AbstractLookism is a term used to describe discrimination based on the physical appearance of a person. We suppose that the social impact of lookism is a philosophical issue, because, from this perspective, attractive people have an advantage over others. The first line of our argumentation involves the issue of lookism as a global ethical and aesthetical phenomenon. A person’s attractiveness has a significant impact on the social and public status of this individual. The common view in society is that it is good to be more attractive and healthier. This concept generates several ethical questions about human aesthetical identity, health, authenticity, and integrity in society. It seems that this unequal treatment causes discrimination, diminishes self-confidence, and lowers the chance of a job or social enforcement for many human beings. Currently, aesthetic improvements are being made through plastic surgery. There is no place on the human body that we cannot improve with plastic surgery or aesthetic medicine. We should not forget that it may result in the problem of elitism, in dividing people into primary and secondary categories. The second line of our argumentation involves a particular case of lookism: Melanie Gaydos. A woman that is considered to be a model with a unique look.

This study proposes a review on hyaluronic acid (HA) known as hyaluronan or hyaluronate and its derivates and their application in cosmetic formulations. HA is a glycosaminoglycan constituted from two disaccharides (N-acetylglucosamine and D-glucuronic acid), isolated initially from the vitreous humour of the eye, and subsequently discovered in different tissues or fluids (especially in the articular cartilage and the synovial fluid). It is ubiquitous in vertebrates, including humans, and it is involved in diverse biological processes, such as cell differentiation, embryological development, inflammation, wound healing, etc. HA has many qualities that recommend it over other substances used in skin regeneration, with moisturizing and anti-ageing effects. HA molecular weight influences its penetration into the skin and its biological activity. Considering that, nowadays, hyaluronic acid has a wide use and a multitude of applications (in ophthalmology, arthrology, pneumology, rhinology, aesthetic medicine, oncology, nutrition, and cosmetics), the present study describes the main aspects related to its use in cosmetology. The biological effect of HA on the skin level and its potential adverse effects are discussed. Some available cosmetic products containing HA have been identified from the brand portfolio of most known manufacturers and their composition was evaluated. Further, additional biological effects due to the other active ingredients (plant extracts, vitamins, amino acids, peptides, proteins, saccharides, probiotics, etc.) are presented, as well as a description of their possible toxic effects.

Today’s media disseminate a narcissistic cultivation of beauty and promote a focus mainly on appearance and satisfaction from its improvement. The human body assumes a form in the media that is often impossible to achieve without surgical intervention. When people are in frequent contact with a utopian vision of the perfect body, this can lead to many disorders in both social functioning and self-perception. In extreme cases, striving to preserve beauty and youth may lead to an addiction to aesthetic medicine treatments. It is an increasingly common phenomenon. People who are addicted to treatments improving their beauty or changing their body shapes are not aware of the problem that affects them. They lose their rational judgement and their assessment is far from the opinions of people around them and socially accepted norms. All signs of concern from others are perceived as an attack on their independence and this further deepens their sense of loneliness and isolation from society. With time, undergoing further beautifying procedures becomes the only way they know to achieve a momentary sense of happiness.

This review describes new engineering solutions for Ti:Sapphire lasers obtained at Laseroptek during the development of laser devices for dermatology and aesthetic medicine. The first device, PALLAS, produces 311 nm radiation by the third harmonic generation of a Ti:Sapphire laser, which possesses similar characteristics to excimer laser-based medical devices for skin treatments. In comparison to excimer lasers, Ti:Sapphire laser services are less expensive, which can save ~10% per year for customers compared to initial excimer laser costs. Here, the required characteristics were obtained due to the application of a new type of diffraction grating for spectral selection. The second device, HELIOS-4, based on the Ti:Sapphire laser, produces 300 mJ, 0.5 ns pulses at 785 nm for tattoo removal. The characteristics of HELIOS-4 exceed those of other tattoo removal laser devices represented in the medical market, despite a simple and inexpensive technical solution. The development of the last laser required the detailed study of a generation process and the investigation of the factors responsible for the synchronization of the generation in Ti: Sapphire lasers with short (several millimeters) cavities. The mechanism that can explain the synchronization in such lasers is suggested. Experiments for the confirmation of this concept are conducted and analyzed.

GalynaViktorovnaKhrushch plastic surgeon, maxillofacial surgeon, international candidat ASPS, Periorbital area is worthily considered as one of the most complicated regions in terms of correction of the age-related changes using injection techniques of medical aesthetics. According to the classification of I.I. Kolgunenko Russian (1974), tired morph type of age-related changes which is the most physiological type of aging, incorporates changes mainly focused in the middle third of the face, including in the periorbital area. These changes include the formation of grooves (tear trough, palpebromalar groove, nasojugal groove) (Figure 1), dark under-eye circles, mimic wrinkles formation, fat compartment displacement, change in mimic muscle tone, stretching of ligamentous structures, overhang of the upper eyelid.

Autoimmune thyroid diseases are the most common organ-specific autoimmune diseases, affecting 2–5% of the world’s population. Due to the autoimmune background of thyroid diseases, we analyzed a wide range of cosmetic procedures, from minimally invasive cosmetic injections (mesotherapy) to highly invasive procedures, such as lifting threads. Out of the seven categories of treatments in aesthetic medicine analyzed by us—hyaluronic acid, botulinum toxin, autologous platelet-rich plasma, autologous fat grafting, lifting threads, IPL and laser treatment and mesotherapy—only two, mesotherapy and lifting threads, are not recommended. This is due to the lack of safety studies and the potential possibility of a higher frequency of side effects in patients with autoimmune thyroid diseases.

Stretch marks are a common problem and a challenge for cosmetologists and aesthetic medicine doctors. Due to the complex etiopathogenesis and difficulties in their reduction, specialists use various therapeutic methods. In this study, the described case confirms the effectiveness of selected combined therapy in reducing stretch marks and skin laxity. Impressive surgical results were obtained with the implementation of combined therapy consisting of the use of non-ablative Nd:YAG fractional laser and alexandrite laser in picosecond technology, CO2 fractional ablative laser and needle mesotherapy treatments.

Актуальность. Глубокие резаные раны являются одной из наиболее распространенных травм при авариях и, по-прежнему, остаются глобальной проблемой общественного здравоохранения. Несмотря на многочисленные успехи в терапии ран, все еще существует необходимость поиска и разработки новых препаратов для их местного лечения. Цель - определить возможности применения местной терапии трансглютаминазой микробного происхождения (МТГ) с вспомогательной экспериментальной субстанцией авторской фитомази при моделировании полнослойных резаных ран на основании изучения особенностей морфогенеза регенераторных процессов кожи. Методы. В эксперименте на 150 белых мышах самцах исследована динамика регенераторных процессов кожи в группах: без лечения; при использовании препарата Солкосерил и авторской фитомази; а также ее комбинации с ферментом МТГ в различных концентрациях. Применяли комплекс морфологических методов, направленных на изучение структуры кожи на органном, тканевом, клеточном и субклеточном уровнях организации: макроскопический метод - для описания и измерения размеров повреждений; гистологические методы на светооптическом уровне - для качественной оценки морфологических преобразований кожи; метод трансмиссионной электронной микроскопии - для выявления ультраструктурных преобразований различных клеток эпидермиса и дермы. Результаты. Наиболее эффективным препаратом для заживления полнослойных резаных ран по итогам визуальной оценки и гистологического исследования является комбинация фитомази с МТГ в концентрации 0,1%, что подтверждается остановкой кровотечения и образованием фибринового сгустка в 3 раза быстрее, чем в других экспериментальных и контрольной группах; ранним началом эпителизации (на 3 сутки эксперимента), окончательным завершением регенераторных процессов к 10 суткам, что свидетельствует о перспективах использования трансглютаминаз в хирургии и эстетической медицине. Заключение. Применение инновационной методики местной терапии глубоких резаных ран кожи ферментом микробной трансглютаминазой с вспомогательной экспериментальной субстанцией авторской фитомази характеризуется ранним началом эпителизации, что свидетельствует о перспективах использования трансглютаминаз в хирургии и эстетической медицине. В то же время, механизмы репарации должны быть дополнительно исследованы в серии клинических испытаний. Background. Deep cut wounds are one of the most common injuries in accidents and still remain a global public health problem. Despite much success in treatment of wounds searching for and developing new medicines for local treatment are still relevant. The study objective was to test a possibility of local treatment with microbial transglutaminase (MTG) supplemented with an auxiliary experimental substance of the author’s herbal ointment by studying morphogenesis of skin regenerative processes on a model of full-thickness cut wounds. Methods. We studied the dynamics of skin regenerative processes using the following groups: no treatment; a standard treatment with the medicine Solcoseryl in combination with the author’s ointment; the ointment alone; and the ointment in combinations with the enzyme MTG at different concentrations. A set of morphological methods was used for studying the skin structure at organ, tissue, cell, and subcellular levels in experiments on 150 white male mice. The methods included macroscopic description and measurement of skin lesions; histological methods at the light-optical level for qualitative evaluation of skin morphologic changes; and transmission electron microscopy for identifying ultrastructural transformations of different epidermal and dermal cells. Results. Visual evaluation and histological study showed that he most effective treatment for healing of full-thickness cut wounds was 0.1% MTG as evidenced by arrest of bleeding and formation of a fibrin clot, which occurred three times faster than in other experimental and control groups; an early onset of epithelialization on day 3 of the experiment; and the completion of regenerative processes by day 10, which suggested prospective benefits of using transglutaminases in surgery and aesthetic medicine. Conclusions. The innovative local treatment of deep cut wounds of the skin with microbial transglutaminase supplemented with an auxiliary experimental substance of the author’s herbal ointment is characterized by an early onset of epithelization, which suggested prospective benefits of using transglutaminases in surgery and aesthetic medicine. At the same time, mechanisms of reparation need further investigation in clinical trials.
