Dermatological problems are the most widely spread skin diseases amongst human beings. They can be infectious, chronic, and sometimes may also lead to serious health problems such as skin cancer. Generally, rural area clinics lack trained dermatologists and mostly rely on the analysis of remotely accessible experts through mobile-based networks for sharing the images and other related information. Under such circumstances, poor image quality introduced due to the capturing device results in misleading diagnosis. Here, a genetic-algorithm- (GA-) based approach used as an image enhancement technique has been explored to improve the low quality of the dermatological images received from the rural clinic. The diagnosis is performed on the enhanced images using convolutional neural network (CNN) classifier for the identification of the diseases. The scope of this paper is limited to only motion blurred images, which is the most prevalent problem in capturing of the images, specifically when any of the two (device or the object) may move unpredictably. Seven types of skin diseases, namely, melanoma, melanocytic nevus, basal cell carcinoma, actinic keratosis, benign keratosis, vascular lesion, and squamous cell carcinoma, have been investigated using ResNet-152 giving an overall accuracy of 87.40% for the blurred images. Use of GA-enhanced images increased the accuracy to 95.85%. The results were further analyzed using a confusion matrix and t-test-based statistical investigations. The advantage of the proposed technique is that it reduces the analysis time and errors due to manual diagnosis. Furthermore, speedy and reliable diagnosis at the earliest stage reduces the risk of developing more severe skin problems.

Background: Skin diseases are very much prevalent in the developing countries. The pattern of skin diseases varies from country to country and even region to region within a country. The histological diagnosis is used by clinicians to aid in the management of patients & most appropriate clinical interventions. Materials and methods: The study included a total of 1040 skin biopsies from January 2016 to December 2018 in the department of pathology, DI skin health and referral centre, Kathmandu, Nepal. Results: The majority of the patients were in the age group of 31-40 years. The most common skin disease is non-infectious vesicobullous and vesicopustular disease (28.6%), followed by non-infectious erythematous papular and squamous disease (25.9%). The most common vesicobullous disease is spongiotic dermatitis (84.8%). Erythema dyschromicum perstans (31.8%) is the commonest non-infectious erythematous papular and squamous disease. The most common microbial disease is fungal infection, followed by leprosy. Among the neoplastic diseases of skin, tumors of epidermis are commonest diseases and the most common epidermal tumor is basal cell carcinoma. The commonest tumor of skin is melanocytic nevus.  The most frequent site is upper extremities. Conclusions: Eczema is predominating non-infectious vesicobullous and vesicopustular disease.  A relatively higher, prevalence of fungal infections was observed. Basal cell carcinoma is the commonest epidermal tumor and melanocytic nevus is commonest of all skin tumors.

The quality of environmental waters such as rivers is often deteriorated by various kinds of trace and unidentified chemicals despite the recent development of sewage systems and wastewater treatment technologies. In addition to contamination by particular toxicants, complex toxicity due to multi-component chemicals could be much more serious. The environmental situation in bodies of water in Japan led us to apply bioassays for monitoring the water quality of environmental waters in order to express the direct and potential toxicity to human beings and ecosystems rather than determinating concentrations of particular chemicals. However, problems arose from the fact that bioassays for pharmaceutical purposes generally required complicated, time-consuming, expert procedures. Also, a methodology for feedback of the resultant toxicity data to water environment management has not been established yet. To this end, we developed a novel bioassay based on the low-density lipoprotein (LDL) uptake activity of human hepatoblastoma cells. The assay enabled us to directly detect the toxicity of environmental waters within 4 hours of exposure. This is a significantly quick and easy procedure as compared to that of conventional bioassays. The toxicity data for 255 selected chemicals and environmental waters obtained by this method were organized by a mathematical equation in order to make those data much more effectively and practically useful to the management of environmental waters. Our methodology represents a promising example of applying bioassays to monitor environmental water quality and generating potential solutions to the toxicity problems encountered.

AbstractAlthough computer-aided diagnosis (CAD) is used to improve the quality of diagnosis in various medical fields such as mammography and colonography, it is not used in dermatology, where noninvasive screening tests are performed only with the naked eye, and avoidable inaccuracies may exist. This study shows that CAD may also be a viable option in dermatology by presenting a novel method to sequentially combine accurate segmentation and classification models. Given an image of the skin, we decompose the image to normalize and extract high-level features. Using a neural network-based segmentation model to create a segmented map of the image, we then cluster sections of abnormal skin and pass this information to a classification model. We classify each cluster into different common skin diseases using another neural network model. Our segmentation model achieves better performance compared to previous studies, and also achieves a near-perfect sensitivity score in unfavorable conditions. Our classification model is more accurate than a baseline model trained without segmentation, while also being able to classify multiple diseases within a single image. This improved performance may be sufficient to use CAD in the field of dermatology.

Human beings have always strived towards excellence and progress since time immemorial. Industrialisation, automation and technological disruptions have led to increased comfort and quality of living of human beings and enhanced output, productivity and efficiency. Consequently, of late, health and wellness are receiving increased attention globally. Stress and stress-related diseases and workplace-related ailments have significantly increased over the last few decades and gained attention from society and industrial organisations. In the recent past, in the coronavirus pandemic context, wellness has been focused upon in many countries, communities and organisations worldwide. Yoga has been part of the Indian ethos for centuries. In this article, the authors discuss the General Yoga Programme (GYP), its broad coverage, and the impact it has had on the participants in terms of their wellness. With the help of a survey of the participants and linking it to relevant literature and research in the field, the authors highlight how GYP is a useful tool for enhancing various wellness dimensions. They recommend that GYP in its simplified form can be extended to the workplace; and also, HR professionals can play a facilitative role in the process.

This paper takes water quality as an ethnographic subject. It looks at how water quality monitors in Boston make sense of the quality of water through mundane engagement with three non-human beings who they encounter during their monitoring activities: herring, bacteria and water lily. Each of these organisms suggests a different understanding of water quality for the monitors and poses a dilemma. Water quality monitors who contribute to the production of water quality data come to know water quality as through direct interactions with these beings, mediated by both sensorial experience and laboratory data. These experiences, at the same time, confuse and redraw relationships between science, water flows, non-human vitality, including that of invasive species, and people.

Vitamin B12 (VitB12) is a naturally occurring compound produced by microorganisms and an essential nutrient for humans. Several papers highlight the role of VitB12 deficiency in bone and heart health, depression, memory performance, fertility, embryo development, and cancer, while VitB12 treatment is crucial for survival in inborn errors of VitB12 metabolism. VitB12 is administrated through intramuscular injection, thus impacting the patients’ lifestyle, although it is known that oral administration may meet the specific requirement even in the case of malabsorption. Furthermore, the high-dose injection of VitB12 does not ensure a constant dosage, while the oral route allows only 1.2% of the vitamin to be absorbed in human beings. Nanocarriers are promising nanotechnology that can enable therapies to be improved, reducing side effects. Today, nanocarrier strategies applied at VitB12 delivery are at the initial phase and aim to simplify administration, reduce costs, improve pharmacokinetics, and ameliorate the quality of patients’ lives. The safety of nanotechnologies is still under investigation and few treatments involving nanocarriers have been approved, so far. Here, we highlight the role of VitB12 in human metabolism and diseases, and the issues linked to its molecule properties, and discuss how nanocarriers can improve the therapy and supplementation of the vitamin and reduce possible side effects and limits.

Daily lifestyle is a fundamental part of human life and its influence accumulates daily in the human body. We observe that a good daily lifestyle has a beneficial impact on our health; however, the actual effects of individual daily lifestyle factors on human skin diseases, especially skin cancers, have not been summarized. In this review, we focused on the influence of daily lifestyle on the development of skin cancer and described the detailed molecular mechanisms of the development or regulation of cutaneous malignancies. Several daily lifestyle factors, such as circadian rhythm disruption, smoking, alcohol, fatty acids, dietary fiber, obesity, and ultraviolet light, are known to be associated with the risk of cutaneous malignancies, malignant melanoma, squamous cell carcinoma, basal cell carcinoma, and Merkel cell carcinoma. Although the influence of some daily lifestyles on the risk of skin cancers is controversial, this review provides us a better understanding of the relationship between daily lifestyle factors and skin cancers.

Pruritus is a relatively common symptom that anyone can experience at any point in their life and is more common in the elderly. Pruritus in elderly can be defined as chronic pruritus in a person over 65 years old. The pathophysiology of pruritus in elderly is still unclear, and the quality of life is reduced. Generally, itch can be clinically classified into six types: Itch caused by systemic diseases, itch caused by skin diseases, neuropathic pruritus, psychogenic pruritus, pruritus with multiple factors, and from unknown causes. Senile pruritus can be defined as a chronic pruritus of unknown origin in elderly people. Various neuronal mediators, signaling mechanisms at neuronal terminals, central and peripheral neurotransmission pathways, and neuronal sensitizations are included in the processes causing itch. A variety of therapies are used and several novel drugs are being developed to relieve itch, including systemic and topical treatments.
