This paper aimed to analyze the analgesic effects of continuous epidural labor analgesia (ELA) at different periods and its effects on postpartum depression, maternal and infant outcomes, and maternal blood pressure. Giving birth in our hospital from September 2017 to August 2019, 119 primiparas with spontaneous delivery were enrolled and divided into an observation group (65 cases) and a control group (54 cases). Patients in the observation group received epidural block analgesia in advance, whereas those in the control group received epidural block analgesia routinely. At 25 days after delivery, breast milk samples were collected, in which miRNA-146b level was detected by PCR. The patients were compared between the two groups with respect to progress of labor, analgesic effects during 3 stages of labor, labor outcomes, adverse reactions, and levels of NO, ANP, and ET-1 in the parturients’ umbilical artery blood. Compared with those in the control group, patients in the observation group had a remarkably higher miRNA-146b level in the breast milk ( P < 0.05 ), remarkably lower average Visual Analogue Scale (VAS) scores during the active phase and the second stage of labor ( P < 0.05 ), and remarkably higher levels of NO, ANP, and ET-1 ( P < 0.05 ). There were no statistically significant differences in adverse reactions and modes of delivery between the two groups ( P < 0.05 ). ELA starting from the latent phase can improve the miRNA-146b level in maternal breast milk, alleviate labor pain of parturients, and shorten stages of labor. Therefore, our study is worthy of clinical promotion. We still need to do more experiments and use more data to conclude more scientific results in future research work.

Abstract BackgroundPostpartum depression (PPD) negatively affects the health of new mothers and can impair infant development. Both labor pain and gestational diabetes mellitus (GDM) are potential risk factors for PPD. However, the effects of epidural labor analgesia in parturients with GDM on PPD and maternal and infant outcomes have not been characterized.MethodsParturients with GDM in our hospital were assigned to the epidural group (n = 133) and no epidural (control) group (n = 130) according to their choice to receive epidural labor analgesia. The Edinburgh Postnatal Depression Scale (EPDS) was used to evaluate maternal mental status at 24 h and 42 days postpartum. Data for relative variables in the perinatal period were collected, and the potential associations of epidural analgesia with PPD and maternal and infant outcomes were analyzed by univariate analysis and multivariate logistic regression analyses.ResultsEpidural analgesia was a protective factor for PPD at 24 h postpartum (odds ratio [OR], 0.301, 95% confidence interval [CI]: 0.104–0.867, p < 0.05), and the EPDS score at 24 h post-delivery showed significant correlation with that at 42 days post-delivery (Pearson correlation coefficient = 0.527, p < 0.001). The rate of neonatal admission to the neonatal intensive care unit (NICU) for hypoglycemia was higher in the epidural group (6.92%) than in the control group (1.50%; p < 0.05). Epidural analgesia and drug-based diabetes control were independent predictors of the rate of neonate transfer to the NICU for hypoglycemia.ConclusionEpidural analgesia was a protective factor for PPD at 24 h postpartum, but associated with an increased risk of neonatal transfer to the NICU for hypoglycemia. Objective: To observe the clinical efficacy of sequential butylphthalide therapy combined with dual antiplatelet therapy in the treatment of elderly patients with acute cerebral infarction (ACI). Methods: One hundred and twenty-two elderly patients with ACI who were admitted to the department of neurology of our hospital at May 2016-August 2018 were selected grouped into a control group and an observation group by random number table method, 61 in each group. On the basis of conventional treatment, the patients in the control group were given dual antiplatelet therapy (aspirin enteric-coated tablets + clopidogrel bisulfate tablets), while the patients in the observation group were given sequential butylphthalide therapy on the basis of the control group. The clinical effects of the two groups were compared after four weeks of treatment, and the changes of National Institutes of Health Stroke Scale (NIHSS), ADL score, plasma 3-mercaptopyruvate sulphurtransferase (3-MST) and Amyloid β42 (Aβ42) levels and the occurrence of adverse reactions during treatment were recorded. Results: The clinical efficacy of the observation group was better than that of the control group (P<0.05). There was no significant difference in NIHSS and ADL scores between the two groups before treatment (P>0.05). After treatment, the NIHSS and ADL scores of the observation group were better than those of the control group (P<0.05). There was no significant difference in plasma levels of 3-MST and AB42 between the two groups before treatment (P>0.05). The level of plasma 3-MST in the observation group was higher than that in the control group, and the level of plasma Aβ42 was lower than that in the control group (P<0.05). No serious adverse reactions occurred during the treatment period in both groups. Conclusion: Butylphthalide sequential therapy combined with dual antiplatelet therapy is effective in the treatment of elderly ACI. Objective: To discuss the specific application of Zhuang Medicine Crystal Sugar Ye’An Decoction in senile skin pruritus and its effects on serum IgE detection level. Methods: 90 patients with senile skin pruritus admitted to our hospital from February 2019 to April 2020 were randomly grouped. The general treatment effect, itching, secondary skin lesion severity, serum IL-6, IgE level and adverse reactions between 45 patients treated with Chlorcyclizine Hydrochloride Tablets (The control group) and 45 patients treated with Crystal Sugar Ye’An Decoction were compared. Results: The total effective rate of the treatment in the observation group was 93.33%, much higher than that in the control group, 75.56%. The difference was verified and statistically significant (P<0.05). Before the treatment, the itching area, frequency, duration and severity of secondary skin lesions were at the same level (P>0.05). After the treatment, the above data were all reduced, and the reduction of the observation group was more significant than that of the control group, indicating the difference was statistically significant (P<0.05). Before the treatment, there was no difference in serum IL-6 and IgE test data between the two groups (P>0.05). After the treatment, the tendency all declined, and that of the observation group was more obvious, indicating the difference was statistically significant (P<0.05). During the experiment, no obvious adverse reactions were observed in both groups. Conclusion: In the treatment of patients with senile pruritus, the application of Zhuang Medicine Crystal Sugar Ye’An Decoction is not only safe and reliable, but can promote the improvement of patients’ symptoms, adjust the level of inflammatory factors, and enhance clinical efficacy.

Objective: To investigate the clinical efficacy of insulin glargine combined with acarbose in the treatment of elderly patients with diabetes. Methods: One hundred and forty-four elderly patients with diabetes who received treatment between December 2016 and December 2017 in Binzhou People’s Hospital, China, were selected and divided into a control group and an observation group, 72 each, using random number table. The control group was treated with insulin glargine, while the observation group was treated with insulin glargine combined with acarbose. The therapeutic effect, improvement of quality of life and adverse reactions were compared between the two groups. Results: After treatment, fasting blood glucose (FBG), 2h postprandial blood glucose (PBG) and glycosylated hemoglobin (Hb Alc) of the two groups were lower than those before treatment, and the decrease degree of the observation group was significantly larger than that of the control group (P<0.05). The time needed for blood glucose reaching the standard level and daily insulin dosage of the observation group were significantly lower than that of the control group, and the differences were statistically significant (P<0.05). SF-36 scale score of the observation group was significantly better than the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of insulin Glargine and Acarbose can significantly control the blood glucose level of elderly patients with diabetes, improve the biochemical indicators, and enhance the quality of life. AbstractBackgroundTo analyze the cardio-protective effects of ticagrelor in patients with acute coronary syndrome with S-T segment elevation.MethodsThe sample was 200 patients who had been diagnosed with acute coronary syndrome accompanied by diabetes Mellitus type II. Only patients having ST segment elevation before the treatment were included. Then, the subjects were further randomly divided into an observation group and a control group. The control group of 100 patients received clopidogrel; the observation group of 100 patients of ticagrelor. The serous creatine kinase CK-MB, functional cardiac indexes of left ventricular end diastolic diameter (LVDD), cardiac troponin I, ventricular ejection fraction, and relevant major adverse cardiovascular events (MACE) were compared between the two groups.ResultsOne month after a percutaneous coronary intervention (PCI) the observation group showed better results against angina, stent thrombosis, and all-cause mortality compared with those of the control subjects. Six months after treatment, both groups suffered adverse reactions. The number of patients who suffered adverse reactions in respiratory tract in the observation group was higher than in the control group. The inhibition of platelet aggregation IPA of ticagrelor was found to be significantly higher than clopidogrel, having a significant p value.ConclusionTicagrelor can effectively protect myocardial function for patients with ST-segment elevation acute coronary syndrome accompanied by diabetes and can reduce the incidence of adverse reactions..

<p><strong>Objective: </strong>To study the clinical effect on the treatment of cardiac arrhythmia, and further guide the clinical treatment. <strong>Method: </strong>From January 2011 to January 2013, 100 patients with arrhythmia were randomly divided into observation group and control group with 50 cases in each group. Observation group patients given oral Stable heart granule treatment, while control group was treated with oral propafenone treatment. The therapeutic effect and adverse reactions of the two groups were observed and compared. <strong>Results: </strong>The total effective rate of the observation group was higher than control group and incidence of adverse reactions was lower than control group, the difference was statistically significant, <em>p</em> &lt; 0.05. The use of stable heart particles in the treatment of arrhythmia produce significant effect. <strong>Conclusion: </strong>Clinical effect for Stable heart granule on arrhythmia was significant and should widely entrenched in clinical practice.</p>

Abstract Objective:The purpose of this study is to retrospectively observe the clinical efficacy and adverse reactions of docetaxel hyperthermic intraperitoneal chemotherapy (HIPEC) combined with endostar in treatment of malignant ascites. Methods:56 cases of malignant ascites admitted to Zhejiang Hospital from July 2019 and October 2020 had received no less than second-line chemotherapy. The observation group (n = 29) was treated with endostar 60mg d1,4,7 Q3W intraperitoneal injection combined with docetaxel 60mg/m2 d4 HIPEC Q3W for 2 cycles. The control group (n = 27) was just treated with docetaxel 60mg/m2 d1 HIPEC Q3W for 2 cycles. The general clinical data and qualitative data of the treatment results were processed by SPSS26.0 using χ2 test, and quantitative data were processed by t test. When P < 0.05, statistical data can be considered statistically significant.Results:There were significant differences between the observation group and the control group in objective response rate (ORR) (65.5% vs 37.0%,P=0.033),in the improvement rate of KPS (48.3% vs 22.2%, P=0.042),also in the median control time (57dvs 45d,P=0.027). The incidence of III to IV adverse reactions was low, and no treatment-related death was observed. There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Endostar combined with docetaxel HIPEC has good clinical effect on malignant ascites and can improve patients’ quality of life, and the adverse reactions can be tolerated.

Objective: To compare curative effect and safety of omeprazole under different treatment courses in treatment of children with peptic ulcer (PU, diameter ≤1.0cm) and helicobacter pylori (HP) infection and its influence on inflammatory cytokines. Methods: The study was a randomized controlled study and conducted at Baoding children’s hospital from June 2015 to June 2018. In this study 100 PU children with positive HP were chosen and classified into two groups at random. The 58 cases in the observation group were given omeprazole + amoxicillin + clarithromycin, and the antibiotics were not used two weeks later. Then, omeprazole was used to treat for two weeks. 42 cases in the control group were given omeprazole + amoxicillin + clarithromycin for two weeks. Curative effect, HP eradication rate, clinical symptoms, incidence of adverse reactions, level of serum inflammatory cytokine interleukin-6 (IL-6) and level of tumor necrosis factor-a (TNF-a) in two groups were compared. Results: Total effective rate, HP eradication rate and clinical symptom relief of observation group were better than those of control group, and the differences showed statistical significance (P<0.05). The differences of two groups in the incidence of adverse reactions had no statistical significance (P<0.05). Serum IL-6 level and TNF-a level of observation group were significantly lower than those of control group and before the treatment, and the differences had statistical significance (P<0.05). Conclusion: The application of omeprazole in treatment of PU patients with positive HP for four weeks can significantly improve PU cure rate and HP eradication rate, relieve clinical symptoms and reduce inflammatory response, so it deserves to be promoted clinically. doi: https://doi.org/10.12669/pjms.36.7.3048 How to cite this:Zhang S, Zhang Y, Dong Q, Gu F. Objective: To explore the impact of comprehensive nursing intervention on the quality of intestinal preparation in the process of intestinal preparation for hospitalized patients undergoing colonoscopy. Methods: A total of 320 patients underwent colonoscopy in our department from January to June 2021. They were grouped by random number table. There were 160 cases in the control group and 160 cases in the observation group. The adverse reactions of intestinal preparation and the cleanliness of intestinal preparation were analyzed. Results: Through comprehensive nursing intervention, it can be found that the incidence of adverse reactions in the preparation of colonoscopy in the control group was 24.38%, and that in the observation group was 13.48%. The results showed that the incidence of adverse reactions in the preparation of colonoscopy was significantly lower than that in the control group (P < 0.05), which indicates that the difference was statistically significant. In the control group, 105 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 65.63%. In the observation group, 139 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 86.88%. The cleanliness qualification rate of enteroscopy preparation was significantly higher than that of the control group, and P < 0.05, which indicates that the difference was statistically significant. Conclusion: Comprehensive nursing intervention can promote the quality of preparation of patients for colonoscopy, improve the compliance of patients and improve the accuracy of examination effect.

Abstract Objective: The purpose of this study is to compare the difference of clinical efficacy between conventional intraperitoneal chemotherapy and HIPEC, so as to explore the clinical application value and advantages of HIPEC.Design: A retrospective analysis was conducted on 80 patients with malignant ascites admitted to our hospital from June 2017 to June 2019. The general clinical data and qualitative data of the treatment results of 80 patients with malignant ascites were processed by SPSS19.0 using χ2 test and quantitative data were processed by t test. P <0.05, statistical data can be considered statistically significant.Results: 1. There was no significant change in vital signs and temperature in the observation group during the treatment, and the difference was not statistically significant 2. The short-term total effective rate of patients in the observation group was 91.11%, and the short-term total effective rate of the patients in the control group was 40%.3. There was no significant difference in the incidence of adverse reactions between the two groups of patients.Conclusion: Intraperitoneal hyperthermic chemotherapy combined with intravenous chemotherapy can significantly control malignant ascites, and has small adverse reactions, which is worthy of clinical promotion and application.
