Relevance. Medicines, taking into account the role of various microbial associations in the development of severe generalized forms of gingivitis and periodontitis, should have high antimicrobial activity, but their use can also cause growth inhibition and representatives of normal microflora. The purpose ― to evaluate the effectiveness of complex treatment of inflammatory periodontal diseases using new prophylactic agents. Material and research methods. For a comparative clinical and laboratory evaluation of the effectiveness of the therapeutic and prophylactic agents used, the patients were divided into 3 groups. With HGPLS - the control group - 18, in which treatment was limited only to hygienic education and the conduct of professional hygiene. The comparison group (20 people with CHPH) received treatment according to the generally accepted standard regimen and prescribed Curasept ADS 712 gel toothpaste. The main group (20 people with CHPH) received the standard therapy with Curaprox Enzycal toothpaste 1450ppm. Microbiological studies of a mixed oral fluid and clinical indices were performed in the work — the SBI bleeding index of the gingival sulcus (Muhlemann, 1971), Silness-Loe hygienic index (S-L, 1964). Evaluation of the cleansing, anti-inflammatory and hemostatic effects of the proposed medications was carried out before treatment and 1,3.6 months after treatment. Research results. The control group, the frequency of occurrence of A.actinomycetemcimitans and P.intermedia three months after the completion of treatment and preventive measures was 5.6 ± 5.4 and 11.1 ± 7.4 %, in the comparison group, where Curasept ADS toothpaste was administered 712, the indicators for the above bacteria dropped to zero. The detected values of the hygienic index three months after treatment in the main, control groups and in the comparison group, respectively, amounted to 1.12 ± 0.038; 1.30 ± 0.028 and 1.13 ± 0.041. In the long-term clinical observations in the main group, the values of the Conclusion. According to the data obtained, the use of Curaprox Enzycal paste with 3 enzymes and sodium fluoride in the minimum amount for use revealed its high prophylactic efficacy.

Goal. Improving the scheme of treatment and prevention of periodontal diseases in workers of the woodworking industry of Chernivtsi region, who in the process of professional activity have long-term contact with unfavorable factors of the production environment. Materials and methods. 70 workers of the woodworking industry with diagnosed chronic generalized periodontitis of the I degree were treated (35 people – the main group (A); 35 people – comparison group (B)), age – from 25 to 45 years. To compare the results of laboratory studies, an additional survey of 25 healthy individuals of the same age with an intact periodontium were conducted. They formed the control group (C). Clinical examination of patients was performed according to standart methods: subjective (complaints, medical history, life history) and objective (examination, periodontal indices, determination of the level of gingival attachment). Laboratory methods of research included determination of urease and lysozyme activity in saliva, degree of dysbiosis of oral cavity. As maintenance therapy, patients of the main group were prescribed the proposed composition DEPE. Results. After the treatment, a significant improvement in the hygienic status of patients in both groups was observed, but difference between groups A and B in the indicators of oral hygiene after treatment was no statistically significant (pA2–B2 > 0.05). There was a significant improvement in periodontal indices after treatment in patients of both groups (PMA index according to C. Parma, bleeding index according to Muhlemann in Cowell I. modification, Russell periodontal index, PSR-test), but the indicators in patients of the main group were significantly better – pA2–B2 < 0.05. In patients of the main group (A) the level of attachment loss decreased by 1.83 times, comparison group (B) – by 1.71 times. The difference in levels of attachment loss after treatment between groups A and B is statistically significant (pA2–B2 < 0.05). The activity of the enzyme urease in patients of the main group (A) and comparison group (B) after treatment was significantly reduced (pA1–A2 < 0.001, pB1–B2 < 0.001), but only in the main group it reached the level in the control group (C) (pA2–C > 0.05). The level of lysozyme after treatment was significantly increased in patients of both observation groups (pA1–A2 < 0.001, pB1–B2 < 0.001), and reached the level of lysozyme in patients of the control group (C) (pA2–C > 0.05, pB2–C > 0.05). The degree of oral dysbiosis in patients of the main group (A) was significantly reduced by 5.43 times (pA1–A2 < 0.001), and its difference from the degree of dysbiosis in patients of the control group (C) is statistically insignificant (pA2–C > 0.05 ). The degree of oral dysbiosis in patients of the comparison group (B) was significantly reduced by 3.04 times (pB1–B2 < 0.001), but still remained significantly different from oral dysbiosis in patients of the control group (C) (pB2–C < 0.05). Conclusion. The proposed pharmacological composition DEPE is an effective antiseptic solution and can be used in the phase of maintenance therapy to improve the treatment and prevention of periodontal disease in workers of woodworking industry. Summary. Inflammatory periodontal diseases continue to be one of the pressing problems of modern dentistry [1]. It is known that there is a close relationship between general-somatic pathology and diseases of the oral cavity [2]. The aim of the study – to learn the effectiveness of the use of anti-dysbiotic hepatoprotector in the complex treatment of patients with periodontal inflammatory diseases on the background of chronic non-calculous cholecystitis. Materials and Methods. The main (group 1) group consisted of 106 people who suffered from inflammatory periodontal disease (IPD) with concomitant chronic non-calculous cholecystitis (CNC). The comparison group included 92 patients with IPD without concomitant pathology (group 2). To compare the research results of the patients with IPD, 30 healthy teethed individuals without periodontal pathology and without somatic diseases (group 3 or the control group) were involved. The state of the hepatobiliary system in patients of the main group was assessed by the doctors of the Gastroenterology Department of Zolochiv District Hospital of Lviv region. Results and Discussion. The symptomatic HCG and the presence of solid and soft dental deposits were diagnosed in all patients. The Green-Vermillion’s index was the highest in patients from the main group (1.67±0.01); it was probably (p <0.05) higher than that in the comparison group (1.54±0.04), as well as in the control group (0.44±0.07). The PMA index in the subgroup 1A immediately after treatment decreased by 9.7 (p <0.001) times. The index of bleeding in the subgroup 1A decreased by 10.7 (p <0.001) times. The PMA index in the subgroup 1A immediately after treatment decreased by 10.4 (p <0.001) times. The index of bleeding in the subgroup 1A under the influence of the proposed therapy decreased by 6.5 (p <0.001) times.The difference regarding the data before treatment remained lower by 2.2 times and 2.0 (p <0.001) times, however, the difference between the subgroups 1A and 1B in 3 months and 6 months was already 2.1 times in both cases. Conclusions. It was found that the clinical course of inflammatory periodontal diseases was much more difficult in these patients. The presence of pathology in the hepatobiliary system in patients increases the risk of periodontal disease. Therefore, in order to improve the efficacy of treatment, it is advisable to use this antidysbiotic drug in the complex treatment, and the results obtained in 3 and 6 months after treatment indicate a long-lasting positive effect.

This article describes the study of the therapeutic efficacy in combining VAC‑therapy with periodic instillations of L‑arginine ("Tivortin") for the local treatment of purulent-inflammatory soft tissue conditions. The article presents the results of the local treatment of 44 patients with purulent-inflammatory soft tissue conditions who made up the main group and received VAC‑therapy in combination with periodic instillations of L‑arginine solution. 45 patients of the control group received standard topical treatment with gauze‑ointment dressings. Local treatment of the main group started in the purulent-necrotic phase and lasted 6 days. Instillations were done every 12 hours, with an exposure at 2 hours with subsequent renewal of special indicators of negative pressure therapy. When studying the microflora of purulent foci in the main group on the first day we found that among the pathogens S. aureus was the most prevalent as it was isolated in 30 (68.2%) cases, Enterobacter was found put in 10 (22.8%) cases, Accinetobacter – in 2 (4.5%) cases, Proteus – in 2 (4.5%) cases. The commonest microbial associations in the main group were presented by the combination of Enterobacter and E. fecalis found in 30 (68.2%) patients, the fungal flora (Candida albicans) was found in 5 (11.4%) individuals. On the 7th day among the patients of the main group no aerobic flora was isolated. Among the patients of the comparison group, monoculture was detected in 14 (31.1%) patients and microbial associations – in 5 (11.1%) patients. The patients of the main group demonstrated faster normalization of laboratory blood parameters than in the comparison group. Since the 3rd day, there was a decrease in the number of leukocytes in the peripheral blood. This tendency was more pronounced among the patients of the main group, where the indicators of leukocytosis were significantly lower (p> 0.05) than in the comparison group and on the 5th day reached the reference values, while in the comparison group the indicators got normalized only on the 7th day. Applying this therapy in the integrated treatment of patients with purulent inflammatory soft tissue conditions promotes the rapid cleansing of wounds from necrotic tissues, the appearance of "juicy" granulations in 3 days, as well as diminishes general intoxication and considerably reduces the length of hospital stay.

The objective: To evaluate the clinical efficacy and safety of Enoxaparin-Pharmex for the prevention of thrombotic complications (pulmonary embolism) in the postoperative period in patients with moderate risk of these complications. Patients and methods. The study included 50 women after a caesarean section had an average degree of risk of pulmonary embolism. Patients were divided into the main group (n=25) and control group (n=25) in accordance with the treatment: patients of the main group received postoperative Еnoxaparin- Pharmex, group comparisons enoxaparin sodium (brand foreign manufacturer’s). Patients in both groups received the drug at a dose of 20 mg for 5 days, 1 time per day subcutaneously. Results. The research data analysis showed identity results of hemostasiogram of patients in the main group and the comparison group, no side effects after treatment in both groups. Conclusion. The clinical studies suggest the drug Enoxaparin-Pharmex is effective, safe LMWH, which can be used to prevent troboembolic complications, including post-operative treatment in obstetric practice. Spectrum of Enoxaparin-Pharmex can be extended to the prevention and treatment of thromboembolic conditions of varying severity with appropriate doses of the drug. The prevalence of comorbidities has been growing for the last decades. Therefore, the detection of biomarkers for diagnostic and prognostic purposes is of great practical importance. The aim of this study was to assess the biomarkers of osteo-defficiency in the course of secondary osteoporosis in patients with comorbid chronic pancreatitis and arterial hypertension. We examined 110 patients with chronic pancreatitis: 70 of them had comorbid hypertension, and 40 patients were found as having no comorbidities. The age of patients ranged from 33.2 ± 2.1 (main group) and 32.9 ± 3.1 years (comparison group); women predominated (72.9% and 70%, respectively). The control group includes 78 healthy individuals of the same age and sex. Diagnostic investigation included studying clinical and anamnestic characteristics of patients (duration of the disease, manifestations of the course, frequency of recrudescence, fractures) and biochemical parameters of bone metabolism: osteocalcin, total bone phosphatase and tartrate-resistant acid phosphatase and the establishment of correlations between these parameters and incidence of complications. It was found that in the isolated course of chronic pancreatitis there is a high (R = 0.60) statistically significant (p <0.01) correlation between the levels of osteocalcin and pancreatic elastase-1. A negative statistically significant (p <0.01) mean correlation (R = -0.49) was found between the content of tartrate-resistant acid phosphatase and age of the patients having comorbidity of chronic pancreatitis and hypertension, as well as there is a moderate correlation between the content of tartrate-resistant acid phosphatase and the duration of hypertension, which is statistically significant (R = 0.36, p <0.01). The levels of total bone phosphatase and tartrate-resistant acid phosphatase in the main group exceeded the reference values in 2.5 and 1.9 times respectively (CMU; U = 866.0; p <0.01), while in the comparison group were 2 times (total bone phosphatase) and 1.3 times higher (tartrate-resistant acid phosphatase) times, respectively (CMU; U = 821.0; p <0.01) that enables to diagnose the development of osteopenic conditions. That is, the combined course of chronic pancreatitis and hypertension should be considered as unfavourable tandem in the development of secondary osteoporosis and requires early osteoporotic screening.

The condition of teeth in ancient populations living on the territory of Ukraine has been little studied. The aim of the work is to study the dental status of the 15th century Nogais, and to carry out a comparative analysis of the data obtained with the state of the dento-jaw system of medieval populations that lived on the territory of modern Ukraine in the 9th – early 15th centuries. The object of the study was 48 Nogai skeletons (main group) and 118 representatives of medieval cultures (control group). The research method was a method for analyzing the skeletal tissue of ancient people, developed by the authors. The prevalence and intensity of caries, the prevalence of dento-jaw anomalies, dental calculus and periodontal diseases were studied. Statistical analysis was performed using the Pearson χ2 test. It was found that the prevalence of dental caries in the main group is lower than in the control group (18.75 % and 37.28 % respectively, p<0.05). It was also found that this indicator significantly differed in men of both groups (p<0.05), while in women it did not differ (p>0.1). It was found that in the main group, compared with the control group, the frequency of primary adentia was higher (16.67 % and 6.68 % respectively, p<0.05), but the prevalence of anomalies in the position of individual teeth and occlusion was lower (4.17 % and 21.18 % respectively, p<0.05,). It was found that the prevalence of dental calculus in the main group is two times higher than in the control group (89.50 % and 41.0 % respectively, p<0.0001). Also, 15.0 % of Nogai men showed a decrease in the height of the alveolar process of the jaws, which is characteristic of periodontitis; in women, such changes were not found due to the low average age of survival. Thus, the 15th century Nogai population had a lower prevalence of caries and dento-maxillary anomalies than the medieval populations that lived on the territory of modern Ukraine in the 9th – early 15th centuries.

Development and introduction of materials influenced on reparation process in a myometrium is actual direction of modern obstetrics in connection with the increase of cesarean section rate. One of such preparation is «Collost», it stimulates the processes of neoangio- and myogenesis and excludes the disorderly growth of scars tissue. Aim: To study the influence of bioplastic material «Collost» on the reparation process in myometrium after cesarean section. Objectives: to estimate the morpho-functional state of scar on uterus after cesarean sections depending on implantation of collagen membrane. Material and methods: There were included 13 female rabbits in experimental part of the study. In clinical part of the study there were included 20 pregnant woman with intraoperative application of «Collost» (main group), 30 pregnant woman without «Collost» implantation (comparison group) and 10 pregnant with vaginal delivery (control group). Results: histological, immunohistological and immunological findings of postoperative uterine scars of female rabbits showed significant differences of expressions of miosin, vascular endothelial growth factor and collagen I type. The myometrium was significantly thicker in the area of uterine scar in main group (with collagen membrane implantation) than in comparison group. Conclusions: bioplastic material «Collost» leads to full formation of muscular wall and vascular ture in the area of postoperative uterine scar.

The abnormalities of maxillodental system take one of the leading places among diseases of the maxillofacial area. Thus, they are diagnosed more than in 50% cases among the younger population and in 30% cases among the adult population.   Most often, there are prerequisites for the incorrect position of individual teeth: anomalies are formed at the genetic level because of heredity of pathologies of size, amount, shape of teeth and size of the jaw bones of the facial skeleton. If the position of the teeth is incorrect, the periodontium retains the occlusal load of the functional injury. Therefore, the aim of our research was to study the prevalence of periodontal tissue diseases in young individuals on the background of crowded teeth. The material and research methods. The study was conducted on the base of Dental Center of Danylo Galytsky Lviv National Medical University during 2015−2017 years. 1146 people at the age from 18 to 44 years were examined. Every participant of research underwent general dental examination, as well as a special examination aimed at clarification of orthodontic treatment. The diagnosis of periodontal tissue diseases was made according to the classification of N. F. Danilevskyi and was determined using paraclinical indicators. The obtained results were worked out statistically using functions of MS Excel and Statistica 6. Results of the research and their discussion. The total number of the examined with orthodontic pathology was found on average in 77.24% of patients (885 people). At the same time, abnormalities of dental jaw system were not detected in about 261 (22.76%) of the examined. As a result of the conducted studies we found that among the examined without dental jaw abnormalities 38.89±.,32% of patients had intact periodontium, that was 1.6 times more for data in individuals with crowding − 23.73±2.00, p<0.01. At the same time, in patients of the main group the prevalence of periodontal tissue diseases was 1.2 times higher than in the examined of the comparative group (76.27±2.00% against 61.11±3.32%, respectively, p<0.01). It should be noted, that with the increase of the age of patients the prevalence of periodontal tissue diseases increased in both study groups, but in patients with crowded teeth this tendency was more pronounced. So, in the younger age group in patients with crowding the prevalence of periodontal diseases was 1.5 times higher in relation to the data of their peers from the comparison group (71.37±3.00% vs. 46.99±5.48%, p<0.01). In 27−35-year-old patients the prevalence of periodontal tissue diseases amounted to 72.38±4.03% of the examined of the main group and 65.62±5.94% of the examined of the comparison group, but the obtained data were not statistically significant, p>0.05. At the same time, in the patients of the main group aged 36−44 years the prevalence of periodontal tissue diseases was maximal and exceeded the value of their peers in the comparison group by 1.2 times (92.08±2.69% vs. 73.91±5.29%, respectively, p<0.01). The analysis of the obtained data allowed to assert that on average crowding of teeth on the upper jaw was found in 237 patients (52.55%) and on the lower jaw in 214 patients (47.45%). Thus, the maximum frequency of crowded teeth on the upper jaw was observed in the examined in the age range of 27−35 years, 60.16±4.41% of patients, with the minimum prevalence of this pathology in the youngest group (18−26 years), 49.34±4, 72%. The analysis of diagnostic structure of periodontal tissue diseases showed that inflammatory diseases of periodontal tissues (gingivitis, localized periodontitis) were found in patients with crowded teeth, on average 1.2 times more often than in patients without orthodontic pathology (42.42±1.88% vs. 36.33±3.54%, respectively, p<0.05). Conclusions. Thus, as a result of the conducted studies, higher prevalence of periodontal tissue diseases was proved in patients with crowding of teeth, which made progress in increasing the age of the examined than in patients without orthodontic pathology.

The article presents the results of a comparative randomized study, the purpose of which was to evaluate the effectiveness of the use of aminodihydrophthalasindione sodium (Galavit, LLC SELVIM, Russia) in the treatment of patients undergoing an abortion. Included in the study, 48 women were divided into two groups, 24 patients of the main group in addition to the standard rehabilitation were treated with aminodihydrophthalasindione sodium in the comparison group – 24 patients underwent only standard rehabilitation. In this study, all patients (100%) of the main group who were treated with aminodihydrophthalasindione sodium in addition to the standard therapy marked reduction of the clinical symptoms of the disease and positive dynamics was observed at ultrasound. In the control group, the full clinical effect of treatment was observed only in 10 patients (52.6%). 9 women (47,4%) required repeated therapy. Ultrasound studies in 12 patients (63.2%) showed changes equivalent to endometritis.

At the present stage of development of dentistry, the leading Ukrainian and foreign scientists devote a considerable part of the research to a deeper study of the etiology and pathogenetic mechanisms of periodontal tissue diseases and the influence of various exogenous and endogenous factors on their course.The aim of the study – to learn the ability and methods of forecasting and early diagnosis of the periodontal tissue lesions in young people. Materials and Methods. During our research we examined 24 young people with periodontal tissue diseases, who were included to the main group, and 15 healthy people, who formed the control group. The complex clinical examination was performed in each research group. It was studied the distribution of polymorphous variants of the type I parathormone receptor and the α1-chain of collagen gene with a help of polymerase chain reaction by restrictase cleavage of DNA fragments and electrophoresis in polyacrylamide gel (AA/BA 29:1). Results and Discussion. The distribution of genotypes by PTHR1 gene in control group was similar to those in main group (p>0.05). Also we didn’t find the difference between frequencies of the separate alleles in people with periodontal tissue pathology and without it (p>0.05). Yes, the repetitions of the allele 5 encoding normal type I parathormone receptor were found more often, comparing with the allele 6 that is responsible for the formation of unfunctional PTHR1 (р<0.001) in both main and control groups. The dominance of the genotype TT, which corresponds to the pathology, was found in young people with the periodontal tissue lesions – (38.46 ± 4.79) %, while among the control group the genotype of norm GG was met the most often – (68.24±5.08) %. Also, the frequency of repetitions of the allele T encoding the imperfect collagen chain was (57.60±3.79) % in young people with periodontal diseases, and in the control group this figure was (13.27±2.81) %, p<0.001.  Conclusions. According to our results the presence of allele T and genotype TT that correspond the imperfect collagen chain may be one of the causes of periodontal tissue pathology.
