Latar belakang. Prevalensi asma makin meningkat, diduga berkaitan dengan kejadian infeksi pada anak yang menurun sehingga menyebabkan pergeseran keseimbangan antara limfosit T helper 1 (Th1) dan T helper 2 (Th2) ke arah predominan Th2. Infeksi mikobakterium dan vaksinasi BCG dapat meningkatkan respon imun Th1 (interferon gamma (IFN-γ)) dan menekan Th2.Tujuan. Mengetahui pengaruh vaksinasi BCG terhadap kadar IFN-γ serum pasien asma ekstrinsik atopi anak setelah vaksinasi BCG satu kali.Metode. Penelitian kuasi-eksperimental pretest posttest control group design pada anak asma atopi. Pengacakan perlakuan dilakukan terhadap subjek ke dalam kelompok BCG dan plasebo. Sebelum dan 8 minggu sesudah perlakuan diukur kadar IFN-γ serum.Hasil. Kadar IFN-γ serum tidak meningkat sesudah vaksinasi BCG (median 1,580 dan 0,780 pg/ml, p= 0,326) dan plasebo (median 1,255 dan 0,670 pg/ml, p= 0,079). Selisih kadar IFN-γ serum kelompok BCG dan plasebo tidak berbeda bermakna (median 0,020 dan -0,420 pg/ml, p= 0,449).Kesimpulan. Kadar IFN-γ serum pasien asma ekstrinsik atopi anak tidak meningkat setelah vaksinasi BCG 1 kali.

Glukokortikoid merupakan salah satu jenis hormon stres yang akan menekan respons imun normal dengan jalan memblokade program sel T helper satu yang merupakan penghasil interferon gamma. Glukokortikoid dapat memberikan dampak terhadap penurunan bobot badan, penurunan sistem imunitas tubuh, dan perubahan diferensiasi leukosit. Pala (Myristica fragrans Houtt) adalah tanaman tradisional Maluku dan mengandung senyawa metabolit sekunder yang dapat bersifat sebagai imunomodulator. Penelitian ini bertujuan untuk mengetahui jumlah dan jenis leukosit mencit yang diinduksi stres dan diberi ekstrak etanol biji pala. Penelitian ini merupakan eksperimen murni dengan desain post test only control group design. Mencit sebanyak 30 ekor di bagi menjadi lima kelompok perlakuan yaitu kelompok kontrol normal (KN), kontrol negatif (K-), kontrol positif (K+), ekstrak etanol biji pala konsentrasi 4% (P1) dan 16% (P2). Pada K-, K+, P1 dan P2 diberi perlakuan stres dengan metode FST (Forced Swim Test) selama enam menit. Kemudian, pada kelompok K+ diberi obat alprazolam dosis 0,2 ml, P1 dan P2 diberi ekstrak etanol biji pala sebanyak 0.5 ml. Perlakuan diberikan selama 7 hari dan pada hari ke-8 dilakukan koleksi darah mencit intrakardial. Data hasil pengamatan dianalisis dengan ANOVA. Hasil penelitian menunjukkan kelompok P1 dapat menurunkan persentase neutrofil, meningkatkan persentase monosit dan limfosit, sedangkan pada P2 dapat menurunkan persentase neutrofil dan monosit, meningkatkan persentase limfosit dan tidak berpengaruh terhadap persentase eosinofil dan basofil. Sehingga dapat disimpulkan bahwa pemberian ektsrak etanol biji pala tidak berpengaruh signifikan terhadap rata-rata persentase neutrofil, monosit, limfosit, eosinofil dan basofil (p>0,05).

ABSTRACTA mouse model to study the effect of cold-induced stress on Chlamydia muridarum genital infection and immune response has been developed in our laboratory. Our previous results show that cold-induced stress increases the intensity of chlamydia genital infection, but little is known about the effect of cold-induced stress on differentiations and activities of T cell subpopulations and bone marrow derived dendritic cells (BMDCs). The factors that regulate the production of T helper 1 (Th1) or T helper 2 (Th2) cytokines is not clear. The objective of this study was to examine whether cold-induced stress modulates the expression of transcription factors and hallmark cytokines of Th1 and Th2 or differentiation of BMDCs during C. muridarum genital infection in mice. Our results show that mRNA level of beta2-adrenergic receptor (β2-AR) compared to β1-AR and β3-AR was high in mixed population of CD4+ T cells and BMDCs. Further, decreased expression of T-bet and low level of interferon-gamma (IFN-γ) production and increased expression of GATA-3 and interleukin-4 (IL-4) production in CD4+ T of stressed mice was observed. Exposure of BMDCs to feroterol (β2-AR agonist) or ICI,118551 (β2-AR antagonist), respectively, revealed significant stimulation or inhibition of β2-AR in stressed mice. Moreover, co-culturing of mature BMDC and naïve CD4+ T cells resulted in increased production of IL-4, IL-10, and IL-17 in culture supernatants, suggesting that stimulation of β2-AR leads to the increased production of Th2 cytokines. Overall, our results show for the first time that cold-induced stress is able to modulate the pattern of Th1 and Th2 cytokine environment, suggesting that it promotes the differentiation to Th2 rather than Th1 by the overexpression of GATA-3 correlated with elevated production of IL-4, IL-10, IL-23, and IL-17 in contrast to a low expression of T-bet correlated with less IFN-γ secretion in the mouse model.

Abstrak     Masalah klinis kesehatan kerja pada pekerja penggilingan padi saat ini adalah penyakit saluran pernafasan. Penelitian ini bertujuan untuk menganalisis peningkatan CD8 dan IFN-γ pada Pneumonitis Hipersensitif (HP) akibat pajanan debu penggilingan padi pada mencit  (Musmusculus) BALB/C. Penelitian yang dilakukan adalah penelitian eksperimental laboratoris dengan hewan coba mencit (Musmusculus) BALB/C,  rancangan penelitian yang digunakan adalah Randomized the post test only control group design. Mencit  (Musmusculus) BALB/C dipajan dengan debu penggilingan padi selama empat jam/hari selama tiga puluh hari dengan konsentrasi 0.50 mg/m3, 0.75 mg/m3, 1.00 mg/m3. Variabel bebas adalah debu penggilingan padi, sedangkan variabel tergantung adalah   CD8 dan IFN-γ, serta gambaran histopatologis paru mencit, sedangkan variabel kendali adalah strain, berat badan, dan umur. Hasil penelitian menunjukkan bahwa terjadi peningkatan kadar CD8 antara kontrol dengan perlakuan, dan secara statistik  ada perbedaan yang bermakna. Kemudian terjadi peningkatan IFN-γ dan secara statistik ada perbedaan yang bermakna antara kontrol dan perlakuan pada mencit  (Musmusculus) BALB/C. Hasil penelitian dapat digunakan sebagai dasar upaya pencegahan dan pemberian terapi penyakit HP lebih dini sehingga dapat meningkatkan kualitas paru.   Kata Kunci: CD8, IFN-γ,  debu penggilingan padi     Abtract   The purpose of this research was to analyze increase of CD8  and IFN-γ of Hypersensitivity Pneumonitis (HP) as a result from the exposure of dust from paddy milling on mice (Musmusculus)BALB/C. The research done was a laboratory experimental research with mice (MusMusculus) as experimental animal.  The research design used was the post only control group design using mice (MusMusculus) Balb/c as experimental animal.  Mice (Musmusculus) Balb/c were exposed to dust from paddy milling for four (4) hours/day and it was done for thirty (30) days with the exposed concentrations respectively were 0.50 mg/m3, 0.75 mg/m3, 1.00 mg/m3.The research variables were free variable, dependent variable, and control variable.  Independent variable was dust from paddy milling, dependent variables were Hypersensitivity Pneumonitis (HP), CD8 and IFN-γ, while control variables were strain, body weight and age of mice (MusMusculus) Balb/c. The research result showed that there was an increase of CD8, and statistically there was significant difference; there was an increase on IFN-γ and statistically there was a significant difference between the study and control on mice BALB/C. Macrophage-derived chemokine (MDC) is a CC chemokine that recognizes the CCR4 receptor and is selective for T helper 2 (Th2) versus T helper 1 (Th1) cells. The present study was designed to investigate the effect of the prototypic Th2/Th1 cytokines, interleukin-4 (IL-4) and interferon-γ (IFN-γ), on the production of MDC by human monocytes. IL-4 and IL-13 caused a time-dependent (plateau at 24 hours) and concentration-dependent (EC50 2 and 10 ng/mL, respectively) increase of MDC mRNA levels in monocytes. Increased expression of MDC mRNA was associated with protein release in the supernatant. MDC expression and production induced by IL-4 and IL-13 were inhibited by IFN-γ. IFN-γ also suppressed the constitutive expression of MDC in mature macrophages and dendritic cells. These results delineate an amplification loop of polarized Th2 responses based on differential regulation of MDC production by IL-4 and IL-13 versus IFN-γ and on the selectivity of this chemokine for polarized Th2 cells. © 1998 by The American Society of Hematology.

Abstract Introduction An abnormal endometrial immune response is involved in the pathogenesis of repeated implantation failure (RIF), so we investigated the effectiveness of tacrolimus treatment on the endometrium of RIF patients. Materials and Methods Ten RIF patients with elevated T-helper 1/T-helper 2 (Th1/Th2) cell ratios were recruited into a clinical study. The expression of p53, leukemia inhibitory factor (LIF), interleukin (IL)-4, IL-10, IL-17, and interferon gamma (IFN-γ) in the endometrium of patients with and without tacrolimus treatment and the association of these factors with assisted reproductive technology (ART) outcomes were investigated. Results Tacrolimus significantly increased the expression of LIF, IL-10, and IL-17 and decreased the expression of IL-4, IFN-γ, and the IFN-γ/IL-10 ratio in RIF patients. Tacrolimus treatment resulted in an implantation rate of 40%, a clinical pregnancy rate of 50%, and a live birth rate of 35% in RIF patients with elevated Th1/Th2 ratios who had previously failed to become pregnant despite at least three transfers of embryos. We also found a significant positive correlation between IL-10 levels and the implantation rate. Conclusions Our findings suggest that RIF patients with a higher Th1/Th2 ratio could be candidates for tacrolimus therapy and that this immunosuppressive drug could be acting through upregulation of LIF, IL-10, and IL-17.

Protective efficiency of a combination of four recombinant Brucella abortus (B. abortus) proteins, namely outer membrane protein (Omp) 16, Omp19, Omp28, and 50S ribosomal protein L7/L12 was evaluated as a combined subunit vaccine (CSV) against B. abortus infection in RAW 264.7 cell line and murine model. The immunoreactivity of these four recombinant proteins as well as pCold-TF vector reacted with Brucella-positive serum individually, but not with Brucella-negative serum by immunoblotting assay. CSV-treated RAW 264.7 cells significantly induced production of IFN-γ and IL-12 while decreased IL-10 production at the late stage of infection compared to PBS-treated control cells. In addition, the enhancement of nitric oxide production together with cytokines secretion profile in CSV-treated cells proved that CSV notably activated bactericidal mechanisms in macrophages. Consistently, mice immunized with CSV strongly elicited production of pro-inflammatory cytokines TNF-α, IL-6 and MCP-1 compared to PBS control group. Moreover, the concentration of IFN-γ was >IL-10 and titers of IgG2a were also heightened compared to IgG1 in CSV-immunized mice which suggest that CSV induced predominantly T helper 1 T cell. These results suggest that the CSV used in the present study is a potential candidate as a preventive therapy against brucellosis.

Abstract Interleukin 12 (IL-12) is a major inducer of interferon gamma (IFN-γ) and the principal mediator of T helper 1 (Th1) differentiation. To identify IL-12–regulated genes, which might contribute to Th1 differentiation and IFNG regulation, we employed microarray analysis. Surprisingly, a ubiquitously expressed proprotein convertase (PC), furin, was one of the most consistently IL-12–induced genes in T cells, and among PCs was the only one regulated by this cytokine. Furin was preferentially expressed in differentiated Th1 cells in a Stat4-dependent manner. Expression of furin enhanced IFN-γ secretion, whereas inhibition of furin interfered with IFN-γ production. Thus, we conclude that IL-12 induction of furin might represent a new aspect of IFN-γ regulation and control of Th1 differentiation.

ABSTRAK Upaya untuk meningkatkan mutu pendidikan keperawatan khususnya untuk mendukung peserta didik menjadi perawat profesional memerlukan proses pembelajaran dengan menggunakan fasilitas keterampilan klinis. Practice based simulation model didasarkan pada teori belajar konstruktif yang menegaskan bahwa pengetahuan tidak pasif ditransfer dari pendidik kepada peserta didik, tetapi dibangun oleh peserta didik melalui pengolahan pengalaman dan interaksi dengan lingkungan mereka. Dengan metode simulasi di laboratorium dapat mendorong mahasiswa untuk menggunakan critical thinking dalam mengambil keputusan dalam mengatasi masalah tanpa merugikan pasien yang sebenarnya. Penelitian ini bertujuan untuk menganalisis pengaruh practice based simulation model terhadap critical thinking pada mahasiswa semester VI di FIK Unissula Semarang. Penelitian ini menggunakan metode Quasy – Experiment dengan pendekatan pretest-posttest with control group design. Pengambilan sampel dengan teknik  simple random sampling dengan jumlah 21 responden baik kelompok intervensi maupun kelompok kontrol. Analisis data menggunakan uji Paired T-Test dan Independent Samples T-Test. Hasil penelitian selisih peningkatan critical thinking pada kelompok intervensi sebanyak 11.95 poin dan pada kelompok kontrol 2.05. Practice based simulation model berpengaruh terhadap critical thinking dengan nilai p=0.00<0.05. Disimpulkan bahwa practice based simulation model mempengaruhi critical thinking pada mahasiswa semester VI FIK Unissula Semarang. Kata kunci: Practice based simulation model, critical thinking
