Many healthcare professionals and professional societies are demanding action to counter ‘burnout’, especially in the acute care medical specialties. This review is intended to empower this laudable ‘call to arms’, while also validating concerns that have been raised about how we typically define, measure and counter this important issue. This review aims to advance the discussion, dispel common misconceptions, add important nuance, and identify common ground. We also encourage the ideas contained within the military term ‘occupational stress injury’, which include a cultural shift away from blame and stigmatization, and towards shared responsibility and empathy. We also outline why mandatory testing can be troublesome and why interventions should be tailored to individuals. While the need for immediate action may seem self-evident, we wish to mitigate the real possibility that good intentions could make a perilous situation worse. ‘Burnout' matters, but how individuals and organizations go forward matters even more.

Objectives: The coronavirus pandemic has forced healthcare staff across all medical specialties to adapt new and different ways of working. A new approach has been set up in the Acute Referral Clinic (ARC) at Musgrove Park Hospital and a survey has been conducted to measure the impact of the new method on patient and healthcare professionals’ satisfaction with the new service. Methods: A telephone-based consultation was introduced in ARC at Musgrove Park Hospital in March 2020 and patients were instructed to fill out a questionnaire containing eight items using a Likert Scale 1 (‘very poor/disagree’) to 4 (‘very good/strongly agree’) plus two boxes for open positive and negative comments respectively. Likewise a questionnaire was designed in order to assess the healthcare professionals’ satisfaction using the new approach. Data collection took place over a two month period between the end of March 2020 and end of May 2020. The data underwent quality control and was analysed using descriptive statistics. Results: Patient responses illustrated high satisfaction scores with an overall rating of very good (89.4%). The healthcare professionals’ rating of the service was good (28.6% – ‘very good/strongly agree’, 57.1% – ‘good/agree’). The safety rating of the new approach was overall rated ‘very good’ with 90.4% and 71.4% of patients and healthcare professionals respectively. Conclusions: The telephone consultations introduced in the wake of COVID-19 are well accepted by both patients and doctors. There are some limitations of the approach, foremost being consultation time and clinic space but these do not outweigh the general benefit of this format amidst a pandemic setting.

Healthcare professionals undergo high levels of occupational stress as a result of their working conditions. Thus, the aim of this study is to develop a model that focuses on healthcare professionals so as to analyze the influence that job demands, control, social support, and recognition have on the likelihood that a worker will experience stress. The data collected correspond to 2,211 healthcare workers from 35 countries, as reported in the sixth European Working Condition Survey (EWCS). The results obtained from this study allow us to infer stress under several working condition scenarios and to identify the more relevant variables in order to reduce this stress in healthcare professionals, which is of paramount importance to managing the stress of workers in this sector. The Bayesian network proposed indicates that emotional demands have a greater influence on raising the likelihood of stress due to workload than do family demands. The results show that the support of colleagues, in general, has less effect on reducing stress than social support from superiors. Furthermore, the sensitivity analysis shows that, in high-demand and low-control situations, recognition clearly impacts stress, drastically reducing it.

Stress was considered by the United Nations as the disease of the 20th Century, being present in more than 90% of the world population, including military police officers. This group of professionals has been in the front-line of the pandemic caused by the Coronavirus and is exposed to daily situations of high emotional stress and traumatic work events, which can cause several health problems, namely occupational stress, sleep problems and obesity. This study aims to conduct a narrative review of the influence of psychosocial environment on the development of occupational stress and consequences on the military police officer´s health. Two platforms, namely PubMed and Google Scholar were used to search for relevant scientific publications on the topic; 148 publications from 2010 to 2020 were analyzed, 71 of which were included in this review. It is estimated that 25% to 35% of the global burden of disease may be due to environmental factors. The stress prevalence in military police officers is higher than the average in the civilian population worldwide; due to an alternate work schedule that does not always respect the circadian characteristics of each individual, and with direct influence on sleep quality and obesity incidence. Public health policies should be taken in the short term in order to improve life quality of these professionals.

Healthcare organizations are commonly known to be high-stress environments due to the life-or-death nature of the work. Thus, examining the quality of life in healthcare professionals is of significance to the field. Quality of life is a common measurement in healthcare, typically in determining the effectiveness of treatment on patients. But, quality of life in healthcare workers is often overlooked. A review of the literature was conducted regarding factors that impact quality of life of workers in healthcare environments. The areas of occupational stress, burnout, and workplace violence were explored in depth in order to understand the potential negative consequences of each on quality of life in healthcare professionals. In comparison to other industries, healthcare professionals are at an increased risk for occupational stress, burnout, and workplace violence. Effectively managing these areas could have a positive impact on quality of life in healthcare workers.

Abstract Background Despite the devastating consequences of Traumatic brain injuries (TBIs), TBI misconceptions are common among healthcare professionals. As an essential member of multi-professional team providing TBI care, it is important that nurses have correct information and adequate skills to achieve the best possible outcomes for TBI. For example, some common misconceptions about TBIs are that a second blow to the head can improve memory functioning and wearing seatbelts can cause as many brain injuries as it prevents. In India, perhaps such misconceptions towards TBI among nursing professionals were not yet documented. As nursing students form the future health workforce, understanding TBI misconceptions among nursing students in resource-limited settings like India will provide useful information for strengthening the nursing curricula for improved care and rehabilitation of TBIs. Methods We used a cross-sectional survey to study the TBI misconceptions among nursing students in India. A Common Misconceptions about Traumatic Brain Injury (CM-TBI) questionnaire was administered to 154 nursing students from a nursing college of a tertiary care neuro-centre in India. The mean percentage of misconceptions were calculated for 7-domains of CM-TBI. T-test for independent samples and ANOVA were used to study the association of misconception with socio-demographic variables using total score for each respondent. Results Of the 143 nursing students who completed the survey, majority of them were female (97%) and in the 19-20 year age-group (95.1%). Domain on brain damage (81.1%) had highest rate, while amnesia domain (42.0%) had lowest rate of misconception. The overall mean-score was 22.73 (Standard Deviation: 4.69) which was significantly higher than the median score of 19.5. The study did not show significant differences on overall misconceptions about TBI for any of the socio-demographic characteristics. Conclusions Misconceptions about TBIs were common among nursing students and it was pervasive irrespective of age, gender, place of residence and year of education. A need to strengthen nursing curriculum in the area of TBIs has been emphasized for improved care and management of TBIs. The study findings also suggest the need for understanding such misconceptions among other healthcare professionals involved in TBI care.

IntroductionThe Journal of the Royal Army Medical Corps (JRAMC) is published with the aim of propagating current knowledge and expertise while also acting as institutional memory for the practice of medicine within the military. This work aimed to examine how the interests of the JRAMC, and by inference the Army Medical Services, have changed over time as reflected by the articles published in the journal.MethodsA text mining analysis of the titles of all published articles in the JRAMC between 1903 and 2019 was performed. The most commonly used terms were identified and their relative frequency over the decades analysed to identify trends. Article content and contemporary events were compared with the observed trends to identify explanatory events and themes of interest.ResultsMedical topics of interest centred around specific infectious diseases, particularly during the early/mid-20th century, and trauma and battle injury. The medical specialties of surgery, anaesthetics and mental health were all well represented in nearly all decades, while primary care only came to prominence as a named specialty from the 1960s onwards. India, France, Egypt and wider Africa were the most commonly cited geographical regions, although there were spikes of interest associated with specific conflicts in the Falklands, Bosnia, Afghanistan and Iraq.ConclusionThe interests of the JRAMC have changed considerably over the years primarily driven by the geopolitical interests of Britain—in particular its colonial interests and the conflicts it has been involved in, but also by medical advances seen in contemporary society.

Medical conference organisers have increasingly used social media to improve interaction and learning amongst healthcare professionals. Despite the increasing use of Twitter at critical care conferences, there remains considerable debate as to its impact and ability to generate meaningful discussions beyond the duration of the conference itself. We aim to analyse the trend in Twitter use at international critical care conferences between 2014 and 2017. Fifteen major, international critical care conferences were identified spanning 2014–2017. They represented the annual congresses of the leading critical care professional societies including the first critical care conference to incorporate social media, Social Media and Critical Care. There has been an increased utilisation of social media at all the conferences analysed. This is reflected both in the number of users and the number of tweets. Tweets from the official conference twitter account contribute only a small proportion to the overall number with the exception of the International Fluid Academy. The potential benefits of social media have resulted in a sustained increased in its use at critical care conference between 2014 and 2017. Our analysis provides a better understanding on the use of social media at critical care conference. Further studies are needed to ascertain if this increase in use translates to enhance learning and patient care amongst colleagues.

Starling from the premise that communication regulation can be made up of informal shared systems of human belief, action and habit, this paper explores the informal political belief systems that helped regulate the Internet during its gestation, before 1984. The article focuses on the odd fact that a decentralised communication system with a strikingly libertarian ethos was created within a military-oriented research and development enterprise famous for its hierarchical, authoritarian culture and organisation. Broadly, my argument is that the larger framework of Internet development was ‘corporate liberal’ — that is, based on the theory that non-profit structures are necessary for advanced forms of experimental technological innovation, but that practical implementation is then best left to the private sector. Within that larger framework, however, a shift in the cultural habits and values within the community of computer scientists between the late 1960s and the early 1980s, partly under the influence of the Viernam War-era counterculture, allowed a distinctive vision of computer communication to take hold: computer communication as a horizontal form of collaboration. This cultural shift then helps explain how a technological system born in the heart of the military–industrial complex came to embody distinctly non-military values.
