ObjectivesExamine the extent that parent gender is associated with supporting children’s physical activity.DesignCross-sectional mixed-methods study.Setting47 primary schools located in Bristol (UK).Participants944 children aged 8–9 years and one of their parents provided quantitative data; 51 parents (20 fathers) were interviewed.MethodsChildren wore an accelerometer, and mean minutes of moderate-to-vigorous physical activity (MVPA) per day, counts per minute (CPM) and achievement of national MVPA guidelines were derived. Parents reported who leads in supporting child activity during the week and weekend. Linear and logistic regression examined the association between gender of parent who supports child activity and child physical activity. For the semistructured telephone interviews, inductive and deductive content analyses were used to explore the role of gender in how parents support child activity.ResultsParents appeared to have a stronger role in supporting boys to be more active, than girls, and the strongest associations were when they reported that both parents had equal roles in supporting their child. For example, compared with the reference of female/mother support, equal contribution from both parents during the week was associated with boys doing 5.9 (95% CI 1.2 to 10.6) more minutes of MVPA per day and more CPM when both parents support on weekday and weekends (55.1 (14.3 to 95.9) and 52.8 (1.8 to 103.7), respectively). Associations in girls were weaker and sometimes in the opposite direction, but there was no strong statistical evidence for gender interactions. Themes emerged from the qualitative data, specifically; parents proactively supporting physical activity equally, mothers supporting during the week, families getting together at weekends, families doing activities separately due to preferences and parents using activities to bond one-to-one with children.ConclusionsMothers primarily support child activity during the week. Children, possibly more so boys, are more active if both parents share the supporting role.

ObjectivesTo investigate factors associated with movement behaviours among White British (WB) and South Asian (SA) children aged 6–8 years during school terms and holidays.DesignCross-sectional.SettingThree primary schools from the Bradford area, UK.ParticipantsOne hundred and sixty WB and SA children aged 6–8 years.Primary and secondary outcomesSedentary behaviour (SB), light physical activity (LPA) and moderate-to-vigorous physical activity (MVPA) measured by accelerometry during summer, winter and spring and during school terms and school holidays. Data were analysed using multivariate mixed-effects multilevel modelling with robust SEs. Factors of interest were ethnicity, holiday/term, sex, socioeconomic status (SES), weight status, weekend/weekday and season.ResultsOne hundred and eight children (67.5%) provided 1157 valid days of data. Fifty-nine per cent of children were WB (n=64) and 41% (n=44) were SA. Boys spent more time in MVPA (11 min/day, p=0.013) compared with girls and SA children spent more time in SB (39 min, p=0.017) compared with WB children in adjusted models. Children living in higher SES areas were more sedentary (43 min, p=0.006) than children living in low SES areas. Children were more active during summer (15 min MVPA, p<0.001; 27 LPA, p<0.001) and spring (15 min MVPA, p=0.005; 38 min LPA, p<0.001) and less sedentary (−42 min and −53 min, p<0.001) compared with winter. Less time (8 min, p=0.012) was spent in LPA during school terms compared with school holidays. Children spent more time in MVPA (5 min, p=0.036) during weekend compared with weekdays. Overweight and obese children spent more time in LPA (21 min, p=0.021) than normal-weight children.ConclusionThe results of our study suggest that significant child level factors associated with movement behaviours are ethnicity, sex, weight-status and area SES. Significant temporal factors are weekends, school holidays and seasonality. Interventions to support health enhancing movement behaviours may need to be tailored around these factors.

Abstract Background The study aimed to examine the cross-sectional associations between 24-hour activity compositions and motor competence in children and adolescents, while stratifying by sex and school type, and investigate the predicted differences in motor competence when time was reallocated between activity behaviours. Methods Data were collected from 359 participants (aged 11.5±1.4 years; 49.3% boys; 96.9% White British). Seven-day 24-hour activity behaviours (sleep, sedentary time, light physical activity (LPA), moderate-to-vigorous physical activity (MVPA)) were assessed using wrist-worn accelerometers. Motor competence outcomes were obtained using the Dragon Challenge (process, product, time, and overall scores). Linear mixed models examined associations between activity behaviour compositions and motor competence outcomes for all participants and stratified by school type (primary or secondary) and sex. Post-hoc analyses modelled the influence of reallocating fixed durations of time between activity behaviours on outcomes. Results In all participants, relative to other activity behaviours, MVPA had the strongest associations with motor competence outcomes. The stratified models displayed that MVPA had the strongest associations with outcomes in both sexes, irrespective of school type. The largest positive, and negative predicted differences occurred when MVPA replaced LPA or sleep, and when LPA or sleep replaced MVPA, respectively. Conclusions Relative to other activity behaviours, MVPA appears to have the greatest influence overall on motor competence outcomes. Reallocating time from LPA or sleep to MVPA reflected the largest positive predicted changes in motor competence outcomes. Therefore, our findings reinforce the key role of MVPA for children’s and adolescents’ motor competence.

Abstract Background: Insufficient physical activity is a serious public health concern. Assessing physical activity accurately in children remains a challenge and valid tools for its assessment in African children are needed. This study compares self-reports versus accelerometry-captured physical activity.Methods: In this cross-sectional study, 51 primary school children aged 9 – 11 years from four primary schools in Kilimanjaro region were enrolled. Children were asked to complete a self-reported questionnaire on physical activity and wore an accelerometer for seven consecutive days. Accelerometry was used as a reference method to assess for child reporting accuracy.Results: Spearman correlation between self-reports and accelerometry was moderate (r=0.41, p=0.002). Children who reported walking to school had higher moderate to vigorous physical activity (MVPA) for both accelerometry and self- reports compared to their counterparts (p < 0.001). Intra-class correlation (ICC) showed that 32% of the variations in accelerometry weekly total MVPA was contributed by the differences in activities between children.Conclusions: Objective measures of physical activity such as accelerometers can be used to accurately measure physical activity in children where feasible. Self-reports have limited validity, and therefore more research is needed to develop better self-reported measures with specific activities which can easily be recalled by children.

ObjectivesThis study investigated physical activity (PA) and sedentary behaviour (SB) among preschool-aged children in Singapore and potential correlates at multiple levels of the socioecological model from in-school and out-of-school settings.DesignA cross-sectional study using a mixed-methods approach.ParticipantsParent–child dyads from six preschools in Singapore.MethodsPA and SB of children (n=72) were quantified using wrist-worn accelerometers for seven consecutive days. Three focus group discussions (FGDs) among 12 teachers explored diverse influences on children’s activities, and System for Observing Play and Leisure Activity in Youth (SOPLAY) assessed PA environment and children’s activity levels at preschools. Seventy-three parents completed questionnaires on home and neighbourhood factors influencing children’s PA and SB. Descriptive analyses of quantitative data and thematic analysis of FGDs were performed.ResultsBased on accelerometry, children (4.4±1.1 years) spent a median of 7.8 (IQR 6.4–9.0) hours/day in SB, and 0.5 (0.3–0.8) hours/day in moderate-to-vigorous physical activity (MVPA). MVPA was similar throughout the week, and SB was slightly higher on non-school days. In preschools, SOPLAY showed more children engaging in MVPA outdoors (34.0%) than indoors (7.7%), and absence of portable active play equipment. FGDs revealed issues that could restrict active time at preschool, including academic requirements of the central curriculum and its local implementation. The teachers had varying knowledge about PA guidelines and perceived that the children were sufficiently active. In out-of-school settings, parents reported that their children rarely used outdoor facilities for active play and spent little time in active travel. Few children (23.5%) participated in extracurricular sports, but most (94.5%) reported watching screens for 1.5 (0.5–3.0) hours/day.ConclusionMVPA was low and SB was high in preschool-aged children in an urban Asian setting. We identified diverse in-school and out-of-school correlates of PA and SB that should be taken into account in health promotion strategies.

Abstract Background: Maximising the level of physical activity in adolescents has been a concern for many researchers given its multiple benefits for health. To verify the association between adolescents compliance with moderate-to-vigorous physical activity (MVPA) and their parental or legal guardian social support (SS) and MVPA compliance. Methods: Cross-sectional study that selected 1390 (59.6% female) adolescents. The questionnaires IPAQ, QAFA and ASAFA were used. Results: In relation to boys, receiving incentives from parents who “assist always” (OR=1.96 – CI95%=1.16–3.32) and having parents/legal guardians who meet the MVPA recommendations (OR=2.78 – CI95%=1.76–4.38) was associated with meeting the MVPA recommendations. The probability increases when adjusted by socioeconomic status (OR=3.47 – CI95%=1.73–6.96) or education level (OR=4.20 – CI95%=1.96–9.02). Regarding girls, parents who “encourage sometimes” was associated negatively (OR=0.61 – CI95%=0.37–0.98) and meet the MVPA recommendations (OR=2.04 – CI95%=1.46–2.87) was associated positively with meeting the recommendations for adolescents. The likelihood raises when adjusted by socioeconomic status (OR=2.11 – CI95%=1.36–3.29) or education level (OR=4.30 – CI95%=2.41–7.69). Conclusions: Having parents who meet the physical activity recommendations demonstrated higher results in adolescents engaging 60 minutes of MVPA daily when compared to receiving social support, for both boys and girls.

Background: Self-reports are commonly used to assess physical activity in children. Existing self-reports for physical activity have not been validated for use among primary school children in, Tanzania. In order to understand if primary school children can accurately report their physical activity, we examined the validity of self-reported physical activity against accelerometer measured physical activity. Methods: A community based cross-sectional study was conducted from May to July, 2018. Four primary schools were conveniently selected in Moshi municipal and Moshi rural districts in Kilimanjaro, Tanzania and from these 51 children aged 9 – 11 years were randomly selected. Self-reported questionnaire was used to collect physical activity related variables. In addition, children wore accelerometers for seven consecutive days to capture physical activity movements. Spearman’s rank test and Bland Altman plots were used for assessing validity and agreement between self-reports and accelerometer moderate to vigorous physical activity (MVPA). Results: The mean age of the study participants was 10 (SD=0.8) years and 32 (63%) were girls. A positive significant correlation was found between self-reports and accelerometer MVPA (rho=0.36, p=0.009). Accelerometer had higher mean MVPA 408 (SD = 66) compared to self-reports 261 (SD = 179). Children who reported walking to school had higher MVPA for both accelerometer and self- reports compared to children who use other means of transport to school, e.g. school buses (p < 0.001). Conclusions: This study found a positive significant correlation between self-reports and accelerometers. Self-reports are prone to errors due to recall bias, and this interferes their validity. More research is needed to develop better self-reported measures with specific activities which can easily be recalled by children. Also, researchers have to be aware of self-reports validity limitation.

The study examined the associations between physical activity and appendicular lean mass (ALM), fat mass, and bone mineral density (BMD) and examined the role of physical activity on these variables. The participants included 497 men (age: 32.2±10.4 years and body mass index: 28.2±5.4 kg/m2). The study was cross-sectional, and measurements included body composition measured on dual-energy X-ray absorptiometry and self-reported physical activity assessed using the International Physical Activity Questionnaire. Physical activity, including light physical activity, was associated with increased ALM (P≤0.05). Fat indicators, including waist circumference, were positively associated with ALM (P≤0.05), but not with BMD. Physical activity, including moderate-to-vigorous physical activity, was not associated with BMD, whereas ALM and handgrip strength were significantly associated with BMD (P≤0.05). Physical activity independent of exercise intensity was directly associated with ALM and indirectly associated with BMD through increased muscle mass and strength.

Background: Most antibiotics are prescribed in primary care. Locum or sessional general practitioners (locums) are perceived as contributing to higher prescribing and may face barriers to engaging with antimicrobial stewardship (AMS). Aim: To identify how locums’ antibiotic prescribing compares to other general practice prescribers, and how they perceive their role in antibiotic prescribing and AMS. Design and Setting: A mixed-methods study in primary care. Methods: Data on antibiotic prescribing, diagnoses, and patient and prescriber characteristics were extracted from The Health Improvement Network database. A mixed-effects logistic model was used to compare locums’ and other prescribers’ antibiotic prescribing for conditions which do not usually benefit from antibiotics. Nineteen semi-structured telephone interviews were conducted with locums in England and analysed thematically. Results: Locums accounted for 11% of consultations analysed. They prescribed antibiotics more often than other GPs and nurse prescribers for cough, sore throat, asthma exacerbations and acute bronchitis. The percentage of patients receiving antibiotics for these conditions was 4% higher (on absolute scale) when consulting with locums compared to other GPs. Four themes capture the perceived influences on prescribing antibiotics and AMS: (1) Antibiotic prescribing as a complex but individual issue; (2) Nature and patterns of locum work; (3) Relationships between practices and locums; (4) Professional isolation. Conclusions: Locums contribute to higher antibiotic prescribing compared to their peers. They experience challenges but also opportunities for contributing to AMS, which should be better addressed. With an increasing proportion of locums, they have an important role in antibiotic optimisation and AMS.

BackgroundCellulitis is a common painful infection of the skin and underlying tissues that recurs in approximately a third of cases. Patients’ ability to recover from cellulitis or prevent recurrence is likely to be influenced by their understanding of the condition.AimTo explore patients’ perceptions of cellulitis and their information needs.MethodMixed methods study comprising semi-structured, face-to-face interviews and cross-sectional survey, recruiting through primary care, secondary care and advertising. Adults aged 18 or over with a history of cellulitis (first or recurrent) were invited to complete a survey, take part in an interview or both. Qualitative data was analysed thematically.ResultsThirty interviews were conducted between August 2016 and July 2017. Qualitative data revealed low prior awareness of cellulitis, uncertainty around diagnosis, concern/surprise at the severity of cellulitis, and perceived insufficient information provision. People were surprised they had never heard of the condition and that they had not received advice or leaflets giving self-care information. Some sought information from the internet and found this bewildering. Two hundred and forty surveys were completed (response rate 17%). These showed that, while most people received information on the treatment of cellulitis (60.0%, n = 144), they reported receiving no information about causes (60.8%, n = 146) or prevention of recurrence (73.3%, n = 176).ConclusionThere is a need for provision of basic information for people with cellulitis, particularly being informed of the name of their condition, how to manage acute episodes, and how to reduce risk of recurrence.
