Abstract Background In recent years, the number of adolescents engaging in premarital sex has increased, and an increasing number of childhood sexual abuse (CSA) cases have been reported in China. Many studies have indicated that CSA has a well-established association with risky sexual activities. However, only a limited number of studies have explored possible reasons for this association among middle school students, a population that may engage in premarital sex, which is critical for the development of interventions to prevent risky sexual behavior. Based on random samples of middle school students from a Chinese city, this article investigated the relationship between CSA and students’ premarital sexual permissiveness (PSP). Methods In a cross-sectional study conducted between 2016 and 2017 in Luzhou, China, 2292 middle school and high school students aged 12–18 years were recruited by multistage random sampling. All students were administered anonymous questionnaires. Multiple linear regression and binary logistic regression analyses were conducted to analyze the relationship between CSA and PSP. Results The prevalence of CSA was 15.4% (354/2292; 95% CI: 14–16.9%). A higher percentage of male respondents (18.2%) than female respondents (12.9%) had experienced CSA. A positive association between CSA and PSP was found among students. Respondents who had suffered CSA exhibited greater PSP, and this relationship was observed in the male sample, female sample and the total student sample in Luzhou (β = − 3.76, P < 0.05; β = − 2.79, P < 0.05; and β = − 2.84, P < 0.05, respectively). Respondents who had suffered CSA were also more likely to express a double standard about premarital sex (odds ratio [OR] =1.41, P < 0.05), especially among male students (odds ratio [OR] =1.63, P < 0.05). Conclusions Sex differences in CSA and the relationship between CSA and PSP were significant among this large sample of middle school students in Luzhou (China). The findings suggest that experiencing CSA may be closely related to youth attitudes toward premarital sex, especially among males. Therefore, it is important to emphasize the prevention of CSA and provide adolescent reproductive health programs to reduce the impact of CSA on sexual cognition and attitude, prevent premarital sex and promote positive attitudes toward sexual equality for middle school students.

The transition period from childhood to adulthood often causes teenagers to experience problems. This period occurs between the ages of 10 to 19 years and is not married. The Indonesian Society of Dermatology and Venereology (INSDV) report the number of sexually transmitted diseases (STDs) for adolescents in Indonesia was increased from 2017. The exposure of adolescents to STDs can be triggered by juvenile delinquency such as premarital sexual relations, drugs, free sex which has an impact on pregnancy out of marriage, dropping out of school, early marriage, abortion, risk of childbirth, STDs. Efforts to anticipate juvenile delinquency are through the establishment of the Youth Integrated Service Post (YISP). It is part of the CAE program (Children, Adolescents and Elderly), as the flagship program of the Ministry of Health, and implemented at the Surabaya City Health Office. This program puts forward a preventive approach because it focuses on primary health care activities holistically. This program is the collaboration with the Benowo Public Health Center (PHC), Surabaya City Government, which aims to foster YISP through health education to solve problems during puberty. This activity was attended by 24  youth assisted by the Benowo PHC. Health education activities were provided through a combination lecture method with the visual education method (documentary video screening), and at the end of the activity, the evaluation of adolescent knowledge was carried out. The results of community service activities showed that the adolescents have increased knowledge about how to deal with problems at puberty, and the best score of 90. Similar coaching activities can be used as preventive measures carried out on an ongoing basis and can be developed towards regular health screenings specifically STDs for teenagers. The spiritual guidance of reproductive health and safety is oriented towards strengthening moral and religious values in addressing adolescent reproductive health problems with an approach on several aspects, such as preventive, curative, rehabilitative, and developmental. The booklet is a medium of guidance/learning that can attract interest and help teenagers understand the guidance material. This study aimed to determine the effect of the spiritual guidance of reproductive health care professionals by booklets on changes in adolescent attitudes towards dating status in premarital sexual behavior. A combination of qualitative and quantitative experimental research with a pre-experimental design, one group pretest-posttest, with a total sample of 26 adolescents, selected a purposive sampling technique. The spiritual guidance intervention used booklets for six meetings with 60-90 minutes each meeting within 2 months. Data were collected through pre-test and post-tests using interview sheets and Likert scale questionnaires. Data were analyzed qualitatively and statistically tests using the Wilcoxon test. The results showed that the sexual behavior of adolescents in dating was including holding hands, embracing the arms/shoulders and waist, hugging, kissing cheeks and lips, stroking the head, laying on the partner's thighs, and having sex with condoms. Then, from the statistical test, the negative ranks between the pre-test and post-test values were 0. Meanwhile, the positive ranks between the pre-test and post-test values indicated that 24 respondents with a positive value and a mean rank value of 13.00, and 2 respondents with a pre-post value remain the same. The Wilcoxon test shows the p-value (sig) 0.000 <0.05. Thus, the spiritual guidance of reproductive health care professionals using booklets significantly affected changes in adolescent attitudes towards dating status in premarital sexual behavior.
