Background: A widespread severe deficiency of vitamin D (vit D) exists in Iraq, especially among women of reproductive age. Depression or anxiety is related to deficiency of vit D. These patients may present with psychosomatic symptoms which are symptoms with no sufficient organic cause in the body but from psychological causality. The study aimed to investigate the serum level of vit D in females with psychosomatic symptoms residing in Mosul City, Iraq and explore the effect of vitamin D3 (vit D3) supplements for treating psychosomatic symptoms. Methods: A total of 73 female patients aged 15–45 years presented with several symptoms. All patients underwent physical examination and psychiatric interviews. Patients with minimal to mild depression were selected. All included patients underwent investigation of first serum vit D level. The supplementation with vitamin D3 was started. The second serum vit D level was investigated at first post-improvement visit. Results: In the improved group, the post-supplementation serum vit D level was higher than that before the supplementation (p = 0.00). In addition, the vit D serum difference before and after its supplementation was greater in the improved group than the nonimproved one (p = 0.00). Patients not only experienced improvement in their mood swings, but also recovered from other recorded symptoms such as headache, fatigue, tiredness, and joint pain. Conclusion: This study revealed a noticeable association concerning serum levels of 25-OH D and mild depression in addition to psychosomatic symptoms in females living in Mosul City. Supplementation with suitable loading vit D3 dosages improved these symptoms showing a probable causal relationship.

Vitamin D deficiency is common in women with polycystic ovarian syndrome. Vitamin D plays an important physiologic role in reproductive functions of ovarian follicular development and luteinization through altering anti-müllerian hormone signaling, follicular stimulating hormone activity and progesterone production in human granulose cells. Vitamin D is precipitated in adipose fat tissues, making it notable to be used for the body as a result; obese people with high body mass index are already highly expected to have low levels of serum vitamin D.

The present study was aimed to study the effect of Sublingual Vitamin D3 on Serum Vitamin D level in Vitamin D deficiency patients. This was a cross-sectional and interventional study. All the Vitamin D deficiency patients of age 18-60years and either gender, willing to participate in the study were included. Patients who had greater than 20 ng/ml were excluded from the study. The total number of participants in our study was 200, out of these 111 males and 89 females, the mean age in our study was 51.07 ± 7.39Yrs. All volunteers were given sublingual vitamin D3 (60,000IU) in six doses every fifteen days of follow up for 3 months. The subject’s serum 25(OH)D levels were estimated before and after the treatment of sublingual vitamin D3. There was a statistically significant difference in serum vitamin D3 level before 16.61±6.71 ng/ml and after 35.80±7.80 ng/ml after treatment with Sublingual Vitamin D3. Six doses of 60,000IU of Vitamin D3 sublingual route having improved the role of serum 25(OH)D levels in the treatment of Vitamin D3 deficiency patients.Keywords: Vitamin D3; Sublingual route

Abstract An analysis of the structure of the causes of infertility shows that up to almost 25% of women of reproductive age ovulation disorders are observed. Hormonal disorders leading to the absence of ovulation, began to play a leading role in the etiology of infertility, special vitamin D deficiency, while today it has been established that its D receptors (VDR) are localized in most organs, including reproductive ones. VDR affect the expression of 1000 genes and play a crucial role in the implementation of sex hormones function. The aim of the study was to assess the vitamin D level in women of reproductive age, depending on the presence of signs of PCOS and waist circumference (WC). 71 women from 19 to 44 years old were examined of these, 41 patients were diagnosed with PCOS and impaired ovulatory function, in 30 patients the ovulatory cycle was preserved, and signs of PCOS were not detected. The level of vitamin 25 (OH) D3 in the serum of the subjects was evaluated by tandem chromato-mass spectrometry at the ArchiMed clinic of new medical technologies, Moscow. In the recommendations of the Canadian “The Vitamin D society”, 40-60 ng/ml the optimal level of serum vitamin D, a concentration of 20-40 ng/ml insufficiency, <20 ng/ml as a deficiency. Vitamin D deficiency was detected in 57% of women in both groups, with a pronounced deficiency (<10 ng/ml) in 17%, insufficiency was observed in 43% of cases, a sufficient level was not detected in any of the subjects. The average level of vitamin D in patients with PCOS was 18.2 (2 to 32.3) ng/ml, the value in patients without signs of PCOS was 18.5 (6.8 to 31) ng/ml. Comparison of the vitamin D value with WC showed that in women with PCOS with an WC <80 cm the value of this indicator was 18.8 ng/ml, and for WC ≥ 80 cm it was statistically significantly lower - 13.3 ng/ml (p<0.05). Conclusion. High prevalence of vitamin D deficiency among patients of reproductive age, with no differences in the magnitude of this indicator depending on the presence of signs of PCOS and persistent ovulation. Difference was established between the levels of vitamin D in patients with PCOS depending on the waist circumference - its insufficiency increases with an increase in WC. The results of the study suggest that the deficiency of vitamin D in the body increases with the aggravation of hormonal dysfunction in PCOS, which should be taken into account by specialists during the management of this category of patients and the treatment of infertility.

Background: Vitamin D plays pleiotropic roles in the body and hence, changes in its metabolism and distribution during starvation could play an important role in the adaptive response to famine. We aimed to identify the responses of some vitamin D metabolites to 8 d of fasting and exercise. Methods: A repeated-measures design was implemented, in which 14 male volunteers fasted for 8 d and performed an exercise test before and after fasting. Serum samples were collected on day 1 after night fasting and after 8 d of complete food restriction, before and 1 h and 3 h after exercise. Results: After 8 d of fasting, compared with baseline values, serum 24,25(OH)2D3 and 3-epi-25(OH)D3 levels significantly increased; those of 25(OH)D3 and 1,25(OH)2D3 were unaffected; and those of 25(OH)D2 decreased. Exercise on the first day of fasting induced an increase in serum 3-epi-25(OH)D3 levels, while exercise performed after 8 d of fasting induced an increase in 25(OH)D3, 24,25(OH)2D3, 25(OH)D2, and 3-epi-25(OH)D3 levels. Conclusion: Increases in 24,25(OH)2D3 and 3-epi-25(OH)D3 levels imply that fasting stimulates vitamin D metabolism. The effects of exercise on serum vitamin D metabolites, which are most pronounced after fasting and in subjects with serum 25(OH)D3 above 25 ng/mL, support the notion that fasting and exercise augment vitamin D metabolism.

Background: Narrowband ultraviolet B (NB-UVB) has been widely used in the treatment of psoriasis. It has been shown that vitamin D is a major regulator of the expression of human antimicrobial peptide cathelicidin LL-37, which has a critical role in inflammatory cascade in psoriasis. Objective: To evaluate the effect of NB-UVB therapy on serum levels of cathelicidin LL-37 and 25-hydroxyvitamin D [25(OH)D] in psoriasis patients. Methods: Ninety-three psoriasis patients and 50 controls were included in the study. For psoriasis patients, serum levels of 25(OH)D and cathelicidin LL-37 were estimated before and after NB-UVB therapy. Results: Before treatment, serum 25(OH)D levels were significantly lower in psoriasis patients (31.5 ± 14.41 nmol/L) compared to controls (53.5 ± 19.6 nmol/L), p = .015. In contrast, serum LL-37 was significantly higher in psoriasis patients (13.24 ± 3.2 ng/mL) than in controls (7.92 ± 5.33 ng/mL), p < .001. After NB-UVB treatment, there was a highly significant elevation of serum 25(OH)D to reach 56.85 ± 5.2 nmol/L ( p < .001) and further elevation of serum LL-37 to reach 29.4 ± 4.2 ( p = .02). Conclusions: The elevation of serum 25(OH)D and cathelicidin LL-37 could be an additional possible mechanism of action of NB-UVB therapy in the treatment of psoriasis.

BACKGROUND: Uterine fibroids (UFs) affect women of reproductive age and lead to major morbidity in premenopausal women. Identifying modifiable risk factors could help develop new UF prevention and treatment strategies. OBJECTIVES: The purpose of this research was to investigate the relationship between serum Vitamin D3 levels and UF in women seeking gynecological services. METHODS: This case–control design was conducted in September 2018 at the outpatient gynecology clinic of Shahid Beheshti University of Medical Sciences, Tehran, Iran. Cases had at least one ultrasound confirmed fibroid lesion with an average volume of 2 cm or greater. The outpatient clinic has enrolled a control group of patients without UF, based on transvaginal ultrasonography or any other gynecologic pathology. Radioimmunoassay techniques were applied to measure serum Vitamin D [25(OH) D3] levels. RESULTS: A total of 148 patients met inclusion criteria, 71 women were had at least one UF and the remaining 77 participants showed normal, UF-free uterine structure. The mean serum concentration of 25-hydroxyvitamin D3 was lower in UF patients (21.37 ± 7.49 ng/mL) than without (24.62 ± 9.21 ng/mL) (p = 0.02). A modified odds ratio derived from a backward logistic regression model for 25-hydroxyvitamin D3 that included positive family history, age, body mass index, bleeding volume, physical activity, sun exposure, and history of abortion was 0.92 (95% CI, 0.88–0.98) (P = 0.02). CONCLUSION: For women with UFs, the serum level of 25-hydroxyvitamin D3 was significantly lower than in controls. Vitamin D3 deficiency is a potential risk factor for UFs to occur.

AbstractThe objective of this study was to examine the cross-sectional relationship between homeostasis model assessment for insulin resistance (HOMA-IR) and serum 25-hydroxyvitamin D (25-OHD) level in Chinese children and adolescents.Anthropometric indices, lipid metabolic profile, and serum levels of glucose, insulin and 25-OHD were determined among 278 healthy prepubertal and pubertal, normal and overweight/obese children and adolescents aged 8–18 years between March 2014 and February 2015.HOMA-IR was significantly different across vitamin D statuses (p<0.001), even after adjusting for body mass index (BMI) (p=0.035) and waist-to-height ratio (p=0.044); the difference was not significant between the vitamin D deficient and insufficient groups (p=0.120). HOMA-IR negatively correlated with serum 25-OHD level for all subjects (ROur findings supported that lower vitamin D status is strongly associated with worse HOMA-IR.

ABSTRACT Objective: Vitamin D has taken center stage in research and treatment of multiple sclerosis (MS). The objective of the present study was to assess the serum vitamin D levels of a large population of patients with MS and controls living in a restricted tropical area. Methods: Data from 535 patients with MS and 350 control subjects were obtained from 14 cities around the Tropic of Capricorn. Results: The mean serum 25-OH vitamin D level was 26.07 ± 10.27 ng/mL for the control subjects, and 28.03 ± 12.19 ng/mL for patients with MS. No correlation was observed between vitamin D levels and the disability of patients over the disease duration. Conclusion: At least for the region around the Tropic of Capricorn, serum levels of vitamin D typically are within the range of 20 to 30 ng/mL for controls and patients with MS.

Abstract Purpose: To assess the effect of oral vitamin D3 supplementation in dry eye after femtosecond laser-assisted in situ keratomileusis (FS-LASIK).Setting: Liuzhou Worker’s Hospital.Design: This prospective study included 90 patients selected between January and December in 2019, who underwent fs-lasik operation in our hospital and had obvious symptoms indicating dry eyes one month after operation. The subjects were randomly divided into two groups: the experimental group (n = 45) received vitamin D3 2000 IU / D continuously for 12 weeks; the control group (n = 45) did not take vitamin D3 orally. Ocular surface disease index(OSDI), tear breakup time(TBUT)and Schirmer’s Test I were evaluated pre-medication and 1,3,6 months after treatment. Serum vitamin D level, and the mean concentration of cytokine IL-6, IL-17, IL-23 in tears were also measured. Results: One month after treatment, the mean OSDI score of the experimental group (11.67 ± 8.53) was significantly lower than that of the control group (23.82 ± 13.22) (P = 0.007). TBUT (10.71±1.02s) and Schirmer I (9.36±0.40mm) of the experimental group were higher than those of the control group (7.49±1.29 s and 7.51±0.44 mm). The OSDI (10.25 ± 5.49), TBUT (10.75±1.09 seconds) and Schirmer I test value (11.34±0.39 mm) of the experimental group were significantly lower than those of the control group (20.22±6.23, 8.36±1.23, 8.12±0.50) at 3 months after treatment. There were significant differences in OSDI, TBUT (P < 0.05) and Schirmer I test value between the two groups at 6 months after treatment. Serum vitamin D3 level was negatively correlated with OSDI score (r=-0.90;P=0.00), and positively correlated with Schirmer I test (r=0.88;P=0.00), TBUT score (r=0.89;P=0.00) and TMH (r=0.80;P=0.00). IL-17 level was shown to be significantly correlated with TBUT (r=-0.25, P=0.014) and Schirmer I test (r=-0.21, P=0.018). IL-6 level was significantly correlated with OSDI (R=0.18, P = 0.020) and TBUT (R=0.20, P = 0.019).

Abstract Context The effect of daily vitamin D supplementation on the serum concentration of vitamin D (the parent compound) may offer insight into vitamin D disposition. Objective To assess the total serum vitamin D response to vitamin D3 supplementation and whether it varies according to participant characteristics. To compare results with corresponding results for total serum 25-hydroxyvitamin D (25(OH)D), which is used clinically and measured in supplementation trials. Design Exploratory study within a randomized trial. Intervention 2,000 International Units of vitamin D3 per day (or matching placebo). Setting Community-based. Participants 161 adults (mean ± SD age 70 ± 6 years; 66% males) with type 2 diabetes. Main Outcome Measures Changes in total serum vitamin D and total serum 25(OH)D concentrations from baseline to year 2. Results At baseline, there was a positive, nonlinear relation between total serum vitamin D and total serum 25(OH)D concentrations. Adjusted effects of supplementation were a 29.2 (95% CI: 24.3, 34.1) nmol/L increase in serum vitamin D and a 33.4 (95% CI: 27.7, 39.2) nmol/L increase in serum 25(OH)D. Among those with baseline 25(OH)D &lt; 50 compared with ≥ 50 nmol/L, the serum vitamin D response to supplementation was attenuated (15.7 vs 31.2 nmol/L; interaction p-value = 0.02), whereas the serum 25(OH)D response was augmented (47.9 vs 30.7 nmol/L; interaction p-value = 0.05). Conclusions Vitamin D3 supplementation increases total serum vitamin D and 25(OH)D concentrations with variation according to baseline 25(OH)D, which suggests that 25-hydroxylation of vitamin D3 is more efficient when serum 25(OH)D concentration is low.
