Preventive anti-inflammatory therapy during the acute respiratory infections in children with mild and moderate bronchial asthma

Objective. To evaluate the efficacy and safety of using anti-inflammatory drugs based on ammonium glycyrrhizinate (AG) (Reglisam, CJSC “VIFITECH”, Russia) in children with BA in the acute period of ARI. Patients and methods. Under observation there were 42 children (from 6 to 10 years old) with a diagnosis of BA in the acute period of ARI (1–2 days from the beginning of clinical manifestations). Patients were randomized into the main group – 20 patients who were prescribed the drug ammonium glycyrrhizinate (Reglisam) in addition to symptomatic ARI therapy, and the control group – 22 children who were prescribed only symptomatic ARI therapy. The observation period was 14 days with 2 checkpoints (1st and 14th days), when the following parameters were evaluated: general clinical examination; asthma symptoms; PCR diagnostics of acute respiratory viral infection and influenza pathogens; external respiration and BA control test (C-ACT), administration of additional medications and development of adverse events. Results. There was a decrease in the average scores of daily and night symptoms of BA in the study group, both in dynamics and in comparison with the control group at visit 2 (p < 0.05). The average duration of ARI in the main group was 6.34 ± ± 1.15 days and was less than that in the control group of patients (10.95 ± 1.45 days) (p < 0.05). The average duration of the use of short-acting β2-agonists in the main group was 3.37 ± 1.05 days and was less in comparison with the control group (6.75 ± 1.6) (p < 0.05). On the 14th day of observation in the main group, the median of C-ACT parameters increased and corresponded to good control of BA, and the level of blood eosinophils decreased to normal values; in the control group, no such dynamics was found. In 78% of patients in the main group, on the 14th day of observation, the persistence of previously detected respiratory viruses and influenza was not determined, the proportion of such patients in the control group was more than 2 times less. Conclusion. The data obtained indicate the feasibility and effectiveness of the inclusion of the drug AG as a preventive therapy for children with asthma in the acute period of ARI to improve control over the course of the disease. Allergic diseases of the respiratory system seriously affect the psychological, physical and social aspects of the live of sick children, morally and financially exhausting members of their families as well. It is known that exacerbations of allergic diseases of the respiratory tract occur due to interaction with numerous triggers, one of which is a respiratory viral infection. At the same time, it is widely known that patients with allergic respiratory diseases are more prone to to acute respiratory infections. One of the reasons for this tendency often is an insufficient activity of non-specific factors of local immunity of the respiratory system – endogenous amphiphilic antimicrobial peptides, in particular the most studied their representatives - the family of defensins and human cathelicidin. Current research proves that these antimicrobial peptides are characterized by broad antiviral, antimicrobial and immunomodulatory activity. The aim of this study was to study the concentrations of local immune factors - human HbD-2 and LL-37 - in the secretion of the mucous membranes of the upper respiratory tract in children with asthma and allergic rhinitis and to clarify their role in protection against respiratory viral infections in this contingent of patients. We performed laboratory and clinical examinations of 76 children aged 7 to 18 years, of whom 24 were diagnosed with allergic rhinitis, 28 children - bronchial asthma, and 24 - bronchial asthma and allergic rhinitis. The control group consisted of 20 healthy children of the appropriate sex and age. In addition to general clinical methods, patterns of respiratory morbidity were analyzed and concentrations of antimicrobial peptides were determined: by ELISA human cathelicidin (LL-37), β-defensin 2 (HbD-2) in the secretion of the upper respiratory tract, statistical analysis was performed. It was found that children with allergic diseases of the respiratory tract are characterized by a higher frequency of acute respiratory infections with more frequent involvement of the lower respiratory tract, which led to an increase in the duration of the disease compared to their healthy peers. In children with allergic rhinitis and bronchial asthma, there was revealed a significant decrease in the concentrations of antimicrobial peptides in the secretion of the upper respiratory tract compared with the control group.

The aim of the study was to identify possible changes in the maxillary sinuses during new coronavirus infection in comparison with the changes in the sinuses occurring during acute respiratory infections and to correlate the gender structure of the study groups. Material and methods. The study included 40 patients (18 women and 22 men) with a confirmed new coronavirus infection, hospitalized at the Covid Hospital Clinics of Samara State Medical University in 2020. All patients underwent multispiral computed tomography of the paranasal sinuses on a Ge revolution Evo 128 tomograph on the 10th day after the appearance of the first clinical symptoms. We also examined a control group of patients with acute respiratory infections with similar complaints of impaired sense of smell, rhinorrhea, and upper maxillary sinus discomfort. This group also included 40 people (24 women and 16 men). Statistical processing of the study results was performed using Microsoft Excel software package, the Mann – Whitney method of statistical significance calculation was used. Results and discussions. In 18 (45%) patients with a confirmed new coronavirus infection, a slight mucosal thickening along the lower wall of the maxillary sinuses was found symmetrically on both sides. Six (15%) of them were women and 12 (30%) were men. The age of the examined patients ranged from 31 to 83 years. Chronic sinusitis was noted in the history of 8 (20%) of the examined patients. In 6 (15%) patients with chronic sinusitis no abnormalities were detected in the maxillary sinuses at the time of examination. Two of them had parietal thickening of the mucous membrane along the lower wall of the maxillary sinuses. In the control group, there was significant symmetric parietal thickening of the mucous membrane of the paranasal sinuses in 24 (60%) examined patients (14 women and 10 men). There were no statistically significant differences in the prevalence of changes in the studied and control groups. Conclusion. According to the results of this study, there were no data on the pattern of occurrence of changes in the mucosa of the maxillary sinuses in new coronavirus infection with gender background or the presence of chronic inflammation of the maxillary sinuses in the history compared with the control group of examined patients after acute respiratory infections.

Background. The effective treatment of the hand-foot syndrome is a necessary component of the complex treatment of patients receiving antitumor therapy with multikinase inhibitors. Aims. The assessment of a new effective method for the treatment of palm-plantar syndrome in patients receiving multi-kinase inhibitors. Materials and methods. There are 27 patients received antitumor treatment with multi-kinase inhibitors and had clinical manifestations of the hand-foot syndrome (HFS) for observation. The main group (14 patients) used a combination of an alpha-lipoic acid at a dose of 600 mg per day per os and an ointment containing 0.005% calcipotriol and 0.05% betamethasone dipropionate 2 times a day. The control group, included 13 patients, used a combination with a cream containing 10% urea and ointment containing 0.05% betamethasone dipropionate. Results. After a week of treatment, the regression of the skin process was observed in both groups, but it was more evident in the main group: significant differences in severity of erythema, paresthesia, pain and burning sensation, the Dermatological Life Quality Index (DLQI) were observed at this stage of therapy (p < 0.05). After 2 weeks of therapy: a more positive trend in the main group in terms of erythema, paresthesia and burning, but the most significant difference is DLQI – 17.4 in the main group and 22.8 in the control group (p < 0.0001). At the end of the 3rd week of therapy: differences with the main characteristics of erythema, desquamation, paresthesia (p < 0.0008), pain (p < 0.0001), DLQI (p < 0.0001). At the end point of the study (after 4 weeks of therapy), significant differences were found in reducing the symptoms and the main parameters for evaluating the effectiveness of DLQI in patients of the main group with a reliability of p < 0.0001. The high correlation of DLQI was found with erythema, pain, burning, paresthesia and desquamation. Conclusions. The treatment of the hand-foot plantar syndrome combined using alpha-lipoic acid inside and topical therapy in the form of an ointment containing calcipotriol and betamethasone dipropionate, can be recommended for patients receiving targeted therapy with multi-kinaseinhibitors.

The high prevalence of inflammatory processes in the periodontitis of children, the ineffective effectiveness of preventive and curative measures can contribute to the development of generalized periodontitis, therefore it remains urgent to develop pathogenetic effects in the treatment of chronic catarrhal gingivitis.The aim of the study – a comparative assessment of the traditional treatment of chronic catarrhal gingivitis in children aged 6–7 years with therapy using an anti-inflammatory gel with neovitin.Materials and Methods. 61 children aged 6–7 years, suffering from chronic catarrhal gingivitis, were taken under clinical supervision and divided into groups: the main (30 children) and the comparative (31 children). All children underwent conventional therapy in accordance with the protocols for the provision of medical care – basic therapy. In the main group, for a month, pathogenetic agents were used: an anti-inflammatory gel with neovitin, hygiene products were recommended. In the comparative group, 3 % hydrogen peroxide solution was used for local therapy, for rinsing of the mouth (during the first week of treatment) – Rotokan, a therapeutic and prophylactic toothpaste.Examination and control examinations were carried out with the hygienic state of the mouth determined by the hygienic index of Yu. A. Fedorov and V. V. Volodkina, prevalence and intensity of the inflammatory process in the gums with the help of the Schiller-Pisarev test and the PMA index, oral fluid index (lysozyme content, formation of oxydradicals, urease activity, degree of dysbiosis, pH and viscosity).Results and Discussion. After the treatment, elimination of gum inflammation in patients of the main group was accompanied by a decrease in the PMA index by 3.8 times; formation of oxyradicals decreased in children of the main group by 6.35 %; in the comparative group – by 6.15 %. Urease activity decreased in children of the main group by 16.37 % (p<0.05), the viscosity of the oral fluid of children decreased in 2.65 times in the main group and in 2.13 times in the comparative group (p<0.05). The lysozyme content increased in children of the main group by 15.61 % (p<0.05) in the comparison group – by 9.63% (p<0.05). The treatment after 0.5 years caused an increase in the lysozyme content in the oral fluid of the children of the main group to the level of the control group. The degree of dysbiosis decreased in the main group, in the comparison group tended to increase.Conclusions. The use of the proposed therapy with gel with neovitin contributed to the best results of treatment: reducing the degree of inflammation of the gums, the number of visits to the doctor, improving the indices of nonspecific protection in the oral fluid of 6-7 years-old children and, to a greater extent, six months after the treatment.

Acute respiratory infections (ARIs) are the most common childhood illnesses worldwide whereby the reported frequency varies widely, often depending on type of assessment. Symptom diaries are a powerful tool to counteract possible under-reporting, particularly of milder infections, and thus offer the possibility to assess the full burden of ARIs. The following analyses are based on symptom diaries from participants of the German birth cohort study LoewenKIDS. Primary analyses included frequencies of ARIs and specific symptoms. Factors, which might be associated with an increased number of ARIs, were identified using the Poisson regression. A subsample of two hundred eighty-eight participants were included. On average, 13.7 ARIs (SD: 5.2 median: 14.0 IQR: 10–17) were reported in the first two years of life with an average duration of 11 days per episode (SD: 5.8, median: 9.7, IQR: 7–14). The median age for the first ARI episode was 91 days (IQR: 57–128, mean: 107, SD: 84.5). Childcare attendance and having siblings were associated with an increased frequency of ARIs, while exclusive breastfeeding for the first three months was associated with less ARIs, compared to exclusive breastfeeding for a longer period. This study provides detailed insight into the symptom burden of ARIs in German infants.

Objective: To study the features of clinical manifestations, cellular and humoral immunity of newborns with intrauterine mixed infections (IUMI). Methods: A comprehensive survey of 45 infants with IUMI was carried out. Depending on the classification of IUMI they were divided into 3 main groups: group I – 24 patients (53%) with the viral-bacterial association, group II – 12 patients (27%) with the viral-viral association, and group III – 9 patients (20%) with the viral-parasitic association. The control group consisted of 10 newborns born from uninfected, somatically healthy mothers. Diagnosis of IUMI was based on the detection of specific antibodies of the IgA, IgG and IgM classes, as well as phagocytic activity and phagocytic index of leukocytes. In addition, general clinical, biochemical, bacteriological and instrumental research methods have been conducted. Results: The blood serum IgA and IgM indices in patients of the main group were significantly higher, and the mean IgG values were lower compared to the control group. Analysis of cellular immunity parameters in the main group showed a decrease in the number of mature T-lymphocytes (CD3), B-lymphocytes (CD20), the number of T-helpers and cells that produce IL-2 in the peripheral blood, compared with the control group. In the main group, there was also an increase in the number of apoptosis cells (CD95), cells with high cytotoxic activity (CD25, CD71) and the percentage of natural killer cells (CD16). A decrease in the phagocytic activity and phagocytic index of neutrophils was recorded, which indicates the insufficiency of the nonspecific component of immunity. Conclusions: In newborns, various changes were found both on the part of specific and nonspecific components of immunity. This indicates the development of secondary immunodeficiency in this category of patients and makes it necessary to add to the main treatment of immune corrective therapy. Background. The objective of this study was to estimate a vascular wall status of large arteries and function of endothelium in patients with different clinical forms of bronchial asthma throughout the disease progression. 220 patients with bronchial asthma have been examined, including 106 persons with moderate asthma ( 1 st group), 61 persons with severe asthma (2 nd group), and 53 persons with steroid-dependent asthma. Control group was formed from 40 healthy donors. Methods. We have assessed parameters of external respiration, arterial rigidity, and the levels of TNFα, IL-6, sCD31 (sPECAM-1), CRP in the peripheral blood at the time of exacerbation and 48 weeks later. Combination of bronchial asthma (BA) and obesity is a difficult-to-control phenotype. Studies of inflammatory process with respect to severity of the disease are important for understanding the potential influence of obesity on the BA clinical course. The objective of this study was to determine cytokine profile in patients with mild BA combined with obesity. The study involved fifty-three patients with partially controlled mild BA. The patients were recruited as volunteers and signed an informed consent. The first observation group consisted of 27 asthma patients with normal body weight, the second observation group consisted of 26 patients with BA combined with obesity. A control group included 25 healthy volunteers. All the patients underwent clinical and laboratory examination in accordance with clinical standards for BA and obesity. The levels of TNFα, IL-2, IL-4, IL-6, IL-10 were evaluated in blood serum by means of flow cytometry. The ratios of proand anti-inflammatory cytokines (TNFα/IL-4, TNFα/IL-10, IL-6/IL-4, IL-6/IL-10) were calculated. Asthma patients with obesity (the 2nd group) had elevated levels of IL-2 over control group and group 1, by 38% and 44% respectively(p < 0.05). The concentration of proinflammatory cytokines TNFα and IL-6 was significanty increased in the both patient groups. Mean TNFα level was increased 2.5 times (p < 0.05), and IL-6 levels were increased by 30% (p < 0.05) in the 1st group as compared to the controls. TNFα and IL-6 concentrations showed a 3-fold increase over control values (p < 0.05) in the 2nd group. The level of antiinflammatory cytokine IL-4 was increased in patients with BA, independently of body mass. It should be noted that the concentration of this cytokine in obese patients was higher by 29% than in patients with normal body weight. IL-10 levels in patients from the 2nd group were reduced more than 2 times than in the 1st group. The patients of the 1st group showed a decrease in the IL-6/IL-10 index, in comparison with control parameters, thus indicative of an imbalance due to the elevation of the anti-inflammatory IL-10 cytokine. Among BA patients with obesity (group 2) the TNFα/IL-10 and IL-6/IL-10 indexes were higher than those of the control group (2.3- and 5.5-fold, respectively) and the group 1 (2.6- and 2.5-fold, respectively). Dynamics of these indexes confirms the systemic nature of inflammation and a predominance of non-atopic  inflammation in asthma patients with obesity. Thus, features of the cytokine profile in BA with obesity consist of a significant increase in pro-inflammatory IL-2, IL-6, TNFα cytokines, and a relative decrease in anti-inflammatory IL- 10 cytokine. The development of BA with obesity, even in mild-severity BA, is accompanied by development of a cytokine disbalance, which is typical for a mixed-type inflammation, with a prevalence of neutrophil inflammation.

The purpose of this research was to study the effect of prolonged craniopuncture on the rate of regression of the neurological deficit, the degree of adaptation to daily life, and the level of motivation among patients in the acute period of ischemic stroke. All patients (60 people) were divided into 2 groups, identical by sex, age and severity of the stroke. In the main group of patients (n=30) prolonged scalp therapy was performed simultaneously with classical acupuncture. The peculiarity of this modification of craniopuncture is the use of seven main areas of the scalp with their symmetrical stimulation and duration of exposure for up to 4 hours. Patients of the control group (n=30) underwent acupuncture in combination with scalp therapy according to the classical method. Assessment of the severity of neurological deficit was carried out on the scale NIHSS, while Barthel ADL index was used to assess the patient's daily live activities after stroke. The level of motivation among the patients was studied by using the questionnaire test Motivation for Success and Motivation for Fear of Failure. Monitoring was carried out on the 1st and 15th day from the start of treatment. In the main group of patients, the reduction in the severity of the neurological deficit occurred on the 15th day according to the data NIHSS scale was 5.0 (64,9%), the Barthel index increased by 31.0 points (55,1%). In the control group, similar indicators were 3.5 (50,7%) (on the NIHSS scale) and 23.5 points (40,5%) (Barthel's index). Both groups of patients experienced an increase in the level of motivation and were already oriented to success, but in the main group, these changes appeared to be more significant. Thus, the use of prolonged craniopuncture significantly accelerates the restoration of lost functions and the adaptation of patients to everyday life, in comparison with the classical method of scalp therapy, and has a beneficial effect on increasing the level of patients motivation.
