Avulsion of posterior primary teeth and space maintaining appliance: case report

A four-year-old child was presented to the Pediatric Dentistry Clinic of the Federal University, 21 days after an incident in which canine first and second primary molar teeth were avulsed, due to a trauma to the face. This was confirmed on radiological examination. The clinical examinations showed that tissues were normal. A removable space-maintaining dental-mucosa supported appliance was made in acrylic resin to replace the three missing teeth. After a period of eight months, the tissues were preserved, the device is helping the child to eat, to speak, and preserving the appearance of the patient. Radiograph examinations have shown that the first molar tooth and canine, first pre molar and second pre molar teeth are erupting normally. Trauma in primary dentition can cause psychological,morphological and functional problems. In the presented case the treatment was planned to recuperate the function and to avoid problems from the premature loss of primary teeth.

Relevance. To study the incidence of different types of resorption of multirooted primary teeth, to specify indications for deciduous molar extraction to prevent eruption abnormalities of permanent posterior teeth in mixed dentition.Materials and methods. Root resorption of 375 multirooted primary teeth (166 first primary molars and 209 second primary molars) was studied on panoramic X-rays of 60 children (30 girls and 30 boys) aged between 7 and 15. Illustrated classification by T.F. Vinogradova (1967) improved by authors was used to determine type and degree of root resorption of multi-rooted primary teeth. Received data were described with absolute values of number of cases and percentage. Chi-square was used to detect differences in sign incidence rate between groups, p<0.05 was considered statistically significant.Results. There were no statistically significant gender differences (p>0,05) in type and degree of root resorption of multirooted primary teeth. Type A resorption prevailed and constituted 53.3% of all primary molars. Disturbances in root resorption of multirooted primary teeth in mixed dentition were related to health condition of primary teeth. Transition of even resorption to unven was considered a risk factor of delayed eruption and aberrant position of permanent teeth, and indication for extraction of a primary molar in question. Conclusions. 1) Even root resorption (type A) was detected in 53.3% of primary molars in mixed dentition by orthopantomography. 2) Transition from even resorption of primary molar roots to uneven resorption was associated with eruption deviations and delayed premolar eruption. 3) Timely extraction of primary molars with uneven root resorption facilitated correct eruption of premolars and increased effectiveness of secondary prevention of malocclusion in children.

Seventy primary molar teeth, carious exposed, symptom free, without any sign of root resorption in children aged from 3 to 6 years (main age 4.3yr) were treated with conventional pulpotomy procedures. Ferric sulfate 15.5% solution (applied for 15 second for 35 teeth) and formocresol solution (five minute procedure of Buckley's formula for next 35 teeth) have been used as pulpotomy agents. In both groups, pulp stumps were covered with zinc-oxide eugenol paste. Permanent restorations were stainless steel crowns. Clinical check up was every three-months and radiographic follow-up time was six and twenty months after treatment. Our results within this period revealed 100% clinical success rate in both groups. Radiographic success rate was in both groups 97.2%, while in 2.8% cases has shown internal root resorption. On the basis of these results, we can recommend ferric sulfate as a pulpotomy agent in primary teeth in substitution for formocresol at the moment.

Objectives: Optimal restoration of endodontically treated teeth is very important for the durability of the endodontic treatment. The focus of this study was to evaluate the fracture strength and microleakage of composite endocrowns compared with Class II composite restorations in endodontically treated primary molar teeth. Materials and Methods: 48 extracted second primary molars were divided into 2 groups. Group 1 (control): teeth with Class II and endodontic access cavities restored with G-aenial composite; group 2: endocrown restorations with G-aenial composite. After completing restorative procedures, teeth were subjected to thermal cycling (500 cycles). Compressive loading was applied to half of the samples, although the other half were immersed in 0.5% basic fucsin solution for 24 h, sectioned, and examined for dye penetration under stereomicroscope. Data were subjected to statistical analysis by the Mann-Whitney U test (α = 0.05). Results: The fracture strength of endocrowns (1741 ± 379.35 N) was significantly higher than that of the control group (1126.5 ± 405.39 N) ( P < .05). No statistical difference was found in microleakage between the 2 groups ( P > .05). Conclusion: Endocrown preparation increases the fracture strength of the final composite restoration when used in primary molar teeth. Composite endocrown restorations can be a practical and an esthetic option for restoring endodontically treated primary molar teeth.

Objective: PTFE tape, which is commonly used as plumber's tape is an inorganic, non-fibrous, ribbon like material. The aim of this study was to evaluate PTFE tape as endodontic spacer material and to compare it with commonly used spacer material that is cotton, in primary teeth. Study Design: Seventeen children undergoing pulpectomy of lower second primary molar bilaterally were included in the study. Cotton and PTFE tape were placed as spacers on each side randomly. Samples were taken from the access cavity at baseline and after seven days to check for microbial leakage. Spacer materials were also checked for microbial contamination. Results: The results revealed that there was a significant increase in the bacterial colony count after seven days in cotton group. The access cavities were also positive for microbial leakage in the cotton group where the spacers showed positive growth. In PTFE group only two samples showed microbial contamination of spacer and out of two only one sample showed contamination of access cavity along with spacer. Conclusion: Within the limitations of this study, it can be concluded that PTFE tape performed better than cotton as endodontic spacer material. Thus, PTFE tape can be recommended as an endodontic spacer material as an alternative to cotton in primary teeth.

ABSTRACT Introduction To assess the amount of debris extruded apically during instrumentation of distal canals of extracted primary molars by three instrument systems [ProTaper Universal (PTU), ProTaper NEXT (PTN), and self-adjusting file (SAF)] compared with conventional stainless steel hand K-files (HF, control). Materials and methods Primary mandibular molars (n = 120) with a single distal canal were selected and randomly divided into four groups (n = 30) for root canal instrumentation using group I, HF (to size 0.30/0.02 taper), group II, PTU (to size F3), group III, PTN (to size X3), and group IV, SAF. Debris extruded during instrumentation was collected in preweighed Eppendorf tubes, stored in an incubator at 70°C for 5 days and then weighed. Statistical analysis was performed by one-way analysis of variance (ANOVA), followed by Turkey's post hoc test (p = 0.05). Results All the groups resulted in extrusion of debris. There was statistically significant difference (p < 0.001) in the debris extrusion between the three groups: HF (0.00133 ± 0.00012), PTU (0.00109 ± 0.00005), PTN (0.00052 ± 0.00008), and SAF (0.00026 ± 0.00004). Conclusion Instrumentation with SAF resulted in the least debris extrusion when used for shaping root canals of primary molar teeth. Clinical significance Debris extrusion in primary teeth poses an adverse effect on the stem cells and may also alter the permanent dental germ. Debris extrusion is rarely reported for primary teeth and it is important for the clinician to know which endodontic instrumentation leads to less extrusion of debris. Ankylosis is a condition frequently associated with primary molars, wherein the ankylosed primary teeth remain in a fixed position, while the adjacent teeth continue to erupt, moving occlusally. In this case report, a five-year-old boy, who had a retained and submerged left lower second primary molar, was presented. Luxation of ankylosed primary molar was considered as a treatment approach. After four months, the tooth erupted to the occlusal level, and there was evidence of further development of a permanent successor in radiographic evaluation. After one year, tooth mobility, bone formation, and development of a permanent successor were in good condition.

ABSTRACT Introduction Chemomechanical caries removal (CMCR) is an effective method of caries removal especially for primary teeth as they cause less discomfort when compared with conventional caries removal. The most significant thing about caries removal is the elimination of cariogenic bacteria. This study compares the antibacterial activity of two CMCR gels. Materials and methods A total of 40 primary molar teeth with carious dentin were split along the long axis in a laboratory. Total viable count (TVC) was taken for the teeth before splitting as a measure of colony-forming units per milliliter (CFU/mL). Each half was treated with either Carisolv or Carie-Care CMCR gels. Clean dentin samples were evaluated for Streptococcus mutans (SM) and Lactobacillus acidophilus (LB) after removal of carious tissue using the caries removal gels using serial dilutions and incubating on specific agar plates. Results The results showed significant reduction in mean TVC after use of both the CMCR gels. Both gels reduced the CFU/mL of SM and LB to a significant level (p < 0.05). However, there was no significant difference between the antibacterial activities of the two CMCR gels. Conclusion The CMCR gels (Carisolv and Carie-Care) significantly reduced the residual TVC as well as SM and LB in carious primary dentin. Both CMCR gels had a similar antibacterial activity on the carious dentin of primary teeth. Clinical significance The CMCR gels tested have a significant antibacterial activity and can be effectively used for elimination of caries-causing bacteria in primary teeth. Introduction. The etiology of exposure determines pulpal response, making it crucial to distinguish between mechanical and carious exposure. This study clinically and radiographically evaluated the success of MTA pulpotomies conducted to treat carious and mechanical pulp exposure. Materials and Methods. This study was conducted with 50 mandibular primary molar teeth. Teeth were divided into 2 groups according to status of the exposure site, with teeth surrounded by carious dentin placed in a carious exposure group and those surrounded by sound dentin in a mechanical exposure group. MTA pulpotomies were performed for both groups. Treatment was followed up clinically and radiographically for 18 months. Results. Clinical and radiographic success rates at 18 months were 100% for both groups. Success rates did not vary significantly between the groups (p=1.000). Pulp canal obliteration was only seen in the carious exposure group, observed in 2 teeth (8.3%). Conclusion. The long term success rates achieved in this study indicate that MTA can be used as a vital pulpotomy material for the long term success in primary teeth with either mechanical or carious exposure. The findings of the present study highlight the fact that treatment prognosis is dependent upon diagnosis and selection of the appropriate materials for treatment.

Aim: This experimental study in vitro aimed to compare the disinfection of the primary teeth root canals system using the rotary and reciprocating file system. Methodology: Forty-eight root canals from twenty-four primary molar teeth were contaminated with the standard strain of Enterococcus faecalis and randomically splitted into 4 groups (n=12). The Group PN comprised the root canals prepared with the ProTaperTM  Next rotary system. The Group WOG was defined by the usage of WaveOneTM Gold reciprocating system. The files from both systems were thermo mechanical processed. The Group PU used the ProTaperTM Universal system, with no thermic treatment, and the Group C (negative control) received no treatment. Samples from the root canals were collected with sterile paper points before and after its preparation, diluted and distributed in plates with Brain Heart Infusion (BHI) agar. The bacterial colonies were counted and the results were statistic analyzed by Wilcoxon and Kruskal-Wallis (Dunn) tests. Results: There was significant bacterial reduction using the systems (p>0,01). However, there was no significant difference among them (p<0,05) (PN=3.38; WOG=3.85; PU=3.26). Conclusion: Both systems (rotary and reciprocating) provide disinfectionon the primary molar teeth root canals system.

Background:  Malnutrition is well-known to yield high morbidities and mortalities and considering its consequence on the oral cavity, malnutrition is shown to have pre-eruptive and post-eruptive outcomes. The objective was to assess the prevalence of hypo-mineralized second primary molar (HSPM), molar–incisor hypo-mineralization (MIH) and dental caries in malnourished children as well as addressing the relation between types of malnutrition of the children and their dental morbidities represented in HSPM, MIH and dental caries. Methods: This is a cross sectional analytical study. Malnourished children aged 5-10 years and presented to the Outpatient Clinic of Pediatric Dentistry Department, Faculty of Dentistry, Cairo University were examined for HSPM. MIH using European Academy of Pediatric Dentistry criteria and dental Caries using def/ DMF indices. Results:  A consecutive sample (a long six months) of 54 malnourished children were enrolled in the study. Dental caries was a greater dental morbidity in the overweight and obese group. Besides, stunting was a greater risk in HSPM and MIH. There was an association between HSPM and MIH in a considerable percentage of the study group. Conclusions: Malnutrition is a risk factor for dental abnormalities. HSPM could expect the presence of MIH.
