<p><em>Quality control is carried out as one of the strategies to measure the capability of the production process. This is to identify the factors that cause products no standardized and propose improvements. During this time, there are often problems in the vacuum packaging of frozen fish. The analysis method used Six Sigma with the stages Define, Measure, Analyze and Improve. The results showed that the Critical to Quality (CTQ) of packaging process is leaking packaging, but the process is working well and it is above the industry average in Indonesia. It can be seen the value of Sigma and the final yield. Packaging leaked based on a fishbone diagram and Failure Modes and Effects Analysis (FMEA) is caused by human factors.The proposed improvement is by monitoring, guidance, and training of human resources. This is done so that the process can be towards the target 6-Sigma.</em></p>

Abstrak. Dunia industri yang semakin berkembang akan mengakibatkan banyaknya persaingan. Perus-ahaan yang dapat bersaing adalah perusahaan yang dapat menjaga  kualitas produknya dengan baik sehingga dapat memenuhi kepuasan konsumen. Maka dari itu diperlukan pengendalian kualitas. Penelitian ini difokuskan pada penurunan cacat pada produk batu bata merah di CV. Ghatan Fatahillah dengan metode Six Sigma-DMAIC (Define, Measure, Analyze, Improve, dan Control). Dari hasil penelitian didapatkan DPMO sebesar 36.212 dengan nilai sigma 3,29. Jenis cacat yang paling sering terjadi pada batu bata merah yaitu pecah/patah yaitu sebesar 4.327 atau 59,81% dari total keseluruhan produk yang cacat.  Hasil dari tahap analyze dengan fishbone diagram, ditetapkan penyebab dari pecah/cacat, yaitu: pekerja kurang paham standar kualitas, tidak adanya pemeriksaan produk, pekerja kurang paham prosedur kerja, kinerja mesin tidak stabil, pekerja kurang teliti,  jumlah penggilingan tidak menentu, dan komposisi batu bata tidak menentu. Maka perlu dilakukan usulan perbaikan agar dapat mengurangi jumlah produk yang cacat pada batu bata merah. Kata kunci: DMAIC, DPMO, Pengendalian Kualitas, Six Sigma Abstract. The growing industrial world will result in a lot of competition. Companies that can compete are companies that can maintain the quality of their products well so that they can meet customer satisfaction. Therefore, quality control is needed. This research is focused on reducing defects in red brick products at CV. Ghatan Fatahillah with the Six Sigma-DMAIC method (Define, Measure, Analyze, Improve, and Control). From the research results obtained DPMO of 36,212 with a sigma value of 3.29. The type of defect that most often occurs in red bricks is broken / broken, which is 4,327 or 59.81% of the total defective products. The results of the Analyze stage with the fishbone diagram showed that the causes of breakage / defects were determined, namely: workers do not understand quality standards, lack of product inspection, workers do not understand work procedures, unstable machine performance, workers are not careful, the number of mills is erratic, and composition erratic bricks. So it is necessary to make improvement proposals in order to reduce the number of defective products in red bricks. Keywords: DMAIC, DPMO, Quality Control, Six Sigma

Quality is a factor that can increase product competitiveness. In improving products quality is necessary to control the quality (Quality Control) of the process activities. This study aims to determine the quality control of granulated sugar packaging process by using the seven tool method consisting of check sheet, histogram, Pareto chart, flow chart, scatter diagram, control chart and fishbone diagram. From 30 days observation concluded that the highest type of damage consist of metal detection by 145 bags and torn packaging by 50 bags. Then, from the results of control chart known that out of control process still happen. Both types of damage then revised, and process capability analysis is carried out, so can be concluded that the process is not capable in making the product according to specifications. Therefore, an analysis using fishbone diagram shows that the causes of damage are human, working methods, and machines. From these factors, Standard Operating Procedures can be done as well as designing new machines in packaging process

The study aimed at acknowledging the Six Sigma methodology and the existence of the essential components for the application, as well as reducing the time for completing the operations, reducing the error rate to the lowest possible level, and improving the quality of operations. For this objective, the analytical descriptive methodology was used on a sample consisted of 300 administrative and medical staff from Khartoum State Hospitals (Khartoum, Omdurman, Bahri). To this end, a questionnaire was used for collecting data and for analyzing it and achieving the results of the study by using the statistical analysis package (SPSS). The study deduced a number of results, the most important of which are that the items of commitment and supreme command support for the senior leadership and the methods of abundant human resources on quality control, and the application of the Six Sigma methodology in government hospitals in Khartoum state achieved a satisfactory level, while continuous improvement paragraphs, processes, and systems on quality control and the application of Six Sigma methodology achieved high quality. There is a significant effect on the extent of commitment and support of the higher leadership on quality control and the application of the Six Sigma methodology. Similarly, there is a statistically significant effect on the application of continuous improvement methods on quality control and Six Sigma methodology. Also, there is a statistically significant effect on the application of the abundance of human resources methods variable on quality control and the application of Six Sigma methodology in governmental hospitals in Khartoum state. The study recommended to take interest in the Six Sigma methodology and assure the possibility of using it in Khartoum State’s hospitals because of its scientific and practical importance in improving the quality of services and provided processes, and the need to provide qualified financial and human resources for using and developing Six Sigma methodology in hospitals, as well as great attention in training and providing departments heads with full knowledge of Six Sigma methodology and the fundamentals on which Six Sigma methodology, is based on its importance for hospitals. The study also recommended associating the promotions system in government hospitals in Khartoum state with the quality control program.

ABSTRAKDinamika dan perkembangan ekonomi global dapat memberikan sinyal positif terhadap pentingnya peningkatan pemberdayaan dan daya saing produk di pasar. Dibutuhkan pemikiran untuk membangun keunggulan kompetitif, yang menjadi dasar bagi peningkatan produksi dan daya saing bisnis. Peningkatan produksi merupakan serangkaian aktifitas untuk membentuk produktifitas. Bila salah satu aktifitas produksi mengalami kegagalam maka akan berpengaruh pada kualitas dan kapasitas produksi. Kegagalan biasanya terindikasi dari waste/ pemborosan yang muncul di sepanjang value stream. Terdapat indikasi defect yang tinggi melebihi 10% di sortir dan packaging process. Permasalahan utama adalah adanya waste/ pemborosan yang mengakibatkan penurunan kualitas dan kapasitas produksi. Paper ini akan mengidentifikasi waste yang terjadi di proses produksi, mencari akar penyebab terjadinya waste, dan membangun alternatif solusi perbaikan.  Diperlukan pemikiran kreatif untuk dapat menaikkan efisiensi sumber daya dan daya saing bisnis di pasar. Metoda yang dipakai untuk menyelesaikan permasalahan adalah Lean six-sigma. Konsep ini menelusuri permasalahan inefisiensi dengan cara mencari nonvalue added activity dari munculnya waste di sepanjang value stream.  Kemunculan waste selanjutnya diidentifikasi lebih dalam dengan root cause analisys (RCA) dan failure mode and effects analysis (FMEA). Hasil akhir dari FMEA adalah risk priority number (RPN) yang menggambarkan prioritas atas mode resiko untuk dilakukan perbaikan. Dari hasil prioritas dirancang 3 alternatif perbaikan yaitu pelatihan operator, pembuatan SOP proses produksi, dan pelatihan karyawan quality control.   Dengan pendekatan net present value (NPV), maka diperoleh nilai NPV yang positif, artinya alternatif tersebut adalah layak untuk dijalankan. Alternatif ini meningkatkan nilai sigma dari sigma awal sebesar 2,80 menjadi 3,30. Kenaikan nilai sigma akan memberi indikasi adanya penurunan tingkat defect per million opportunities (DPMO), dan berakibat pada penurunan biaya sampai 20%.  Kata kunci: daya saing, keunggulan kompetitif, Lean Six Sigma, RCA, FMEA, DPMO

Since the establishment of the KPHL Kota Ambon in 2010 until nearly a decade, how has the development been managed? How management performance. Therefore, through this study, the performance of the management of the KPHL Kota Ambon  aims to determine the management performance of KPHL Kota Ambon. This research was conducted at the KPHL Kota Ambon, November-December 2019. The research was conducted using Criteria and indicators made by Forest Wach Indonesia ((FWI), 2014), document verification,  and in-depth interviews with the Head of the UPTD. Pulau Ambon dan Pulau-Pulau Lease. Quantitative analysis method based on calculation procedures made by FWI, 2014. The index results are then described using descriptive methods. The results of the study concluded that 1) The performance of the management of the KPHL Kota Ambon was in the medium criteria index; 2) Management of KPHL Kota Ambon is not yet optimal because the RPHJP has not been ratified as a basis for the preparation of the RPHJPd and Business Plan; 3) The organizational structure has not been filled and the available human resources are not yet competent in the management of FMUs because they have not attended special education and training on FMU management; 4) KPHL Kota Ambon management activities is still limited to socialization activities that have not been routinely so that not many people know about KPHL Kota Ambon

<p>This paper discusses a conducted research in the food industry with a variety of snack products such as bread, cream wafers, cookies, wafer sticks, chocolate, and dry cake. The company should always improve the quality of processes and products according to customer demands. Six Sigma method is expected to improve the quality of process and product in the company. Six Sigma is a quality system approach to be used in the industrialization function engaged in the manufacturing and services sectors. Generally, six sigma strategy utilizes DMAIC approach (Define-Measure-Analyze-Improve-Control). This research focused on product wafers abon, because it contributed a large number of defects. Moreover, this paper discusses two main processes:1. oven and cooling sheet process, 2. creaming and pressing process. The improvement is focused in the creaming and pressing process, because it presents 65% of defects. Based on the obtained data processing, COPQ for 1 month is Rp. 65,473,200, - and there are 3 priority CTQ’s in the issues, Pasta Tidak Rata (53.68%), Crashed Edge (16.29%), and Crack (16.12%). Results of capability process are 91.51% and sigma level of wafer abon product is 3.647628 sigma. An analysis has been done by brainstorming to find the cause of the defect and then arranged in a fishbone diagram and five why’s diagram. Human resource development is essential for any organization that wants to be a dynamic and growth-oriented. Unlike other resources, human resources have the potential ability rather limited. This potential can be used only by creating a climate that can continuously identify, bring to the surface, maintain and use the ability. An HRD number of techniques have been developed in recent years to carry out the above tasks based on certain principles.This study aims to know the extent of the influence of Education and Training toward the Achievement of Employees Performance at PT. ANTAM Tbk, Nickel Ore Processing Unit Pomalaa. The population in this study was all the employees of PT. ANTAM Tbk, Nickel Ore Processing Unit Pomalaa, totaled 1,060 people, sampling is done by using the formula Slovin, so the sample is 91 people. The analysis method used in this study is a simple linear regression analysis.The results of this study showed that the variables of the Education and Training toward the Achievement of Employees Performance at PT. ANTAM Tbk, Nickel Ore Processing Unit Pomalaa. Is positive and significant impact on the Achievement of Employees Performance. The magnitude of the relationship between education and training variables to the variable of achievement of employee performance at PT. ANTAM Tbk, Nickel Ore Processing Unit Pomalaa, in the amount of 85.60% and the other influenced by other factors outside of education and training.

This research aims to manage the amount of defective packaging of capsule products in the department of capsule product at PT SM. It is important because the number of defective packaging of capsule products is high and exceeds the limits of maximum, i.e. 5% of the total production. In addition, the defective packaging of capsule products cannot be reworked and will cause losses in terms of time and finances. This research uses the Six Sigma method to find out the capabilities of the production process from the defective packaging of capsule products. Application of the method of Six Sigma through DMAIC process to eliminate the number of defective products and Fishbone diagram is used to identify the cause of the defect and provide recommendations on improvements to the production process. The data for this research is the amount of production, defective packaging of capsule products and interviews with the department of quality control, department of capsule product and operator. Data processing using Six Sigma methods showed that the value of sigma is 3.41σ with the number of DPMO is 27,951.202. Using Fishbone diagram, there are 9 causes of the defective packaging of capsule products.

ABSTRAK Persaingan industri manufaktur di Indonesia semakin ketat. Dengan adanya pengendalian kualitas yang baik kemudian dikaitkan dengan perbaikan berkelanjutan perlu dilakukan oleh banyak perusahaan agar dapat mendorong peningkatan pasar dan memenangkan persaingan. PT. Roman Ceramics International, merupakan perusahaan perintis dalam produksi keramik lantai di Indonesia. Perusahaan seringkali masih mendapati kejadian banyak Defect yang terjadi pada produk keramik lantai walaupun perusahaan telah melakukan usaha dalam meminimumkan jumlah Defect nya. Maka dari itu dilakukan penelitian yang bertujuan untuk mengurangi jumlah Defect dengan melakukan suatu perbaikan berkelanjutan menggunakan metode Six Sigma dan Kaizen. Berdasarkan data Defect akumulatif terbesar terjadi pada bulan April 2019 dengan jumlah produksi 421.422pcs kemudian jumlah Defect sebesar 20.521pcs dengan presentase Defect sebesar 18,50%. Dengan jenis Defect yang sering terjadi adalah Gupil. Berdasarkan analisa hasil yang didapatkan diketauhi bahwa dengan metode Six Sigma perusahaan mendapati hasil nilai rata-rata DPMO sebesar 10.670 yang berarti level sigma nya sebesar 3,803 atau sekelas dengan rata-rata industri Indonesia. Dari analisa hasil tersebut didapatkan hasil identifikasi akar penyebab terjadinya Defect dari jenis Defect terbesar yaitu Gupil menggunakan fishbone diagram berdasarkan 5 faktor yaitu faktor manusia, faktor lingkungan, faktor mesin, faktor metode dan faktor bahan baku. Berdasarkan fishbone diagram dapat diketauhi suatu usulan perbaikan dengan metode Kaizen yang direkomendasikan yaitu perusahaan memerlukan analisis keputusan untuk memilih supplier batch material maupun keramik lantai yang cacat dan masih memiliki kualitas baik.
