Treating skin conditions: a holistic approach in dermatology

This article is a review of current evidence to both support and refute holistic approaches to dermatological cases, including analysis of data linked to psychodermatology, psychosocial and lifestyle influences. A case for taking an integrated approach adjuvant to traditional medicine in skin disease therapy, how this could save time, money and reduce pressure on resources will be discussed. Overall, the research is promising; however, there are gaps in knowledge and there is a need for long-term, larger scale studies in the future. Also, an improved understanding of the processes that lead to inflammatory cutaneous diseases in the future will enable practitioners to make better treatment choices.

Abstract. Regional/global-scale information on coastline rates of change and trends is extremely valuable, but national-scale studies are scarce. A widely accepted standardized methodology for analysing long-term coastline change has been difficult to achieve, but is essential to conduct an integrated and holistic approach to coastline evolution and hence support coastal management actions. Additionally, databases providing knowledge on coastline evolution are of key importance to support both coastal management experts and users. The main objective of this work is to present the first systematic, global and consistent long-term coastline evolution data of Portuguese mainland low-lying sandy. The methodology used quantifies coastline evolution using an unique and robust coastline indicator (the foredune toe), which is independent of short-term changes. The dataset presented comprises: 1) two polyline sets, mapping the 1958 and 2010 sandy beach-dune systems coastline, both optimized for working at 1:50 000 scale or smaller, and 2) one polyline set representing long-term change rates between 1958 and 2010, estimated at each 250 m. The science data produced here are in Open Access at doi:10.1594/PANGAEA.853654 and can be used in other studies. Results show beach erosion as the dominant trend, with a mean change rate of −0.24 ± 0.01 m/year for all mainland Portuguese beach-dune systems. Although erosion is dominant, this evolution is variable in signal and magnitude in different coastal sediment cell and also within each cell. The most relevant beach erosion issues were found in the coastal stretches of Espinho – Torreira and Costa Nova – Praia da Mira, both at sub-cell 1b; Cova Gala – Leirosa, at sub-cell 1c and Cova do Vapor – Costa da Caparica, at cell 4. Cells 1 and 4 exhibit a history of major human interventions interfering with the coastal system, many of which originated and maintained a sediment deficit. In contrast, cells 5 and 6 have been less intervened and show stable or moderate accretion behaviour.

Holistic approaches involve integration of thinking and activities, inclusive ways of working with complex, cross-curricular, interdisciplinary and transcendental connections (Learning for the future, 2011, p. 7). Assessment of existing situations shows that teachers struggle with diversifying learning form and content and with varying the pace of work as well as the difficulty level of tasks. The holistic approach determines that one particular learning or activity sphere cannot exclude the others and that all of them are interconnected in a complex and nuanced way; therefore it is essential to identify the existing links between them. In pre-school education praxis the learning process is still divided into certain subjects with little emphasis on the integrated approach. In the process of implementing the learning content, the curriculum is composed of more than one particular situation or a case. The aim of this article is to describe the positive experience of teachers in terms of diversification of their activities and to determine the diversification principles which can help the pre-school children to discover their uniqueness and enable them to share their subjective experience as well as recognize the link between their lives and the outside world. As the result of this study, the diversification principles of teachers’ activities have been determined for the holistic development of children.

They can give a low-cost and society friendly. Researchers also do researches to find out if there are substances that are not allowed to be included. Researchers also have result of their research that majority of instant jamu in society have composition of chemical substances such as corticosteroid, paracetamol, and mefenamic acid. Those compounds are very dangerous to be consumed in long-term period due to the side effects which are able to deliver metabolism abnormalities. People among society, especially in village, have no awareness in consuming that instant jamu. So that, doctor has a major role to deliver education to those people in order to decrease risk factor of metabolism abnormalities in the future.

IntroductionCurrent evidence indicates an alarming increase in topical steroid (TS) misuse in India. Data regarding the magnitude and characteristics of this problem in rural India, where 68% of the population resides, are insufficient. This study analyses the magnitude, causes, characteristics and consequences of TS misuse in rural India. It also examines the association between TS misuse and patients’ perception of skin disease.MethodsA mixed-method observational study was conducted among the attendees of the dermatology outpatient department in a rural North Indian hospital. Those with a history of TS misuse were analysed for behaviour patterns and outcome.ResultsOut of 723 patients, 213 (29.2%) misused TS. Clobetasol propionate (58.2%) was most commonly misused. Seventy brands of inappropriate fixed drug combination steroid creams were recovered from the patients. Pharmacists and local healers together contributed to 78% of the sources for steroid misuse. Almost 58% of participants perceived their skin conditions to be allergic reactions to food, when in fact 70.1% were tinea, 10% scabies and 9% acne. Eighty per cent of the respondents having tinea had tinea incognito and 97% had extensive lesions. Eighty-five per cent of the participants with scabies had atypical lesions and 80% with acne had steroid rosacea or aggravation of acne. The median expenditure incurred in purchasing these potentially harmful steroid creams was Rs 1000 (US$14.1, equivalent to 3 days’ wages of a labourer).ConclusionSteroid misuse is a problem of epidemic proportion in rural India. This practice is changing the profile of many common and infective skin conditions, which portends diagnostic dilemmas and therapeutic challenges for clinicians. Misconceptions about skin disease drive the public to seek ‘quick fixes’ from non-allopathic providers who have unrestricted access to potent steroids. There is an urgent need to tighten regulatory controls over the manufacturing, sale and prescription of irrational TS combinations.

This should be undertaken with a view to ensuring that the Convention fulfils its mission and meets the future challenges, taking into account the ever-increasing pace of change and technological advancements made since 1974. This paper considers that a Goal-Based-Safety Level approach will provide the key to success and discusses the possibility to take a holistic approach to build a far reaching Convention that will help the stakeholders who deal with these regulations to be more effective.

The forest-game conflict – how can forest economics contribute to solve it? (Essay) Core parameters of forest economics such as land expectation value or highest revenue show that damage caused by wild ungulates can critically influence the economic success of forest enterprises. When assessing and evaluating the damage in order to calculate damage compensation, methods are applied in Germany that look either into the past (“cost value methods”) or into the future (“expected value methods”). The manifold uncertainties related to this evaluation over long-term production periods are taken into account within a framework of conventions through strongly simplifying assumptions. Only lately, the increased production risk due to game-induced loss of species diversity is also considered. Additional aspects that should be taken into account in the future are the loss of climate-adapted species, the change of the insurance values of forest ecosystems and the impossibility of specific management systems such as single-tree selection forestry due to the influence of game. Because of high transaction costs when assessing the damage, financial compensation should only be the “ultimate measure” and a meditation between stakeholder groups with the goal to find a cooperative solution before the damage occurs should be preferred.
