Limited Adverse Occurrence Screening: A Program with Significant Benefits for the Medical Record Department

Quality is a major issue in industry. However, the performance of hospitals is predominantly measured by quantity. There is little accurate measurement of, and control over, the quality of patient care provided. Traditional medical quality assurance methods do not meet the basic criteria of an effective control system as defined in management theory. Occurrence screening is a method of medical quality control that overcomes many of these deficiencies. It detects adverse patient occurrences by screening medical records using outcome criteria and selective medical record review. The implementation of an occurrence screening program using a small number of criteria and retrospective review in the Medical Record Department of a 200 bed base hospital is described. Screening has been integrated into daily work practices in an efficient and cost effective manner. Medical record staff have become more aware of the importance of complete documentation and the profile of the department in the hospital has risen. Significant patient care problems have been detected by the screening process.

This paper presents the General Electric Aerospace Control Systems Department (ACSD) approach to hardware screening using random vibration as a stimulus. The proposed techniques were developed to achieve a screening program that is cost effective and is supportable in dollar payoff with increased productivity. Cost savings are realized by avoiding the assignment of a costly combined environment facility to each product line and reducing the number of test cycles required in the screening process.

An explosion of knowledge and technology is revolutionizing medicine and patient care. Novel testing must be brought to the clinic with safety and accuracy, but also in a timely and cost-effective manner, so that patients can benefit and laboratories can offer testing consistent with current guidelines. Under the oversight provided by the Clinical Laboratory Improvement Amendments, laboratories have been able to develop and optimize laboratory procedures for use in-house. Quality improvement programs, interlaboratory comparisons, and the ability of laboratories to adjust assays as needed to improve results, utilize new sample types, or incorporate new mutations, information, or technologies are positive aspects of Clinical Laboratory Improvement Amendments oversight of laboratory-developed procedures. Laboratories have a long history of successful service to patients operating under Clinical Laboratory Improvement Amendments. A series of detailed clinical examples illustrating the quality and positive impact of laboratory-developed procedures on patient care is provided. These examples also demonstrate how Clinical Laboratory Improvement Amendments oversight ensures accurate, reliable, and reproducible testing in clinical laboratories.

Objectives- A woman is pivot of a family, but they hardly pay attention to their health. The present study aimed to screen the women for common health problems under one roof. A programme “Swasth Mahila, Swasth Pariwar” was conducted in a small hospital set up and cost effective manner. Material and Methods- 408 women, above 18 years were screened for the study after obtaining their consent, 294 were enrolled and were subjected to a questionnaire regarding sociodemographic factors, dietary habits and medical history. Anthropometry and clinical examination was followed by laboratory investigations including Hb%, TLC, DLC, P/S for anaemia, Urine analysis, Blood Glucose. Vaginal cytology with special staining in patients complaining of vaginal discharge (n=128). After screening, patients were referred to separate special clinics accordingly for homoeopathic treatment and follow up. Collaboration with family planning department was done to educate them about family planning practices. Results- The prevalence of anaemia was 40.14% (mean haemoglobin 10.1g/dl). 64.4% had mild, 33.1% had moderate and 2.5% had severe anaemia. 92 % women had iron deficiency anaemia. The prevalence of diabetes was 11.57% and pre-diabetes was 28.9 %. 13 new diabetic and 35 UTI cases were identified. 45.3% women had bacterial vaginosis, 10.9% had trichomoniasis, 17.2% had candidiasis and 0.8% had candidiasis with bacterial vaginosis. All the facilities including consultation, medical examination and laboratory testing were free of charge. The expenditure on laboratory testing for each patient was Rs 300 (approx). Conclusion- This programme helped to identify nutritional, lifestyle disorders and to generate awareness to improve the sexual health status of women in a cost effective manner.Int. J. Appl. Sci. This chapter reflects on the core concept of the digital twin, evaluates the state-of-the-art modelling and analysis technologies, and presents a pragmatic approach to develop high-fidelity digital twin for large complex enterprises.

There are a multitude of reasons that a teletinnitus program can be beneficial, not only to the patients, but also within the hospital and audiology department. The ability to use technology for the purpose of tinnitus management allows for improved appointment access for all patients, especially those who live at a distance, has been shown to be more cost effective when the patients travel is otherwise monetarily compensated, and allows for multiple patient's to be seen in the same time slots, allowing for greater access to the clinic for the patients wishing to be seen in-house. There is also the patient's excitement in being part of a new technology-based program. The Gulf Coast Veterans Health Care System (GCVHCS) saw the potential benefits of incorporating a teletinnitus program and began implementation in 2013. There were a few hurdles to work through during the beginning organizational process and the initial execution of the program. Since the establishment of the Teletinnitus program, the GCVHCS has seen an enhancement in patient care, reduction in travel compensation, improvement in clinic utilization, clinic availability, the genuine excitement of the use of a new healthcare media amongst staff and patients, and overall patient satisfaction.

Three levels of planning can be distinguished in grassland farming: strategic, tactical and operational. The purpose of strategic planning is to achieve a sustainable long-term fit of the farm business with its physical, social and financial environment. In pastoral farming, this essentially means developing plans that maximise and best match pasture growth with animal demand, while generating sufficient income to maintain or enhance farm resources and improvements, and attain personal and financial goals. Strategic plans relate to the whole farm business and are focused on the means to achieve future needs. They should be routinely (at least annually) reviewed and monitored for effectiveness through key performance indicators (e.g., Economic Farm Surplus) that enable progress toward goals to be measured in a timely and cost-effective manner. Failure to link strategy with control is likely to result in unfulfilled plans. Keywords: management, performance

The wastewater from the city of Thessaloniki is discharged without treatment to the nearby inner part of the Thessaloniki Gulf. The existing, since 1989, treatment plant offers only primary treatment and did not operate since the expected effluent quality is not suitable for safe disposal to the available recipients. Upgrading of the plant for advanced biological treatment, including seasonal nitrogen removal, is due from 1995. In the mean time, after minor modifications completed in February 1992, the existing plant was put into operation as a two-stage chemical-biological treatment plant for 40 000 m3 d−1, which corresponds to about 35% of the total sewage flow. The operational results obtained during the two years operation period are presented and evaluated. All sewage and sludge treatment units of the plant perform better than expected, with the exception of the poor sludge settling characteristics, due to severe and persistent bulking caused by excessive growth of filamentous microorganisms, particularly M. Parvicella. Effective control of the bulking problem could lead to more cost-effective operation and increased influent flows.

To date, surface plasmon resonance (SPR) biosensors have been exploited in numerous different contexts while continuously pushing boundaries in terms of improved sensitivity, specificity, portability and reusability. The latter has attracted attention as a viable alternative to disposable biosensors, also offering prospects for rapid screening of biomolecules or biomolecular interactions. In this context here, we developed an approach to successfully regenerate a fiber-optic (FO)-SPR surface when utilizing cobalt (II)-nitrilotriacetic acid (NTA) surface chemistry. To achieve this, we tested multiple regeneration conditions that can disrupt the NTA chelate on a surface fully saturated with His6-tagged antibody fragments (scFv-33H1F7) over ten regeneration cycles. The best surface regeneration was obtained when combining 100 mM EDTA, 500 mM imidazole and 0.5% SDS at pH 8.0 for 1 min with shaking at 150 rpm followed by washing with 0.5 M NaOH for 3 min. The true versatility of the established approach was proven by regenerating the NTA surface for ten cycles with three other model system bioreceptors, different in their size and structure: His6-tagged SARS-CoV-2 spike fragment (receptor binding domain, RBD), a red fluorescent protein (RFP) and protein origami carrying 4 RFPs (Tet12SN-RRRR). Enabling the removal of His6-tagged bioreceptors from NTA surfaces in a fast and cost-effective manner can have broad applications, spanning from the development of biosensors and various biopharmaceutical analyses to the synthesis of novel biomaterials.

AbstractBackgroundIn Cuba, no screening program for cystic fibrosis (CF) has been implemented yet. The ultramicro enzyme-linked immunosorbent assay (UMELISA)® TIR NEONATAL has been developed for the measurement of immunoreactive trypsin (IRT) in dried blood spots on filter paper. The analytical performance of the kit was evaluated in the national network of laboratories.MethodsNewborn dried blood samples (DBS) were evaluated in 16 laboratories. An IRT/IRT/DNA protocol was followed using a cut-off value of 50 ng/mL. The mean, median and percentiles of the distribution were calculated and a two-sample t-test with unequal variance was used for statistical analysis. Influence of perinatal factors on IRT levels was analyzed.ResultsFrom January to June 2018, 6470 newborns were studied, obtaining a mean IRT value of 12.09 ng/mL (ranging 0–358 ng/mL) and a median of 8.99 ng/mL. Fifty-two samples (0.78%) were above the cut-off level and 16 samples (0.24%) were elevated in the re-screening process. One of them was confirmed positive by molecular biology (phe508del/c.3120 + 1G > A), constituting the first newborn screened and diagnosed early in Cuba. Second DBS samples were collected on average at 14 days and processed in the laboratory at 16 days of birth. Significant differences were observed (p < 0.05) when evaluating the influence of gender, birth weight (BW) and gestational age (GA) on the IRT values. Lower IRT concentrations were found in samples processed after 10 days of collection.ConclusionsThe performance of UMELISA® TIR NEONATAL in the laboratories has been satisfactory; hence CF newborn screening (NBS) was extended throughout the country from January 2019.
