The Effect of Different Implant-Abutment Connections on Screw Joint Stability

Dental implants with an internal connection have been designed to establish a better stress distribution when lateral external forces act on the prosthesis and minimize the forces transmitted to the fastening screw. In the present study, 10 externally and 10 internally hexed implants were tested with a compressive force applied with an Instron Universal machine. Four cycles of loading-unloading were applied to each specimen to achieve displacements of 0.5, 1, 2, and 2.5 mm. The mean loads for the first cycle were 256.70 N for the external connection and 256 N for the internal connection implants. The independent t test did not reveal any significant differences among the 2 tested groups (P = .780). For the second cycle, the mean loads needed for a displacement of 1 mm were 818.19 N and 780.20 N for the external connection and the internal connection implants, respectively. The independent t test revealed significant differences among the 2 tested groups (P < .001). In the third cycle, the mean load values for a 2-mm displacement were 1394.10 N and 1225.00 N. The independent t test revealed significant differences among the 2 tested groups (P < .001). The mean loads for the fourth cycle were 1488.00 N for the external connection and 1029.00 N for the internal connection implants. These loads were required for a displacement of 2.5 mm. The independent t test revealed significant differences among the 2 tested groups (P < .001). The results of this in vitro study suggest that the internal connection design of the examined implant system could not prevent screw loosening during overloading. No implant or prosthesis failure was noticed in either group.

(1) Background: In today's dentistry, implantology has become a therapeutic resource of choice in certain clinical situations. The design of implants has evolved in several aspects since their inception. Dental implants were initially designed with an external hex connection, although due to force transmission and security in the adjustment of the prosthesis, later implants featured an internal hex connection. This study aims to analyse the mechanical properties of two types of implants (an internal connection and an external connection) from the same manufacturer and their different prosthetic components (union screw between implant and prosthetic abutment, and the abutment itself) when subjected to different types of load. (2) Materials and methods: Intraosseous dental implants of similar shape, design and size, although different in type of connection (external vs. internal), were studied. The specifications of the UNI EN ISO 14801 test standard were used, with all determinations being carried out three times. Finally, the dimensional characterisation of the samples analysed after the dynamic load study was carried out, and the values of both study groups were compared by means of the non-parametric Mann–Whitney U test to find statistically significant differences (p < 0.05). (3) Results: For the static characterisation test, we found between 610.9 N and 986.1 N for the external connection and between 1263.6 N and 1324 N for the internal connection (p = 0.011). All of the dynamic load tests were positive and there was no failure in any of the components studied. (4) Conclusions: After the analysis of the samples studied in vitro, satisfactory results were obtained, demonstrating that both connections can support considerable mechanical loads according to international standards (UNI EN ISO 14801).

ABSTRACT Objective: The apical constriction (AC) and the apical foramen (AF) are the principal reference points used to determine the apical limit for instrumentation and root canal filling. AC varies in different races, and the aim of the current study was to measure and compare the distances from AC to AF and the anatomical apex (AA) in incisor and molar teeth in the Iranian population. Materials and Methods: Forty-five roots of incisor teeth and 45 roots of molar teeth were selected randomly in Isfahan Province, Iran. If the foramen was located toward the mesial or distal side of the apex, the cut was made mesiodistally, and if it was toward the buccal or lingual side, the section was made accordingly. Roots were examined under a microscope at ×25 magnification. The distances from AC to AF and AA were then estimated using a Motic camera. Descriptive statistics were used. The independent t-test was also used to compare distances in incisors and molars, and P = 0.05 was deemed to indicate statistical significance. Results: The mean distances between AC and AF were 0.847 ± 0.33 mm in incisors and 0.709 ± 0.27 mm in molars. The mean distances between AC and AA were 1.23 ± 0.39 mm in incisors and 1.01 ± 0.38 mm in molars. In an independent t-test, the distances between AC and AF differed significantly in incisors and molars (P = 0.035), but the distances between AC and AA did not (P = 0.172). Conclusion: The end points for root canal therapy should be 0.85 mm in incisors and 0.70 mm in molars.

ABSTRACT Objective: The purpose of this study was to investigate the importance of environments (dry and wet) to dislodge the clasp. Materials and Methods: Mandibular test models with natural premolar and molar teeth were used to test four types of clasp (each 12) (Akers, rest plate Akers [RPA], half and half [H-H], and ring clasp) in dry and natural fresh saliva environments. Each clasp was pulled out 10 times with a crosshead speed of 10 mm/min and the force required to withdraw each was measured. Statistical Analysis Used: A paired sample t-test and Wilcoxon test were used. Results: There were significant differences between the dry and wet (natural fresh saliva) environment. However, while the mean of the environment for RPA and ring clasp type was significantly different, the H-H and Akers clasp type was not. Conclusion: The environment has an effect on dislodging the clasp but differs according to the type of clasp.

Abstract Objective: To study the shear bond strength (SBS), sites of failure, and micromorphology of bonded molar tubes used on teeth affected by dental fluorosis. Materials and Methods: This in vitro study included 140 first molars classified according to Dean's index for dental fluorosis. Samples were divided into seven groups: (1) healthy teeth etched for 15 seconds, (2) teeth with moderate fluorosis (MOF) etched for 15 seconds, (3) teeth with MOF etched for 150 seconds, (4) teeth with MOF microabrasion etched for 15 seconds, (5) teeth with severe fluorosis (SEF) etched for 15 seconds, (6) teeth with SEF etched for 150 seconds, and (7) teeth with SEF microabrasion etched for 15 seconds. All samples were incubated and were then submitted to the SBS test and evaluated with the modified adhesive remnant index (ARI) and analyzed by using a scanning electronic microscope. Results: The SBS mean value for healthy enamel was 20 ± 10.2 MPa. For the group with MOF, the etched 150-second mean value was the highest (19 ± 7.6 MPa); for the group with SEF treated with microabrasion and etched for 15 seconds, the mean value was (13 ± 4.1 MPa). Significant differences (P ≤ .05) were found in the ARI between healthy and fluorosed groups. Conclusions: Fluorotic enamel affects the adhesion of bonded molar tubes. The use of overetching in cases of MOF and the combination of microabrasion and etching in SEF provides a suitable adhesion for fixed appliance therapy.

This in vitro study was designed to compare the forces generated by commercially available elastomeric chains and NiTi closed coil springs, and to determine their force decay pattern. Forty elastomeric chains and forty NiTi closed coil springs were divided into 4 groups according to the following manufacturers: (1) Morelli®, (2) Abzil®, (3) TP Orthodontics® and (4) American Orthodontics®. The specimens were extended to twice their original length and stored in artificial saliva at 37°C. Initial force was measured by means of an Instron universal testing machine and then at 1, 4, 7, 14, 21, and 28 days. The results revealed that the elastomeric chains delivered a mean initial force of 347 g for Morelli®, 351 g for American Orthodontics®, 402 g for Abzil®, and 404 g for TP Orthodontics®. The NiTi closed coil springs generated a mean initial force of 196 g for American Orthodontics®, 208 g for TP Orthodontics®, 216 g for Abzil®, and 223 g for Morelli®. The mean percentage of force decay observed after 28 days for the elastomeric chains was 37.4% for TP Orthodontics®, 48.1% for American Orthodontics®, 65.4% for Morelli®, and 71.6% for Abzil®. After 28 days, the NiTi closed coil springs presented a mean percentage of force decay of 22.6% for American Orthodontics®, 29.8% for Abzil®, 30.6% for Morelli®, and 45.8% for TP Orthodontics®. At the end of the study, significant differences were observed between the elastomeric chains and the NiTi closed coil springs. The results indicated that the studied NiTi closed coil springs are more adequate for dental movement than the elastomeric chains.

Background: Dual-arch impression technique allows the simultaneous recording of  tooth preparation, opposing anatomic tooth and maxillomandibular relationship. The  accuracy of reproduction of this easy and quick technique, however, has not been  studied in detail in past. Objective: To compare the accuracy of the impressions  made by using the same impression material in dual arch plastic trays, dual-arch  metal trays and acrylic resin custom trays. Methods: The dies obtained from the  addition silicone impressions made in dual-arch plastic trays, dual-arch metal trays  and full arch acrylic resin custom trays were compared for the dimensional accuracy  with the prepared typodont tooth as a control. Student’s paired t-test and unpaired  t-test were used for the data analyses using the Statistical Package for Social Studies  (SPSS) version 11.5. Results: The dies obtained from all the impression combinations  showed increased dimension (acrylic resin custom trays 9.4 mm±0.048, dual-arch  plastic trays 9.5 mm±0.035, dual-arch metal trays 9.41 mm±0.017) as compared to  the dimension of control (9.39 mm±0.007). Conclusion: All the tray-impression  material combinations showed variable accuracies. Full arch acrylic resin trays  resulted in greatest accuracy whereas dual-arch plastic trays the least accuracy.Health Renaissance 2014;12(2):  pp: 111-117

Abstract The aim of this in vitro study was to determine the fluoride concentration in silver diamine fluoride (SDF) products and their bioavailability with demineralized dentine. The products evaluated (expected fluoride concentrations) were: I: Saforide 38% (45,283 ppm F); II: Advantage Arrest 38.3 to 43.2% (45,283 to 51,013 ppm F); III: Ancárie 12% (14,100 ppm F); IV: Ancárie 30% (35,400 ppm F), V: Cariestop 12% (14,100 ppm F) and VI: Cariestop 30% (35,400 ppm F). The fluoride concentration was evaluated using an ion-specific electrode (ISE) by direct technique, which was confirmed after microdiffusion. The pH of the products was determined with a pH test strip. For the bioavailability test, demineralized dentine slabs were treated with one of the products for 1 min. Loosely (CaF2-like) and firmly-bound fluoride (FAp) were determined. The fluoride concentration found in the products (mean±SD; ppm F) by the ISE direct technique was: I:53,491±554; II:57,249±1,851; III:4,814±268; IV:5,726±43; V:10,145±468; VI:11,858±575; these values were confirmed after microdiffusion (t-test; p>0.05) and disagree with the declared by the manufacturers. The pH of Ancárie 12 and 30% was 6.0 and 4.5, respectively, in disagreement with the alkaline pH expected for SDF solution and found in the other products evaluated. There was no correlation between either CaF2-like (r=0.221; p=0.337) or FAp (r=-0.144; p=0.830) formed in demineralized dentine and fluoride concentration found in the products. The problems of pH and fluoride concentration found in available professional commercial SDF products suggest that they are not under sanitary surveillance.

Background. The present in vitro study aimed to compare the effectiveness of the WaveOne and ProTaper Gold systems in removing the Enterococcus faecalis biofilm. Methods. Thirty-eight mandibular premolars were selected. The root canals were assigned to standard control (canals serially enlarged with ProTaper Gold S1-S2-F1-F2, n=15) and experimental (canals enlarged with Primary WaveOne file, n=15) groups. Following the instrumentation procedure, the root canals underwent a sampling procedure, and the colonyforming unit (CFU) counts were determined. The samples were also evaluated under a fluorescent microscope to evaluate viable bacteria. The data were analyzed using independent samples t test and paired samples t test. Results. The results showed that, compared with the ProTaper group, the WaveOne group exhibited the least viable bacteria (P=0.004). Conclusion. It was concluded that comparison with the ProTaper Gold rotary system, the WaveOne reciprocating file is more successful in reducing intratubular viable bacteria counts.

Background: A successful restorative material forms a better adhesion, resist the microleakage and releases fluoride. However, existing glass ionomer cements cannot be used as a posterior restorative material in stress bearing areas. A new ionomer called Zirconomer, zirconia reinforced glass ionomer claims to exhibit high physical and biological properties. Aim: To assess and compare the microleakage, compressive strength, flexural strength and fluoride release from zirconomer with ketac silver. Materials & Methods: On twenty caries free premolar teeth (10 per each group), a class v cavity was restored with zirconomer and ketac silver. The microleakage was assessed using dye penetration test and stereomicroscope. The compressive and flexural strengths of these materials were measured using Instron Universal testing machine. The amount of fluoride released from the modified ionomers at pH 5 and pH 7 was estimated using Orion fluoride specific electrode. The obtained data was then subjected to statistical analysis. Results: Data was analysed using paired t-test for intergroup comparisons and unpaired t-test for intragroup comparisons. The overall microleakage (0.5±0.48) of zirconomer was significantly less (p=0.000) compared to ketac silver (1.9±0.83). Zirconomer demonstrated a significant higher compressive strength (330.25±60.14), flexural strength (33.058±2.36) than ketac silver (p= 0.000). Zirconomer demonstrated high fluoride release from day 1 to day 7 at both pH 5 and pH 7. Conclusion: Zirconomer demonstrated better physical and biological properties compared with ketac silver.
