[Formula: see text] [Formula: see text] [Formula: see text] The formation of a wide range of excessive scars following various skin injuries is a natural consequence of healing. Scars resulting from surgery or trauma affect approximately 100 million people per annum in the developed world and can have profound physical, aesthetic, psychological and social consequences. Thus, scar treatment is a priority for the plastic surgeon. We aim to explore new approaches to the management of such scarring. The senior authors current use of laser technology, chemotherapeutic agents, pharmacotherapy and cryosurgery will be reviewed. This is placed in the context of the current literature and evidence base and is illustrated with case studies, starting with burns scars in part I, and focusing on keloid and hypertrophic scars in part II, acne scars in part III and finally pigmented scars in part IV. In Part I we focus on burns scar treatment with fractional ablative 10,600 nm wavelength carbon dioxide (CO2) laser therapy.

Thus, scar treatment is a priority for patient and physician alike. Laser treatment plays an important role in scar management with additional support from ancillary modalities. Subsequent to part 1: Burns scars, part 2 focuses on our strategies and literature review of treatment of keloid, hypertrophic, pigmented and acne scars where lasers are used in conjunction with other measures, and illustrated with case studies.

The horse has been used as a therapeutic medium since the time of the ancient Greeks and Xenophon is quoted as saying that ‘the best thing for the inside of a man is the outside of the horse’ (Longden 1998, p44). Man has been using horses for more than 3,500 years as draught horses or for riding; therefore, the man-horse relationship may be perceived ‘as a close psychophysical attachment’ (Schulz 1997, p65). The horse and its environment has been and continues to be used as a therapeutic tool for people with a wide range of needs and abilities, as is evidenced in the literature. This paper identifies some of the benefits and limitations of therapeutic horse riding and its relationship with occupational therapy, through a review of the literature and the use of case summaries from the author's personal experience.

Abstract. Intimate partner violence (IPV) is a global social concern: many women are affected by this phenomenon and by the difficulty of putting an end to it. This review of the literature aims to identify help-seeking facilitating and inhibiting factors in response to IPV. It was carried out on the PsycINFO and Medline databases using the following keywords: “intimate partner violence,” “domestic violence,” “help-seeking,” and “help-seeking barrier.” Ninety out of 771 eligible publications were included on the basis of inclusion criteria. The results highlight that (1) research on this phenomenon is very recent and underdeveloped in Europe, (2) theoretical and conceptual frameworks are poorly developed and extended, (3) there is a significant impact of violence characteristics (e.g., severity, type) on help-seeking, and (4) help-seeking is a complex and multifactorial process influenced by a wide range of factors simultaneously individual and social. To conclude, these findings lead us to propose a psychosocial conceptualization of the help-seeking process by indicating how the levels of explanation approach in social psychology can be applied to this field of research in order to increase our understanding of this phenomenon.

Background: In spite of the availability of various treatment approaches including surgery, radiotherapy, and hormonal therapy, the steroidal aromatase inhibitors (SAIs) play a significant role as chemotherapeutic agents for the treatment of estrogen-dependent breast cancer with the benefit of reduced risk of recurrence. However, due to greater toxicity and side effects associated with currently available anti-breast cancer agents, there is emergent requirement to develop target-specific AIs with safer anti-breast cancer profile. Methods: It is challenging task to design target-specific and less toxic SAIs, though the molecular modeling tools viz. molecular docking simulations and QSAR have been continuing for more than two decades for the fast and efficient designing of novel, selective, potent and safe molecules against various biological targets to fight the number of dreaded diseases/disorders. In order to design novel and selective SAIs, structure guided molecular docking assisted alignment dependent 3D-QSAR studies was performed on a data set comprises of 22 molecules bearing steroidal scaffold with wide range of aromatase inhibitory activity. Results: 3D-QSAR model developed using molecular weighted (MW) extent alignment approach showed good statistical quality and predictive ability when compared to model developed using moments of inertia (MI) alignment approach. Conclusion: The explored binding interactions and generated pharmacophoric features (steric and electrostatic) of steroidal molecules could be exploited for further design, direct synthesis and development of new potential safer SAIs, that can be effective to reduce the mortality and morbidity associated with breast cancer.

Background: Functional foods, neutraceuticals and natural antioxidants have established their potential roles in the protection of human health and diseases. Thymoquinone (TQ), the main bioactive component of Nigella sativa seeds (black cumin seeds), a plant derived neutraceutical was used by ancient Egyptians because of their ability to cure a variety of health conditions and used as a dietary food supplement. Owing to its multi targeting nature, TQ interferes with a wide range of tumorigenic processes and counteracts carcinogenesis, malignant growth, invasion, migration, and angiogenesis. Additionally, TQ can specifically sensitize tumor cells towards conventional cancer treatments (e.g., radiotherapy, chemotherapy, and immunotherapy) and simultaneously minimize therapy-associated toxic effects in normal cells besides being cost effective and safe. TQ was found to play a protective role when given along with chemotherapeutic agents to normal cells. Methods: In the present study, reverse in silico docking approach was used to search for potential molecular targets for cancer therapy. Various metastatic and apoptotic targets were docked with the target ligand. TQ was also tested for its anticancer activities for its ability to cause cell death, arrest cell cycle and ability to inhibit PARP gene expression. Results: In silico docking studies showed that TQ effectively docked metastatic targets MMPs and other apoptotic and cell proliferation targets EGFR. They were able to bring about cell death mediated by apoptosis, cell cycle arrest in the late apoptotic stage and induce DNA damage too. TQ effectively down regulated PARP gene expression which can lead to enhanced cancer cell death. Conclusion: Thymoquinone a neutraceutical can be employed as a new therapeutic agent to target triple negative breast cancer which is otherwise difficult to treat as there are no receptors on them. Can be employed along with standard chemotherapeutic drugs to treat breast cancer as a combinatorial therapy.

Beliefs and feelings about death are excerpted from interviews conducted with elderly English men and women, who were viewed as spiritually mature by those in their community. Respondents reported a wide range of beliefs about death, reflecting their personal experience, but none reported fear of death. Subtle sex differences were noted: men tended to picture death in spatial terms, of moving into a new dimension, while women tended to describe death in terms of relationships. Overall the respondents indicated that they placed a positive value on death, viewing it as a continuation of, and source of meaning for their present life.

AbstractCancer is one of the most common causes of death in the world. Despite the importance of combating cancer in healthcare systems and research centers, toxicity in normal tissues and the low efficiency of anticancer drugs are major problems in chemotherapy. Nowadays the aim of many medical research projects is to discover new safer and more effective anticancer agents. 1,3,4-Thiadiazole compounds are important fragments in medicinal chemistry because of their wide range of biological activities, including anticancer activities. The aim of this study was to determine the capacity of newly synthesized 1,3,4-thiadiazole compounds as chemotherapeutic agents. The structures of the obtained compounds were elucidated using 1H-NMR, 13C-NMR and mass spectrometry. Although the thiadiazole derivatives did not prove to be significantly cytotoxic to the tumour tissue cultures, compound 4i showed activity against the C6 rat brain cancer cell line (IC50 0.097 mM) at the tested concentrations.

Abstract The objective of this study was to examine the effects of diet quality on greenhouse gas emissions and dry matter intake (DMI). We used 42 mature, gestating Angus cows (600±69 kg; and BSC 5.3±1.1) with a wide range in DMI EPD (-1.36 to 2.29). Cows were randomly assigned to 2 diet sequences forage-concentrate (FC) or concentrate-forage(CF) determined by the diet they consumed in each period (forage or concentrate). The cows were adapted to the diet and the SmartFeed individual intake units for 14 d followed by 45 d of intake data collection for each period. Body weight was recorded on consecutive weigh days at the beginning and end of each period and then once every two wk for the duration of a period. Cows were exposed to the GreenFeed Emission Monitoring (GEM) system for no less than 9 d during each period. The GEM system was used to measure emissions of carbon dioxide (CO2) and methane (CH4). Only cows with a minimum of 20 total >3-m visits to the GEM were included in the data set. Data were analyzed in a crossover design using GLIMMIX in SASv.9.4. Within the CF sequence there was a significant, positive correlation between TMR DMI and CH4 (r=0.81) and TMR DMI and CO2 (r=0.69), however, gas emissions during the second period on the hay diet were not correlated with hay intake. There was a significant, positive correlation between hay DMI and CO2 (r=0.76) and hay DMI and CH4 (r=0.74) when cows first consumed forage (FC). In comparison to the CF sequence, cows on the FC sequence showed a positive correlation between CO2 and TMR DMI during the second period. There was also a significant positive correlation between hay and TMR DMI when assessed across (r=0.43) or within sequence (FC r=0.41, CF r=0.47).

Private practice is a growing field for occupational therapists, and fieldwork placements in this area can provide occupational therapy students with a unique and timely learning experience. As non-traditional placements are being considered more frequently, practitioners in private practice have a greater mandate to respond and to provide opportunities for student field-work experience. This article is designed to highlight some of the unique considerations, for both therapist and student, related to fieldwork experience in a private practice setting. It is based on the personal experience of the authors, a review of the literature, and discussions with other private practitioners.
