The problem of vitamin D deficiency is one of the most actual today, because, according to the results of numerous studies, its deficiency is registered in half the worlds population. The article presents the results of a study on the level of vitamin D in children of first year of life, suffering from cows milk proteins allergy, as well as in children who have formed a secondary food tolerance to this allergen. In the article was evaluated the effectiveness of the additional use of vitamin D against the background of the therapeutic dairy-free diet use. It was found that the 57.1 4.8% children of first year of life with cows milk proteins allergy have vitamin D insufficiency. Normal vitamin D level was found in only 36.2 4.7% of patients. In turn, vitamin D deficiency was found in 6.7 2.4% children, suffering from cows milk proteins allergy. It was found that the additional daily intake of vitamin D on the background of the use of therapeutic dairy-free diet may reduce the time of development of food tolerance to cows milk proteins and achieve the formation of secondary food tolerance for the three years in 92.0 3.8% of children with cows milk proteins allergy. It was found, that the use of only therapeutic dairy-free diet allows to achieve the formation of secondary food tolerance only in 76.0 6.0% children (p 0.05). The normal level of vitamin D were established in 97.6 1.7 % children, formed a secondary food tolerance to cows milk proteins.

The article presents the results of a study on the level of vitamin D in children of first year with cow's milk proteins allergy. The aim of the work is to evaluate the effectiveness of using an aqueous solution of vitamin D in addition to a therapeutic elimination diet in children who are allergic to cow's milk proteins. It was found that the 57.1±4.8 % children of first year with cow's milk proteins allergy have vitamin D insufficiency. Normal vitamin D levels were found in only 36.2±4.7 % % patients. In turn, vitamin D deficiency were found in 6,7±2,4 % children. The additional intake of vitamin D on the background of the use of elimination therapeutic dairy-free diet can reduce the time of development of tolerance to cow's milk proteins and achieve tolerance for three years in 92 % of children with allergies to cow's milk proteins were found.

The aim of the research is to study the availability of vitamin D among children of different age and pregnant women living in the Amur region. 339 residents of Blagoveshchensk (279 children of different age and 60 adults) were examined. Among them there were 129 toddlers, 90 children of preschool age of 3-6 years old, 60 adolescents of 15-17 years old and 60 adults (pregnant women) of 18-40 years old. In the blood serum the content of the metabolite of vitamin D [25(OH)D] was studied by high performance liquid chromatography. The assessment of risk factors for failure and deficiency of vitamin D was conducted by clinical and anamnestic data. The first results of the study have been presented. They indicate a high frequency of various vitamin D deficiency in the population of the Amur region. Among 339 residents of the Amur region 92 (27.1%) had an adequate availability of vitamin D, 144 (42.4%) had an insufficiency of 25(OH)D; at the same time vitamin D deficiency was identified in 90 (26.5%) with 1/3 of the child population and 1/4 of pregnant women. The level of 25(OH)D within normal limits found in 37.5% of children in the first year of life decreased till 3.6 % by 3 years old, with its deficit increasing from 29.1% to 50%, persisting at this level (45.5 percent) in 3-6 year-old children, declining till 23.3% in adolescents and till 23.3% in pregnant women. The average vitamin D in the studied age periods was the highest in the first year of life (36.14±4.3 ng/ml), and the lowest in 2-3 year-old children (19.31±14.68 ng/ml), with a gradual increase in 3-6 years old (21.77±0.96 ng/ml); in 15-17 years old (23.89±0.66 ng/ml) and in pregnant women it has reached a level of 27.75±0.18 ng/ml, remaining below normal values. Respiratory infections, digestion, bone and joint systems pathologies are the most important risk factors contributing to the low status of 25 (OH) D; their prevention, alongside with the correction of vitamin D deficiency can improve the health of the population of the Amur region.

Vitamin D is essential for calcium absorption and for maintaining bone health in the pediatric population. We conducted a retrospective study to establish the profile of a child aged under 3 years old with vitamin D deficiency in the context of correct prophylaxis, on a cohort of 49 children from two general practitioner offices. From the study group 30.6% of children (15 cases) had low vitamin D levels. The mean serum 25(OH)D level was 41.5±16.6 ng/ml. Regarding nutrition in the first year of life, breastfeeding predominated (83.7% of patients), and only 8.16% of patients had clinical signs of rickets. So, low serum levels of vitamin D can also be found in children who have successfully received correct prophylaxis with vitamin D. Keywords: vitamin D, children, rickets

ABSTRACT Objective: To determine the prevalence of vitamin D deficiency/insufficiency in children 0-18 years of age with recurrent wheezing and/or asthma residing in the microregion of Viçosa, Minas Gerais, Brazil, and treated at a referral center, and to determine its association with major risk factors for wheezing. Methods: A cross-sectional study was performed using a semi-structured questionnaire, which was administered by trained interviewers to the legal guardians of the study participants. Data were obtained regarding general characteristics of recurrent wheezing; general sociodemographic, environmental, and biologic factors; and atopy-related factors. The magnitude of the statistical association was assessed by calculating ORs and their corresponding 95% CIs by using multiple logistic regression. Results: We included 124 children in the study. The prevalence of vitamin D deficiency/insufficiency in the sample was 57.3%. Vitamin D deficiency/insufficiency was found to be associated with wheezing in the first year of life, personal history of atopic dermatitis, environmental pollution, and vitamin D supplementation until 2 years of age. Conclusions: The prevalence of vitamin D deficiency/insufficiency was high in our sample. Vitamin D concentrations were directly associated with vitamin D supplementation until 2 years of age and were inversely associated with wheezing events in the first year of life, personal history of atopic dermatitis, and environmental pollution.

We aimed to investigate the changes in vitamin D levels and factors associated with vitamin D deficiency (VDD) during the first year of life in Korean preterm infants. We enrolled 333 preterm infants who were born at Kyungpook National University Children’s Hospital between March 2013 and December 2019. 25-hydroxyvitamin D (25-OHD) levels and medical records were collected at birth, 6 months, and 12 months of age. The mean gestational age was 33.4 ± 2.3 weeks and mean 25-OHD levels at birth were 18.2 ± 13.5 ng/mL. The incidence of VDD was 82.8%, 30.6%, and 27.0% at birth, 6 months, and 12 months, respectively. The incidence of severe VDD (25-OHD < 10 ng/mL) was 31.5%, 1.5%, and 0%, at birth, 6 months, and 12 months, respectively. Among infants with severe VDD, the deficiency persisted in 49.6% at 6 months, and 35.3% at 12 months. The strongest predictor of VDD during follow-up was 25-OHD concentration at birth. Vitamin D supplementation at 400 IU/day did not affect vitamin D levels during the first year of life. Therefore, it is important to prevent neonatal VDD through maternal vitamin D supplementation during pregnancy. Further research is needed to determine the optimal vitamin D supplementation dose for Korean preterm infants.

SummaryAim of the study was to analyse the influence of a concomitant vitamin D deficiency on the results of 99mTc-MIBI studies in patients (pts) with primary hyperparathyroidism (pHPT). Patients, methods: Between January 1998 and May 2004, 71 pts with pHPT had undergone operation after a 99mTc-MIBI study of whom 54 pts (76%) had normal values of 25-OH-vitamin D3 and 17 pts (24%) had vitamin D deficiency. Results of a dual-phase 99mTc-MIBI protocol with SPECT were compared with histopathology. Results: In 54 pts with normal vitamin D values late SPECT images identified more lesions (n = 51, sensitivity 91%) than early planar (n = 45, sensitivity 82%) or late planar images (n = 50, sensitivity 88%). In 17 pts with vitamin D deficiency late SPECT images identified more lesions (n = 13, sensitivity 72%) than early planar (n = 10, sensitivity 56%) or late planar images (n = 10, sensitivity 56%) too. In pts with vitamin D deficiency the sensitivity of a 99mTc-MIBI SPECT study was lower than in those with normal vitamin D status (72% vs. 91%) and dependent on the value for PTH. However, the results did not reach statistical significance: early planar: p = 0.1625; late planar: p = 0.0039; 99mTc-MIBI SPECT: p = 0.1180. Conclusion: The likelihood of a pathological 99mTc-MIBI study being obtained in pts with pHPT is dependent on the parathyroid hormone level. However, a negative influence of a low vitamin D level on the scintigraphic detection rate of a parathyroid adenoma could not be proven which may be due to the low number of pts with vitamin D deficiency.

Introduction: Vitamin D is an important micronutrient having crucial role in calcium and phosphate homeostasis but there is emerging evidence to suggest its role in prevention of infectious, inflammatory and neoplastic diseases. The vitamin D insufficiency/deficiency is a widely recognized problem among children in developed as well as developing countries.Objective: To find out the prevalence of vitamin D deficiency/insufficiency among children aged 2 months to 13 yrs and its correlation with serum calcium, phosphate and demographic factors.Methodology: Prospective, cross sectional, hospital based study conducted at Birat Medical College Teaching Hospital for a period of one year among children presented to pediatric OPD in whom pediatrician had a suspicion of Vitamin D Insufficiency /Deficiency. Venous blood Samples were taken for the estimation of 25(OH) Vitamin D, Calcium and Phosphate. Statistical analysis was done using SPSS software version 16. Results Among 174 children aged 2 months to 13 years, the prevalence of vitamin D insufficiency at the cutoff of <30ng/ml was 82.75% where as no cases of vitamin D deficiency was noted at cut off level of < 10 ng/ml. Children residing in urban area, samples taken in the winter season had higher prevalence of vitamin D insufficiency (p value <0.05). Serum calcium and phosphate level were found positively correlated with Vitamin D level (p<.05). However the relationship was very strong between Vitamin D and Calcium (r=0.402, p-value <0.001) and weak positive correlation was noted with phosphate(r=0.155, p value <0.05).Conclusion: There is very high prevalence of Vitamin D insufficiency among children. The vitamin D insufficiency was significantly higher in winter season as compared to summer. Children living in the urban area are more prone to Vitamin D insufficiency. Moderately strong positive correlation was observed between Vitamin D level and calcium but weak correlation existed with phosphate. BJHS 2018;3(1)5 : 381-384

Objective. Early feeding and infant exposures have been suggested as potential risk factors for immunoglobulin E- (IgE-) mediated food allergy (FA). We aimed to evaluate the association between IgE-mediated FA in children and early exposures including the child’s nutritional status, breastfeeding and its duration, the age at which the solid food was first introduced, antibiotic exposure during the first year of life, and the child’s vitamin D status during infancy. Design. A case-control study. Setting and Subjects. Children aged 0–13 years were recruited from pediatric allergy and immunology clinics (PAICs) located at major government hospitals in Kuwait (total FA cases: n=100; boys = 67%), and healthy controls (n=100, boys 55%) were recruited from various vaccination units at primary healthcare centers. Results. Cow’s milk allergy was the most common type of FA. FA status was independently associated with the early exposures of exclusive breastfeeding (aOR = 15.55 (3.26–74.19), p=0.001), vitamin D deficiency or insufficiency during infancy (aOR = 5.42 (1.92–15.30), p=0.001), and antibiotic exposure during the first year of life (aOR = 5.00 (1.58–15.84), p=0.006). Conclusions. FA is highly prevalent among children in Kuwait, and our data indicate that early nutrition-related and antibiotic exposures are associated with FA risk.

Cord blood 25-hydroxyvitamin D (25OHD) has been reported in association with risk of early life recurrent wheeze. In a subset of infants who participated in the Vitamin D Antenatal Asthma Reduction Trial, we demonstrated that higher cord blood 25OHD at birth (>31 ng/mL) was associated with a reduced risk of recurrent wheeze in the first year of life. We then identified a module of co-expressed genes associated with cord blood 25OHD levels >31 ng/mL. Genes in this module are involved in biological and immune pathways related to development and progression of asthma pathogenesis including the Notch1 and transforming growth factor-beta signalling pathways.

AbstractThe incidence of vitamin D deficiency is high globally, and vitamin D supplementation draws particular attention. The objective of this study was to investigate the effects of stratified vitamin D supplementation in middle-aged and elderly individuals with vitamin D insufficiency in Beijing. A total of 448 subjects aged over 40 years old were selected from a community in Beijing. Among them, 100 middle-aged and elderly people with vitamin D insufficiency were randomly selected on a voluntary basis. They were further divided into control group and intervention group. The control group received health education and lifestyle guidance, and the intervention group received lifestyle guidance and vitamin D supplementation for nine months. The doses were stratified as follows: for vitamin D insufficiency, oral vitamin D3 supplement was given at 5000 IU/w; for mild vitamin D deficiency, oral vitamin D3 supplement was given at 10 000 IU/w; for severe vitamin D deficiency, oral vitamin D3 supplement was given at 15 000 IU/w. Safety evaluation was conducted after three-month treatment. The intervention group consisted of 8%, 62%, and 30% of cases who had vitamin D insufficiency, mild vitamin D deficiency, and severe vitamin D deficiency, respectively, which were similar with the control group. It showed that the blood 25(OH)D level increased significantly in the intervention group, from 14.30±4.30 ng/ml to 33.62±6.99 ng/ml (p<0.001), in contrast to insignificant change in the control group. Stratified vitamin D supplementation effectively increased the blood 25(OH)D level, as well as the number of cases with corrected vitamin D insufficiency or deficiency.
