The paper deals with an evaluation of the quality of services provided by healthcare organizations. First, an index representing a patient’s health condition is described, then its changes before and after being treated by a given entity are employed as a criterion to assess the operations of this entity. The index of a patient’s health condition is based on the theory of survival analysis, while a model of random effects is used to determine the quality of services based on health value added

TThe distribution of the limited financial resources in the state hospitals in Kyrgyzstan is uneven. The problems associated with the current method of distribution of resources: the poor quality of services at the level of polyclinics and high hospitalization rates that require an evaluation of the budget allocation of healthcare organizations operating in the Single Payer system. In order to improve the efficiency of resource use it is suggested to review the principles of allocation of resources to the primary level of patient care.

Background. Chronic mesenteric ischemia (CMI) is characterized by long-standing abdominal symptoms due to insufficient mesenteric circulation. Data on the effect of revascularisation on quality of life (QoL) for CMI are scarce. This study is the first to evaluate the impact of revascularisation on quality of life. Methods. Seventy-nine patients with CMI or acute-on-chronic mesenteric ischemia (AoCMI) underwent an intervention of one or more mesenteric arteries between January 2010 and July 2012. QoL before and after intervention was measured with the EuroQol-5D. Preintervention questionnaires were of standard care. Postintervention data were obtained by resending a questionnaire to the patients between February and May 2013. To investigate the clinical relevance of our findings, the minimal clinically important difference (MCID) was used. Since there is no established MCID for CMI, we used the literature reference MCID of inflammatory bowel syndrome (IBS) of 0.074. Results. Fifty-five (69.6%) of 79 patients returned their questionnaire and 23 (29.1%) were completely filled out. There was a significant increase of the median EQ-index score from 0.70 to 0.81 (p=0.02) and a significant reduction of symptoms in the domains usual activities (34.4%) and pain/discomfort (32.3%). There was a significant improvement of 17% in overall current health condition (VAS) (p=0.001). The MCID between baseline and postoperative EQ-5D index score was 0.162, indicating a clinically relevant improvement of quality of life after revascularisation. Conclusion. Quality of life of CMI patients is improved after mesenteric artery revascularisation.

Epistaxis is the most prevalent clinical symptom in Hereditary Haemorrhagic Telangiectasia (HHT), causing anaemia and decreasing the quality of life (QOL). Since 2013, in Hospital Universitario Fundación Alcorcón, more than 150 HHT patients have been treated by nose sclerotherapy on demand. This study shows the results of 105 patients treated with sclerotherapy between 2017 and 2019. HHT-ESS (epistaxis severity score) was used to measure the severity and frequency of epistaxis. QOL was determined before and after treatment by EuroQol-5D (EQ-5D) and the visual analogue scale (VAS) on the health condition. According to HHT-ESS before treatment, 22 patients presented mild, 35 moderate, and 47 severe epistaxes. Sclerotherapy significantly decreased the frequency and severity of epistaxis, with a significant drop of HHT-ESS in 4.6 points, from 6.23 ± 2.3 to 1.64 ± 1.6. Furthermore, the QOL significantly improved, the EQ-5D scale raised from 0.7 ± 0.26 pre- to 0.92 ± 0.16 post-treatment (p < 0.05). Additionally, VAS mean value showed a significant increase from 4.38 ± 2.4 to 8.35 ± 1.2. The QOL improvement was correlated with the ESS decrease. In conclusion, this study shows that on-demand sclerotherapy at the office significantly reduces HHT epistaxis as well as improved the patients’ QOL.

Cerebrovascular  accident  (CVA)  is one of the foremost reasons leading to mortality and morbidity throughout the world. It is the third biggest killer in India after a heart attack and cancer.  It is like a chronic health condition which negatively impacts on quality of life. Dysphagia is one of the most successive side effects in patients with a stroke which is a loss of motion of throat muscles. This condition can disturb the gulping procedure and make eating, drinking, taking prescription and breathing trouble. Dysphagiaexercises are designed toenhancing muscles and coordinatingthe nervesandmuscles involved in swallowing. The pre-experimental design was used with 60 samples who matched the inclusion criteria who were selected by purposive sampling technique. The study aims is to assess the swallowing ability before and after dysphagia exercisesamong patients with cerebrovascular accidents. Demographical variables were collected by using self- structured questionnaires and Modified Mann Assessment of Swallowing Ability Scale. The study uncovered that the pre-test mean swallowing ability score was 42.45 with a standard deviation of 8.63 among the cerebrovascular accident patients. The post-test swallowing ability mean score of 50.54% with a standard deviation of 8.23 among the cerebrovascular accident patients. The study findings concluded thatDysphagia practices which was an effective, inexpensive, simple measure for improving swallowing ability among patients with cerebrovascular accident.

Background: Hospitals are one of the most important units in providing healthcare services. The assessment of the quality of provided services is essential in these settings. Patient satisfaction is one of the key indicators of service quality in healthcare organizations. Objectives: This study aimed at investigating the gap between the perceptions and expectations of service recipients in Shohada-e-Tajrish Hospital (Tehran, Iran), using the SERVQUAL model in 2020. Methods: This cross-sectional study was conducted on patients with cancer, who were upon discharge from Shohada-e-Tajrish Hospital. Patients (> 18 years) with a cancer diagnosis, who were being discharged from the oncology ward of the hospital, were included in this study. The sample size was estimated at 118. The required information was collected, using the modified SERVQUAL questionnaire. Statistical analyses were performed in SPSS version 20, using a paired t test and one-sample t-test. Results: The results showed that the quality of the provided services from the patients’ perspective was significantly lower than their expectations. The mean gap between the expectations and perceptions of patients was significantly above zero (P < 0.001). The most influential factor in improving patient satisfaction with the quality of services was assurance, followed by reliability, while the least effective factor was empathy. The correlation coefficient between the general status of provided services from the patients’ point of view and loyalty was 0.740 (P < 0.001). Conclusions: The findings of the present study indicated that patient expectations were higher than their satisfaction. Therefore, hospital authorities should take major steps to improve the quality of services, especially reliability and responsiveness, through proper planning, prioritization of services, and review of processes contributing to patient expectations. The existing gaps can also be addressed by considering the perspectives of patients as the clients of healthcare organizations.
