Background: HCV is known for its chronicity and leads to cirrhosis in about 10 to 20 per cent of patients and may further progress to hepatocellular carcinoma (HCC). The global seroprevalence of HCV among blood donors varies from 0.4 to 19.2 per cent and the estimated risk for HCV transmission is between 0.10 to 2.33 per million units transfused. ELISA is the most commonly used initial assay for detecting HCV antibodies. The purpose of the present analysis was to monitor the seroprevalence of anti-HCV antibodies in the blood donor population in a hospital based blood bank in north India for a period of 10 years (2007-2016), and to evaluate the trends over the years.Methods: The purpose of the present analysis was to monitor the seroprevalence of anti-HCV antibodies in the blood donor population in a hospital based blood bank in north India for a period of 10 years (2007-2016), and to evaluate the trends over the years.Results: Of the total 340078, 298421 (87.75%) collections were voluntary and 41657 (12.25%) were replacement collections. A prevalence of 0.72% of hepatitis C virus infection was seen among the donors from the period 2007-2016. The trend of hepatitis C prevalence among donors has been fluctuating while ranging from 0.51% to 0.89%.Conclusions: For a safe blood service in our country, where comprehensive laboratory tests are neither possible nor pragmatic, it is best to switch over to 100% voluntary donations, as it is now established that only voluntary non-remunerated regular donation is the safest. Thus, one of our key strategies to enhance blood safety is to focus on motivating non-remunerated blood donors and phasing out even replacement donors. Since, no vaccine is presently available for immunization against HCV infection, transfusion transmitted HCV infection remains a potential threat to the safety of the blood supply.

The existence and sustenance of the blood bank depends on blood donors. It is imperative that the donation experience is satisfactory for the donors. Therefore this study was carried out to determine the frequency of undesirable events experienced by the blood donor as part of donor haemovigilance. This was a retrospective descriptive study of the events that occurred amongst the blood donors of the blood bank of a tertiary institution. The blood donor incident book was reviewed for the period of six months. Negative undesirable events occurred in 2% of the donor populations, of which 45.8% could not complete the blood donation process while only 16.7% completed the blood donation process. Mild vasovagal attack occurred in 0.2% of the donor population. Undisclosed deferrable risk factors/ behaviours were identified by the phlebotomist in the bleeding room which made donors unfit for donation even though they had passed the donor screening criteria. This accounted for 20.8% of those with negative experience. Guidelines are required to identify donors that are not likely to complete donation to avoid wastage of time, blood, resources and reduce undesirable experiences.

Background: Blood transfusion saves lives and improves health, but many patients requiring transfusion do not have timely access to safe blood. Providing safe and adequate blood should be an integral part of every country’s national health care policy and infrastructure. Aim: The main aim of the study was to analyze the reasons for pre-donation deferral amongst blood donors attending our blood bank and outdoor camp organized by our blood bank and to evaluate and categorize the reasons into temporary and permanent deferral. Materials and methods: It is a Record based Retrospective Study conducted in Department of Immunohematology and blood transfusion over a period of two years from July 2015 to July 2017. The reasons of deferral were analyzed after classification into following categories, namely, temporary- permanent deferral, in-house –camp blood donors, gender-based (male -female), and various age group categories. Results: A total of 30,022 prospective blood donors registered for blood donation out of which 2241 blood donors were deferred (7.46%) with low hemoglobin level, medication and hypertension as top three causes. The majority of deferral was seen in 18-25 years of age group which mainly comprises the youth population. The female donors were rejected eight times the male donors. Conclusion: The knowledge of the deferral incidences and their causes in a particular region helps in deciding the magnitude and the direction of the blood donor recruitment efforts. Background: Safe blood is a critical component in improving health care and in preventing the spread of infectious diseases. Aims & Objectives: Blood transfusion can cause the transmission of infectious to recipients. This is an important mode of infection. The aim of the study was to assess the prevalence of such type of infection among blood donors and to compare the seroprevalence of transfusion transmitted diseases in blood donors. Retrospective study was conducted for 5 years from January-2009 to December-2013 at KIMS Blood Bank, Secunderabad. Materials and methods: All donors reporting to the blood bank during the period January-2009 to December-2013 were screened for human immunodeficiency virus (HIV) 1 & 2, hepatitis C viruses, malaria and syphilis. Screening of HIV, hepatitis B and hepatitis C viruses were done by chemillumiencies and syphilis was screened by RPR method. Results: A total of 39780 voluntary blood donors were screened, of which 38697 were males and 1083 were females. Seropositivity of HIV, hepatitis B, hepatitis C viruses & syphilis were 0.26%, 1.28%, 0.51% and 0.03% respectively. No blood donors test showed positivity for malaria parasite. Conclusion: With the implementation of strict donor selection criteria and use of sensitive screening test, it may be possible to reduce the incidence of TTIs.

Introduction. Syphilis is one of infectious diseases that can be transmitted by blood transfusion. Determining the frequency of syphilis among blood donors and the relevant risk factors is an important aspect of safe blood transfusion. Objective. To determine the frequency and the trend of syphilis infection among blood donors in South Backa Region, to classify the confirmed infection and to describe demographic characteristics of seropositive blood donors and their risk factors. Methods. This retrospective study analyzed results of routine EIA syphilis screening and blood donor data from South Backa Region between 2004 and 2008. Results. Thirteen out of 153,431 tested blood units were confirmed as syphilis-infected. The study found that the overall frequency of infection was 8.47 per 100,000 blood donations, with a trend increase of 2.4%; there was a statistically significant (p<0.001) higher frequency among first-time blood donors (51.93/100,000) than regular ones (2.84/100 000). The highest prevalence was noted in donors aged over 45 years. All cases were classified as past syphilis infections. Post-donation interviews revealed exposure risk in 69.23% of the donors. Conclusion. A higher frequency of past infections detected in older donors is probably due to a higher prevalence and incidence of syphilis in the past. Further continual epidemiologic follow-up of syphilis in blood donors is crucial in obtaining safe blood.

Background: In India, it is mandatory to screen blood donors for HIV, hepatitis B, hepatitis C, syphilis and malaria. The donor screening strategies include taking the elaborate medical history, performing preliminary clinical examination and screening for infectious markers. The infectious markers include anti-HIV (1 and 2) antibodies, hepatitis B surface antigen (HBsAg), anti-hepatitis C virus antibodies, and malaria antigens, such as histidine rich protein (HRP) and pan-aldolase. National adult (15-49 years) HIV prevalence is estimated at 0.26% (0.22%-0.32%) in 20155 (0.30% among males and 0.22% among females). The objective of this study was to be carried out with the aim to find out the seroprevalence of HIV infection and its trend among the blood donors a hospital based blood transfusion service set up in north India over a period of six years.Methods: It was record based retrospective study from 2010 to 2015. Before donation "blood donation form" had been filled by every donor and this form had particulars about age, gender, address, and occupation, date of previous donation, any illness and medical treatment taken. The donor blood samples were collected at the time of blood donation from the primary bag and tested for the presence of HIV using enzyme linked immunosorbent assay (ELISA). All donors were subjected to a pre-test counselling which was done by qualified staff trained to screen donors for blood donation.Results: A prevalence of 0.21% of HIV was seen among the donors from the period 2010-2015. The trend of HIV prevalence among donors has been fluctuating while ranging from 0.13% to 0.34%.Conclusions: HIV remains the major threat for transfusion transmitted infections in India. For a safe blood service in our country, where comprehensive laboratory tests are neither possible nor pragmatic, it is best to switch over to 100% voluntary donations, as it is now established that only voluntary non-remunerated regular donation is the safest. Background: There are a limited number of studies regarding the prevalence of hepatitis B, hepatitis C and HIV infections among the drug addicts in Himachal Pradesh; C virus (HCV) infection in north India especially Himachal with very high rates of substance abuse. The present study was attempted to study the prevalence of hepatitis C among the injectable drug users, which is more important in a country like India where viral hepatitis is estimated to be among the top ten causes of deaths.Methods: A study was conducted in 2019-2020 (July-April). HBsAg, anti-HBs, anti-HCV and anti-HIV tests in 235 drug addicts were studied. Urine samples obtained from drug addicts were analyzed for cannabis, opiate and cocaine metabolites.Results: The subjects included were 235 IDUs who were opioid dependent. All the 235 drug users were males, and their mean age was 30.69±9.494 years; 112 (47.7%) of them were in the age group ranging 20 - 29 years (p <0.05). Of 235 drug addicts, 113 (48.1%) and 115 (48.9%) were only cannabis and opiate users, respectively. The frequencies of HBsAg, anti-HBs and anti-HCV among drug addicts were 2.6%, 38.3%, and 9.4%, respectively.Conclusions: The obtained results showed that HCV infection was an alarming problem among opiate users in this part of Himachal. It is suggested to rapidly diagnose the infected persons; thus preventive measures and appropriate control may limit further transmission of these infections.

Background: Blood donor programme is vital to any transfusion service. This process of blood donation involves voluntary and non remunerated blood donors. The objective of donor screening is to minimize the chances of transmitting infectious agents to recipients. Material and Method: The study was conducted during one year duration from 2017-2018. A total of 700 donors are deferred against 9556 blood donors recruited on the basis of clinical history and brief medical examination in blood bank and voluntary blood donation camps. Donors were deferred temporarily or permanently. Result: Out of 700 deferred donors; 546 were voluntary and 154 were replacement blood donors. Male (601) outnumbered female (99). There were 682 temporary and 18 permanent deferral. Most common age group for deferral was 17-30 yrs (490). Common causes included anemia(142), on medication(108), underweight(83) and high blood pressure(72). Conclusion: Donor selection process is a most important preliminary step in blood banking and should follow stringent medical and regulatory rules. Studying and analysing the profile of blood donors will help to identify sections of the population which could be targeted to increase the pool of voluntary blood donors. Every attempt should be made to decrease the donor deferral keeping in view the safety of both donor and recipients. Keywords: Blood donor, Deferral, Voluntary, Temporary, Permanent.

BACKGROUND The primary aspect of blood transfusion lies in the availability of blood and blood components which are tested and reported negative for transfusion transmitted infections (TTI’s). The increased seroprevalence of TTI’s indicate their prevalence in general public. The study was conducted to analyse the increasing seroprevalence of hepatitis C virus (HCV) over a period of 3 years. The objectives of the study determine the seroprevalence of hepatitis B (HBV), hepatitis C (HCV) and human immunodeficiency virus (HIV) among the voluntary blood donors at a tertiary care hospital & associated blood bank. METHODS This cross-sectional study was done at the Department of Transfusion Medicine, GSVM Medical College, Kanpur, Uttar Pradesh, over a period of 3 years from July 2017 to June 2020. The samples collected were screened by enzyme-linked immunosorbent assay (ELISA) tests. RESULTS A total of 69561 blood units were collected during this 3-year study from July 2017 to June 2020. Of all the blood bags screened, the highest prevalence was seen for HBV followed by HCV and HIV. 949 (1.4 %) were reactive for HBV, 75 (0.1 %) were reactive for HIV and 332 (0.5 %) were reactive for HCV. The results show a remarkable increase in the seropositivity for hepatitis C infection. CONCLUSIONS The seroprevalence of HCV has seen an increase in case load over the years as compared to HBV and HIV. This increase in case volume is indicative of impending liver diseases progressing to chronic stage over the years. Thus, it is imperative to create awareness in the general population to help reduce the risk factors responsible for HCV infection.
