the calculated expectation. The above research results are valuable to further prediction of concrete temperature in different periods, the pre-study of the effect of temperature control measures, and these could offer guidance of the adjustment of temperature control measures in the case of abnormal conditions.

Aim: Morpho-molecular analyses for taxonomic characterization of nine predominant Penicillium species present in the soil of different parts of India. Methodology: Fifteen Penicillium isolates were isolated from the soil samples collected from the experimental field of ICAR-Indian Agricultural Research Institute (IARI), New Delhi. Another twenty-six isolates were procured from Indian Type Culture Collection (ITCC), Division of Plant Pathology, ICAR-IARI, New Delhi which were isolated from the soil of different parts of India. Total 41 isolates were characterized following distinct macroscopic (colony texture, colony colour exudate production; soluble pigmentation; reverse coloration and mycelial growth) and microscopic observations (type of penicillus; shape of phialides; conidial shape, size and pigmentation). Molecular characterization was done using partial β-tubulin gene sequence which is considered an excellent marker in differentiating Penicillium species. Results: The morphological data in species description perfectly matched with molecular data generated using β-tubulin gene marker and nine different species viz., P. aethiopicum, P. chrysogenum, P. crustosum, P. janthinellum, P. mononematosum, P. oxalicum, P. polonicum, P. singorense and Talaromyces pinophilus (Syn. P. pinophilum) were confirmed. Interpretation: Above nine Penicillium species are found predominantly in the soil collected from different parts of India. The β-tubulin gene can be considered as an excellent marker to differentiate Penicillium species as confirmed from this study. The combined morpho- molecular analyses can be further utilized to delineate morphologically similar Penicillium species and also helpful to establish new species of Penicillium.
