Objective: to determine a set of signs that are prognostically significant for identifying a high-risk axial skeletal lesion in early psoriatic arthritis (ePsA).Patients and methods. Examinations were made in 95 patients (47 men and 48 women) with peripheral arthritis lasting for ≤2 years, who met the 2006 Classification Criteria for Psoriatic Arthritis (CASPAR). The clinical characteristics of the patients were presented in our previously published work. In all the patients, a standard examination was made and the signs of inflammatory back pain (IBP) were identified according to the Assessment of SpondyloArthritis International Society (ASAS) criteria, the presence of human leukocyte antigen B27 (HLA-B27) was determined, and pelvic bone X-ray was done; regardless of whether they had IBP, 79 patients underwent magnetic resonance imaging (MRI) of the sacroiliac joints using a low-field Signa Ovation 0.35 T. Sacroiliitis (SI) diagnosed based on radiography (rSI) was considered reliable if there were bilateral changes corresponding to at least Stage II or unilateral changes corresponding to at least Stage III according to the Kellgren system. SI diagnosed based on MRI (MRI-SI) was regarded as active when osteitis was detected in the STIR mode in the bones adjacent to the joint on at least two slices or in the presence of two signals in a slice. X-ray and MRI results were assessed by an independent radiologist. The extent of a skin lesion was determined from the body surface area (BSA): the extent was regarded insignificant, moderate, and significant with involvements of <3%, 3–10%, and >10%, respectively.The patients were divided into two groups. Group 1 included 65 (68.4%) patients with the manifestations of axial PsA (axPSA): IBP, and/or rSI, and/or MRI-SI; Group 2 consisted of 30 (31.6%) patients without axial manifestations, only with peripheral PsA (pPsA). Multivariate stepwise discriminant analysis was used to identify a group of signs that were most characteristic of axPsA.Results and discussion. Comparative analysis of the two groups showed that there were more males among patients with axPsA than among those with pPsA (39 (60%) and 8 (26.7%), respectively) (p=0.003). HLA-B27 positivity was also more often detected in patients with axPSA than in those with pPsA (30 (46.6%) and 7 (23.3%) patients, respectively) (p=0.02). In the axPSA group, there were significantly more individuals with moderate and high DAS, high CRP levels, and a more severe skin lesion (BSA >3%).The investigators obtained the following discriminant classification rule associated with axPSA: 1.566 (if CRP is >5 mg/L) + 0.957 (if HLA-B27 is positive) + 0.986 (if BSA is >3%) + 1.845 (if DAS is moderate or high) + 0.6 (if the sex is male) >3.751 (p=0.0025). The sensitivity and specificity of the model were 68% and 73%, respectively.Conclusion. The combination of signs, such as male sex, HLA-B27 positivity, high or moderate DAS, CRP >5 mg/L, the extent of skin lesions according to BSA >3%, is prognostically significant for identifying high-risk axial skeletal lesion in ePSA. The proposed mathematical model can be used to screen patients for the early diagnosis of an axial lesion in ePSA.

Background:Axial Involvement in psoriatic arthritis (PsA) is quite common [1]. Predictors of axial involvement at early-stage of disease haven’t been sufficiently studied.Objectives:To identify predictors of axial involvement in PsA patients (pts) at early-stage of disease.Methods:95 patients (pts) (M/F–47/48) with early PsA fulfilling the CASPAR criteria were included. All pts had peripheral arthritis for≤2 years; no inflammatory back pain (IBP) pts were specifically selected. Mean (Me) age 36.5±10.7 yrs, disease duration 12.2±10.3 mo. Pts underwent standard clinical examination of PsA activity. Me disease activity indexes DAS=4.0±1.4, DAS28=4.2±1.1, BASDAI=4.5±1.6; Me pts global disease activity VAS 56.9±17.1. All patients were evaluated for the presence of IBP by ASAS criteria, underwent sacroiliac joints (SIJs) X-ray (pelvic radiographs) and HLA B27 antigen status study. MRI of SIJs was performed in 79 pts, regardless of IBP presence, on Signa Ovation 0.35T. Radiographic sacroiliitis (R-SI) was identified according to New York criteria (unilateral grade≥3 or bilateral grade≥2). Bone marrow edema/ osteitis on MRI (STIR) was considered active MRI sacroiliitis (MRI-SI). X-ray and MRI results were evaluated by an independent reader. IBP was observed in 63 (66.3%) cases, MRI-SI in 28 of 79 (35.4%) examined cases, R-SI in 29 (30.5%) cases. Skin lesion severity was evaluated as body surface area (BSA) affected: minor at <3%, mild at 3-10%, severe at >10%. Pts were split into 2 groups (gr.): those with axial involvement (axPsA), that is with IBP and/or R-SI and/or MRI-SI; and those without axial involvement (having only peripheral PsA [pPsA]).The axPsA gr. included 65 (68.4%) cases, the pPsA gr. 30 (31.6%) cases. Multi-dimensional step-by-step discriminant analysis was used to identify a group of features that are more typical for the axPsA patients.Results:The following features proved to be the most informative: male sex (р = 0.300), presence of HLA B27 (р = 0.107), mild or high DAS (р = 0.098), skin lesion severity BSA>3% (р = 0.118), and CRP > 5 mg/L (0.038).Sensitivity of model 79.0%, specificity of model 57.7%. Area under ROC curve 0.756; 95% CI (0.642-0.869).Conclusion:It is a combination of features – male sex, HLA-B27 positivity, mild or high activity of peripheral arthritis according to DAS, CRP > 5 mg/L, and BSA> 3% – that constitutes a clinical predictor for the development of axial involvement in early psoriatic arthritis.References:[1]V. Psoriatic arthritis (PsA) is a heterogeneous disease manifested by peripheral arthritis, dactylitis, spondylitis, and enthesitis. PsA is often undiagnosed by dermatovenerologists because of the difficulty in identifying a variety of clinical signs. The early diagnosis of PsA and the accurate assessment of all its symptoms are necessary for the timely choice of optimal therapy.Objective: to assess the detectability of clinical signs of PsA in patients with psoriasis in dermatological practice.Patients and methods. The investigation enrolled 103 patients (47 men and 56 women) (mean age, 44.0±13.7 years) with psoriasis (its mean duration, 10.7±10.2 years), the average prevalence and severity according to the Body Surface Area (BSA) and the Psoriasis Area and Severity Index (PASI) were 9.3±13.6% and 15.4±12.5 scores, respectively. All the patients completed the Psoriasis Epidemiology Screening Tool (mPEST) and were examined by a dermatovenerologist and a rheumatologist. The diagnosis of PsA was based on the Classification Criteria for Psoriatic Arthritis (CASPAR). The investigators evaluated arthritis, dactylitis, enthesitis, and inflammatory back pain (IBP) according to the rheumatological standards: IBP by the Assessment of SpondyloArthritis International Society (ASAS) criteria, and enthesitis by the Leeds Enthesitis Index (LEI).Results and discussion. Sixty-one (59.2%) of the 103 patients with psoriasis were found to have PsA on the basis of the CASPAR criteria and the rheumatologist's examination. The dermatovenerologist diagnosed arthritis in a significantly smaller number of cases than did the rheumatologist: in 15 (24.6%) and 35 (57.4%) of the 61 patients (p<0.001), respectively. The dermatovenerologist and the rheumatologist demonstrated no significant differences in their clinical evaluation of dactylitis: it was detected in 37 (60.7%) and 40 (65.6%) of the 61 patients, respectively (p=0.32). Based on patient complaints and mPEST findings, the dermatovenerologist recorded pain in the calcaneal region in 32 (52.5%) patients. The rheumatologist identified ulnar, knee, and calcaneus enthesitis in 11 (18%), 8 (13.1%), and 25 (41%) patients, respectively. Based on complaints and mPEST findings, the dermatovenerologist detected back pain in 30 (49.2%) of the 61 patients. The rheumatologist diagnosed IBP in 21 (70%) of these 30 patients and mechanical back pain in 9 (30%). Thus, IBP was noted in 34.4% of PsA patients. Tendonitis was undiagnosed by the dermatovenerologist; the rheumatologist identified wrist tendonitis in 13 (21.3%) of the 61 patients with PsA.Conclusion. Dermatovenerologists frequently underestimate damage to the spine and entheses in patients with psoriasis. The introduction of the ASAS criteria for IBP and methods for assessing enthesitis in dermatological practice can improve the early diagnosis of axial lesion in PsA in patients with psoriasis.

Reconstructive tomography is a new technique in diagnostic radiology for imaging cross-sectional planes of the human body /1/. A collimated beam of X-rays is scanned through a thin slice of the body and the transmitted intensity is recorded by a detector giving a linear shadow graph or projection (see fig. 1). Many of these projections at different angles are used to reconstruct the body-layer, usually with the aid of a computer. The picture element size of present tomographic scanners is approximately 1.1 mm2.Micro tomography can be realized using the very fine X-ray source generated by the focused electron beam of a scanning electron microscope (see fig. 2). The translation of the X-ray source is done by a line scan of the electron beam on a polished target surface /2/. Projections at different angles are produced by rotating the object.During the registration of a single scan the electron beam is deflected in one direction only, while both deflections are operating in the display tube.

Spontaneous mammary tumors in females of a high tumor strain of C3H mice were cut into small fragments that were Implanted into the subcutaneous tissue of the back of males of the same strain, where they grew as transplantable tumors. When about Cm. In diameter daily fractional radiation was begun, applied to the tumors, the rest of the body being shielded by a lead shield. Two groups were treated with 150 and 200 r X-ray dally, of half value layer 0.6mm. copper; a third group was treated with 500 r cobalt radiation dally. The primary purpose was to examine the enzyme changes during radiation, with histochemlcal technics.

BACKGROUND Tuberculosis is a widespread and in many cases a fatal, infectious disease caused by many strains of mycobacterium complex usually mycobacterium tuberculosis. It can affect any part of the body but mainly the lungs hence called pulmonary tuberculosis. Tuberculosis in children has been less of a public health priority in the recent years despite the fact that TB has been a major cause of childhood morbidity and mortality worldwide with difficulties in diagnosis. Data on trends of childhood TB is rarely in published literature hence need for this research which will help in publishing OBJECTIVE To establish the trends in TB among children aged 1-12 years and its management over a period of five years from 2011- 2015 at Arthur Davison children’s hospital in Ndola and also to determine the proportion of TB in these children and to establish the age group most affected by TB over the period of five years and lastly to assess the proportion of TB and HIV as a co-morbidity METHODS The study reviewed all records that were registered in the MOH TB register at ADH from 2011 to 2015 coming up with a total number of 483 TB records and these records had satisfied the inclusion and exclusion criteria. The data capturing tool was used to collect the data. RESULTS : This study determined the trends of TB in children aged 1 to 12 over the five years period from a total of 483 reviewed paediatric TB cases , its proportion in these children, the age range most affected by TB and lastly assessed TB and HIV as a co-morbidity at Arthur Davison Children’s hospital. It revealed that the trends were decreasing and that out of 483 TB patient, the majority 157(32.5%) of the TB patients were seen in the year 2011, followed by 129(26.7%), 84(17.4%), 57(11.8%) and 56(11.6%) representing the year 2012, 2013, 2014 and 2015 respectively. Findings on the age range most affected are that the majority 282(58.4%) of the TB patients were aged (5-12) years old while 201(41.6%) were aged between (1-5) years old. Therefore, the age range most affected by TB was between (5-12) years. The TB proportion results showed that Out of 483, the majority 467(96.7%) of the TB patient type were newly diagnosed, followed by 13(2.7%), 2(.4%) and the rest 1(.2%) that were Relapse, Trans-in and Treatment resumed respectively. And that many 467(96.7%) of TB condition types were PTB while 16(3.3%) were EPTB. Furthermore, the majority of 463(95.9%) were diagnosed by X-ray while 20(4.1%) were diagnosed by smear microscopy. And lastly In terms of HIV as a co-morbidity, the majority 175(36.2%) of the TB patients were HIV positive, followed by 173(35.8%) that were HIV negative while the rest 135(28%) of the TB patients were not tested for HIV CONCLUSIONS This study reports that the trends of TB in children aged 1-12 years have been decreasing from 2015 to 2011 .And the age range most affected was found to have been 5-12 years and the proportion of TB cases were mostly newly diagnosed and the diagnosis was made by use x-ray and lastly the large proportion of TB patients had tested positive for HIV as a co-morbidity. Therefore efforts should be made to sustain this decreasing trend of childhood TB.

Nanoparticles may invade directly into the internal body through the respiratory or digestive system and diffuse inside body. The behavior of nanoparticles in the internal body is also essential to comprehend for the realization of DDS. Thus it is necessary to reveal the internal dynamics for the proper treatments and biomedical applications of nanoparticles. In the present study the plural methods with different principles such as X-ray scanning analytical microscope (XSAM), MRI and Fluorescent microscopy were applied to enable the observation of the internal diffusion of micro/nanoparticles in the (1) whole body level, (2) inner organ level and (3) tissue and intracellular level. Chemical analysis was also done by ICP-AES for organs and compared with the results of XSAM mapping.

Background:Evidence on the efficacy of biologics in the treatment of psoriatic arthritis (PsA) patients with axial manifestations affecting 30-70% of PsA patients is limited. Secukinumab (SEC) has provided significant and sustained improvement in the signs and symptoms of active PsA and ankylosing spondylitis.Objectives:This study aims to analyze the experience of using SEC for PsA patients with axial involvement in real-world setting.Methods:Multicentric observational, longitudinal, retrospective study conducted in a tertiary hospital between January 2016 and December 2019. Patients with PsA (CASPAR criteria) and clinical and/or image diagnosis of axial involvement receiving at least one dose of SEC were included. Patients with non-pathological sacroiliacs x-ray and MRI had to have spinal pain VAS ≥4/10 after failure to NSAIDs, prior to the onset of SEC, to be included. Medical records were reviewed to collect demographic and clinical data, features of PsA (manifestations, treatments and activity assessment). Descriptive statistics and then a comparative analysis with the Studentt-test to analyze the effectiveness of SEC were performed.Results:Of 98 PsA patients treated with SEC, 58 (59.2%) had axial involvement, of which 41 (71%) female. Mean age was 54 y.o (SD 10) and average duration of the disease was 10 years (SD 8). All 58 patients had peripheral disease (33% joint erosions), 55 (95%) had psoriasis, 20 (34%) showed dactilitis and 39 (67%) had enthesitis. Sacroiliacs x-ray was damaged in 38 (66%) patients (grade I-IV) and 23 (40%) pathological MRI, with HLAB27+ at 8 (14%) patients. Average BMI was 29 (SD 8), with an obesity rate of 33% (19 pt). Observed comorbidities were hypertension (27 pt, 47%), diabetes mellitus (6 pt, 10%), dyslipidemia (23 pt, 40%), active smoking (18 pt, 31%) and malignancy (6 pt, 10%). Regarding previous treatments, 90% had received cDMARDs, particularly methotrexate (86%) and 40 (69%) had been exposed to at least one bDMARD (15 pt to one, 9 to two, 6 to three and 10 to four or more). 7 patients were on 300 mg dose and 51 patients on 150 mg dose (dose escalation to 300 mg was performed in 16 patients and 44% respond and maintain SEC). Average drug survival time was 1.4 (SD 1) years. At 6 months of SEC therapy, tender and swollen joint count, spinal pain VAS, CRP, ASDAS-CRP and DAPSA had significantly decreased (Table 1). 29 (50%) patients suspended SEC during follow-up due to primary ineffectiveness (8), secondary ineffectiveness (16), adverse events (3), latex allergy (1) and remission (1).
