Ancillary techniques play an essential role in pulmonary cytopathology. Immunoperoxidase and special stains are by far the most common ancillary techniques used in cytopathology; however, the role of molecular diagnosis is growing, especially in the fields of pulmonary oncology and infectious disease. In this article, we review the uses of ancillary techniques in lung tumor diagnosis, lung tumor classification, predictive marker determination, primary versus metastasis differential diagnosis, and infectious organism detection.

SUMMARYChronic pain is a phenomenon with important psychiatric aspects from a diagnostic as well as a therapeutic point of view. The place of chronic pain in the different versions of the Diagnostic and Statistical Manual of Mental Disorders, and the differential-diagnosis are critically discussed. The comorbidity with depression, anxiety disorders, substance abuse and personality disorders is extensively treated. Finally, the essential role of the psychiatrist in the multidisciplinary therapeutic approach of these patients is emphasised.

A 55-year-old woman had a right-sided neck mass overlying the angle of the mandible, which proved to be a chemodectoma. The occurrence, differential diagnosis, and relatively high frequency of incorrect preoperative diagnosis of carotid body tumors is reviewed. The essential role of angiography to substantiate the diagnosis is discussed.

This chapter provides a review of infectious disease to date and the challenges they may present in the future. The main pandemics that have driven the history of humanity are described, from the first to be recorded in 3180 BC to more recent ones such as AIDIS, SARS and others associated with emerging pathogens. The essential role of emerging scientific specialisms (particularly microbiology, public health and sanitary engineering) to our understanding of the causes of these diseases (and how they may be better monitored, controlled and prevented) is presented. Globalization and climate change, determining factors for the ecology of infectious diseases and their emergence and re-emergence, are discussed and point to the urgent need for research to deal with these threats that continue to have a significant impact on human development and wellbeing.

An Ayurveda is an ancient system of medicine. The primary purpose of Ayurveda is to maintain the health of healthy individuals and cure the disease of the diseased one. 'Rasayanchikitsa’ plays a vital role in keeping the health of an individual. Rasayanchikitsa also plays an essential role in infectious disease which spreads directly or indirectly from man to man, animal to animal or from the environment to animal or man. Rasayana promotes physical health, improve dhatus (tissues), acts as immunomodulation and rejuvenate the system. Rasayana, by its name, means obtaining good qualities of rasa & nourishing the rasadidhatus(tissues). The excellent quality of rasa directly governs the state of health of individuals. Rasayana therapy deals with the delaying process of ageing, prolongation of life and curing the disease. Acharya Sushruta says, "for better results, Rasayan therapy should be done after Panchakarma procedures like Vaman, Virechan and Basti depending on the individual's Vata, pitta and Kapha predominance. Strength, improving complexion, prolongation of life, energy, enhancing memory, immunity booster, etc. is obtained from Rasayan therapy. Rasayan drugs help by modulating the neuro-endocrine-immune system and also a rich source of antioxidants. Review of the role of rasayan in the prevention of new infectious disease like Covid 19.

SummarySynthetic peptides (TRAP or Thrombin Receptor Activating Peptide) corresponding to at least the first five aminoacids of the new N-terminal tail generated after thrombin proteolysis of its receptor are effective to mimic thrombin. We have studied two different TRAPs (SFLLR, and SFLLRN) in their effectiveness to induce the different platelet responses in comparison with thrombin. Using Indo-1/AM- labelled platelets, the maximum rise in cytoplasmic ionized calcium was lower with TRAPs than with thrombin. At threshold concentrations allowing maximal aggregation (50 μM SFLLR, 5 μM SFLLRN and 1 nM thrombin) the TRAPs-induced release reaction was about the same level as with thrombin, except when external calcium was removed by addition of 1 mM EDTA. In these conditions, the dense granule release induced by TRAPs was reduced by over 60%, that of lysosome release by 75%, compared to only 15% of reduction in the presence of thrombin. Thus calcium influx was more important for TRAPs-induced release than for thrombin-induced release. At strong concentrations giving maximal aggregation and release in the absence of secondary mediators (by pretreatment with ADP scavengers plus aspirin), SFLLRN mobilized less calcium, with a fast return towards the basal level and induced smaller lysosome release than did thrombin. The results further demonstrate the essential role of external calcium in triggering sustained and full platelet responses, and emphasize the major difference between TRAP and thrombin in mobilizing [Ca2+]j. Thus, apart from the proteolysis of the seven transmembrane receptor, another thrombin binding site or thrombin receptor interaction is required to obtain full and complete responses.

SummaryThe hypothesis that platelet ADP is responsible for collagen-induced aggregation has been re-examined. It was found that the concentration of ADP obtaining in human PRP at the onset of aggregation was not sufficient to account for that aggregation. Furthermore, the time-course of collagen-induced release in human PRP was the same as that in sheep PRP where ADP does not cause release. These findings are not consistent with claims that ADP alone perpetuates a collagen-initiated release-aggregation-release sequence. The effects of high doses of collagen, which released 4-5 μM ADP, were not inhibited by 500 pM adenosine, a concentration that greatly reduced the effect of 300 μM ADP. Collagen caused aggregation in ADP-refractory PRP and in platelet suspensions unresponsive to 1 mM ADP. Thus human platelets can aggregate in response to collagen under circumstances in which they cannot respond to ADP. Apyrase inhibited aggregation and ATP release in platelet suspensions but not in human PRP. Evidence is presented that the means currently used to examine the role of ADP in aggregation require investigation.
