IT IS PROVERBIAL that a fool can ask more questions than a wise man can answer. It is not proverbial that for the question-asking fool there is some hope and for the others, none. Now it is my intention on this occasion to play the fool awhile, to ask a great number of questions, and I cordially invite you to join me in this game. I want to inquire into The World of the Rheumatic Child, into his internal as well as into his external world, or, as Claude Bernard has phrased it, into his milieu interieur and into his milieu exterieur. Now there is some method to my folly, and it amounts to this. We know a great deal about the disease rheumatic fever and about its devastating effects within the body of its victim. But we do not know a great deal, indeed only a very little, about the victim within whose body the disease effects its devastations. I said—we know a great deal about the disease itself. In preparation for this talk I "re-surveyed the literature" and I found it, as I have known it to be, not only enormous in quantity but most impressive in quality. It is literally studded with masterpieces of etiologic research, of clinical surveys, of pathologic studies, of follow-up surveys, of epidemiologic analyses, and of therapeutic enterprises. In my review of the literature I came upon some old and esteemed friends whose works I had witnessed "in the making," the studies, for example, of Wyckoff, and those of Alfred Cohn; Claire Ling's penetrating statistical analyses, Pearl Raymond's biologic speculations, May Wilson's classical and encyclopedic résumé of knowledge—and upon a host of others, too numerous, really, to catalogue.

BACKGROUND The world is hit by a global pandemic caused by severe acute respiratory syndrome coronavirus, a new genotype of the virus, which causes coronavirus disease, Covid19. The situation has challenged the entire scientific community nationally as well as internationally to fight back this deadly disease. Since its beginning in November 2019, it has disseminated throughout the human race, regardless of all the measures taken by healthcare sectors, governments, and world health organizations as well. Numerous investigations show that this virus uses air as a passage to commute and spread, the disease most commonly spreads through droplet infections and when comes in contact with the mucous membrane, enters the body. Entire medical staff along with scientists of various nations are working perpetually to develop successful vaccines and drugs to fight back this virus. Amongst various vaccines developing across the world, many of them are in their clinical trials and human trial phases and those which have succeeded in all the trial phases are getting delivered to citizens since December 2020. The present article aims to provide a review of the literature on the type of vaccinations that have been developed so far with their mechanism of action and their basic formulations. Objective. Granular cell tumors arise from neurogenic mesenchymal stem cells and can occur anywhere throughout the body. They rarely present as breast masses and should be included in the differential diagnosis of pediatric breast neoplasms. We report a rare presentation of a pediatric breast granular cell tumor and a review of the literature.Participant. A 15-year-old female presented with an enlarging breast mass. She underwent ultrasound imaging and excisional biopsy, which revealed a granular cell tumor. Granular cell tumors of the breast are difficult to diagnose using ultrasound and mammography due to numerous similarities to other breast masses. Histopathologic staining best differentiates breast granular cell tumors from other breast masses with their positive staining for S100, CD68, and neurospecific enolase.Conclusion. Although rare, granular cell tumors of the breast should be considered as a possible diagnosis for pediatric breast masses to allow for proper management and follow-up for these patients. Although rare, these tumors do have malignant potential necessitating a correct and timely diagnosis.

Saddharmasmṛtyupasthānasūtra is an Indian Buddhist sutra dating to the first half of the first millennium. Chapter 7 of the sutra consists of a very long meditation on the body, unusual in Buddhist literature for its anatomical, especially osteological, detail. The meditation also includes extensive descriptions of many internal worms as well as the internal winds that destroy the worms at the moment of death. The sutra has several elements not found in other Buddhist texts. For example, the Saddharmasmṛtyupasthānasūtra meditation on the body includes extensive descriptions of things in the external world (e.g., rivers, mountains, flowers) and designates them as the “external body”. Most strikingly, the meditation on the body found in Saddharmasmṛtyupasthānasūtra differs from the general scholarly perception of Buddhist meditations on the body in that it does not emphasize impurity or generate repulsion. Instead, the sutra guides the meditator through a dispassionate and “scientific” observation of the body and the world.

As well as being an extremely widespread addiction, tobacco smoking is a well-known cause of numerous deaths around the world. Most (80–90%) of lung cancer cases occur among smokers. The awareness of harmful effects of smoking is growing constantly. Manufacturers of tobacco products have recently opted to face this challenge by marketing heated tobacco products. Based on the performed research, tobacco companies maintain that these products are less harmful. They advertise their products as a smokeless, unconventional, less harmful cigarette. Due to the short follow-up period, the studies on consequences of using heated tobacco products are still scarce. This article presents a review of the literature focused on the use of heated tobacco products as an alternative to traditional cigarettes.

Abstract Background Male circumcision is one of the oldest and most commonly performed surgical procedures in the world; in fact, one of every 3 males in the world is circumcised. Plastibell circumcision is the commonest procedure performed worldwide with higher rates of satisfaction and lower rates of complications. Case presentation We present two cases, 4 and 5 months old with proximal migration of Plastibell ring with penile incarcerations that were managed successfully. We aim to highlight the necessary precautions needed to avoid serious complications after a minor surgical procedure. Conclusions Plastibell circumcision is a minor surgical procedure that is underestimated with the potential for major and serious complications. The implementation of the law against the practice of non-professional individuals, standardization of the procedure, and improvement of communication are highly recommended to avoid unnecessary and serious complications. Plastibell ring circumcision still needs further studies to examine ring antimigration, re-designing, and the best types and sizes of threads used. Lastly, studies are also needed to determine a cost-effective routine follow-up visit post-Plastibell circumcision procedures.

Over the last one-year COVID 19 has affected millions of people around the world. Though initially it was thought to be an infection of respiratory system, but it involves any organ of the body. Its morbidity and mortality are more when it involves patients with preexisting cardiovascular diseases and also when it causes a cardiovascular complication. COVID 19 can produce a number of cardiovascular complications including myocardial infarction, heart failure, myocarditis, arrhythmia, thromboembolism etc. Following recovery from COVID 19 infection evaluation, follow up and management of these cardiovascular conditions, which may be sometimes in subclinical form, is essential. Though there is lack of adequate studies or guidelines, we tried to discuss the different aspects of Post- COVID cardiovascular care depending on the existing evidence and previous guidelines. In this paper, I give an account of a hitherto neglected kind of ‘here’, which does not work as an intentional indexical. Instead, it automatically refers to the immediate perceptual environment of the subject’s body, which is known as peripersonal space. In between the self and the external world, there is something like a buffer zone, a place in which objects and events have a unique immediate significance for the subject because they may soon be in contact with her. I argue that seeing objects as being here in a minimal sense means seeing them in the place in which the perceptual system expects the world and the body to collide. I further argue that this minimal notion of here-content gives rise to a tactile sense of presence. It provides a unique experiential access to the reality of the seen object by making us aware of its ability to have an effect on us.

The lymphatic system is very important in the immunological activities of the body. Clinicians confirm the diagnosis of infectious diseases by palpating the involved cutaneous lymph node for changes in size, heat, and consistency. Clinical pathologists diagnose systemic diseases through biopsies of superficial lymph nodes. In many parts of the world the goat is considered as an important source of milk and meat products. The lymphatic system has been studied extensively. These studies lack precise information on the natural morphology of the lymph nodes and their vascular and cellular constituent. This is due to using improper technique for such studies. A few studies used the SEM, conducted by cutting the lymph node with a blade. The morphological data collected by this method are artificial and do not reflect the normal three dimensional surface of the examined area of the lymph node. SEM has been used to study the lymph vessels and lymph nodes of different animals. No information on the cutaneous lymph nodes of the goat has ever been collected using the scanning electron microscope.
