In the Department of Gastroenterology, poor compliance, psychological anxiety, fear, and other reasons result in a high rate of breach of appointment. The failure rate of colonoscopy was 6.68. This paper proposes mastering the psychological characteristics of patients and implement humanistic care services to alleviate patients’ psychological anxiety. The study found that telephone follow-up 2 days before diagnosis and treatment can let us know whether patients can come for examination and reduce the rate of nonappointment. Second, preoperative education can significantly reduce the degree of anxiety and improve the anxiety of patients. Third, the cleanliness of intestinal preparation before colonoscopy directly affects the results of colonoscopy. The incidence of missing flat adenomas due to inadequate intestinal preparation can be as high as 27%. In this paper, 7250 patients were analyzed, of which 4% failed to complete the examination, of which 32.7% were caused by unqualified intestinal preparation. The combination of telephone and network communication should be taken before colonoscopy to give education and guidance to patients, which is expected to improve intestinal preparation. To sum up, digestive endoscopic treatment technology is widely used to guide the nursing of patients after endoscopic treatment. The three effective schemes proposed in this paper can achieve higher patient satisfaction and improve the quality of nursing at the same time.

The high rate of deaths caused by serious illnesses has led the World Health Organization (WHO) to recommend palliative care that is considered to be able to improve the quality of patient’s live Palliative care or service is service for patients with serious illnesses, such as cancer (stadium or end-stage). In this activity, however, palliative care and service is extended so that it includes diseases that are not contagious but deadly (thus, contributed significantly to mortality rate), including such illnesses as diabetes, high-blood pressure, cholesterol, stroke and other similar illnesses. These illnesses are now increasingly being suffered by and become the major cause of death (caused by illnesses) amongst Indonesians. In order to increase the availability of palliative services in the target area, this community project provides training and assistance to develop women’s capacity in the villages of Kambingan and Ngembung, Cerme, Gresik, so that they are ready to volunteers for palliative care and service in their respective communities. The projects were conducted by providing material through lectures, and modules; followed by assistance to form a team of palliative case and service. At the end, this project resulted in the increasing understanding, and capacity of women (and mothers or PKK member), as the target groups about palliative care and services. In turn, the women were then ready to become volunteers for palliative care and program.abstrakTingginya tingkat kematian akibat penyakit serius membuat WHO menyarankan untuk melakukan perawatan paliatif yang dianggap dapat meningkatkan kualitas hidup pasien. Program atau layanan paliatif adalah pelayanan kepada pasien dengan penyakit berat, yaitu kanker (stadium akhir). Dalam kegiatan pengabdian masyarakat (pengmas) ini, layanan paliatif diperluas sehingga meliputi pula penyakit-penyakit yang tidak menular namun mematikan, seperti penyakit diabates, darah tinggi, kolesterol, stroke dan sejenisnya yang dewasa ini semakin banyak di derita masyarakat Indonesia. Kegiatan pengmas ini memberikan pelatihan dan pendampingan untuk mengembangkan kapasitas ibu-ibu tim penggerak dan anggota PKK di Desa Kambingan dan Desa Ngembung, Kecamatan Cerme, Kabupaten Gresik agar siap untuk menjadi relawan program paliatif di lingkungan masyarakat masing-masing. Metode yang digunakan dalam kegiatan ini adalah dengan pemberian materi melalui ceramah, pemberian modul, serta pendampingan pembentukan tim paliatif sebagai follow-up kegiatan. Hasil yang dicapai dari kegiatan pengabdian masyarakat ini adalah meningkatnya pemahaman masyarakat khususnya ibu-ibu tentang layanan paliatif, serta meningkatnya kapasitas mereka,sehingga siap menjadi relawan program paliatif.

Objective:To compare a specialized interprofessional team approach to community-based stroke rehabilitation with usual home care for stroke survivors using home care services.Methods:Randomized controlled trial of 101 community-living stroke survivors (<18 months post-stroke) using home care services. Subjects were randomized to intervention (n=52) or control (n=49) groups. The intervention was a 12-month specialized, evidence-based rehabilitation strategy involving an interprofessional team. The primary outcome was change in health-related quality of life and functioning (SF-36) from baseline to 12 months. Secondary outcomes were number of strokes during the 12-month follow-up, and changes in community reintegration (RNLI), perceived social support (PRQ85-Part 2), anxiety and depressive symptoms (Kessler-10), cognitive function (SPMSQ), and costs of use of health services from baseline to 12 months.Results:A total of 82 subjects completed the 12-month follow-up. Compared with the usual care group, stroke survivors in the intervention group showed clinically important (although not statistically significant) greater improvements from baseline in mean SF-36 physical functioning score (5.87, 95% CI -3.98 to 15.7; p=0.24) and social functioning score (9.03, CI-7.50 to 25.6; p=0.28). The groups did not differ for any of the secondary effectiveness outcomes. There was a higher total per-person costs of use of health services in the intervention group compared to usual home care although the difference was not statistically significant (p = 0.76).Conclusions:A 12-month specialized, interprofessional team is a feasible and acceptable approach to community-based stroke rehabilitation that produced greater improvements in quality of life compared to usual home care. AbstractPedicle subtraction osteotomy (PSO) is a powerful tool for the management of sagittal misalignment. However, this procedure has a high rate of implant failure, particularly rod breakages. The four-rod technique diminishes this complication in the lumbar spine. The aim of the present study is to provide a case report regarding PSO and four-rod technique stabilization in the treatment of short-angle hyperkyphosis in the thoracolumbar (TL) junction. The authors describe the case of a patient with TL hyperkyphosis secondary to spinal tuberculosis treated with L1 PSO and fixation with a four-rod technique. There were no major surgical complications. The self-reported quality of life questionnaires (the Short-Form Health Survey 36 [SF-36] and the Oswestry disability index) and radiological parameters were assessed preoperatively, as well as 6, 12 and 24 months after surgery, and they showed considerable and sustained improvements in pain control and quality of life. No hardware failure was observed at the two-year follow-up.

ObjectivesPatients with haematological malignancies (HM) receive more aggressive treatments near the end-of-life (EOL) than patients with solid tumours. Palliative care (PC) needs are less widely acknowledged in patients with multiple myeloma (MM) than in other HM. The main objective of our study was to describe EOL care and PC referral in a population of older patients with MM.MethodsWe retrospectively included deceased inpatients and outpatients with an MM previously diagnosed at the age of 70 and over in two tertiary centres in France. We reported EOL characteristics regarding treatments considered to be aggressive—antimyeloma therapies, hospitalisations, blood product transfusions, intensive care units (ICUs) or emergency admissions—and PC referral.ResultsWe included 119 patients. In their last month of life, 75 (63%) were hospitalised for fever, pain, asthenia, anaemia or bleeding, 49 (41%) were admitted in the emergency department and 12 (10%) in ICU, 76 (64%) still received antimyeloma therapy and 45 (38%) had at least two transfusions. Only 24 (20%) received PC intervention for pain, global care, family support, anxiety, social care or confusion. Median follow-up until death was 20 days.ConclusionsOur study found a high rate of hospitalisations and antimyeloma therapies in the last month of life. The PC referral rate was low, often once specific treatments were stopped. Our results suggest the need for more effective collaboration between PC teams and haematologists in order to respond to the specific needs of these patients and to improve their quality of care at EOL.

AIM to show on the results of our study the influence of the quality of preparationfor videocapsule endoscopy (VCE) on the accuracy of diagnosis of diseases of the small and large intestine. MATERIALS AND METHOD. During the period from September 2014 to December 2016, a videocapsule study was performed on 100 patients of treatment at the State Scientific Center of Coloproctology. Final data processing was carried out based on the results of a survey of 96 patients (52 men and 44 women aged 18 to 78 years). To adequately prepare patients for the study, we used a specific scheme with the use of a «splitdose» of intestinal cleansers based on polyethylene glycol and stimulation solutions. The quality of preparation of the small and / or large intestine for the study was assessed according to the scale of Leighton J.A., Rex D.K. RESULTS. The completeness of the study and the full examination (the ability to visualize all the sections of the small and / or large intestine) are important criteria for conducting the VCE. In our study, a complete study of the small and large intestine was performed in 87 (91 %)patients, and a full study was performed in 86 (90 %) of 96 patients. In the study of the small and large intestine using small intestine or large intestinal videocapsules, in 87 (91 %) patients the intestinal preparation was rated «good» or «excellent». CONCLUSION. The experience of our study showed that in order to obtain reliable and informative results, careful follow-up of the methodology of preparation for the study should be done.

<p><strong>Objective: </strong>To summarize and discuss the nursing of debridement and suturing operation of emergency trauma in children. <strong>Methods:</strong> Retrospect on the preoperative, intraoperative and postoperative nursing of 125 cases on debridement and suturing operation of emergency trauma in children between January and December 2012. <strong>Results:</strong> Through a telephone follow-up lasting 3−4 weeks after the operation, there was no complications such as wound infection, wound dehiscence and delayed union among 125 cases of this group, and the wounds were primary healed. <strong>Conclusion:</strong> Psychological nursing throughout the preoperative, intraoperative and postoperative process, sufficient preoperative preparation, strict aseptic intraoperative operation and close observation, adept nursing cooperation and effective anesthesia can shorten the operation time, reduce the complications and improve the quality of nursing, which are the keys to a successful operation.<strong></strong></p>

Introduction. The matter of the replacement of acetabular defects is becoming increasingly relevant today. In their clinic the authors used custom-made implants with trabecular coating for replacement of defects in the acetabulum where and when it was necessary to ensure extended fixation of the component. Purpose — to evaluate short-term clinical and functional outcomes, osteointegration of porous coated custom-made implants, the overall survival of structural components and to identify the causes of failures and complications. Materials and Methods. During the study the authors interviewed and evaluated radiographs of 48 patients operated from October 2015 to June 2018. The patients were interviewed before and after the surgery using the Oxford hip joint scale, quality of life EQ-5D and visual analogue pain scale. Radiographs were evaluated for a period of at least 12 months for the signs of osteointegration. Results. The average OHS scores increased from 14.9 (±7) to 37.6 (±7) (p<0.01). Quality of life according to the EQ-5D index increased from 0.2 (±0.2) to 0.7 (±0.2) (p<0.01). The average value by general health scales: before operations — 49 (±17) and 73 (±18) scores after surgeries (p<0.01). The average VAS pain score decreased from 73 (±10) to 19 (±19) points (p<0.01). Osteointegration was observed in 98 percent of cases. Migration of the implant with fracture of a flange was observed in one case. Conclusions. In the short-term follow up the use of custom-made implants significantly improves hip joint function and the patients’ life quality. Custom-made implants enable primary reliable fixation in case of complex acetabular defects. High rate of osteointegration is observed for custom-made implants with porous coating within at least 12 months. Longer follow-up is needed to evaluate long-term results.

The implementation of supervising the head of a room in one hospital in Semarang has not been optimal due to the absence of a schedule, assessment instruments, guidance, documentation of supervision results, and standard operating procedures (SOP) supervision. Supervision activities are incidental in accordance with needs and have not been implemented in a structured and well-documented manner. The writing of this article aims to find out the description of the implementation of head supervision in a hospital in Semarang. This study uses descriptive research design. The subjects in this study were all heads of inpatient rooms. The object of this research is the implementation of the supervision of the head of the room according to the SOP and the results of documentation of the implementation of supervision. The instrument in this study used a draft sheet for evaluation of the superficial room leader evaluation. Shows that there is a change in the implementation of supervision based on the SOP before and after the dissemination of supervision is carried out to the head of the room. Documented supervision results can help the head of the room to see the extent of the ability of staff and can jointly improve capabilities, correct errors in improving the quality of nursing care services. The implementation of supervising the head of a room in one of the Semarang hospitals needs to be improved, especially in terms of post-supervision documentation, development of thematic supervision themes, and structured supervision scheduling.

Abstrak Kualitas perawat ditentukan oleh kompetensi hard skills dan soft skills. Caring sebagai bagian dari soft skills adalah esensi mendasar pada profesi perawat. Penilaian pasien mengenai soft skills caring perawat adalah indikator dari kualitas pelayanan keperawatan. Tujuan penelitian untuk mengembangkan model pelatihan soft skills caring dan mengidentifikasi model tersebut terhadap kualitas keperawatan dan kepuasan pasien. Desain pra-eksperimental dengan pretest-posttest tanpa kontrol dengan melibatkan 53 perawat dan 53 pasien pada dua rumah sakit swasta di Kota Bandung. Instrumen penelitian diadaptasi dari Caring Nurse Patient Interactions Scale (CNPI) dan kepuasan pasien diadaptasi dari patient satisfaction with nursing care. Intervensi terdiri dari pemberian materi selama 3 hari, post pelatihan 2, 4 dan 6 minggu. Analisis data dilakukan untuk melihat perubahan penilaian soft skills caring perawat serta kepuasan pasien sebelum dan sesudah intervensi-pelatihan menggunakan uji paired t-test. Analisis General Linier Model Repeated Measure (GLM-RM) dipergunakan untuk analisis follow-up 4 dan 6 minggu. Hasil penelitian menunjukkan model pelatihan soft skills caring terbukti efektif meningkatkan penilaian perawat dan kepuasan pasien, serta dapat dimanfaatkan bagi perawat di rumah sakit. Soft skill of caring was the basic competence of nurses, it was affecting patient satisfaction. This study aimed to: a) develop the model of training soft skills caring, b)identify the training model for increasing the quality of nursing care and patient satisfaction. The study was pretest-posttest pre-experimental design without control. The sampples including 53 nurses and 53 patients at two private hospitals in Bandung, West Java Province, Indonesia. The instrument was adapted from Caring Nurse Patient Interactions Scale (CNPI) while patient satisfaction assesment was adapted from patient satisfaction with nursing care. The interventions was soft skills training for 3 days, while the soft skill were assesed at in 2, 4 and 6 weeks post intervention. The pre and 2 weeks post intervention were analysed using paired t-test, while the 4 and 6 weeks follow-up soft skill post itervention was analyzed by GLM-RM. The results show that soft skill training are effective for improving nurse’s competence and increasing patient satisfaction. Futher training can be implemented to increase quality of nursing care and patients satisfaction. Keywords: soft skills, training, quality of nursing, patient satisfaction
