Abstract Background The aim of this study was to show the influence of cognitive impairment and dementia on oral health and on the utilization of dental services. Methods A cross-sectional analyzation of data of the OrBiD (Oral Health, Bite Force and Dementia) pilot study was conducted. 137 subjects were stratified into five dementia groups on the basis of the Mini Mental State Examination (MMSE) (1—no dementia (MMSE 28–30), 2—mild cognitive impairment (MMSE 25–27), 3—mild dementia (MMSE 18–24), 4—moderate dementia (MMSE 10–17), and 5—severe dementia (MMSE < 10)). Information on the utilization of dental services and oral health parameters (DMFT index, degree of restoration, Periodontal Screening Index, Bleeding on Probing, Oral Hygiene Index, Denture Hygiene Index) were collected. Results An increase in dementia resulted in significant reduction in utilization. Moreover, with increasing cognitive impairment/dementia there was a significant difference in the number of teeth that were decayed, but not in the number of filled or missing teeth or the DMF/T index itself. With increasing dementia, the degree of restoration decreased and oral/denture hygiene deteriorated significantly. Nevertheless, periodontal therapy was required for all subjects independent of their degree of dementia while bleeding on probing was increasing with increasing dementia. Conclusions An influence of cognitive impairment and dementia on oral health and on the utilization of dental services was shown. However, no conclusions about the influence of the utilization behavior of people with dementia on oral health parameters can be drawn. Further longitudinal studies are needed. Kehamilan dan kesehatan gigi-mulut merupakan aspek yang berkaitan satu sama lain. Ibu hamil perlu secara rutin memeriksakan kesehatan gigi dan mulutnya ke dokter gigi setiap trimester. Angka utilisasi pelayanan kesehatan gigi dan mulut yang ditandai dengan data kunjungan ibu hamil ke dokter gigi di Indonesia masih sangat rendah. Faktor-faktor antecendent dalam teori perubahan perilaku ABC (antecendent behavior consequences) sangat penting dalam mempengaruhi terbentuknya sebuah perilaku termasuk perilaku kunjungan ibu hamil untuk pemeriksaan ke dokter gigi. Tujuan penelitian ini adalah untuk mengindetifikasi faktor-faktor antecendent yang menyebabkan rendahnya perilaku kunjungan ibu hamil ke dokter gigi. Penelitian pendahuluan dilakukan di wilayah kerja puskesmas Serpong Tangerang Selatan secara cross sectional pada 27 ibu hamil, wawancara dengan kuisioner dan pemeriksaan klinis menghitung skor DMFT dan MGI (modified gingival index) kemudian dianalisis deskriptif dan statistik dengan uji chi-Square. Hasil penelitian menunjukkan 96% (26 responden) tidak pernah memeriksakan ke dokter gigi saat hamil. Faktor antecendent rendahnya kunjungan bumil ke dokter gigi: belum mengetahui manfaat ke dokter gigi saat hamil (33%; 9 responden), takut ke dokter gigi (26%; 7 responden), merasa mahal (19%; 5 responden), merasa tidak butuh (11%; 3 responden), takut membahayakan janin (11%; 3 responden). Rata-rata indeks DMFT sebesar 2.78 dan skor rata-rata MGI sebesar 1.00. Tidak terdapat perbedaan yang bermakna skor DMFT dan MGI dari masing-masing faktor antecendent (p>0.05). Kesimpulannya dua faktor yang mempengaruhi utilisasi pelayanan kesehatan gigi dan mulut selama kehamilan adalah 1) pengetahuan dan 2) perceived need.Antecendent Affecting Utilization of Dental Services in Pregnant Women (Pilot Study at Serpong District, South Tangerang). Pregnancy and oral health are an aspect that is related to each other. Pregnant women need to regularly check oral health to a dentist every trimester. The number of utilization of dental service which is indicated by pregnant women’s dental visit data in Indonesia is still very low. Antecendent factors in ABC behavior change theory (Antecendent Behavior Consequences) are very important to influence the formation of a behavior including pregnant women dental’s visit. The objective of this study is to identify and evaluate the antecendent factors affecting the utilization of dental services during pregnancy. The preliminary research was conducted at comunity health center in Serpong district, South Tangerang with a cross sectional study. Subjects are 27 pregnant women with gestational age of 4-38 weeks.Data were collected through interview and clinical examination to find out about the score of DMFT index and modified gingival index, which was then analyzed descriptively and statistically using chi-Square test. Results: most of the respondents (96%; 26 respondents) did not visit the dentist during pregnancy. Antecendent factor of pregnant women’s dental visit: not knowing the benefits of dental visit during pregnancy (33%; 9 respondents), having fear of a dentist (26%; 7 respondents), visiting a dentist is thought to be expensive (19%; 5 respondents), having no need (11%; 3 respondents), having fear of causing harm to the fetus (11%; 3 respondents). The mean of DMFT is 2.78 ± 3.23 and the mean of modified gingival index (MGI) 1.00 ± 1.07. There were no significant differences in DMFT and MGI scores of each antecendent (p> 0.05). Conclusion: two major factors predicting the utilization of dental services during pregnancy are 1) knowledge and 2) perceived need.

Good oral health during pregnancy can possibly reduce complications and this will improve the quality of life of the pregnant woman. In addition, this will prevent the risk of the child to develop early childhood dental caries in the future. The main objective of this study was to determine if there is an association existingbetween the level of knowledge on oral health and utilization of dental services among pregnant women. Furthermore, the study determined the (a) proportion among pregnant women who had high level of knowledge regarding dental careduring pregnancy, (b) proportion of pregnant women who ever visited the dentist during pregnancy, (c) proportions of expecting women who are regularly brushing her teeth and whouses dental floss during pregnancy, and (d) factors that hinders the expecting women from visiting the dentist during pregnancy. A descriptive cross-sectional study was conducted at El Minia University Maternal and Child Health Hospital, El Minia, Egypt. A cross-sectional self-administered questionnaire-based survey was conducted among 200 expectant mothers, approached 156 out of them returned completed questionnaires, representing a response rate of 78%. Results revealed that: (a) nearly 65% of pregnant mothers had a high level of poor knowledge regarding dental care during pregnancy, (b) roughly 24% of pregnant women ever visited the dentist during pregnancy, (c) about 80% and 43% of expecting women regularly brush their teeth at least twice per day and use dental floss during pregnancy, respectively, and (d) among the factors that hinder the expecting women from visiting the dentist during pregnancy include having no time to visit, the consultation cost, fear and anxiety about the procedure, long waiting time, transportation problem and the idea of not needing the dental care. Moreover, the results showed that there was no sufficient evidence to say that the level of knowledge regarding dental service during pregnancy is associated with the utilization of dental service during pregnancy (Pearson Chi-square p-value = .160). The study concludes that health promotion on oral care during prenatal visits should address the mythologies and misconceptions among pregnant women. Therefore, healthcare providers should raise awareness among expectant mothers about giving special attention to oral health needs due to the higher risk of oral diseases that may affect pregnancy and the child inside the womb.

Abstract Background Children with developmental disorders (DD) have higher rates of oral diseases and tooth decay compared to the general population. Despite that, they often face more barriers to access the needed health care, especially dental care. This study aims to describe the oral health needs (OHN), utilization of dental services, and unmet dental needs of children with developmental disorders (DD) compared to children without developmental disorders (DD); and to assess the impact of access to healthcare factors on the oral health of children with DD. Methods This cross-sectional study utilized a sample of 30,530 noninstitutionalized children from the 2018 National Survey of Children’s Health (NSCH). Descriptive and inferential statistics were used to compare and analyze healthcare barriers in children with and without OHN. Results Overall, there were 6,501 children with DD compared to 24,029 children without DD. A significantly higher proportion of children with DD relative to children without DD were found with OHN (20.3% vs. 12.2%, respectively), unmet dental needs (3.5% vs 1.2%), and utilization of any dental visits (86.1% vs 76.1%), (P-value <. 001). The adjusted logistic model found that poverty (< 100% of the Federal Poverty Level (AOR = 2.27, CI: 1.46–3.51), being uninsured (AOR = 2.12, 95% CI: 1.14–3.95), a great deal of disability (AOR = 1.89, CI: 1.23–2.78), and living in the West (AOR = 1.61, CI: 1.09–2.37) contributed to the largest odds of OHN among children with DD. Conclusion Despite the high utilization of dental services, children with DD had poor oral health and more unmet dental needs compared to children without DD. Advocacy efforts and policy changes are needed for the development of an integrated model of care that addresses health insurance, poverty and considers children’s levels of disability.

OBJECTIVE: To make a comparative assessment of primiparous and multiparous mothers' knowledge about dental caries, access to information and use of dental services during pregnancy. METHODS: This cross-sectional, quantitative study was conducted in a Reference Maternity center for the Seridó region of Rio Grande do Norte, with 100 women aged between 12 and 45 years, who answered a structured questionnaire. RESULTS: The majority of patients received information on oral health (70%) (p=0.24), but only 42% sought dental care during pregnancy (p = 0.45). The interviewees considered caries (80%) (p=0.80), a communicable disease (64%) (p=0.29) arising due to inadequate brushing (60%) (p=0.37). Pregnant women considered a brush with soft bristles and small head appropriate (59%) (p=0.83) and reported that the amount of toothpaste should extend over the entire brush (46%) (p=0.04). "Brush your teeth" was the most cited response (70.9%) in their speeches on caries prevention. CONCLUSIONS: In general the mothers in this study presented similar characteristics as regards knowledge about dental caries, access to information and use of dental services, however, there was significant difference between the groups for the variable quantity of toothpaste used. Nevertheless, some aspects such as the multifactorial etiology of dental caries, biofilm, use of dentifrices, and seeking information and dental services from the perspective or oral health prevention and promotion still need to be worked on among these mothers. These are simple aspects that must be considered in daily care by both pregnant and multiparous women

The aim is to analyze protective and modifying factors (e.g., vulnerability, resilience, sense of coherence) in relation to the utilization of dental services by seniors at different levels of the healthcare system. Terminological imprecision in the use and transfer of existing terms (sense of coherence, resilience, salutogenesis) to gerodontology is clarified. Factors influencing a reduced utilization (static/dynamic factors) can occur isolated or in combination and, thus, model the risk of a reduced utilization of dental services (influencing-factor mechanism). Protective factors of utilization include patient-specific factors for self-motivation and factors that promote oral-health-related resilience. Resistance forces that counteract can be identified as oral-health-related resilience factors. Achieving social and individual appreciation and establishing a prevention-oriented approach to utilization will be increasingly challenging, since the population is becoming older and access is not equal in terms of opportunity. Resistance forces need to be strengthened in an ethical context. Studies should increasingly present resilience processes, determinants and modes of action at the various interfaces in the healthcare system, which can ensure sustainable medical care in old age. The concepts conveyed here are generally valid and able to point out inequalities and ageism in access to dental services.

Introduction: Implant supported prosthetic rehabilitation could re-establish the comfort and bring a confident smile to patients, allowing them to eat properly and to lead a social life free of traumas and complexes. Aim: To verify which type of implant-supported rehabilitation- overdenture or fixed denture effectively restores the comfort of the lower arch of partial edentulous patients. Materials and Methods: A cross-sectional clinical study was performed in patients with completely edentulous lower arch treated with either overdenture (2 implants) or fixed denture (5-6 implants), obtained from a sample from Southern Brazil. The study sample was composed of 37 volunteers who met the inclusion and exclusion criteria. Half of them were rehabilitated with overdenture (O- ring and bar) and the other half with fixed denture (Branemark protocol). A clinical evaluation was performed to obtain the Index of Plaque (PI) and bleeding on probing indexes (BI). Three questionnaires were applied to quantify the patient’s level of satisfaction with their prosthetic treatment (OHIP-EDENT and masticatory ability questionnaires) up to one year after installation of the prosthesis. Results were analysed by the Mann-Whitney test (α=0.05). Results: All treatments were considered satisfactory regarding masticatory ability. However, fixed dentures had a statistically higher level of satisfaction compared to overdenture. There were significant differences in the mean PI and BI index between the groups. Conclusion: Both rehabilitation treatments increased the satisfaction and improved the oral health and masticatory ability of edentulous patients. However, although patients rehabilitated with fixed dentures presented higher level of satisfaction, they also showed higher plaque and bleeding on probing indexes.

Background: The study aims to determine the spatiotemporal gait parameters and/or their combination(s) that best differentiate between cognitively healthy individuals (CHI), patients with mild cognitive impairment (MCI) and those with mild and moderate dementia, regardless of the etiology of cognitive impairment. Methods: A total of 2099 participants (1015 CHI, 478 patients with MCI, 331 patients with mild dementia and 275 with moderate dementia) were selected from the intercontinental “Gait, cOgnitiOn & Decline” (GOOD) initiative, which merged different databases from seven cross-sectional studies. Mean values and coefficients of variation (CoV) of spatiotemporal gait parameters were recorded during usual walking with the GAITRite® system. Results: The severity of cognitive impairment was associated with worse performance on all gait parameters. Stride velocity had the strongest association with cognitive impairment, regardless of cognitive status. High mean value and CoV of stride length characterized moderate dementia, whereas increased CoV of stride time was specific to MCI status. Conclusion: The findings support the existence of specific cognitive impairment-related gait disturbances with differences related to stages of cognitive impairment, which may be used to screen individuals with cognitive impairment.

ABSTRACT Introduction: Although there has been an improvement in the oral health status of the population, tooth loss still aggravates the oral health of adults and is a matter of great relevance to dentistry. Aim: To determine the spatial distribution of tooth loss in adults and correlate this with the Social Exclusion Index and proximity to public dental services. Material and Method: This ecological study was based on epidemiological data of adults from Piracicaba municipality and from the Piracicaba Research and Planning Institute (IPPLAP). Data on dental evaluations were extracted from the Piracicaba epidemiological survey, which was a cross-sectional study with probabilistic sampling of 248 adults aged 20-64 years, representative of adults living in Piracicaba, Brazil. Oral examinations of the DMFT index were in accordance with the World Health Organization codes and criteria and were performed by a single examiner calibrated for this purpose. Data on social exclusion and the municipal health units that have dental services were extracted from IPPLAP. Georeferencing was performed of census tracts selected by draw, and the city health facilities that have dental services. For Spearman correlation analysis (p <0.05), we used the mean value of teeth lost per district, the Social Exclusion Index (IEX), and proximity to public dental service categorized according to radius: <500m, between 500-1000m, and >1000m the census tract. Result: There was a correlation between tooth loss and higher IEX, and r=−0.51 (p=0.01), but no correlation with proximity to public dental services (p=0.42). Conclusion: Tooth loss in adults was distributed according to social exclusion, however, it was unrelated to proximity to the public dental services.

Abstract Background Approximately half of all patients with multiple sclerosis (MS) experience cognitive impairment, most commonly with regard to new learning and memory. Cognitive dysfunction is a leading cause of disability in MS and it can have profound social and economic consequences for patients and their families. Objective This study was conducted to discover the early cognitive domains affected in multiple sclerosis patients concomitant with the postulated brain atrophy in an Egyptian sample of multiple sclerosis patients. Patients and Methods A cross-sectional observational case-control study conducted on seventy (60) patients who came for follow-up in Ain Shams University hospitals. 40 patients were taken as cases that followed up in MS unit in Ain-Shams university hospitals. 20 participants were taken as controls taken from the general medicine clinics age and sex matched to our patient group. An informed written consent was taken from parents of each person included in the study. Results There was a statistically significant difference between the two groups in brain volumetric changes and in the parameters of cognitive assessment Conclusion Early detection and examination of cognitive functions is important for patient evaluation, follow up and treatment regimen used.

ObjectivesTo investigate the prevalence of mild cognitive impairment and the relationship with plasma aluminium among aluminium workers.DesignThis was a cross-sectional case-control study in the SH Aluminium Factory, China.SettingThe university and affiliated hospital cooperated in the study.ParticipantsThere were 910 aluminium workers on duty, among whom 853 participated in our study. Participants, such as those with cerebral vascular disease, epilepsy, brain trauma, Parkinson’s and mental diseases, aluminium-containing drug and mental drug use, and any family history of dementia in first-degree relatives were excluded.Primary and secondary outcome measuresBlood samples were collected, and plasma aluminium was measured by inductively coupled plasma-mass spectrometry. For each case, four age-matched controls were evaluated to determine the relationship between aluminium exposure and mild cognitive impairment. Conditional logistic regression was used to explore influential factors in mild cognitive impairment.ResultsAmong 910 workers, 93.74% participated in stage 1; 53 cases were finally diagnosed. The crude prevalence of mild cognitive impairment among aluminium workers on duty was 6.21%. There was a significant difference in plasma aluminium concentration between the two groups. In the multivariate analysis, we found that a higher level of plasma aluminium was associated with a high risk of cognitive impairment when compared with a lower aluminium level (AOR=2.24, 95% CI=1.17 to 4.26), and a high education level was a protective factor (AOR=0.36, 95% CI=0.18 to 0.70). No other factor was statistically significant.ConclusionsMild cognitive impairment is no longer a disease specific to elderly people. High plasma aluminium exposure might be associated with an increased risk of cognitive impairment, but a reduced risk was observed with a high education level. The cognitive function of aluminium workers on duty must be considered seriously.
