AbstractTrends of cesarean section in Indonesia (2007-2012) have double increase at risk of the emergence of long-term and short-term health problems. This study aimed to determine relation between antenatal care provider and cesarean section. This quantitative study used cross-sectional design with a total sample of 5,143 women aged 15-49 years who gave birth to the last child through cesarean section or not as in urban areas selected in samples of 2012 Indonesia Demographic and Health Survey. Logistic regression multivariate analysis was used to determine relation between antenatal care provider and section cesarean, which was controlled by maternal age, antenatal care facility, parity, and place of birth. The results showed that antenatal care at obstetrician was 6.6 times higher, while antenatal care at obstetrician and midwife was 2.1 times higher for cesarean section compared to women who had antenatal care at midwife after controlled by maternal age, antenatal care facility, parity, and place of birth. There is interaction between socioeconomic status and obstetrician for a cesarean section. Implementation of rules on cesarean section by health institutions, as well as protective and preventive labor efforts on the high economic community group may reduce the occurrence of unnecessary cesarean section.

Background: Disturbance of haematological parameters is prevalent in pregnancy globally leading to a range of feto-maternal complications. Covid-19 infection has potential to enhance the severity of and complications impending due to anaemia in pregnancy. Objective: To determine the effects of Covid-19 infection on haematological parameters during antenatal care among pregnant women in rural Sindh. Study Design: Cross sectional study. Setting: Shaikh Zaid Institute, Chandka Medical College, Larkana. Duration: From March 1, 2021, and May 31, 2021. Materials and Method: A sample of 110 pregnant women, of 18-50 years of age, presenting in first through third trimester with single alive pregnancy, were included in the study after taking a valid written consent. The Research Evaluation Committee of SMBBMU- Larkana granted approval. Women having any haemoglobinopathy were excluded. Covid-19 test conducted through rt-PCR method. A standard questionnaire was used to collect data of two groups. Analysis was performed through SPSS Version-23. Descriptive and inferential statistics were calculated. Results: Mean ± SD age was 30.3 ± 6.99 (Range: 19 to 45) years. Both groups were identical in age. Mean ± SD gestational age was 28.12 ± 4.66 weeks (Range: 20-38). Covid-19 positive women had lower mean Hb (9.7 ± 2.11) than the covid-19 negative (10.54 ± 2.51; P = 0.158). Other haematological parameters like MCV, serum ferritin, TIBC, TLC & platelets were statistically different between groups. Women of eldest group i-e; in 41-50 years, from urban areas, second trimester (gest. age 13-24 weeks), and nulliparous had been affected more from covid-19 (P = 0.271, 0.748, 0.290 & 0.053). Frequency and severity of anaemia was more among Covid-19 positive women. Conclusion: Covid‐19 demands maternal healthcare to be extra vigilant to the haematological parameters of pregnant women during antenatal care till delivery. Keywords: Antenatal care, pregnancy, anaemia, haemoglobin, Covid‐19.

Introduction The World Health Organization and the United Nations International Children’s Emergency Fund jointly recommend colostrum feeding immediately following delivery of the newborn. Colostrum avoidance is the practice of discarding colostrum at least once within the first three days after delivery of the newborn. Colostrum contains antibodies that protect the newborn against disease. Despite this fact, many Ethiopian mothers see colostrum feeding as a cause of neonatal morbidity and mortality, and they believe colostrum should be discarded to alleviate its effects. However, the cause of this misconception about colostrum is not well researched in Ethiopia, particularly in the study area. Objective To assess the prevalence of colostrum avoidance and associated factors among mothers of children aged less than six months in Bure district, Amhara Region, North West, Ethiopia. Methods A community-based cross-sectional study was conducted from March 1st to 30th, 2019 in Bure district. Structured questionnaires and face to face interviews were used to collect data from 621 study participants. Multistage sampling technique was used to select study participants. Data were entered into Epi Data version 4.2.0 and then exported to Statistical Package for the Social Sciences version 23. Bivariate and multivariate logistic regression analyses were performed to identify predictors of colostrum avoidance practice. Results Six hundred twenty-one (621) mothers of children aged less than six months participated. The prevalence of colostrum avoidance was 14.5% (95%CI; 11.5–17.4). The multivariate analysis indicated that home delivery [AOR = 3.350, (95%CI; 2.517–14.847)], giving birth through cesarean section [AOR = 3.368, (95%CI; 1.339–8.471)], no participation in an antenatal care group [AOR = 1.889, (95%CI; 1.144–3.533)], poor knowledge of mothers about colostrum [AOR = 3.44, (95%CI; 2.45–4.977)], and poor attitude of mothers towards colostrum [AOR = 3.053, (95%CI; 1.157–8.054)] were important predictors of colostrum avoidance practice. Conclusion and recommendations Home delivery, giving birth through cesarean section, no participation in an antenatal care group, poor knowledge of mothers about colostrum, and poor attitude of mothers towards colostrum were significantly associated with colostrum avoidance practice. Therefore, health care workers in the district should promote institutional delivery, reduce the magnitude of cesarean section, encourage pregnant mothers to participate in an antenatal care group, and enhance maternal awareness about the merits of colostrum feeding. Moreover, health educations for mothers to have a positive attitude towards colostrum are important recommendations to be taken to prevent the further practice of colostrum avoidance.

Background: Bangladesh is a small South Asian country which became independent in 1971 after a bloody war. Rapid urbanisation in Bangladesh (26% of the 147.1 million inhabitants live in urban areas) is fuelling a growth in urban poverty, particularly in the urban slums where the quality of life is extremely poor. The average population density in slums was reported in 2005 as 831 persons per acre or 205,415 people per square kilometre.1 Early commencement of antenatal care by pregnant women as well as regular visits has the potential to affect maternal and foetal outcome. Objective: To assess the status of ANC service used by the pregnant mothers and their socio-demographic characteristics.Materials and Methods: A community-based cross-sectional study was conducted at Moghbazar slum area in Dhaka district of Bangladesh, during January to June 2014. A total of 161 slum dwellers were enrolled in the study. Information regarding education, occupation, monthly family income, antenatal care was gathered using a pretested structured questionnnare and data were analysed. Results: The majority respondents had knowledge about antenatal care and of them 89 (55.2%) completed ?3 visits. Forty five (47.8%) pregnant women received ANC from government hospitals. Nearly 72% mothers received ANC service from doctors and 16.9% received from family welfare visitors (FWV). Half of mothers were satisfied with the overall care provided to them. About 86.2% mothers said that they had to wait for more than two hours for check-ups. More than 50% received information about exercise and 36% were reassured about discussing fear and anxiety about pregnancy. Conclusion: This study reveals that antenatal care provided was not up to the mark of standard care and measures should be taken to improve it.J Enam Med Col 2015; 5(3): 166-169

Kata kunci : ASI satu jam pertama, faktor risiko, SDKI 2002 – 2003.Infant Mortality Rate (IMR) in Indonesia is still the highest among the other ASEAN countries. The major cause for infant and children mortality is infections, espe- cially the upper respiratory tracts infection and diarrhea. The prevention efforts for reducing the infections are a good nutrition management for infant and children such as adequate and appropriate breastfeeding. A good start for breastfeeding is about 30 minutes after delivery. The Indonesia DHS 2002-2003 showed that 95.5% children under five have already have breast-milk, but only 38.7% of them are having the first breast-milk within one hour after delivery. The Objective of this study is to know the factors related to the breastfeeding given within one hour after delivery. The study uses secondary source of data of the Indonesia DHS 2002-2003 with a cross-sectional design. The number of sample is 6.018, which are mothers who have the latest life child aged 0 to 24 months and still having breastfed and de- livered without surgery. Data are analyzed using the application multivariate logistic regression. The study has found that the proportion of breastfeeding given within one hour after delivery as high as 38.28%. The dominant factor related to the breastfeeding given within one hour after delivery is the antenatal care provider. Other factor are: the residential location, wanted pregnancy, the antenatal care provider, birth attendance, accessibility on radio, and newborn’s weight. There is an inter-action between residential location and the antenatal care provider, wanted pregnancy and the antenatal care provider, and newborn’s weight and the birth atten- dance. There is a need to make an effort on: increasing the knowledge and motivation for the health provider about the importance of the immediate administration of breastfeeding to the newborn and the exclusive breastfeeding. The efforts should be supported by government policy.Key words : Breastfed in first one hour, risk factor, IDHS 2002-2003.

Abstract Background: Cesarean section is an effective technique for preventing maternal and perinatal mortality when performed properly. Evidence suggested that pregnant women who have had good knowledge about their condition are able to participate in shared decision-making and can alleviate fears related to anesthesia. However, only a few studies conducted in Ethiopia assess the level of knowledge and attitude of pregnant women towards anesthesia for cesarean section. Methods: Hospital based cross-sectional study was conducted from April 18 to June 25, 2021. A systematic random sampling technique was used to select a total of 362 pregnant women attending antenatal care. Pretested and structured questionnaires were used to collect the data using a face-to-face interview. Both bivariate and multivariable binary logistic regression models were used for statistical analysis. The adjusted odds ratio was used as the measure of association. P-value less than 0.05 at 95% CI was considered as statistically significant. Results: A total of 354 pregnant women attending at ANC clinic participated with a response rate of 97.8%. The overall proportion of good knowledge and positive attitude towards anesthesia for cesarean section were 56.5% (95% CI: 51.4%, 61.9%) and 50.8% (95% CI: 45.8%, 56.2%), respectively. Pregnant women who had previous operations (AOR=4.47, 95% CI: 1.77, 11.32) and had any health information about anesthesia (AOR=10.06, 95% CI: 5.41, 18.69) were significantly associated with good knowledge towards anesthesia for cesarean section. While, pregnant women who have had secondary education (AOR=6.71, 95% CI: 1.19, 37.99), college and above education (AOR=13.63, 95% CI: 2.26, 82.21), and had health information about anesthesia (AOR=2.02, 95% CI: 1.09, 3.77) significantly associated with a positive attitude towards anesthesia for CS.Conclusion: This finding means that a significant number of pregnant women still have poor knowledge and attitude towards anesthesia for CS. Previous operation and health information about anesthesia were significantly associated with knowledge while higher education and health information about anesthesia were significantly associated with a positive attitude towards anesthesia for cesarean section. Health professionals need to provide health information during ANC visits and in the operating theater areas for mothers who have indications for CS.

Continuity of breastfeeding process when mothers return to work is a serious issue that immediately must be followed up, so that exclusive breastfeeding program within the first six months can be achieved. Beside providing many benefits for babies, breastfeeding is also beneficial for mothers and entrepreneurs. This study aimed to determine relation of working mothers to exclusive breastfeeding. This study used was cross- sectional design with secondary data of Indonesia Demographic and Health Survey 2012 with samples as many as 1,193 mothers aged 15 – 49 years who had 0 – 5-month-old babies. Based on multivariate analysis, working mothers could decrease opportunity of exclusive breastfeeding in which mother who worked all the time were 1.54 times more likely not to give exclusive breastfeeding than mothers who did not work after controlled by maternal age at childbirth, household wealth index, and antenatal care frequency (p = 0.038; 95% CI = 1.0 to 2.3). Fulltime working mothers are twofold more likely to not be able to give exclusive breasfedding than unemployed mothers after being controlled by counfounder variable.
