CONTEXT: The curative treatment of peptic ulcer is made available nowadays through the eradication of the bacterium Helicobacter pylori, which is associated with it, but the best therapeutic regimen is yet to be determined. OBJECTIVE: To assess the efficacy of a therapeutic regimen with 400 mg ranitidine bismuth citrate associated with 500 mg clarithromycin given twice a day for seven days in a cohort of Brazilian patients with peptic ulcer. TYPE OF STUDY: Cross-sectional study. SETTING: Tertiary-care hospital. PATIENTS: One hundred and twenty nine outpatients, with active or healed peptic ulcers infected by Helicobacter pylori, diagnosed via endoscopy with confirmation via the urease test and histological examination, who had never undergone a regimen for the eradication of the bacterium. PROCEDURE: Administration of 400 mg ranitidine-bismuth and 500 mg clarithromycin twice a day, for seven days. MAIN MEASUREMENTS: Efficacy of the treatment, with a check on the cure done via another endoscopy eight weeks after drug administration. The eradication of the bacterium was determined via the urease test and histological examination. Patients who were negative for both were considered to be cured. RESULTS: Eight patients failed to complete the study. The eradication rate according to intention to treat was 81% (104/129) and per protocol was 86% (104/121). CONCLUSION: The bismuth ranitidine compound associated with clarithromycin used for one week was shown to be a simple, effective and well-tolerated therapeutic regimen for the eradication of Helicobacter pylori.

Background: Helicobacter pylori (H. pylori) infection is a medical encumber especially in the developing countries, ranging from acute inflammation to malignancy. The study is intended to explore the prevalence of H. pylori infection and also to compute its associated risk factors.Methods: Sixty two (n=62) symptomatic patients attending the tertiary care hospital, were included in the study. Two antral biopsies collected from each patient were subjected to rapid urease test and Gram staining. IgG antibodies were detected by rapid test based on the principle of Immunochromatography. To evaluate the risk factors, various determinants were asked to each patient in the form of questionnaire. Statistical analysis was done using Fisher exact test. P value < 0.05 was considered significant.Results: Prevalence rate of H. pylori infection was 24.19 % (15/62). More than half (53%) of the infected patients had complained of persistent burning abdominal pain. Prevalence rate was found highest in the patients suffering from peptic ulcer disease (66.6%, P=0.0237). Consumption of unfiltered water (P=0.007, Odd Ratio (OR)-1.06), eating spicy food, meat, fish and chili pepper (P=0.052, OR-1.01) was found the major risk factors. Addictive habits of either smoking/alcohol/tobacco (P=0.904) were not associated with infection.Conclusions: Prevalence of H. pylori infection was found low comparatively and was mainly associated with symptom of persistent burning abdominal pain and peptic ulcer. Use of unfiltered water, spicy food, chili pepper and meat undoubtedly increase the risk of acquiring H. pylori infection. Addictive habits were not significantly associated with infection.

BACKGROUND: With the outstanding & revolutionary discovery of Helicobacter Pylori(H.Pylori) organism as a causative agent in Peptic Ulcer Disease (PUD), Gastric carcinoma and Malt Lymphoma, there has been a lot of interest generated about H.Pylori in modern times. The role of NonSteroidal Anti-Inammatory Drugs(NSAIDs) in the causation of Gastric Ulcer (GU) & Duodenal Ulcer (DU) is also an important point of study now-a-days. AIM & OBJECTIVES: To assess the prevalence of H.Pylori in patients of PUD diagnosed with Upper Gastro-Intestinal Endoscopic (UGIE) examination. MATERIALS & METHODS: Total 100 patients of the study underwent UGIE examination. Rapid Urease Test of gastric biopsy specimens of all patientswas done forthepresenceofH.Pylori. RESULTS:Out of 100PUDpatients, 74weremales&26were females.Out of 54H.Pylori associatedPUDpatients, 41(76%)weremales and 13(24%) were females. About 47% patients had GU, 43% had DU and 10% had both GU & DU. About 34(79%) DU patients were associated with H.Pylori infection. Out of 47 GU patients, 11(23%) are associated with NSAID usage, 7(15%) are associated with H.Pylori & other 53% had associated comorbidities like HTN, DM, CAD and CLD. Descriptive analysis in percentage was done by using MS Excel. CONCLUSION:Thendings ofthisstudy revealthatH.Pyloriinfection ismore common inmalesthan females.High prevalence ofH.Pyloriinfection isseeninDUpatients.NSAIDabusewas highinGUpatients.Thisstudyhighlightsthe signicanceofH.Pylori&NSAIDusage inPUDpatients.

Background: Gastritis is one of the commonest clinical conditions encountered by a physician. The most common cause is said to be infection with Helicobacter pylori. The present study was aimed to diagnose the prevalence of H. pylori infection among the cases of gastritis and its correlation with histopathological findings and associated rapid urease test.Methods: A one year prospective study at a tertiary care hospital was conducted and all cases of gastritis were included and socio demographic data, clinical complaints and duration were noted. Endoscopy was performed on all cases. Biopsy was performed histopathological examination with staining and graded by Houston-updated Sydney system. Rapid urease test was performed and findings noted.Results: 325 cases with 215 male and 110 female cases were included. 26.15% were in age group of more than 60 years and number of cases increased with age. Mean age of study participants was 39.12±2.8 years and heart burn was the common complaint in the study cases. 81.54% of the cases revealed endoscopic gastritis on endoscopy and hyperaemia was commonest. 83.69% were positive by rapid urease test. Rapid urease test is more sensitive than histological staining in confirmation of H. pylori infection.Conclusions: To conclude on the present study, the prevalence of H. pylori infection is on a global rise and appropriate measures to reduce the prevalence is quite an urgent necessity. Histopathological interpretation of gastric biopsies is a reliable indicator of H. pylori infection as well as gastritis grading according to the Sydney grading system.

Background: Numerous studies have evaluated the impact of Helicobacter pylori (H. pylori) eradication on the number, severity, and recurrence of migraine attacks. But the association of migraine, H. pylori, and gastrointestinal (GI) presentation is challenging. The aim of the current study was to investigate the correlation between migraine, H. pylori, and peptic ulcers among patients with dyspepsia undergoing upper GI endoscopy. Methods: 305 patients with dyspepsia referring to our endoscopy ward, Shahid Beheshti Hospital affiliated to Qom University of Medical Sciences, Qom, Iran, for upper GI endoscopy filled out the study questionnaire. If a patient was experiencing headaches and the migraine was confirmed by neurologists, he/she was asked to answer the questions related to migraine, which were prepared exactly from Migraine Disability Assessment (MIDAS) questionnaire. The relation between migraine and confirmed H. pylori contamination was investigated using statistical models. Results: Of all the 305 patients, 133 (43.6%) had confirmed episodic migraine headaches (MHs) and 177 patients (58.04%) had positive RUT for confirming H. pylori contamination, of which 123 (69.5%) had confirmed migraine. 52 (17.0%) had duodenal peptic ulcer(s), of which, 49 (94.2%) had a positive rapid urease test (RUT) (P < 0.001). 20 (6.5%) of all patients had the gastric peptic ulcer(s) which did not have a significant relation with H. pylori contamination. There was a significant relationship between the peptic ulcer site and migraine. In total, 177 patients (58.0%) had a positive RUT. History of migraine was significantly positive in those with positive H. Pylori contamination. Notably, multivariable analysis demonstrated a significant relation of H. pylori and migraine at younger ages. Conclusion: The prevalence of H. pylori and migraine in patients with dyspepsia seems to be high. Moreover, there is a meaningful association between migraine, duodenal peptic ulcers, and H. pylori infection, too.

Background: The idiopathic peptic ulcers (IPU) or non-Helicobacter pylori, non-steroidal anti-inflammatory drug non-NSAIDs associated peptic ulcers are a serious therapeutic challenge. The prevalence of IPU is increasing in some parts of the world. Objectives: This study aimed to determine the prevalence of IPU in southeastern Iran. Methods: In this cross-sectional study, a total of 367 patients with active peptic ulcer were diagnosed by endoscopy between Jan 2018 and Feb 2019. The patients were assessed for history of medication use (including NSAIDs) in the past month, smoking, alcohol, as well as a complete history of health problems related to underlying disease such as cardiopulmonary, hepatic, renal problems, and chronic pancreatitis. Biopsy samples from antrum were performed for rapid urease test (RUT), and if RUT was negative, then biopsies were sent for histopathology. If both RUT and pathologic findings were negative for H. pylori, in patients who had treatment for H. pylori eradication, anti-H. pylori IgG antibody of blood samples was assessed. Patients were considered infected with H. pylori if any of the diagnostic tests had a positive result. Results: According to the results, 336 (91.3%) cases had at least one of the two main etiologic factors (H. pylori and NSAIDs) for peptic ulcer. While 323 (87.7%) patients were H. pylori-positive, 45 (12.3%) patients were H. pylori-negative. However, out of 45 H. pylori-negative cases, 13 patients had a positive history of using NSAIDs, and 32 (8.69%) patients were IPU. Conclusions: Our study showed that both H. pylori infection and NSAIDs use remain the main cause of peptic ulcers, and the prevalence of IPU is relatively high in Zahedan.

Background: Dermatophytosis is a commonly encountered superficial fungal infection in the tropical and subtropical countries. The present study was undertaken to study the clinicomycological profile of dermatophytosis and perform antifungal susceptibility testing for the isolated dermatophytes.Methods: This is 2 years cross- sectional observational study including 433 clinically suspected cases of dermatophytoses. Skin, hair and nails were collected, subjected to direct microscopy by Potassium hydroxide (KOH) mount and culture on Sabouraud’s Dextrose Agar (SDA) with chloramphenicol and cycloheximide. Positive growth on culture media was further identified by LPCB mount, slide culture test, growth on Dermatophyte Test Medium, pigment production in corn meal agar with 1% dextrose, urease test and hair perforation test. The isolated dermatophytes were subjected to antifungal susceptibility testing by agar based disc diffusion method.Results: Out of 433 samples, fungal filaments were seen in 308 (71.1%) samples by KOH mount, 259 (59.8%) dermatophytes were isolated from culture. Males (60.5%) were more commonly affected than females (39.5%). Most common age group affected was 21-30 years. Trichophyton mentagrophytes (57.5%) was the most common isolate followed by Trichophyton rubrum (30.1%). Tinea corporis was the most common clinical presentation (52.7%) followed by Tinea unguium (14.1%). Antifungal susceptibility testing showed itraconazole as the most sensitive antifungal agent, while fluconazole was least sensitive.Conclusions: This study provides a scope for assessment of prevalence and clinicomycological profile, which could help in estimation of the problem and hence prevent spread of dermatophytoses with adequate control measures.
