Morphological features of the mother–placenta–fetus system on the background of sexually transmitted infections

The objective: to identify morphological substrate of placental disorders in women with urogenital infections and miscarriages and spontaneous abortions, as well as women diagnosed with «sexually transmitted infections» who gave birth at term. Materials and methods. A morphological study of samples from 80 women of reproductive age was performed. Patients were divided into 4 groups. At the first stage, histological and immunofluorescent studies of abortuses were performed in two groups of women with spontaneous abortions and missed abortions. The I group consisted of samples from 20 patients with urogenital infections, the II group – samples from 20 uninfected women. At the first stage, a morphological (histological and immunofluorescent) study of abortions was performed in two groups of women with missed and spontaneous abortions. The III group consisted of samples from 20 patients with urogenital infections, the IV group (control) – 20 samples from uninfected women. The second stage involved organometric and histostereometric examination of the placenta from women diagnosed with «sexually transmitted infections» without differentiating the forms of the infectious process (main group, n=10), who gave birth at term, and women who were uninfected (control group, n=10) and also gave birth at term. Results. Morphological study of abortuses from infected patients showed that urogenital infections are characterized by pronounced inflammatory changes in the form of deciduitis (9 specimens – 45%); chorionitis (5 specimens – 25%) and syncytial endometritis (2 specimens – 10%). The morphological picture of necrobiosis was revealed in 25% of samples (5 specimens). In addition, amnionitis (6 specimens – 30%), villisitis (4 specimens – 20%) and basal deciduitis (3 specimens – 15%) were detected. In the group of uninfected women, the leading pathomorphological changes were signs of placental immaturity and changes in vascularization and sclerosis of the villi. Most of the studies of placenta and membranes from women who gave birth at term and had urogenital infection showed significant changes in the villous chorion with the involvement of villi of different calibers. There was a significant increase in the proportion of micropathology. The presence of small necrosis of villi was established in almost 1/3 of all cases, extensive areas of necrosis occurred, but there was no significant difference between two groups. There was a decrease in the volume of the chorionic epithelium compared with the control group. Conclusions. Pathomorphological changes of abortuses and placentas in women with STIs reflected the main links in the pathogenesis of infectious and inflammatory processes and were different from such changes in the absence of this pathology. In particular, urogenital infection is characterized by changes associated with inflammatory manifestations in abortuses, and in the group of uninfected women - with signs of placental immaturity and changes in vascularization and sclerosis of the villi. Histostereometry also revealed significant differences in the structure of the placentas of women of the main and control groups, in particular, a significant increase in the proportion of micropathology. Therefore, the prevention of pregnancy complications and pregravid preparation in women diagnosed with STIs is essential for preventing reproductive failures. Abstract Background Bilateral salpingectomy has been proposed to reduce the risk of ovarian cancer, but it is not clear whether the surgery affects ovarian reserve. This study compares the impact of laparoscopic hysterectomy for benign disease with or without prophylactic bilateral salpingectomy on ovarian reserve. Methods Records were reviewed for 373 premenopausal women who underwent laparoscopic hysterectomy with ovarian reserve for benign uterine diseases. The serum anti-Müllerian hormone (AMH), follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), and three-dimensional antral follicle count (AFC) were assessed before surgery and 3 and 9 months postoperatively to evaluate ovarian reserve. Patients were divided into two groups according to whether they underwent prophylactic bilateral salpingectomy. The incidence of pelvic diseases was monitored until the ninth month after surgery. Results There was no significant difference between the two surgery groups in terms of baseline AMH, E2, FSH, LH, and AFC (all P > 0.05). There was no difference in potential bias factors, including patient age, operative time, and blood loss (all P > 0.05). There was also no significant difference between the two groups 3 months after surgery with respect to AMH (P = 0.763), E2 (P = 0.264), FSH (P = 0.478), LH (P = 0.07), and AFC (P = 0.061). Similarly, there were no differences between groups 9 months after surgery for AMH (P = 0.939), E2 (P = 0.137), FSH (P = 0.276), LH (P = 0.07) and AFC (P = 0.066). At 9 months after the operation, no patients had malignant ovarian tumors. The incidences of benign ovarian tumors in the salpingectomy group were 0 and 2.68 % at 3 and 9 months after surgery, respectively, and the corresponding values in the control group were 0 and 5.36 %. The incidences of pelvic inflammatory disease in the salpingectomy group were 10.72 and 8.04 % at 3 and 9 months after surgery, respectively, while corresponding values in the control group were 24.13 and 16.09 %. Conclusions Prophylactic bilateral salpingectomy did not damage the ovarian reserve of reproductive-age women who underwent laparoscopic hysterectomy. Prophylactic bilateral salpingectomy might be a good method to prevent the development of ovarian cancer. Larger clinical trials with longer follow-up times are needed to further evaluate the risks and benefits.

Abstract: Premarital sexual behavior in adolescence is a high risk factor for sexually transmitted infections. Sexually transmitted infections (STIs) are infections that are generally transmitted through sexual contact. Differences in social, cultural, and economic factors were found to affect the incidence and prevalence of sexually transmitted infections between different groups in a population. These are likely caused by differences in the knowledge and attitudes of adolescents living in urban and rural areas. This study was aimed to obtain the differences in the level of knowledge and attitudes toward STIs of adolescents in urban and rural area schools. This was a descriptive study with a cross-sectional design conducted on 50 adolescents of urban senior high schools and 50 adolescents of rural senior high schools/vocational high schools. Questionnaires were distributed via email by using Google form. The results showed that adolescents living in urban areas had good knowledge about STIs meanwhile adolescents living in rural areas had fair knowledge. The attitudes about STIs of most adolescents living in urban areas and rural areas were good. In conclusion, adolescents living in urban areas had better knowledge about STIs than those living in rural areas, however, there was no significant difference in attitudes about STI between the two regions. Equal distribution of education in Indonesia is needed in urban as well as in rural areas.Keywords: sexually transmitted infections, adolescents, knowledge, attitudes, urban and rural Abstrak: Perilaku seksual pranikah pada usia remaja merupakan faktor risiko tinggi terhadap infeksi menular seksual (IMS). Infeksi menular seksual merupakan infeksi yang umumnya ditularkan melalui hubungan seksual. Perbedaan faktor sosial, kultural maupun ekonomi dapat memengaruhi insiden dan prevalensi IMS antara kelompok yang berbeda dalam suatu populasi. Hal tersebut kemungkinan besar disebabkan oleh adanya perbedaan pengetahuan dan sikap remaja yang tinggal di wilayah perkotaan dan pedesaan. Penelitian ini bertujuan untuk mengetahui perbedaan tingkat pengetahuan dan sikap remaja terhadap IMS di sekolah wilayah perkotaan dan pedesaan. Jenis penelitian ialah deskriptif dengan desain potong lintang dan kuesioner didistribusikan secara daring menggunakan Google Form. Responden ialah 50 remaja di SMA wilayah perkotaan dan 50 remaja di SMA/SMK wilayah pedesaan. Hasil penelitian mendapatkan bahwa pengetahuan remaja tentang IMS pada siswa SMA perkotaan sebagian besar berada dalam kategori baik sedangkan pada siswa SMA/SMK pedesaan sebagian besar berada dalam kategori cukup. Sikap remaja tentang IMS pada siswa SMA/SMK perkotaan dan pedesaan sebagian besar baik. Simpulan penelitian ini ialah remaja perkotaan memiliki pengetahuan yang lebih baik mengenai IMS dibandingkan remaja di pedesaan namun tidak terdapat perbedaan sikap remaja yang bermakna mengenai IMS antara kedua wilayah. Pemerataan pendidikan di Indonesia dibutuhkan di wilayah perkotaan dan pedesaan.Kata kunci: infeksi menular seksual, remaja, pengetahuan, sikap, perkotaan dan pedesaan

Chlamydia trachomatis is a Gram-negative bacterium that causes urogenital tract infections, and ocular infections including trachoma, neonatal conjunctivitis and adult chlamydial inclusion conjunctivitis. A positive C. trachomatis diagnosis in children often raises suspicions of sexual abuse. While outer membrane protein A (ompA) genotypes A–C are non-invasive and are associated with trachoma; ompA genotypes D–K are often associated with sexually transmitted urogenital infections or sexually acquired chlamydial conjunctivitis. A 10-year-old female presented with a 7-month history of unilateral conjunctivitis with itching, watering and hyperaemia. She had recently moved from an urban centre in Afghanistan to the UK. A conjunctival swab taken from the child tested positive for C. trachomatis. Application of ompA genotyping to conjunctival swab chlamydial DNA demonstrated that the C. trachomatis had an ompA genotype C. Chlamydial strains with this ompA genotype cause trachoma and have never previously been associated with urogenital infection. This result supported cessation of child protection investigations.

Background Pancreatic cancer is characterized initially by non-specific abdominal symptoms followed by rapid tumor progression. Although chronic pancreatitis is a benign disorder, it can be one of the causative factors of pancreatic cancer. The level of the tumor marker carbohydrate antigen 19–9 (CA 19–9) in pancreatic cancer does not correlate with the stage of the neoplasm. Soluble interleukin 2 receptor (sIL-2R) is a cytokine that shows increased levels during some inflammatory processes and malignant disorders. Aim Our aim in this study was to investigate whether sIL-2Rα levels can be used in association with CA 19–9 in the early diagnosis of pancreatic cancer and chronic pancreatitis. Patients Serum samples were obtained from the blood of 21 pancreatic cancer patients without distant metastasis who were deemed inoperable, 16 chronic pancreatitis patients and 20 normal volunteers. Results We did not find any significant differences in CA 19–9 levels between normal controls and patients with chronic pancreatitis. There was a significant difference in the levels between the control group and the pancreatic cancer group (p=0.003) and between patients with chronic pancreatitis and those with pancreatic cancer (p=0.004). Although there was no significant difference in sIL-2Rα levels between the control group and the patient groups, we found a slight correlation between sIL-2Rα and CA 19–9 levels in the pancreatic cancer group (p=0.003, r=0.623) and a more marked correlation in the chronic pancreatitis group (p<0.01, r=0.751). Conclusion According to our results, sIL-2Rα alone is not a good candidate marker in the diagnosis of pancreatic cancer; it can, however, be used in association with CA 19–9 for this purpose.

Background: Sexually transmitted infections (STIs) present a huge burden of disease and adversely affect the reproductive health of people. The disease prevalence is about 6% in India. This study is done to determine the prevalence of STIs in women of reproductive age (15-49yrs) attending gynaec outpatient block at Institute of Obstetrics and Gynaecology, Chennai and to identify the risk factors.Methods: It is a prospective analytical study conducted at IOG from Febraury 2010 to January 2011 where 1000 women of reproductive age attending gynec op were included of which 500 women were asymptomatic and 500 women were symptomatic for STIs. A well-structured proforma was prepared for selection of women, history, examination, investigations and treatment. Asymptomatic women were also screened and treated. Partners were also screened and treated. Depending upon the statistical data, the risk factors were identified and analysed.Results: Overall prevalence of STI was 27.2% of which 22.5% was in symptomatic group and 4.7% in asymptomatic group. Bacterial vaginosis was the commonest STI. The important risk factors identified were age group between 26-30 years, high risk sexual behaviours, poor socioeconomic factors, poor menstrual hygiene and lack of contraception.Conclusions: STIs cause major health problem and it is important to diagnose and treat them at the earliest. The importance of STIs has been more widely recognised since the advent of the HIV/AIDS epidemic, and there is good evidence that their control can reduce HIV transmission. Women diagnosed with one STI should be screened for other STI due to coexistant infections. Screening and treatment of partners and follow-up tests of cure should be performed wherever possible.

Male circumcision is highly protective against urinary tract infections, inflammatory conditions of the penis, sexually transmitted infections, and urogenital cancers. We aimed to reintroduce newborn male circumcision through the creation of a training program in Port-au-Prince, Haiti—an area with a considerable burden of preventable urogenital infections, sexually transmitted infections, and low circumcision rate—after an earlier study reported that a majority of Haitian medical providers were in need of and wanted newborn circumcision training. The program was conducted at the GHESKIO Health Centers, a large, non-governmental clinic offering comprehensive pediatric and adult health services. Two Haitian obstetricians and seven nurses learned circumcision procedures. On training completion, one of two obstetricians achieved surgical competence. Introduction of a newborn male circumcision training program was feasible, achieving an acceptable rate of procedural competency and high-quality services. Permanent resources now exist in Haiti to train additional providers to perform newborn male circumcisions.

Objective. Polycystic ovarian syndrome (PCOS) is the most common endocrine disorder affecting 5–10% of women in reproductive age. Insulin resistance, dyslipidemia, glucose intolerance, hypertension, and obesity are metabolic disorders accompanying the syndrome. PCOS is a chronic proinflammatory state and the disease is associated with endothelial dysfunction. In diseases with endothelial damage, hearing in high frequencies are mostly effected in early stages. We evaluated extended high frequency hearing loss in PCOS patients.Material Methods. Forty women diagnosed as PCOS and 25 healthy controls were included in this study. Age and BMI of PCOS and control groups were comparable. Each subject was tested with low (250–2000 Hz), high (4000–8000 Hz), and extended high frequency audiometry (8000–20000). Hormonal and biochemical values including LH, LH/FSH, testosterone, fasting glucose, fasting insulin, HOMA-I, and CRP were calculated.Results. PCOS patients showed high levels of LH, LH/FSH, testosterone, fasting insulin, glucose, HOMA-I, and CRP levels. The hearing thresholds of the groups were similar at frequencies of 250, 500, 1000, 2000, and 4000 Hz; statistically significant difference was observed in 8000–14000 Hz in PCOS group compared to control group.Conclusion. PCOS patients have hearing impairment especially in extended high frequencies. Further studies are needed to help elucidate the mechanism behind hearing impairment in association with PCOS.
